d3556babd7 ocfs2: fix d_splice_alias() return code checking
d_splice_alias() can return a valid dentry, NULL or an ERR_PTR.
Currently the code checks not for ERR_PTR and will cuase an oops in
ocfs2_dentry_attach_lock().  Fix this by using IS_ERR_OR_NULL().

2bd63329cb ocfs2: replace strnicmp with strncasecmp
The kernel used to contain two functions for length-delimited,
case-insensitive string comparison, strnicmp with correct semantics and
a slightly buggy strncasecmp.  The latter is the POSIX name, so strnicmp
was renamed to strncasecmp, and strnicmp made into a wrapper for the new
strncasecmp to avoid breaking existing users.

To allow the compat wrapper strnicmp to be removed at some point in the
future, and to avoid the extra indirection cost, do
s/strnicmp/strncasecmp/g.

b246d3d11e ocfs2: fix a deadlock while o2net_wq doing direct memory reclaim
Fix a deadlock problem caused by direct memory reclaim in o2net_wq.  The
situation is as follows:

1) Receive a connect message from another node, node queues a
   work_struct o2net_listen_work.

2) o2net_wq processes this work and call the following functions:

o2net_wq
-> o2net_accept_one
  -> sock_create_lite
    -> sock_alloc()
      -> kmem_cache_alloc with GFP_KERNEL
        -> ____cache_alloc_node
          ->__alloc_pages_nodemask
            -> do_try_to_free_pages
              -> shrink_slab
                -> evict
                  -> ocfs2_evict_inode
                    -> ocfs2_drop_lock
                      -> dlmunlock
                        -> o2net_send_message_vec

   then o2net_wq wait for the unlock reply from master.

3) tcp layer received the reply, call o2net_data_ready() and queue
   sc_rx_work, waiting o2net_wq to process this work.

4) o2net_wq is a single thread workqueue, it process the work one by
   one.  Right now it is still doing o2net_listen_work and cannot handle
   sc_rx_work.  so we deadlock.

Junxiao Bi's patch "mm: clear __GFP_FS when PF_MEMALLOC_NOIO is set"
(http://ozlabs.org/~akpm/mmots/broken-out/mm-clear-__gfp_fs-when-pf_memalloc_noio-is-set.patch)
clears __GFP_FS in memalloc_noio_flags() besides __GFP_IO.  We use
memalloc_noio_save() to set process flag PF_MEMALLOC_NOIO so that all
allocations done by this process are done as if GFP_NOIO was specified.
We are not reentering filesystem while doing memory reclaim.

f775da2fc2 ocfs2: fix deadlock due to wrong locking order
For commit ocfs2 journal, ocfs2 journal thread will acquire the mutex
osb->journal->j_trans_barrier and wake up jbd2 commit thread, then it
will wait until jbd2 commit thread done. In order journal mode, jbd2
needs flushing dirty data pages first, and this needs get page lock.
So osb->journal->j_trans_barrier should be got before page lock.

But ocfs2_write_zero_page() and ocfs2_write_begin_inline() obey this
locking order, and this will cause deadlock and hung the whole cluster.

70e82a12db ocfs2: fix deadlock between o2hb thread and o2net_wq
The following case may lead to o2net_wq and o2hb thread deadlock on
o2hb_callback_sem.
Currently there are 2 nodes say N1, N2 in the cluster. And N2 down, at
the same time, N3 tries to join the cluster. So N1 will handle node
down (N2) and join (N3) simultaneously.

5046f18d5b ocfs2: don't fire quorum before connection established
Firing quorum before connection established can cause unexpected node to
reboot.

Assume there are 3 nodes in the cluster, Node 1, 2, 3.  Node 2 and 3 have
wrong ip address of Node 1 in cluster.conf and global heartbeat is enabled
in the cluster.  After the heatbeats are started on these three nodes,
Node 1 will reboot due to quorum fencing.  It is similar case if Node 1's
networking is not ready when starting the global heartbeat.

The reboot is not friendly as customer is not fully ready for ocfs2 to
work.  Fix it by not allowing firing quorum before the connection is
established.  In this case, ocfs2 will wait until the wrong configuration
is fixed or networking is up to continue.  Also update the log to guide
the user where to check when connection is not built for a long time.

Note that the code in and using sc_common_open() has been quite
extensively changed.  Not least because there was a latent memory leak in
the code as was: if sc_common_open() failed, the previously allocated
buffer was not freed.

7fa05c6e46 ocfs2: fix shift left operations overflow
ocfs2_inode_info->ip_clusters and ocfs2_dinode->id1.bitmap1.i_total are
defined as type u32, so the shift left operations may overflow if volume
size is large, for example, 2TB and cluster size is 1MB.

c53f755d33 ocfs2: Back out change to use OCFS2_MAXQUOTAS in ocfs2_setattr()
ocfs2_setattr() actually needs to really use MAXQUOTAS and not
OCFS2_MAXQUOTAS since it will pass the array over to VFS. Currently
this isn't a problem since MAXQUOTAS == OCFS2_MAXQUOTAS but it would
be once we introduce project quotas.

5760a97c71 ocfs2/dlm: do not get resource spinlock if lockres is new
There is a deadlock case which reported by Guozhonghua:
  https://oss.oracle.com/pipermail/ocfs2-devel/2014-September/010079.html

This case is caused by &res->spinlock and &dlm->master_lock
misordering in different threads.

It was introduced by commit 8d400b81cc ("ocfs2/dlm: Clean up refmap
helpers").  Since lockres is new, it doesn't not require the
&res->spinlock.  So remove it.

MAXQUOTAS value defines maximum number of quota types VFS supports.
This isn't necessarily the number of types ocfs2 supports and with
addition of project quotas these two numbers stop matching. So make
ocfs2 use its private definition.

8e9801dfe3 ocfs2: o2net: set tcp user timeout to max value
When tcp retransmit timeout(15mins), the connection will be closed.
Pending messages may be lost during this time.  So we set tcp user
timeout to override the retransmit timeout to the max value.  This is OK
for ocfs2 since we have disk heartbeat, if peer crash, the disk
heartbeat will timeout and it will be evicted, if disk heartbeat not
timeout and connection idle for a long time, then this means the cluster
enters split-brain state, since fence can't happen, we'd better keep the
connection and wait network recover.

c43c363def ocfs2: o2net: don't shutdown connection when idle timeout
This patch series is to fix a possible message lost bug in ocfs2 when
network go bad.  This bug will cause ocfs2 hung forever even network
become good again.

The messages may lost in this case.  After the tcp connection is
established between two nodes, an idle timer will be set to check its
state periodically, if no messages are received during this time, idle
timer will timeout, it will shutdown the connection and try to
reconnect, so pending messages in tcp queues will be lost.  This
messages may be from dlm.  Dlm may get hung in this case.  This may
cause the whole ocfs2 cluster hung.

This is very possible to happen when network state goes bad.  Do the
reconnect is useless, it will fail if network state is still bad.  Just
waiting there for network recovering may be a good idea, it will not
lost messages and some node will be fenced until cluster goes into
split-brain state, for this case, Tcp user timeout is used to override
the tcp retransmit timeout.  It will timeout after 25 days, user should
have notice this through the provided log and fix the network, if they
don't, ocfs2 will fall back to original reconnect way.

This patch (of 3):

Some messages in the tcp queue maybe lost if we shutdown the connection
and reconnect when idle timeout.  If packets lost and reconnect success,
then the ocfs2 cluster maybe hung.

To fix this, we can leave the connection there and do the fence decision
when idle timeout, if network recover before fence dicision is made, the
connection survive without lost any messages.

This bug can be saw when network state go bad.  It may cause ocfs2 hung
forever if some packets lost.  With this fix, ocfs2 will recover from
hung if network becomes good again.

2b462638e4 ocfs2: do not write error flag to user structure we cannot copy from/to
If we failed to copy from the structure, writing back the flags leaks 31
bits of kernel memory (the rest of the ir_flags field).

In any case, if we cannot copy from/to the structure, why should we
expect putting just the flags to work?

Also make sure ocfs2_info_handle_freeinode() returns the right error
code if the copy_to_user() fails.

bba1cb17d9 ocfs2: race between umount and unfinished remastering during recovery
Orabug: 19074140

When umount is issued during recovery on the new master that has not
finished remastering locks, it triggers BUG() in
dlm_send_mig_lockres_msg().  Here is the situation:

 1) node A has a lock on resource X mastered by node B.

 2) node B dies ->  node A sets recovering flag for res X

 3) Node C becomes the new master for resources owned by the
    dead node and is remastering locks of the dead node but
    has not finished the remastering process yet.

 4) umount is issued on node C.

 5) During processing of umount, ignoring unfished recovery,
    node C attempts to migrate resource X to node A.

 6) node A finds res X in DLM_LOCK_RES_RECOVERING state, considers
    it a logic error and sends back -EFAULT.

 7) node C asserts BUG() upon seeing EFAULT resp from node B.

Fix is to delay migrating res X till remastering is finished at which
point recovering flag will be cleared on both A and C.

981035b47d ocfs2: correctly check the return value of ocfs2_search_extent_list
ocfs2_search_extent_list may return -1, so we should check the return

is still queued for master assert.  it may trigger BUG() as follows.

dlmlock_remote() send create_lock message to N2, but receive DLM_IVLOCKID,
if it is RECOVERY lockres, it triggers the BUG().

Another BUG() is triggered when N3 become the new master and send
assert_master to N1, N1 will trigger the BUG() because owner doesn't
match.  So we should not purge lockres when it is queued for assert
master.

f7a14f32e7 ocfs2: fix a tiny race when running dirop_fileop_racer
When running dirop_fileop_racer we found a dead lock case.

2 nodes, say Node A and Node B, mount the same ocfs2 volume.  Create
/race/16/1 in the filesystem, and let the inode number of dir 16 is less
than the inode number of dir race.

Node A                            Node B
mv /race/16/1 /race/
                                  right after Node A has got the
                                  EX mode of /race/16/, and tries to
                                  get EX mode of /race
                                  ls /race/16/

In this case, Node A has got the EX mode of /race/16/, and wants to get EX
mode of /race/.  Node B has got the PR mode of /race/, and wants to get
the PR mode of /race/16/.  Since EX and PR are mutually exclusive, dead
lock happens.

This patch fixes this case by locking in ancestor order before trying
inode number order.

a270c6d3c0 ocfs2/dlm: fix misuse of list_move_tail() in dlm_run_purge_list()
When a lockres in purge list but is still in use, it should be moved to
the tail of purge list.  dlm_thread will continue to check next lockres in
purge list.  However, code list_move_tail(&dlm->purge_list,
&lockres->purge) will do *no* movements, so dlm_thread will purge the same
lockres in this loop again and again.  If it is in use for a long time,
other lockres will not be processed.

          -> check whether osb->recovery_map->rm_used is zero

Because osb->recovery_map is already freed, rm_used can be any other
values, so it may yield umount hang.

27bf6305cf ocfs2: fix deadlock when two nodes are converting same lock from PR to EX and idletimeout closes conn
Orabug: 18639535

Two node cluster and both nodes hold a lock at PR level and both want to
convert to EX at the same time.  Master node 1 has sent BAST and then
closes the connection due to idletime out.  Node 0 receives BAST, sends
unlock req with cancel flag but gets error -ENOTCONN.  The problem is
this error is ignored in dlm_send_remote_unlock_request() on the
**incorrect** assumption that the master is dead.  See NOTE in comment
why it returns DLM_NORMAL.  Upon getting DLM_NORMAL, node 0 proceeds to
sends convert (without cancel flg) which fails with -ENOTCONN.  waits 5
sec and resends.

This time gets DLM_IVLOCKID from the master since lock not found in
grant, it had been moved to converting queue in response to conv PR->EX
req.  No way out.

   -> ocfs2_recover_orphans
  The inode of the file b will be
  put with iput().

  ocfs2_evict_inode
   -> ocfs2_delete_inode
   -> ocfs2_wipe_inode
   -> ocfs2_remove_inode
  OCFS2_VALID_FL in the inode
  i_flags will be cleared.

                                   The file b still can be accessed
                                   on node B.
                                   ls /mnt/ocfs2
                                   When first read the file b with
                                   ocfs2_read_inode_block(). It will
                                   validate the inode using
                                   ocfs2_validate_inode_block().
                                   Because OCFS2_VALID_FL not set in
                                   the inode i_flags, so the file
                                   system will be readonly.

So we should add inode into orphan dir after updating entry in
ocfs2_rename().

17bf1418b7 ocfs2: fix incorrect i_size of global bitmap inode after resize
Ocfs2 cluster size may be 1MB, which has 20 bits.  When resize, the
input new clusters is mostly the number of clusters in a group
descriptor(32256).

Since the input clusters is defined as type int, so it will overflow
when shift left 20 bits and then lead to incorrect global bitmap i_size.

Revert commit 75f82eaa50 ("ocfs2: fix NULL pointer dereference when
dismount and ocfs2rec simultaneously") because it may cause a umount
hang while shutting down the truncate log.

fix NULL pointer dereference when dismount and ocfs2rec simultaneously

The situation is as followes:
ocfs2_dismout_volume
-> ocfs2_recovery_exit
  -> free osb->recovery_map
-> ocfs2_truncate_shutdown
  -> lock global bitmap inode
    -> ocfs2_wait_for_recovery
	  -> check whether osb->recovery_map->rm_used is zero

Because osb->recovery_map is already freed, rm_used can be any other
values, so it may yield umount hang.

To prevent NULL pointer dereference while getting sys_root_inode, we use
a osb_tl_disable flag to disable schedule osb_truncate_log_wq after
truncate log shutdown.

happened to crash before send DLM_PROXY_AST_MSG for a downconverting
lock.  The situation is as follows:

and call ocfs2_cluster_unlock
to decrease ex_holders.
                                                 converting lock_3 from

                           send DLM_PROXY_AST_MSG
                           to Node1, asking Node 1

receiving DLM_PROXY_AST_MSG,
thread ocfs2dc send
DLM_CONVERT_LOCK_MSG
to Node2 to downconvert
lock_1(EX->NL).
                           lock_1 can be granted and
                           put it into pending_asts
                           list, return DLM_NORMAL.
                           then something happened
                           and Node2 crashed.
received DLM_NORMAL, waiting
for DLM_PROXY_AST_MSG.
                                               selected as the recovery
                                               master, receving migrate
                                               lock from Node1, queue
                                               lock_1 to the tail of
                                               converting list.

After dlm recovery, converting list in the master of lockresA(Node3)
will be: converting list head <-> lock_3(NL->EX) <->lock_1(EX<->NL).
Requested mode of lock_3 is not compatible with the granted mode of
lock_1, so it can not be granted.  and lock_1 can not downconvert
because covnerting queue is strictly FIFO.  So a deadlock is created.
We think function dlm_process_recovery_data() should queue_ast for
lock_1 or alter the order of lock_1 and lock_3, so dlm_thread can
process lock_1 first.  And if there are multiple downconverting locks,
they must convert form PR to NL, so no need to sort them.

55b465b668 ocfs2: limit printk when journal is aborted
Once JBD2_ABORT is set, ocfs2_commit_cache will fail in
ocfs2_commit_thread.  Then it will get into a loop with mass logs.  This
will meaninglessly consume a larger number of resource and may lead to
the system hanging.  So limit printk in this case.

676d23690f net: Fix use after free by removing length arg from sk_data_ready callbacks.
Several spots in the kernel perform a sequence like:

	skb_queue_tail(&sk->s_receive_queue, skb);
	sk->sk_data_ready(sk, skb->len);

But at the moment we place the SKB onto the socket receive queue it
can be consumed and freed up.  So this skb->len access is potentially
to freed up memory.

Furthermore, the skb->len can be modified by the consumer so it is
possible that the value isn't accurate.

And finally, no actual implementation of this callback actually uses
the length argument.  And since nobody actually cared about it's
value, lots of call sites pass arbitrary values in such as '0' and
even '1'.

So just remove the length argument from the callback, that way there
is no confusion whatsoever and all of these use-after-free cases get
fixed as a side effect.

Based upon a patch by Eric Dumazet and his suggestion to audit this
issue tree-wide.

91b0abe36a mm + fs: store shadow entries in page cache
Reclaim will be leaving shadow entries in the page cache radix tree upon
evicting the real page.  As those pages are found from the LRU, an
iput() can lead to the inode being freed concurrently.  At this point,
reclaim must no longer install shadow pages because the inode freeing
code needs to ensure the page tree is really empty.

Add an address_space flag, AS_EXITING, that the inode freeing code sets
under the tree lock before doing the final truncate.  Reclaim will check
for this flag before installing shadow pages.

The problem here is:

We cached a block, but the buffer_head got reused.  When we are to pick
up this block again, a new buffer_head created with UPTODATE flag
cleared.  ocfs2_buffer_uptodate() returned false since no UPTODATE is
set on the buffer_head.  so we set this block to cache as a NEW block,
then it failed at asserting block is not in cache.

The fix is to add a new parameter indicating the bucket is a new
allocated or not to ocfs2_init_xattr_bucket().
ocfs2_init_xattr_bucket() assert block not cached accordingly.

                                    at the moment, B thread also gets
                                    two refs, so lead to one more
                                    inode ref.

So add mutex lock to avoid multi thread set two inode ref once at the
same time.

da8ded405d ocfs2/o2net: o2net_listen_data_ready should do nothing if socket state is not TCP_LISTEN
Orabug: 17330860

When accepting an incomming connection o2net_accept_one clones a child
data socket from the parent listening socket.  It then proceeds to setup
the child with callback o2net_data_ready() and sk_user_data to NULL.  If
data arrives in this window, o2net_listen_data_ready will be called with
some non-deterministic value in sk_user_data (not inherited).  We panic
when we page fault on sk_user_data -- in parent it is
sock_def_readable().

The fix is to recognize that this is a data socket being set up by
looking at the socket state and do nothing.

ocfs2_do_flock() calls ocfs2_file_lock() to get the cross-node clock and
then call flock_lock_file_wait() to compete with local processes.  In
case flock_lock_file_wait() failed, say -ENOMEM, clean up work is not
done.  This patch adds the cleanup --drop the cross-node lock which was
just granted.

missed that ocfs2 is calling kfree(xattr->name).  As a result, kernel

ocfs2_mknod() and ocfs2_symlink().

3ed2be719e ocfs2: allow for more than one data extent when creating xattr
Orabug: 18108070

ocfs2_xattr_extend_allocation() hits panic when creating xattr during
data extent alloc phase.  The problem occurs if due to local alloc
fragmentation, clusters are spread over multiple extents.  In this case
ocfs2_add_clusters_in_btree() finds no space to store more than one
extent record and therefore fails returning RESTART_META.  The situation
is anticipated for xattr update case but not xattr create case.  This
fix simply ports that code to create case.

caused performance regression in the remote unlink() calls.

  f7b1aa69be - ocfs2: Fix deadlock on umount
  5fd1318937 - ocfs2: Don't oops in ocfs2_kill_sb on a failed mount

Previous patches in this series removed the possible deadlocks from
downconvert thread so the above patches shouldn't be needed anymore.

The regression is caused because these patches delay the iput() in case
of dentry unlocks.  This also delays the unlocking of the open lockres.
The open lockresource is required to test if the inode can be wiped from
disk or not.  When the deleting node does not get the open lock, it
marks it as orphan (even though it is not in use by another
node/process) and causes a journal checkpoint.  This delays operations
following the inode eviction.  This also moves the inode to the orphaned
inode which further causes more I/O and a lot of unneccessary orphans.

The following script can be used to generate the load causing issues:

84d86f83f9 ocfs2: avoid blocking in ocfs2_mark_lockres_freeing() in downconvert thread
If we are dropping last inode reference from downconvert thread, we will
end up calling ocfs2_mark_lockres_freeing() which can block if the lock
we are freeing is queued thus creating an A-A deadlock.  Luckily, since
we are the downconvert thread, we can immediately dequeue the lock and
thus avoid waiting in this case.

Move dquot_initalize() call in ocfs2_delete_inode() after the moment we
verify inode is actually a sane one to delete.  We certainly don't want
to initialize quota for system inodes etc.  This also avoids calling
into quota code from downconvert thread.

Add more details into the comment why bailing out from
ocfs2_delete_inode() when we are in downconvert thread is OK.

765aabbbc7 ocfs2: add dlm_recover_callback_support in sysfs
This is a part of the nocontrold feature which was incorporated sometime
back.

This is required for backward compatibility of the tools, specifically
the scenario where the tools with recovery callback is used with a
kernel not using the recovery callbacks (older kernel + newer tools).
The tools look for this file to understand if the kernel supports DLM
recovery callbacks.

For kernels which support recovery callbacks but will miss this patch,
ocfs2 will continue to use the older API and would still be able to
mount the filesystem.

ded2cf7141 ocfs2: dlm: fix recovery hung
There is a race window in dlm_do_recovery() between dlm_remaster_locks()
and dlm_reset_recovery() when the recovery master nearly finish the
recovery process for a dead node.  After the master sends FINALIZE_RECO
message in dlm_remaster_locks(), another node may become the recovery
master for another dead node, and then send the BEGIN_RECO message to
all the nodes included the old master, in the handler of this message
dlm_begin_reco_handler() of old master, dlm->reco.dead_node and
dlm->reco.new_master will be set to the second dead node and the new
master, then in dlm_reset_recovery(), these two variables will be reset
to default value.  This will cause new recovery master can not finish
the recovery process and hung, at last the whole cluster will hung for
recovery.

A similar race may happen between new recovery master and normal node
which is in dlm_finalize_reco_handler(), also fix it.

34aa8dac48 ocfs2: dlm: fix lock migration crash
This issue was introduced by commit 800deef3f6 ("ocfs2: use
list_for_each_entry where benefical") in 2007 where it replaced
list_for_each with list_for_each_entry.  The variable "lock" will point
to invalid data if "tmpq" list is empty and a panic will be triggered
due to this.  Sunil advised reverting it back, but the old version was
also not right.  At the end of the outer for loop, that
list_for_each_entry will also set "lock" to an invalid data, then in the
next loop, if the "tmpq" list is empty, "lock" will be an stale invalid
data and cause the panic.  So reverting the list_for_each back and reset
"lock" to NULL to fix this issue.

Another concern is that this seemes can not happen because the "tmpq"
list should not be empty.  Let me describe how.

2931cdcb49 ocfs2: improve fsync efficiency and fix deadlock between aio_write and sync_file
Currently, ocfs2_sync_file grabs i_mutex and forces the current journal
transaction to complete.  This isn't terribly efficient, since sync_file
really only needs to wait for the last transaction involving that inode
to complete, and this doesn't require i_mutex.

Therefore, implement the necessary bits to track the newest tid
associated with an inode, and teach sync_file to wait for that instead
of waiting for everything in the journal to commit.  Furthermore, only
issue the flush request to the drive if jbd2 hasn't already done so.

This also eliminates the deadlock between ocfs2_file_aio_write() and
ocfs2_sync_file().  aio_write takes i_mutex then calls
ocfs2_aiodio_wait() to wait for unaligned dio writes to finish.
However, if that dio completion involves calling fsync, then we can get
into trouble when some ocfs2_sync_file tries to take i_mutex.

There is a problem that waitqueue_active() may check stale data thus miss
a wakeup of threads waiting on ip_unaligned_aio.

The valid value of ip_unaligned_aio is only 0 and 1 so we can change it to
be of type mutex thus the above prolem is avoid.  Another benifit is that
mutex which works as FIFO is fairer than wake_up_all().

181a9a043b ocfs2: fix null pointer dereference when access dlm_state before launching dlm thread
When mounting an ocfs2 volume, it will firstly generate a file
/sys/kernel/debug/o2dlm/<uuid>/dlm_state, and then launch the dlm thread.
So the following situation will cause a null pointer dereference.
dlm_debug_init -> access file dlm_state which will call dlm_state_print ->
dlm_launch_thread

Move dlm_debug_init after dlm_launch_thread and dlm_launch_recovery_thread
can fix this issue.

02b9984d64 fs: push sync_filesystem() down to the file system's remount_fs()
Previously, the no-op "mount -o mount /dev/xxx" operation when the
file system is already mounted read-write causes an implied,
unconditional syncfs().  This seems pretty stupid, and it's certainly
documented or guaraunteed to do this, nor is it particularly useful,
except in the case where the file system was mounted rw and is getting
remounted read-only.

However, it's possible that there might be some file systems that are
actually depending on this behavior.  In most file systems, it's
probably fine to only call sync_filesystem() when transitioning from
read-write to read-only, and there are some file systems where this is
not needed at all (for example, for a pseudo-filesystem or something
like romfs).

15c34a7606 ocfs2: fix quota file corruption
Global quota files are accessed from different nodes.  Thus we cannot
cache offset of quota structure in the quota file after we drop our node
reference count to it because after that moment quota structure may be
freed and reallocated elsewhere by a different node resulting in
corruption of quota file.

Fix the problem by clearing dq_off when we are releasing dquot structure.
We also remove the DB_READ_B handling because it is useless -
DQ_ACTIVE_B is set iff DQ_READ_B is set.

0e048316ff ocfs2: check existence of old dentry in ocfs2_link()
System call linkat first calls user_path_at(), check the existence of
old dentry, and then calls vfs_link()->ocfs2_link() to do the actual
work.  There may exist a race when Node A create a hard link for file
while node B rm it.

         Node A                          Node B
user_path_at()
  ->ocfs2_lookup(),
find old dentry exist
                                rm file, add inode say inodeA
                                to orphan_dir

call ocfs2_link(),create a
hard link for inodeA.

                                rm the link, add inodeA to orphan_dir
                                again

When orphan_scan work start, it calls ocfs2_queue_orphans() to do the
main work.  It first tranverses entrys in orphan_dir, linking all inodes
in this orphan_dir to a list look like this:

	inodeA->inodeB->...->inodeA

When tranvering this list, it will fall into loop, calling iput() again
and again.  And finally trigger BUG_ON(inode->i_state & I_CLEAR).

c7d2cbc364 ocfs2: update inode size after zeroing the hole
fs-writeback will release the dirty pages without page lock whose offset
are over inode size, the release happens at
block_write_full_page_endio().  If not update, dirty pages in file holes
may be released before flushed to the disk, then file holes will contain
some non-zero data, this will cause sparse file md5sum error.

To reproduce the bug, find a big sparse file with many holes, like vm
image file, its actual size should be bigger than available mem size to
make writeback work more frequently, tar it with -S option, then keep
untar it and check its md5sum again and again until you get a wrong
md5sum.

d62e74be12 ocfs2: fix issue that ocfs2_setattr() does not deal with new_i_size==i_size
The issue scenario is as following:

- Create a small file and fallocate a large disk space for a file with
  FALLOC_FL_KEEP_SIZE option.

- ftruncate the file back to the original size again.  but the disk free
  space is not changed back.  This is a real bug that be fixed in this
  patch.

In order to solve the issue above, we modified ocfs2_setattr(), if
attr->ia_size != i_size_read(inode), It calls ocfs2_truncate_file(), and
truncate disk space to attr->ia_size.

fb951eb5e1 ocfs2: free allocated clusters if error occurs after ocfs2_claim_clusters
Even if using the same jbd2 handle, we cannot rollback a transaction.
So once some error occurs after successfully allocating clusters, the
allocated clusters will never be used and it means they are lost.  For
example, call ocfs2_claim_clusters successfully when expanding a file,
but failed in ocfs2_insert_extent.  So we need free the allocated
clusters if they are not used indeed.

Adjust minlen with discard_granularity for FITRIM ioctl(2) if the given

For FITRIM ioctl(2), we should not keep silence if the given range
length ls less than a block size as there is no data blocks would be
discareded.  Hence it should return EINVAL instead.  This issue can be
verified via xfstests/generic/288 which is used for FITRIM argument
handling tests.

c994c2ebdb ocfs2: use the new DLM operation callbacks while requesting new lockspace
Attempt to use the new DLM operations.  If it is not supported, use the
traditional ocfs2_controld.

To exchange ocfs2 versioning, we use the LVB of the version dlm lock.
It first attempts to take the lock in EX mode (non-blocking).  If
successful (which means it is the first mount), it writes the version
number and downconverts to PR lock.  If it is unsuccessful, it reads the
version from the lock.

If this becomes the standard (with o2cb as well), it could simplify
userspace tools to check if the filesystem is mounted on other nodes.

Dan: Since ocfs2_protocol_version are two u8 values, the additional
checks with LONG* don't make sense.

24aa338611 ocfs2: shift allocation ocfs2_live_connection to user_connect()
We perform this because the DLM recovery callbacks will require the
ocfs2_live_connection structure to record the node information when
dlm_new_lockspace() is updated (in the last patch of the series).

Before calling dlm_new_lockspace(), we need the structure ready for the
.recover_done() callback, which would set oc_this_node.  This is the
reason we allocate ocfs2_live_connection beforehand in user_connect().

[AKPM] rc initialization is not required because it assigned in case of
errors.  It will be cleared by compiler anyways.

66e188fc31 ocfs2: add DLM recovery callbacks
These are the callbacks called by the fs/dlm code in case the membership
changes.  If there is a failure while/during calling any of these, the
DLM creates a new membership and relays to the rest of the nodes.

 - recover_prep() is called when DLM understands a node is down.
 - recover_slot() is called once all nodes have acknowledged
   recover_prep and recovery can begin.
 - recover_done() is called once the recovery is complete.  It returns
   the new membership.

c74a3bdd9b ocfs2: add clustername to cluster connection
This is an effort of removing ocfs2_controld.pcmk and getting ocfs2 DLM
handling up to the times with respect to DLM (>=4.0.1) and corosync
(2.3.x).  AFAIK, cman also is being phased out for a unified corosync
cluster stack.

fs/dlm performs all the functions with respect to fencing and node
management and provides the API's to do so for ocfs2.  For all future
references, DLM stands for fs/dlm code.

The advantages are:
 + No need to run an additional userspace daemon (ocfs2_controld)
 + No controld device handling and controld protocol
 + Shifting responsibilities of node management to DLM layer

For backward compatibility, we are keeping the controld handling code.
Once enough time has passed we can remove a significant portion of the
code.  This was tested by using the kernel with changes on older
unmodified tools.  The kernel used ocfs2_controld as expected, and
displayed the appropriate warning message.

This feature requires modification in the userspace ocfs2-tools.  The
changes can be found at: https://github.com/goldwynr/ocfs2-tools branch:

but that would change soon.

41ecc34598 ocfs2: simplify ocfs2_invalidatepage() and ocfs2_releasepage()
Ocfs2 doesn't do data journalling.  Thus its ->invalidatepage and
->releasepage functions never get called on buffers that have journal
heads attached.  So just use standard variants of functions from
buffer.c.

b1214e4757 ocfs2: fix possible double free in ocfs2_write_begin_nolock
When ocfs2_write_cluster_by_desc() failed in ocfs2_write_begin_nolock()
because of ENOSPC, it goes to out_quota, freeing data_ac(meta_ac).  Then
it calls ocfs2_try_to_free_truncate_log() to free space.  If enough
space freed, it will try to write again.  Unfortunately, some error
happenes before ocfs2_lock_allocators(), it goes to out and free
data_ac(meta_ac) again.

fae477b6f0 ocfs2: delay migration when the lockres is in migration state
We trigger a bug in __dlm_lockres_reserve_ast() when we parallel umount 4
nodes.  The situation is as follows:

1) Node A migrate all lockres it owned(eg.  lockres A) to other nodes
   say node B when it umounts.

2) Receiving MIG_LOCKRES message from A, Node B masters the lockres A
   with DLM_LOCK_RES_MIGRATING state set.

3) Then we umount ocfs2 on node B.  It also should migrate lockres A to
   another node, say node C.  But now, DLM_LOCK_RES_MIGRATING state of
   lockers A is not cleared.  Node B triggered the BUG on lockres with
   state DLM_LOCK_RES_MIGRATING.

750e3c6581 ocfs2: skip locks in the blocked list
A parallel umount on 4 nodes triggered a bug in
dlm_process_recovery_date().  Here's the situation:

Receiving MIG_LOCKRES message, A node processes the locks in migratable
lockres.  It copys lvb from migratable lockres when processing the first
valid lock.

If there is a lock in the blocked list with the EX level, it triggers the
BUG.  Since valid lvbs are set when locks are granted with EX or PR
levels, locks in the blocked list cannot have valid lvbs.  Therefore I
think we should skip the locks in the blocked list.

Call fiemap ioctl(2) with given start offset as well as an desired mapping
range should show extents if possible.  However, we somehow figure out the
end offset of mapping via 'mapping_end -= cpos' before iterating the
extent records which would cause problems if the given fiemap length is
too small to a cluster size, e.g,

Cluster size 4096:

This patch lets it wait on o2hb_callback_sem when another thread is firing
callbacks.  And for performance consideration, we only call
o2hb_run_event_list when there is an event queued.

6cae6d3189 ocfs2: fix possible double free in ocfs2_reflink_xattr_rec
In ocfs2_reflink_xattr_rec(), meta_ac and data_ac are allocated by calling
ocfs2_lock_reflink_xattr_rec_allocators().

Once an error occurs when allocating *data_ac, it frees *meta_ac which is
allocated before.  Here it mistakenly sets meta_ac to NULL but *meta_ac.
Then ocfs2_reflink_xattr_rec() will try to free meta_ac again which is
already invalid.

dlm_do_local_recovery_cleanup() should force clean refmap if the owner of
lockres is UNKNOWN.  Otherwise node may hang when umounting filesystems.
Here's the situation:

  -> dlm_get_lock_resource()
send DLM_MASTER_REQUEST_MSG to
other nodes.

                                       trying to master this lockres,
                                       return MAYBE.

selected as the master of lockresA,
set mle->master to Node1,
and do assert_master,
send DLM_ASSERT_MASTER_MSG to Node2.
                                       Node 2 has interest on lockresA
                                       and return
                                       DLM_ASSERT_RESPONSE_MASTERY_REF
                                       then something happened and
                                       Node2 crashed.

Receiving DLM_ASSERT_RESPONSE_MASTERY_REF, set Node2 into refmap, and keep
sending DLM_ASSERT_MASTER_MSG to other nodes

o2hb found node2 down, calling dlm_hb_node_down() -->
dlm_do_local_recovery_cleanup() the master of lockresA is still UNKNOWN,
no need to call dlm_free_dead_locks().

Set the master of lockresA to Node1, but Node2 stills remains in refmap.

When Node1 umount, it found that the refmap of lockresA is not empty and
attempted to migrate it to Node2, But Node2 is already down, so umount
hang, trying to migrate lockresA again and again.

7e9b793707 ocfs2: ac_bits_wanted should be local_alloc_bits when returns -ENOSPC
There is an issue in reserving and claiming space for localalloc, When
localalloc space is not enough, it would claim space from global_bitmap.
And if there is not enough free space in global_bitmap, the size of
claiming space would set to half of orignal size and retry.

The issue is as follows: osb->local_alloc_bits is set to half of orignal
size in ocfs2_recalc_la_window(), but ac->ac_bits_wanted is set to
osb->local_alloc_default_bits which is not changed.  localalloc always
reserves and claims local_alloc_default_bits space and returns ENOSPC.

So, ac->ac_bits_wanted should be osb->local_alloc_bits which would be
changed.

                                     send DLM_LOCK_REQUEST_MSG to nodeA

It happened that NodeA cannot alloc memory when it processes this
message.  dlm_request_all_locks_handler() do not queue
dlm_request_all_locks_worker and returns -ENOMEM.  It will never send
locks and DLM_RECO_DATA_DONE_MSG to NodeB.

                                    NodeB do not deal with the status
                                    sent from nodeA, and will fall in
                                    endless loop waiting for the
                                    recovery state of NodeA to be
                                    changed.

2b1e55c389 ocfs2: lighten up allocate transaction
The issue scenario is as following:

When fallocating a very large disk space for a small file,
__ocfs2_extend_allocation attempts to get a very large transaction.  For
some journal sizes, there may be not enough room for this transaction,
and the fallocate will fail.

The patch below extends & restarts the transaction as necessary while
allocating space, and should work with even the smallest journal.  This
patch refers ext4 resize.

Test:

7b7a8665ed direct-io: Implement generic deferred AIO completions
Add support to the core direct-io code to defer AIO completions to user
context using a workqueue.  This replaces opencoded and less efficient
code in XFS and ext4 (we save a memory allocation for each direct IO)
and will be needed to properly support O_(D)SYNC for AIO.

The communication between the filesystem and the direct I/O code requires
a new buffer head flag, which is a bit ugly but not avoidable until the
direct I/O code stops abusing the buffer_head structure for communicating
with the filesystems.

Currently this creates a per-superblock unbound workqueue for these
completions, which is taken from an earlier patch by Jan Kara.  I'm
not really convinced about this use and would prefer a "normal" global
workqueue with a high concurrency limit, but this needs further discussion.

73a7075e3f aio: Kill aio_rw_vect_retry()
This code doesn't serve any purpose anymore, since the aio retry
infrastructure has been removed.

This change should be safe because aio_read/write are also used for
synchronous IO, and called from do_sync_read()/do_sync_write() - and
there's no looping done in the sync case (the read and write syscalls).

    set reg(A)->item.ci_name to NULL
    in function config_item_cleanup().

                                    begin mounting, call

                                    regions. When reading
                                    reg(A)->item.ci_name, it causes
                                    NULL pointer dereference.

  call o2hb_region_release() and
  del reg(A) from list.

So we should skip accessing regions that is going to release when
tranverse o2hb_all_regions.

e873fdb525 ocfs2: submit disk heartbeat bio using WRITE_SYNC
Under heavy I/O load, writing the disk heartbeat can be forced to wait for
minutes, and this causes the node to be fenced.

This patch tries to use WRITE_SYNC in submitting the heartbeat bio, so
that writing the heartbeat will have a priority over other requests.

ef962df057 ocfs2: xattr: fix inlined xattr reflink
Inlined xattr shared free space of inode block with inlined data or data
extent record, so the size of the later two should be adjusted when
inlined xattr is enabled.  See ocfs2_xattr_ibody_init().  But this isn't
done well when reflink.  For inode with inlined data, its max inlined
data size is adjusted in ocfs2_duplicate_inline_data(), no problem.  But
for inode with data extent record, its record count isn't adjusted.  Fix
it, or data extent record and inlined xattr may overwrite each other,
then cause data corruption or xattr failure.

b5a8bb717e ocfs2: fix readonly issue in ocfs2_unlink()
While deleting a file with ocfs2_unlink(), there is a bug in this
function.  This bug will result in filesystem read-only.

After calling ocfs2_orphan_add(), the file which will be deleted is
added into orphan dir.  If ocfs2_delete_entry() fails, the file still
exists in the parent dir.  And this scenario introduces a conflict of
metadata.

If a file is added into orphan dir, when we put inode of the file with
iput(), the inode i_flags is setted (~OCFS2_VALID_FL) in
ocfs2_remove_inode(), and then write back to disk.

But as previously mentioned, the file still exists in the parent dir.
On other nodes, the file can be still accessed.  When first read the
file with ocfs2_read_blocks() from disk, It will check and avalidate
inode using ocfs2_validate_inode_block().  So File system will be
readonly because the inode is invalid.  In other words, the inode
i_flags has been set (~OCFS2_VALID_FL).

493098413b ocfs2: rework transaction rollback in ocfs2_relink_block_group()
In ocfs2_relink_block_group(), we roll back all those changes if notify
intent to modify buffers for metadata update failed even if the relevant
buffer has not yet been modified/got dirty at that point, that are not
quite right because of:

 - None buffer has been modified/dirty if failed to call
   ocfs2_journal_access_gd() against the previous block group buffer

 - Only the previous block group buffer has got dirty if failed to call
   ocfs2_journal_access_gd() against the block group buffer

 - There is no need to roll back the change for file entry buffer at all

Those problems will not cause anything wrong but unnecessary.  This
patch fix them and kill the useless bg_ptr variable as well.

ea45466aec ocfs2: need rollback when journal_access failed in ocfs2_orphan_add()
While adding a file into orphan dir in ocfs2_orphan_add(), it calls
__ocfs2_add_entry() before ocfs2_journal_access_di().  If
ocfs2_journal_access_di() failed, the file is added into orphan dir, and
orphan dir dinode updated, but file dinode has not been updated.
Accordingly, the data is not consistent between file dinode and orphan
dir.

So, need to call ocfs2_journal_access_di() before __ocfs2_add_entry(),
and if ocfs2_journal_access_di() failed, orphan_fe and
orphan_dir_inode->i_nlink need rollback.

This bug was added by 3939fda4 ("Ocfs2: Journaling i_flags and
i_orphaned_slot when adding inode to orphan dir.").

so it will retry in dlmlock().  And this may cause dlm_thread fall into an
infinite loop

	Thread1                                  dlm_thread

  calls dlm_lock->dlmlock_master,
  if lockresA is in state
  DLM_LOCK_RES_RECOVERING, calls
  __dlm_wait_on_lockres() and waits
  until others threads clear this
  state;

  If cannot grant this lock,
  adding lock to blocked list,
  and return DLM_RECOVERING;

                                        Grant this lock and move it to
                                        grant list;

  After a while, retry and
  calls list_add_tail(), adding lock
  to blocked list again.

Granted and blocked list of this lockres will become the following
conditions:

    lock_res->granted.next = dlm_lock->list_head;
    lock_res->blocked.next = dlm_lock->list_head;
    dlm_lock->list_head.next = dlm_lock_resource->blocked;

When dlm_thread traverses the granted list, it will fall into an endless
loop, checking dlm_lock.list_head, dlm_lock->list_head.next
(i.e.lock_res->blocked), lock_res->blocked.next(i.e.dlm_lock.list_head
again) .....

While adding extends to a file, the credits are calculated incorrectly
and if the requested clusters is more than one (or more because we used
a conservative limit) then we run out of journal credits and we hit an
assert in journalling code.

The function parameter bits_wanted variable was not used at all.
