b0d9efba3e USB: centralize -EREMOTEIO handling
This patch (as969) continues the ongoing changes to the way HCDs
report URB statuses.  The programming interface has been simplified by
making usbcore responsible for clearing urb->hcpriv and for setting
-EREMOTEIO status when an URB with the URB_SHORT_NOT_OK flag ends up
as a short transfer.

By moving the work out of the HCDs, this removes a fair amount of
repeated code.

e39ab592f1 USB: remove unnecessary tests in isp116x and sl811
This patch (as962) cleans up some code I forgot to remove earlier in
the isp116x and sl811 HCDs.  There is no longer any need to check for
unlink-during-submit; it can't happen since the endpoint queues are
now under the protection of the HCD-private spinlock.

e9df41c5c5 USB: make HCDs responsible for managing endpoint queues
This patch (as954) implements a suggestion of David Brownell's.  Now
the host controller drivers are responsible for linking and unlinking
URBs to/from their endpoint queues.  This eliminates the possiblity of
strange situations where usbcore thinks an URB is linked but the HCD
thinks it isn't.  It also means HCDs no longer have to check for URBs
being dequeued before they were fully enqueued.

In addition to the core changes, this requires changing every host
controller driver and the root-hub URB handler.  For the most part the
required changes are fairly small; drivers have to call
usb_hcd_link_urb_to_ep() in their urb_enqueue method,
usb_hcd_check_unlink_urb() in their urb_dequeue method, and
usb_hcd_unlink_urb_from_ep() before giving URBs back.  A few HCDs make
matters more complicated by the way they split up the flow of control.

In addition some method interfaces get changed.  The endpoint argument
for urb_enqueue is now redundant so it is removed.  The unlink status
is required by usb_hcd_check_unlink_urb(), so it has been added to
urb_dequeue.

7d12e780e0 IRQ: Maintain regs pointer globally rather than passing to IRQ handlers
Maintain a per-CPU global "struct pt_regs *" variable which can be used instead
of passing regs around manually through all ~1800 interrupt handlers in the
Linux kernel.

The regs pointer is used in few places, but it potentially costs both stack
space and code to pass it around.  On the FRV arch, removing the regs parameter
from all the genirq function results in a 20% speed up of the IRQ exit path
(ie: from leaving timer_interrupt() to leaving do_IRQ()).

Where appropriate, an arch may override the generic storage facility and do
something different with the variable.  On FRV, for instance, the address is
maintained in GR28 at all times inside the kernel as part of general exception
handling.

Having looked over the code, it appears that the parameter may be handed down
through up to twenty or so layers of functions.  Consider a USB character
device attached to a USB hub, attached to a USB controller that posts its
interrupts through a cascaded auxiliary interrupt controller.  A character
device driver may want to pass regs to the sysrq handler through the input
layer which adds another few layers of parameter passing.

I've build this code with allyesconfig for x86_64 and i386.  I've runtested the
main part of the code on FRV and i386, though I can't test most of the drivers.
I've also done partial conversion for powerpc and MIPS - these at least compile
with minimal configurations.

This will affect all archs.  Mostly the changes should be relatively easy.
Take do_IRQ(), store the regs pointer at the beginning, saving the old one:

	struct pt_regs *old_regs = set_irq_regs(regs);

And put the old one back at the end:

	set_irq_regs(old_regs);

Don't pass regs through to generic_handle_irq() or __do_IRQ().

In timer_interrupt(), this sort of change will be necessary:

	-	update_process_times(user_mode(regs));
	-	profile_tick(CPU_PROFILING, regs);
	+	update_process_times(user_mode(get_irq_regs()));
	+	profile_tick(CPU_PROFILING);

I'd like to move update_process_times()'s use of get_irq_regs() into itself,
except that i386, alone of the archs, uses something other than user_mode().

Some notes on the interrupt handling in the drivers:

 (*) input_dev() is now gone entirely.  The regs pointer is no longer stored in
     the input_dev struct.

 (*) finish_unlinks() in drivers/usb/host/ohci-q.c needs checking.  It does
     something different depending on whether it's been supplied with a regs
     pointer or not.

 (*) Various IRQ handler function pointers have been moved to type
     irq_handler_t.

38e2bfc94e USB: Dealias -110 code (more complete)
The purpose of this patch is to split off the case when a device does
not reply on the lower level (which is reported by HC hardware), and
a case when the device accepted the request, but does not reply at
upper level. This redefinition allows to diagnose issues easier,
without asking the user if the -110 happened "immediately".

The usbmon splits such cases already thanks to its timestamp, but
it's not always available.

I adjusted all drivers which I found affected (by searching for "urb").
Out of tree drivers may suffer a little bit, but I do not expect much
breakage. At worst they may print a few messages.

185849991d PM: USB HCDs use PM_EVENT_PRETHAW
This teaches several USB host controller drivers to treat PRETHAW as a chip
reset since the controller, and all devices connected to it, are no longer in
states compatible with how the snapshotted suspend() left them.

6a8e87b23f [PATCH] USB core and HCDs: don't put_device while atomic
This patch (as640) removes several put_device and the corresponding
get_device calls from the USB core and HCDs.  Some of the puts were done
in atomic contexts, and none of them are needed since the core now
guarantees that every endpoint will be disabled and every URB completed
before a USB device is released.

1c50c317e2 [PATCH] USB: central handling for host controllers that were reset during suspend/resume
This patch (as515b) adds a routine to usbcore to simplify handling of
host controllers that lost power or were reset during suspend/resume.
The new core routine marks all the child devices of the root hub as
NOTATTACHED and tells khubd to disconnect the device structures as soon
as possible.

0c0382e32d [PATCH] USB: Rename hcd->hub_suspend to hcd->bus_suspend
This patch (as580) is perhaps the only result from the long discussion I
had with David about his changes to the root-hub suspend/resume code.  It
renames the hub_suspend and hub_resume methods in struct usb_hcd to
bus_suspend and bus_resume.  These are more descriptive names, since the
methods really do suspend or resume an entire USB bus, and less likely to
be confused with the hub_suspend and hub_resume routines in hub.c.

It also takes David's advice about removing the layer of bus glue, where
those methods are called.  And it implements a related change that David
made to the other HCDs but forgot to put into dummy_hcd.

9480e307cd [PATCH] DRIVER MODEL: Get rid of the obsolete tri-level suspend/resume callbacks
In PM v1, all devices were called at SUSPEND_DISABLE level.  Then
all devices were called at SUSPEND_SAVE_STATE level, and finally
SUSPEND_POWER_DOWN level.  However, with PM v2, to maintain
compatibility for platform devices, I arranged for the PM v2
suspend/resume callbacks to call the old PM v1 suspend/resume
callbacks three times with each level in order so that existing
drivers continued to work.

Since this is obsolete infrastructure which is no longer necessary,
we can remove it.  Here's an (untested) patch to do exactly that.

4b2e790a4d [PATCH] USB: sl811-hcd minor fixes
Three minor sl811-hcd fixes:

 - Elminate memory leak on one (rare) disable/shutdown path.

 - For periodic transfers that don't need to be scheduled, update
   urb->start_frame to represent the transfer phase correctly.

 - Report the (single) port as removable, by default.

Since no drivers yet use start_frame or that part of the hub descriptor,
only that leak is likely to ever matter.

ca078bae81 [PATCH] swsusp: switch pm_message_t to struct
This adds type-checking to pm_message_t, so that people can't confuse it
with int or u32.  It also allows us to fix "disk yoyo" during suspend (disk
spinning down/up/down).

[We've tried that before; since that cpufreq problems were fixed and I've
tried make allyes config and fixed resulting damage.]

1e9a47b62f [PATCH] USB: sl811-hcd fixes
Various fixes to the sl811-hcd driver:

  * Fix small glitches that crept in during recent evolution of usbcore's hcd
    glue layer, coupling endpoint state records to usbcore and active urbs.
    (As noted by folk whose boards weren't stuck on 2.6.9 kernels...)

  * Cope with various system-specific issues:
      - Some configurations (e.g. a CF-card uses this chip) have iospace
        addresses for the two registers, rather than memory mapped ones.
      - Some configurations do interesting things with IRQs; maybe the
        line is shared, or it doesn't support level triggering.
      - Not all boards can drive the chip reset line in software.

  * Address a potential race during unlinking.

  * Tweak probe/remove section info to handle the case where this segment
    of a platform bus is hotpluggable (e.g. CF card).  (The basic problem
    is that CONFIG_HOTPLUG is global, which is wrong since not all busses
    can hotplug even on hotplug-friendly systems...)  Also export the
    driver, so that the CF driver can depend on it.

Also removed some annoying end-of-line whitespace.

1da177e4c3 Linux-2.6.12-rc2
Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.

Let it rip!
