f2db7361cb VMCI: Support upto 64-bit PPNs

45412befe8 VMCI: Remove non-blocking/pinned queuepair support
We added this for a special case that doesn't exist on Linux.  Remove
the non-blocking/pinned queuepair code and simplify the driver in
preparation for adding virtual IOMMU support.
