25d4054cc9 mm: make arch_get_unmapped_area() take vm_flags by default
Patch series "mm: Care about shadow stack guard gap when getting an
unmapped area", v2.

As covered in the commit log for c44357c2e7 ("x86/mm: care about shadow
stack guard gap during placement") our current mmap() implementation does
not take care to ensure that a new mapping isn't placed with existing
mappings inside it's own guard gaps.  This is particularly important for
shadow stacks since if two shadow stacks end up getting placed adjacent to
each other then they can overflow into each other which weakens the
protection offered by the feature.

On x86 there is a custom arch_get_unmapped_area() which was updated by the
above commit to cover this case by specifying a start_gap for allocations
with VM_SHADOW_STACK.  Both arm64 and RISC-V have equivalent features and
use the generic implementation of arch_get_unmapped_area() so let's make
the equivalent change there so they also don't get shadow stack pages
placed without guard pages.  The arm64 and RISC-V shadow stack
implementations are currently on the list:

Given the addition of the use of vm_flags in the generic implementation we
also simplify the set of possibilities that have to be dealt with in the
core code by making arch_get_unmapped_area() take vm_flags as standard. 
This is a bit invasive since the prototype change touches quite a few
architectures but since the parameter is ignored the change is
straightforward, the simplification for the generic code seems worth it.


This patch (of 3):

When we introduced arch_get_unmapped_area_vmflags() in 961148704a ("mm:
introduce arch_get_unmapped_area_vmflags()") we did so as part of properly
supporting guard pages for shadow stacks on x86_64, which uses a custom
arch_get_unmapped_area().  Equivalent features are also present on both
arm64 and RISC-V, both of which use the generic implementation of
arch_get_unmapped_area() and will require equivalent modification there. 
Rather than continue to deal with having two versions of the functions
let's bite the bullet and have all implementations of
arch_get_unmapped_area() take vm_flags as a parameter.

The new parameter is currently ignored by all implementations other than
x86.  The only caller that doesn't have a vm_flags available is
mm_get_unmapped_area(), as for the x86 implementation and the wrapper used
on other architectures this is modified to supply no flags.

No functional changes.

213aa67015 parisc: Delay write-protection until mark_rodata_ro() call
Do not write-protect the kernel read-only and __ro_after_init sections
earlier than before mark_rodata_ro() is called.  This fixes a boot issue on
parisc which is triggered by commit 91a1d97ef4 ("jump_label,module: Don't
alloc static_key_mod for __ro_after_init keys"). That commit may modify
static key contents in the __ro_after_init section at bootup, so this
section needs to be writable at least until mark_rodata_ro() is called.

8b1d723956 parisc: Fix random data corruption from exception handler
The current exception handler implementation, which assists when accessing
user space memory, may exhibit random data corruption if the compiler decides
to use a different register than the specified register %r29 (defined in
ASM_EXCEPTIONTABLE_REG) for the error code. If the compiler choose another
register, the fault handler will nevertheless store -EFAULT into %r29 and thus
trash whatever this register is used for.
Looking at the assembly I found that this happens sometimes in emulate_ldd().

To solve the issue, the easiest solution would be if it somehow is
possible to tell the fault handler which register is used to hold the error
code. Using %0 or %1 in the inline assembly is not posssible as it will show
up as e.g. %r29 (with the "%r" prefix), which the GNU assembler can not
convert to an integer.

This patch takes another, better and more flexible approach:
We extend the __ex_table (which is out of the execution path) by one 32-word.
In this word we tell the compiler to insert the assembler instruction
"or %r0,%r0,%reg", where %reg references the register which the compiler
choosed for the error return code.
In case of an access failure, the fault handler finds the __ex_table entry and
can examine the opcode. The used register is encoded in the lowest 5 bits, and
the fault handler can then store -EFAULT into this register.

Since we extend the __ex_table to 3 words we can't use the BUILDTIME_TABLE_SORT
config option any longer.

935d4f0c6d mm: hugetlb: add huge page size param to set_huge_pte_at()
Patch series "Fix set_huge_pte_at() panic on arm64", v2.

This series fixes a bug in arm64's implementation of set_huge_pte_at(),
which can result in an unprivileged user causing a kernel panic.  The
problem was triggered when running the new uffd poison mm selftest for
HUGETLB memory.  This test (and the uffd poison feature) was merged for
v6.5-rc7.

Ideally, I'd like to get this fix in for v6.6 and I've cc'ed stable
(correctly this time) to get it backported to v6.5, where the issue first
showed up.


Description of Bug
==================

arm64's huge pte implementation supports multiple huge page sizes, some of
which are implemented in the page table with multiple contiguous entries. 
So set_huge_pte_at() needs to work out how big the logical pte is, so that
it can also work out how many physical ptes (or pmds) need to be written. 
It previously did this by grabbing the folio out of the pte and querying
its size.

However, there are cases when the pte being set is actually a swap entry. 
But this also used to work fine, because for huge ptes, we only ever saw
migration entries and hwpoison entries.  And both of these types of swap
entries have a PFN embedded, so the code would grab that and everything
still worked out.

But over time, more calls to set_huge_pte_at() have been added that set
swap entry types that do not embed a PFN.  And this causes the code to go
bang.  The triggering case is for the uffd poison test, commit
99aa77215a ("selftests/mm: add uffd unit test for UFFDIO_POISON"), which
causes a PTE_MARKER_POISONED swap entry to be set, coutesey of commit
8a13897fb0 ("mm: userfaultfd: support UFFDIO_POISON for hugetlbfs") -
added in v6.5-rc7.  Although review shows that there are other call sites
that set PTE_MARKER_UFFD_WP (which also has no PFN), these don't trigger
on arm64 because arm64 doesn't support UFFD WP.

If CONFIG_DEBUG_VM is enabled, we do at least get a BUG(), but otherwise,
it will dereference a bad pointer in page_folio():

Fix
===

The simplest fix would have been to revert the dodgy cleanup commit
18f3962953 ("mm: hugetlb: kill set_huge_swap_pte_at()"), but since
things have moved on, this would have required an audit of all the new
set_huge_pte_at() call sites to see if they should be converted to
set_huge_swap_pte_at().  As per the original intent of the change, it
would also leave us open to future bugs when people invariably get it
wrong and call the wrong helper.

So instead, I've added a huge page size parameter to set_huge_pte_at(). 
This means that the arm64 code has the size in all cases.  It's a bigger
change, due to needing to touch the arches that implement the function,
but it is entirely mechanical, so in my view, low risk.

I've compile-tested all touched arches; arm64, parisc, powerpc, riscv,
s390, sparc (and additionally x86_64).  I've additionally booted and run
mm selftests against arm64, where I observe the uffd poison test is fixed,
and there are no other regressions.


This patch (of 2):

In order to fix a bug, arm64 needs to be told the size of the huge page
for which the pte is being set in set_huge_pte_at().  Provide for this by
adding an `unsigned long sz` parameter to the function.  This follows the
same pattern as huge_pte_clear().

This commit makes the required interface modifications to the core mm as
well as all arches that implement this function (arm64, parisc, powerpc,
riscv, s390, sparc).  The actual arm64 bug will be fixed in a separate
commit.

No behavioral changes intended.

426b313f35 parisc: mm: convert to GENERIC_IOREMAP
By taking GENERIC_IOREMAP method, the generic generic_ioremap_prot(),
generic_iounmap(), and their generic wrapper ioremap_prot(), ioremap() and
iounmap() are all visible and available to arch.  Arch needs to provide
wrapper functions to override the generic versions if there's arch
specific handling in its ioremap_prot(), ioremap() or iounmap().  This
change will simplify implementation by removing duplicated code with
generic_ioremap_prot() and generic_iounmap(), and has the equivalent
functioality as before.

Here, add wrapper function ioremap_prot() for parisc's special operation
when iounmap().

This happens because patching static key code temporarily maps it via
fixmap and if it happens before page allocator is initialized set_fixmap()
cannot allocate memory using pte_alloc_kernel().

Make sure that fixmap page tables are preallocated early so that
pte_offset_kernel() in set_fixmap() never resorts to pte allocation.

ea3f827287 parisc: fix expand_stack() conversion
In commit 8d7071af89 ("mm: always expand the stack with the mmap write
lock held") I tried to deal with the remaining odd page fault handling
cases.  The oddest one is ia64, which has stacks that grow both up and
down.  And because ia64 was _so_ odd, I asked people to verify the end
result.

But a close second oddity is parisc, which is the only one that has a
main stack growing up (our "CONFIG_STACK_GROWSUP" config option).  But
it looked obvious enough that I didn't worry about it.

I should have worried a bit more.  Not because it was particularly
complex, but because I just used the wrong variable name.

The previous vma isn't called "prev", it's called "prev_vma".  Blush.

9471f1f2f5 Merge branch 'expand-stack'
This modifies our user mode stack expansion code to always take the
mmap_lock for writing before modifying the VM layout.

It's actually something we always technically should have done, but
because we didn't strictly need it, we were being lazy ("opportunistic"
sounds so much better, doesn't it?) about things, and had this hack in
place where we would extend the stack vma in-place without doing the
proper locking.

And it worked fine.  We just needed to change vm_start (or, in the case
of grow-up stacks, vm_end) and together with some special ad-hoc locking
using the anon_vma lock and the mm->page_table_lock, it all was fairly
straightforward.

That is, it was all fine until Ruihan Li pointed out that now that the
vma layout uses the maple tree code, we *really* don't just change
vm_start and vm_end any more, and the locking really is broken.  Oops.

It's not actually all _that_ horrible to fix this once and for all, and
do proper locking, but it's a bit painful.  We have basically three
different cases of stack expansion, and they all work just a bit
differently:

 - the common and obvious case is the page fault handling. It's actually
   fairly simple and straightforward, except for the fact that we have
   something like 24 different versions of it, and you end up in a maze
   of twisty little passages, all alike.

 - the simplest case is the execve() code that creates a new stack.
   There are no real locking concerns because it's all in a private new
   VM that hasn't been exposed to anybody, but lockdep still can end up
   unhappy if you get it wrong.

 - and finally, we have GUP and page pinning, which shouldn't really be
   expanding the stack in the first place, but in addition to execve()
   we also use it for ptrace(). And debuggers do want to possibly access
   memory under the stack pointer and thus need to be able to expand the
   stack as a special case.

None of these cases are exactly complicated, but the page fault case in
particular is just repeated slightly differently many many times.  And
ia64 in particular has a fairly complicated situation where you can have
both a regular grow-down stack _and_ a special grow-up stack for the
register backing store.

So to make this slightly more manageable, the bulk of this series is to
first create a helper function for the most common page fault case, and
convert all the straightforward architectures to it.

Thus the new 'lock_mm_and_find_vma()' helper function, which ends up
being used by x86, arm, powerpc, mips, riscv, alpha, arc, csky, hexagon,
loongarch, nios2, sh, sparc32, and xtensa.  So we not only convert more
than half the architectures, we now have more shared code and avoid some
of those twisty little passages.

And largely due to this common helper function, the full diffstat of
this series ends up deleting more lines than it adds.

That still leaves eight architectures (ia64, m68k, microblaze, openrisc,
parisc, s390, sparc64 and um) that end up doing 'expand_stack()'
manually because they are doing something slightly different from the
normal pattern.  Along with the couple of special cases in execve() and
GUP.

So there's a couple of patches that first create 'locked' helper
versions of the stack expansion functions, so that there's a obvious
path forward in the conversion.  The execve() case is then actually
pretty simple, and is a nice cleanup from our old "grow-up stackls are
special, because at execve time even they grow down".

The #ifdef CONFIG_STACK_GROWSUP in that code just goes away, because
it's just more straightforward to write out the stack expansion there
manually, instead od having get_user_pages_remote() do it for us in some
situations but not others and have to worry about locking rules for GUP.

And the final step is then to just convert the remaining odd cases to a
new world order where 'expand_stack()' is called with the mmap_lock held
for reading, but where it might drop it and upgrade it to a write, only
to return with it held for reading (in the success case) or with it
completely dropped (in the failure case).

In the process, we remove all the stack expansion from GUP (where
dropping the lock wouldn't be ok without special rules anyway), and add
it in manually to __access_remote_vm() for ptrace().

Thanks to Adrian Glaubitz and Frank Scheiner who tested the ia64 cases.
Everything else here felt pretty straightforward, but the ia64 rules for
stack expansion are really quite odd and very different from everything
else.  Also thanks to Vegard Nossum who caught me getting one of those
odd conditions entirely the wrong way around.

Anyway, I think I want to actually move all the stack expansion code to
a whole new file of its own, rather than have it split up between
mm/mmap.c and mm/memory.c, but since this will have to be backported to
the initial maple tree vma introduction anyway, I tried to keep the
patches _fairly_ minimal.

Also, while I don't think it's valid to expand the stack from GUP, the
final patch in here is a "warn if some crazy GUP user wants to try to
expand the stack" patch.  That one will be reverted before the final
release, but it's left to catch any odd cases during the merge window
and release candidates.

8d7071af89 mm: always expand the stack with the mmap write lock held
This finishes the job of always holding the mmap write lock when
extending the user stack vma, and removes the 'write_locked' argument
from the vm helper functions again.

For some cases, we just avoid expanding the stack at all: drivers and
page pinning really shouldn't be extending any stacks.  Let's see if any
strange users really wanted that.

It's worth noting that architectures that weren't converted to the new
lock_mm_and_find_vma() helper function are left using the legacy
"expand_stack()" function, but it has been changed to drop the mmap_lock
and take it for writing while expanding the vma.  This makes it fairly
straightforward to convert the remaining architectures.

As a result of dropping and re-taking the lock, the calling conventions
for this function have also changed, since the old vma may no longer be
valid.  So it will now return the new vma if successful, and NULL - and
the lock dropped - if the area could not be extended.

that would repeat the entire thing without making any progress; what we need
instead is to treat that as failed (user) memory access.

d92725256b mm: avoid unnecessary page fault retires on shared memory types
I observed that for each of the shared file-backed page faults, we're very
likely to retry one more time for the 1st write fault upon no page.  It's
because we'll need to release the mmap lock for dirty rate limit purpose
with balance_dirty_pages_ratelimited() (in fault_dirty_shared_page()).

Then after that throttling we return VM_FAULT_RETRY.

We did that probably because VM_FAULT_RETRY is the only way we can return
to the fault handler at that time telling it we've released the mmap lock.

However that's not ideal because it's very likely the fault does not need
to be retried at all since the pgtable was well installed before the
throttling, so the next continuous fault (including taking mmap read lock,
walk the pgtable, etc.) could be in most cases unnecessary.

It's not only slowing down page faults for shared file-backed, but also add
more mmap lock contention which is in most cases not needed at all.

To observe this, one could try to write to some shmem page and look at
"pgfault" value in /proc/vmstat, then we should expect 2 counts for each
shmem write simply because we retried, and vm event "pgfault" will capture
that.

To make it more efficient, add a new VM_FAULT_COMPLETED return code just to
show that we've completed the whole fault and released the lock.  It's also
a hint that we should very possibly not need another fault immediately on
this page because we've just completed it.

This patch provides a ~12% perf boost on my aarch64 test VM with a simple
program sequentially dirtying 400MB shmem file being mmap()ed and these are
the time it needs:

  Before: 650.980 ms (+-1.94%)
  After:  569.396 ms (+-1.38%)

I believe it could help more than that.

We need some special care on GUP and the s390 pgfault handler (for gmap
code before returning from pgfault), the rest changes in the page fault
handlers should be relatively straightforward.

Another thing to mention is that mm_account_fault() does take this new
fault as a generic fault to be accounted, unlike VM_FAULT_RETRY.

I explicitly didn't touch hmm_vma_fault() and break_ksm() because they do
not handle VM_FAULT_RETRY even with existing code, so I'm literally keeping
them as-is.

67c35a3b64 parisc: Disable debug code regarding cache flushes in handle_nadtlb_fault()
Change the "BUG" to "WARNING" and disable the message because it triggers
occasionally in spite of the check in flush_cache_page_if_present.

The pte value extracted for the "from" page in copy_user_highpage is racy and
occasionally the pte is cleared before the flush is complete.  I assume that
the page is simultaneously flushed by flush_cache_mm before the pte is cleared
as nullifying the fdc doesn't seem to cause problems.

I investigated various locking scenarios but I wasn't able to find a way to
sequence the flushes.  This code is called for every COW break and locks impact
performance.

This patch is related to the bigger cache flush patch because we need the pte
on PA8800/PA8900 to flush using the vma context.
I have also seen this from copy_to_user_page and copy_from_user_page.

The messages appear infrequently when enabled.

Currently, the parisc kernel does not fully support non-access TLB
fault handling for probe instructions. In the fast path, we set the
target register to zero if it is not a shadowed register. The slow
path is not implemented, so we call do_page_fault. The architecture
indicates that non-access faults should not cause a page fault from
disk.

This change adds to code to provide non-access fault support for
probe instructions. It also modifies the handling of faults on
userspace so that if the address lies in a valid VMA and the access
type matches that for the VMA, the probe target register is set to
one. Otherwise, the target register is set to zero.

This was done to make probe instructions more useful for userspace.
Probe instructions are not very useful if they set the target register
to zero whenever a page is not present in memory. Nominally, the
purpose of the probe instruction is determine whether read or write
access to a given address is allowed.

This fixes a problem in function pointer comparison noticed in the
glibc testsuite (stdio-common/tst-vfprintf-user-type). The same
problem is likely in glibc (_dl_lookup_address).

V2 adds flush and lpa instruction support to handle_nadtlb_fault.

9129886b88 parisc: Drop __init from map_pages declaration
With huge kernel pages, we randomly eat a SPARC in map_pages(). This
is fixed by dropping __init from the declaration.

However, map_pages references the __init routine memblock_alloc_try_nid
via memblock_alloc.  Thus, it needs to be marked with __ref.

memblock_alloc is only called before the kernel text is set to readonly.

The __ref on free_initmem is no longer needed.

Comment regarding map_pages being in the init section is removed.

36ef159f44 mm: remove redundant check about FAULT_FLAG_ALLOW_RETRY bit
Since commit 4064b98270 ("mm: allow VM_FAULT_RETRY for multiple
times") allowed VM_FAULT_RETRY for multiple times, the
FAULT_FLAG_ALLOW_RETRY bit of fault_flag will not be changed in the page
fault path, so the following check is no longer needed:

	flags & FAULT_FLAG_ALLOW_RETRY

So just remove it.

9d90a90855 parisc: Don't call faulthandler_disabled() in do_page_fault()
It is dangerous to call faulthandler_disabled() when user_mode(regs)
is true.  The task pagefault_disabled counter is racy and it is not
updated atomically on parisc. As a result, calling faulthandler_disabled()
may cause erroneous termination.

We now handle execption fixups and termination when user_mode(regs) is
false in handle_interruption(). Thus, we can just remove the
faulthandler_disabled() check from do_page_fault().

4b9d2a731c parisc: Switch user access functions to signal errors in r29 instead of r8
Use register r29 instead of register r8 to signal faults when accessing
user memory. In case of faults, the fixup routine will store -EFAULT in
this register.

This change saves up to 752 bytes on a 32bit kernel, partly because the
compiler doesn't need to save and restore the old r8 value on the stack.

This series tries to disable huge pmd unshare of hugetlbfs backed memory
for uffd-wp.  Although uffd-wp of hugetlbfs is still during rfc stage,
the idea of this series may be needed for multiple tasks (Axel's uffd
minor fault series, and Mike's soft dirty series), so I picked it out
from the larger series.

It is a preparation work to be able to behave differently in the per
architecture huge_pte_alloc() according to different VMA attributes.

Pass it deeper into huge_pmd_share() so that we can avoid the find_vma() call.

b7795074a0 parisc: Optimize per-pagetable spinlocks
On parisc a spinlock is stored in the next page behind the pgd which
protects against parallel accesses to the pgd. That's why one additional
page (PGD_ALLOC_ORDER) is allocated for the pgd.

Matthew Wilcox suggested that we instead should use a pointer in the
struct page table for this spinlock and noted, that the comments for the
PGD_ORDER and PMD_ORDER defines were wrong.

Both suggestions are addressed with this patch. Instead of having an own
spinlock to protect the pgd, we now switch to use the existing
page_table_lock.  Additionally, beside loading the pgd into cr25 in
switch_mm_irqs_off(), the physical address of this lock is loaded into
cr28 (tr4), so that we can avoid implementing a complicated lookup in
assembly for this lock in the TLB fault handlers.

The existing Hybrid L2/L3 page table scheme (where the pmd is adjacent
to the pgd) has been dropped with this patch.

Remove the locking in set_pte() and the huge-page pte functions too.
They trigger a spinlock recursion on 32bit machines and seem unnecessary.

What this series did:

  - Correct page fault accounting: we do accounting for a page fault
    (no matter whether it's from #PF handling, or gup, or anything else)
    only with the one that completed the fault.  For example, page fault
    retries should not be counted in page fault counters.  Same to the
    perf events.

  - Unify definition of PERF_COUNT_SW_PAGE_FAULTS: currently this perf
    event is used in an adhoc way across different archs.

    Case (1): for many archs it's done at the entry of a page fault
    handler, so that it will also cover e.g.  errornous faults.

    Case (2): for some other archs, it is only accounted when the page
    fault is resolved successfully.

    Case (3): there're still quite some archs that have not enabled
    this perf event.

    Since this series will touch merely all the archs, we unify this
    perf event to always follow case (1), which is the one that makes most
    sense.  And since we moved the accounting into handle_mm_fault, the
    other two MAJ/MIN perf events are well taken care of naturally.

  - Unify definition of "major faults": the definition of "major
    fault" is slightly changed when used in accounting (not
    VM_FAULT_MAJOR).  More information in patch 1.

  - Always account the page fault onto the one that triggered the page
    fault.  This does not matter much for #PF handlings, but mostly for
    gup.  More information on this in patch 25.

Patchset layout:

Patch 1:     Introduced the accounting in handle_mm_fault(), not enabled.
Patch 2-23:  Enable the new accounting for arch #PF handlers one by one.
Patch 24:    Enable the new accounting for the rest outliers (gup, iommu, etc.)

This patch (of 25):

This is a preparation patch to move page fault accountings into the
general code in handle_mm_fault().  This includes both the per task
flt_maj/flt_min counters, and the major/minor page fault perf events.  To
do this, the pt_regs pointer is passed into handle_mm_fault().

PERF_COUNT_SW_PAGE_FAULTS should still be kept in per-arch page fault
handlers.

So far, all the pt_regs pointer that passed into handle_mm_fault() is
NULL, which means this patch should have no intented functional change.

After removal of CONFIG_HAVE_MEMBLOCK_NODE_MAP we have two equivalent
functions that call memory_present() for each region in memblock.memory:
sparse_memory_present_with_active_regions() and membocks_present().

Moreover, all architectures have a call to either of these functions
preceding the call to sparse_init() and in the most cases they are called
one after the other.

Mark the regions from memblock.memory as present during sparce_init() by
making sparse_init() call memblocks_present(), make memblocks_present()
and memory_present() functions static and remove redundant
sparse_memory_present_with_active_regions() function.

Also remove no longer required HAVE_MEMORY_PRESENT configuration option.

ca15ca406f mm: remove unneeded includes of <asm/pgalloc.h>
Patch series "mm: cleanup usage of <asm/pgalloc.h>"

Most architectures have very similar versions of pXd_alloc_one() and
pXd_free_one() for intermediate levels of page table.  These patches add
generic versions of these functions in <asm-generic/pgalloc.h> and enable
use of the generic functions where appropriate.

In addition, functions declared and defined in <asm/pgalloc.h> headers are
used mostly by core mm and early mm initialization in arch and there is no
actual reason to have the <asm/pgalloc.h> included all over the place.
The first patch in this series removes unneeded includes of
<asm/pgalloc.h>

In the end it didn't work out as neatly as I hoped and moving
pXd_alloc_track() definitions to <asm-generic/pgalloc.h> would require
unnecessary changes to arches that have custom page table allocations, so
I've decided to move lib/ioremap.c to mm/ and make pgalloc-track.h local
to mm/.

This patch (of 8):

In most cases <asm/pgalloc.h> header is required only for allocations of
page table memory.  Most of the .c files that include that header do not
use symbols declared in <asm/pgalloc.h> and do not require that header.

As for the other header files that used to include <asm/pgalloc.h>, it is
possible to move that include into the .c file that actually uses symbols
from <asm/pgalloc.h> and drop the include from the header file.

The process was somewhat automated using

	sed -i -E '/[<"]asm\/pgalloc\.h/d' \
                $(grep -L -w -f /tmp/xx \
                        $(git grep -E -l '[<"]asm/pgalloc\.h'))

where /tmp/xx contains all the symbols defined in
arch/*/include/asm/pgalloc.h.
