Remove the file name from the comment at top of many files. In most
cases the file name was wrong anyway, so it's rather pointless.
Also unify the IBM copyright statement. We did have a lot of sightly
different statements and wanted to change them one after another
whenever a file gets touched. However that never happened. Instead
people start to take the old/"wrong" statements to use as a template
for new files.
So unify all of them in one go.

A number of architectures have identical asm/mman.h files so they can all
be merged by using the new generic file.
The remaining asm/mman.h files are substantially different from each
other.

Add a flag for mmap that will be used to request a huge page region that
will look like anonymous memory to user space. This is accomplished by
using a file on the internal vfsmount. MAP_HUGETLB is a modifier of
MAP_ANONYMOUS and so must be specified with it. The region will behave
the same as a MAP_ANONYMOUS region using small pages.
The patch also adds the MAP_STACK flag, which was previously defined only
on some architectures but not on others. Since MAP_STACK is meant to be a
hint only, architectures can define it without assigning a specific
meaning to it.

The existing asm-generic versions are incomplete and included
by some architectures. New architectures should be able
to use a generic version, so rename the existing files and
change all users, which lets us add the new files.

After TASK_SIZE now gives the current size of the address space the
upgrade of a 64 bit process from 3 to 4 levels of page table needs
to use the arch_mmap_check hook to catch large mmap lengths. The
get_unmapped_area* functions need to check for -ENOMEM from the
arch_get_unmapped_area*, upgrade the page table and retry.
