We need to convert the dm code to the new bvec_iter primitives which
respect bi_bvec_done; they also allow us to drastically simplify dm's
bio splitting code.
Also, it's no longer necessary to save/restore the bvec array anymore -
driver conversions for immutable bvecs are done, so drivers should never
be modifying it.
Also kill bio_sector_offset(), dm was the only user and it doesn't make
much sense anymore.

This patch doesn't itself have any functional changes, but immutable
biovecs are going to add a bi_bvec_done member to bi_iter, which will
need to be saved too here.

Device mapper saves and restores various fields in the bio, but it doesn't save
bi_io_vec. If the device driver modifies this after a partially successful
request, dm-raid1 and dm-multipath may attempt to resubmit a bio that has
bi_size inconsistent with the size of vector.
To make requests resubmittable in dm-raid1 and dm-multipath, we must save
and restore the bio vector as well.
To reduce the memory overhead involved in this, we do not save the pages in a
vector and use a 16-bit field size if the page size is less than 65536.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
