The SCSI specification requires that the second Command Data Byte
should contain the LUN value in its high-order bits if the recipient
device reports SCSI level 2 or below. Nevertheless, some USB
mass-storage devices use those bits for other purposes in
vendor-specific commands. Currently Linux has no way to send such
commands, because the SCSI stack always overwrites the LUN bits.
Testing shows that Windows 7 and XP do not store the LUN bits in the
CDB when sending commands to a USB device. This doesn't matter if the
device uses the Bulk-Only or UAS transports (which virtually all
modern USB mass-storage devices do), as these have a separate
mechanism for sending the LUN value.
Therefore this patch introduces a flag in the Scsi_Host structure to
inform the SCSI midlayer that a transport does not require the LUN
bits to be stored in the CDB, and it makes usb-storage set this flag
for all devices using the Bulk-Only transport. (UAS is handled by a
separate driver, but it doesn't really matter because no SCSI-2 or
lower device is at all likely to use UAS.)
The patch also cleans up the code responsible for storing the LUN
value by adding a bitflag to the scsi_device structure. The test for
whether to stick the LUN value in the CDB can be made when the device
is probed, and stored for future use rather than being made over and
over in the fast path.

Add a use_cmd_list flag in struct Scsi_Host to request keeping track of
all outstanding commands per device.
Default behaviour is not to keep track of cmd_list per sdev, as this may
introduce lock contention. (overhead is more on multi-node NUMA.), and
only enable it on the two drivers that need it.

The SYNCHRONIZE_CACHE command is a medium write command and hence can
fail when the device is write protected. Avoid sending such commands by
making sure that write-cache-enable is disabled even though the device
claim to support it.

Just like CNIC, tristate of these two modules is also dependent on IPV6.
These need to be handled separately as they select CNIC, which can override
tristate for CNIC from 'm' to 'y', which can cause build failures when ipv6 is
compiled as a module even if CNIC's Kconfig will only 'm' or 'n' when ipv6 is
compiled as a module.

This patches fixes a potential buffer overrun in __iscsi_conn_send_pdu.
This function is used by iscsi drivers and userspace to send iscsi PDUs/
commands. For login commands, we have a set buffer size. For all other
commands we do not support data buffers.

The blk_get_request function may fail in low-memory conditions or during
device removal (even if __GFP_WAIT is set). To distinguish between these
errors, modify the blk_get_request call stack to return the appropriate
ERR_PTR. Verify that all callers check the return status and consider
IS_ERR instead of a simple NULL pointer check.
For consistency, make a similar change to the blk_mq_alloc_request leg
of blk_get_request. It may fail if the queue is dead, or the caller was
unwilling to wait.

Code like this appears in e.g. sg_start_req() in drivers/scsi/sg.c (for
large CDBs only). Without this patch, scsi_mq_prep_fn() will set
rq->cmd back to rq->__cmd, causing the wrong CDB to be sent to the device.

If a scsi host driver specifies .cmd_len in it's scsi_host_template, a driver's
private command pool is needed. scsi_find_host_cmd_pool() will locate it, but
scsi_alloc_host_cmd_pool() isn't saving the pool address in the host template.
This will result in an access error when the host is removed.
Avoid the problem by saving the address of a new allocated command pool where
it is expected.

This patch set consists of the usual driver updates (ufs, storvsc, pm8001
hpsa). It also has removal of the user space target driver code (everyone is
using LIO now), a partial PCI MSI-X update, more multi-queue updates,
conversion to 64 bit LUNs (so we could theoretically cope with any LUN
returned by a device) and placeholder support for the ZBC device type (Shingle
drives), plus an assortment of minor updates and bug fixes.

way to allow different functions to call directly different trampolines
instead of all calling the same "mcount" one.
The only user of this for now is the function graph tracer, which always
had a different trampoline, but the function tracer trampoline was called
and did basically nothing, and then the function graph tracer trampoline
was called. The difference now, is that the function graph tracer
trampoline can be called directly if a function is only being traced by
the function graph trampoline. If function tracing is also happening on
the same function, the old way is still done.
The accounting for this takes up more memory when function graph tracing
is activated, as it needs to keep track of which functions it uses.
I have a new way that wont take as much memory, but it's not ready yet
for this merge window, and will have to wait for the next one.
Another big change was the removal of the ftrace_start/stop() calls that
were used by the suspend/resume code that stopped function tracing when
entering into suspend and resume paths. The stop of ftrace was done
because there was some function that would crash the system if one called
smp_processor_id()! The stop/start was a big hammer to solve the issue
at the time, which was when ftrace was first introduced into Linux.
Now ftrace has better infrastructure to debug such issues, and I found
the problem function and labeled it with "notrace" and function tracing
can now safely be activated all the way down into the guts of suspend
and resume.
Other changes include clean ups of uprobe code.
Clean up of the trace_seq() code.
And other various small fixes and clean ups to ftrace and tracing.

If scsi_remove_host() is called while an rport is in the blocked state
then scsi_remove_host() will only finish if the rport is unblocked
from inside a timer function. Make sure that an rport only enters the
blocked state if a timer will be started that will unblock it. This
avoids that unloading the ib_srp kernel module after having
disconnected the initiator from the target system results in a
deadlock if both the fast_io_fail_tmo and dev_loss_tmo parameters have
been set to "off".

The current implementation may mix the negative value returned from
pm8001_set_nvmd with count. -(-ENOMEM) could be interpreted as bytes
programmed, this patch fixes it.

When a iscsi nop as ping timedout we were failing with the
common connection error code, ISCSI_ERR_CONN_FAILED. This
patch adds a new error code for this problem so can properly
track/distinguish in userspace.

When the get_host_stats call was not supported we were
returing EINVAL. This has us return ENOSYS, because for
software iscsi drivers where there is no host it is ok to not

Some devices don't like REPORT SUPPORTED OPERATION CODES and will
simply timeout causing sd_mod init to take a very very long time.
Introduce BLIST_NO_RSOC scsi scan flag, that stops RSOC from being

As result of deprecation of MSI-X/MSI enablement functions
pci_enable_msix() and pci_enable_msi_block() all drivers
using these two interfaces need to be updated to use the
new pci_enable_msi_range() or pci_enable_msi_exact()
and pci_enable_msix_range() or pci_enable_msix_exact()
interfaces.

- add host_lock option whose default value is 0 which removes the
host_lock around all queued commands
- accept delay=-1 (_hi_) or -2 which use a tasklet to invoke
the scsi_done callback into the mid-layer. The default
is still delay=1 which uses a timer to delay 1 jiffy
- wire .change_queue_depth and .change_queue_type
functions to better simulate queueing in a modern LLD
- add SCSI_DEBUG_OPT_Q_NOISE (0x200) mask to only produce
debug output associated with queue full, plus from
.change_queue_depth and .change_queue_type functions
- add SCSI_DEBUG_OPT_ALL_TSF (0x400) mask which reports
all queued_arr fulls at TASK_SET_FULL, otherwise
SCSI_MLQUEUE_HOST_BUSY is returned
- add SCSI_DEBUG_OPT_RARE_TSF (0x800) mask which works
together with the every_nth option (> 0) to count
occurrences of num_in_q==queue_depth. When every_nth

With fio and four (pseudo) devices I have observed 1.2 M IOPS
on my equipment. Rob Elliott who has done much testing and made
numerous suggestions, has better IOPS results than mine.

Add blist flags to permit the reading of the VPD pages even when
the target may claim SPC-2 compliance. MSFT targets currently
claim SPC-2 compliance while they implement post SPC-2 features.
With this patch we can correctly handle WRITE_SAME_16 issues.

Fix many warnings with incorrect endian assumptions
which makes the code unportable to new architectures.
The UFS specification defines the byte order as big-endian
for UPIU structure and little-endian for the host controller
transfer/task management descriptors.

The driver checks the return valu, but after he tries to wait_for_completion
which might never happen. Also the ioctl buffer is freed at the end of the
function, so the first removal is not needed.

During hibernation, the HBA firmware may lose power and forget the device
id info. This causes the HBA to reject IO upon resume. The fix is
to call the libsas power management routines to make the domain device
forgetful.

In _scsih_probe, delay the call to scsi_add_host until the host has been
fully set up.
Otherwise, the default .can_queue value of 1 causes scsi-mq to set the block
layer request queue size to its minimum size, resulting in awful performance.
In _scsih_probe error handling, call mpt3sas_base_detach rather than
scsi_remove_host to properly clean up in reverse order.
In _scsih_remove, call scsi_remove_host earlier to clean up in reverse order.

On some Windows hosts on FC SANs, TEST_UNIT_READY can return SRB_STATUS_ERROR.
Correctly handle this. Note that there is sufficient sense information to
support scsi error handling even in this case.

Correctly set SRB flags for all valid I/O directions. Some IHV drivers on the
Windows host require this. The host validates the command and SRB flags
prior to passing the command down to native driver stack.

On Azure, we have seen instances of unbounded I/O latencies. To deal with
this issue, implement handler that can reset the timeout. Note that the
host gaurantees that it will respond to each command that has been issued.

Based on the negotiated VMBUS protocol version, we adjust the size of the storage
protocol messages. The two sizes we currently handle are pre-win8 and post-win8.
In WS2012 R2, we are negotiating higher VMBUS protocol version than the win8
version. Make adjustments to correctly handle this.

Going forward it is possible that some of the commands that are not currently
implemented will be implemented on future Windows hosts. Even if they are not
implemented, we are told the host will corrrectly handle unsupported
commands (by returning appropriate return code and sense information).
Make command filtering depend on the host version.

Hyper-V hosts can support multiple targets and multiple channels and larger number of
LUNs per target. Update the code to reflect this. With this patch we can correctly
enumerate all the paths in a multi-path storage environment.

If the controller doesn't support 64-bit addressing mode, it must not
set the DMA mask to 64-bit. But it's unconditionally trying to set to
64-bit without checking 64-bit addressing support in the controller
capabilities.
It was correctly checked before commit 3b1d05807a
("[SCSI] ufs: Segregate PCI Specific Code"), this aims to restores
the correct behaviour.
To achieve this in a generic way, firstly we should push down the DMA
mask setting routine ufshcd_set_dma_mask() from PCI glue driver to core
driver in order to do it for both PCI glue driver and Platform glue
driver. Secondly, we should change pci_ DMA mapping API to dma_ DMA
mapping API because core driver is independent of glue drivers.

The data byte count field of PRDT indicates the length of data block
which is a segment of data transfer for SCSI commands. The value of
this field shall have Dword granularity and the the maximum of length
is 256KB.
This adjusts dma pad mask and max segment size to the above-mentioned
PRDT limitations.

UFS 1.1 specification does not support MAINTENANCE IN(0xA3) SCSI
command and hence it doesn't support REPORT SUPPORTED OPERATION CODES
as well.

In interrupt context, after reading and comparing the UTRLDBR to
hba->outstanding_request and before resetting the interrupt aggregation,
there might be completion of another transfer request (TR). Such TRs might
get stuck, pending, until the next interrupt is generated (if any).
Changing the sequence of resetting the interrupt aggregation first and
then reading UTRLDBR status, will assure that completed TRs won't get
stuck pending.

Some UFS devices may expose bLUQueueDepth field as zero indicating
that the queue depth depends on the number of resources available
for LUN at a particular instant to handle the outstanding transfer
requests. Currently, when response for SCSI command is TASK_FULL
the LLD decrements the queue depth but fails to increment when the
resources are available. The scsi mid-layer handles the change in
queue depth heuristically and offers simple interface with
->change_queue_depth.

Some of the UFS devices may support different number of commands
that can be queued per LU. At the current implementation,
SW configure each of the UFS devices LU's according to the
controller capability.
In this patch the queue depth available per LU is read and updated in
the LU's SW structure.

Introduces the API for sending queries with descriptors.
A descriptor is a block or page of parameters that describe the device.
The descriptors are classified into types and can range in size
from 2 bytes through 255 bytes.
All descriptors have a length value as their first element, and a type
identification element as their second byte.
All descriptors are readable and some may be write once.
They are accessed using their type, index and selector.

change_queue_depth allows changing per-target queue depth via sysfs.
It also allows the SCSI midlayer to ramp down the number of concurrent
inflight requests in response to a SCSI BUSY status response and allows
the midlayer to ramp the count back up to the device maximum when the
BUSY condition has resolved.

The spinlock of tgt_lock is only for serializing read and write
req_vq, one lockless seqcount is enough for the purpose.
On one 16core VM with vhost-scsi backend, the patch can improve
IOPS with 3% on random read test.

a 6-byte READ/WRITE CDB with a 0 block data transfer really
means a 256 block data transfer. The RAID mapping code failed
to handle this case. For 10/12/16 byte READ/WRITEs, 0 just means
no data should be transferred, and should not trigger BUG_ON.

code failed to distribute interrupts correctly. Rework the code to
handle arbitrary processor counts.
Also remove the hardcoded upper limit on the number of processors so we
can boot on large systems.

In _scsih_{slave,target}_alloc, an incorrect structure type is passed
to sizeof() when allocating storage for hostdata. Luckily larger
structure types were used, so at least the wrong sizes were safe:

In _scsih_{slave,target}_alloc, an incorrect structure type is passed
to sizeof() when allocating storage for hostdata. Luckily larger
structure types were used, so at least the wrong sizes were safe:

The NCR53c406a scsi driver normally does not use DMA, unless
the USE_PIO macro is disabled by modifying the source code.
The call to free_dma() for some reason uses #ifdef USE_DMA,
which does not do the right thing, since USE_DMA is defined
as a boolean that is either 0 or 1, but always present.
One case where it gets in the way is randconfig builds on ARM,
which depending on the configuration does not provide a free_dma()
function, causing this build error:

The custom stats is an array with custom_length indicating the length
of the array. This patch fixes bnx2i and be2iscsi's setting of the
custom stats length. They both just have the one, eh_abort_cnt, so that should
be in the first entry of the custom array and custom_length should then
be one.

Despite supporting modern SCSI features some storage devices continue to
claim conformance to an older version of the SPC spec. This is done for
compatibility with legacy operating systems.
Linux by default will not attempt to read VPD pages on devices that
claim SPC-2 or older. Introduce a blacklist flag that can be used to
trigger VPD page inquiries on devices that are known to support them.

We currently set the field in common code based on the device type,
but then only use it in the cdrom driver which also overrides the
value previously set in the generic code.
Just leave this entirely to the CDROM driver to make everyones life
simpler.

Current the midlayer fakes up a struct request for the explicit reset
ioctls, and those don't have a tag allocated to them. The fnic driver pokes
into midlayer structures to paper over this design issue, but that won't
work for the blk-mq case.
Either someone who can actually test the hardware will have to come up with
a similar hack for the blk-mq case, or we'll have to bite the bullet and fix
the way the EH ioctls work for real, but until that happens we fail these
explicit requests here.

This patch adds support for an alternate I/O path in the scsi midlayer
which uses the blk-mq infrastructure instead of the legacy request code.
Use of blk-mq is fully transparent to drivers, although for now a host
template field is provided to opt out of blk-mq usage in case any unforseen
incompatibilities arise.
In general replacing the legacy request code with blk-mq is a simple and
mostly mechanical transformation. The biggest exception is the new code
that deals with the fact the I/O submissions in blk-mq must happen from
process context, which slightly complicates the I/O completion handler.
The second biggest differences is that blk-mq is build around the concept
of preallocated requests that also include driver specific data, which
in SCSI context means the scsi_cmnd structure. This completely avoids
dynamic memory allocations for the fast path through I/O submission.
Due the preallocated requests the MQ code path exclusively uses the
host-wide shared tag allocator instead of a per-LUN one. This only
affects drivers actually using the block layer provided tag allocator
instead of their own. Unlike the old path blk-mq always provides a tag,
although drivers don't have to use it.
For now the blk-mq path is disable by defauly and must be enabled using
the "use_blk_mq" module parameter. Once the remaining work in the block
layer to make blk-mq more suitable for slow devices is complete I hope
to make it the default and eventually even remove the old code path.

Blk-mq drivers usually preallocate their S/G list as part of the request,
but if we want to support the very large S/G lists currently supported by
the SCSI code that would tie up a lot of memory in the preallocated request
pool. Add support to the scatterlist code so that it can initialize a
S/G list that uses a preallocated first chunks and dynamically allocated
additional chunks. That way the scsi-mq code can preallocate a first
page worth of S/G entries as part of the request, and dynamically extend
the S/G list when needed.

Replace the calls to the various blk_end_request variants with opencode
equivalents. Blk-mq is using a model that gives the driver control
between the bio updates and the actual completion, and making the old
code follow that same model allows us to keep the code more similar for
both paths.

This saves us an atomic operation for each I/O submission and completion
for the usual case where the driver doesn't set a per-target can_queue
value. Only a few iscsi hardware offload drivers set the per-target
can_queue value at the moment.

Seems like these counters are missing any sort of synchronization for
updates, as a over 10 year old comment from me noted. Fix this by
using atomic counters, and while we're at it also make sure they are
in the same cacheline as the _busy counters and not needlessly stored
to in every I/O completion.
With the new model the _busy counters can temporarily go negative,
so all the readers are updated to check for > 0 values. Longer
term every successful I/O completion will reset the counters to zero,
so the temporarily negative values will not cause any harm.

Avoid taking the queue_lock to check the per-device queue limit. Instead
we do an atomic_inc_return early on to grab our slot in the queue,
and if necessary decrement it after finishing all checks.
Unlike the host and target busy counters this doesn't allow us to avoid the
queue_lock in the request_fn due to the way the interface works, but it'll
allow us to prepare for using the blk-mq code, which doesn't use the
queue_lock at all, and it at least avoids a queue_lock round trip in
scsi_device_unbusy, which is still important given how busy the queue_lock
is.

We should call the device handler prep_fn for all TYPE_FS requests,
not just simple read/write calls that are handled by the disk driver.
Restructure the common I/O code to call the prep_fn handler and zero
out the CDB, and just leave the call to scsi_init_io to the ULDs.

During IO with fabric faults, one generally sees several "Unhandled error
code" messages in the syslog as shown below:

This comes from scsi_io_completion (in scsi_lib.c) while handling error
codes other than DID_RESET or not deferred sense keys i.e. this is
actually handled by the SCSI mid layer. But what gets displayed here is
"Unhandled error code" which is quite misleading as it indicates
something that is not addressed by the mid layer.
The description string is based on the sense key and sometimes on the
additional sense code;
since the ACTION_FAIL case always prints the sense key and the
additional sense code, this patch removes the description string
completely because it does not add useful information.

Now that the ibmvstgt driver as the only user of scsi_tgt is gone, the
scsi_tgt kernel module, the CONFIG_SCSI_TGT, CONFIG_SCSI_SRP_TGT_ATTRS and
CONFIG_SCSI_FC_TGT_ATTRS kbuild variable, the scsi_host_template
transfer_response method are no longer needed.

scsilun_to_int() has an error which prevents it from generating
correct LUN numbers for 64bit values.
Also we should remove the misleading comment about portions of
the LUN being ignored; the initiator should treat the LUN as
an opaque value.
And, finally, the example given should use the correct
prefix (here: extended flat space addressing scheme).
This patch includes the modifications suggested by

Sequential scan for more than 256 LUNs is very fragile as
LUNs might not be numbered sequentially after that point.
SAM revisions later than SCSI-3 impose a structure on
LUNs larger than 256, making LUN numbers between 256
and 16384 illegal.
SCSI-3, however allows for plain 64-bit numbers with
no internal structure.
So restrict sequential LUN scan to 256 LUNs and add a
new blacklist flag 'BLIST_SCSI3LUN' to scan up to
max_lun devices.

This addresses a problem reported by Vaughan Cao concerning
the correctness of the O_EXCL logic in the sg driver. POSIX
doesn't defined O_EXCL semantics on devices but "allow only
one open file descriptor at a time per sg device" is a rough
definition. The sg driver's semantics have been to wait
on an open() when O_NONBLOCK is not given and there are
O_EXCL headwinds. Nasty things can happen during that wait
such as the device being detached (removed). So multiple
locks are reworked in this patch making it large and hard
to break down into digestible bits.
This patch is against Linus's current git repository which
doesn't include any sg patches sent in the last few weeks.
Hence this patch touches as little as possible that it
doesn't need to and strips out most SCSI_LOG_TIMEOUT()
changes in v3 because Hannes said he was going to rework all
that stuff.
The sg3_utils package has several test programs written to
test this patch. See examples/sg_tst_excl*.cpp .
Not all the locks and flags in sg have been re-worked in
this patch, notably sg_request::done . That can wait for
a follow-up patch if this one meets with approval.

When the SG_IO ioctl was copied into the block layer and
later into the bsg driver, subtle differences emerged.
One difference is the way injected commands are queued through
the block layer (i.e. this is not SCSI device queueing nor SATA
NCQ). Summarizing:
- SG_IO in the block layer: blk_exec*(at_head=false)
- sg SG_IO: at_head=true
- bsg SG_IO: at_head=true
Some time ago Boaz Harrosh introduced a sg v4 flag called
BSG_FLAG_Q_AT_TAIL to override the bsg driver default.
This patch does the equivalent for the sg driver.

Until now the per-command transfer length has exclusively been gated by
the max_sectors parameter in the scsi_host template. Given that the size
of this parameter has been bumped to an unsigned int we have to be
careful not to exceed the target device's capabilities.
If the if the device specifies a Maximum Transfer Length in the Block
Limits VPD we'll use that value. Otherwise we'll use 0xffffffff for
devices that have use_16_for_rw set and 0xffff for the rest. We then
combine the chosen disk limit with max_sectors in the host template. The
smaller of the two will be used to set the max_hw_sectors queue limit.

We must notify the block layer via q->flush_flags after a temporary change
of the cache_type to write through. Without this, a SYNCHRONIZE CACHE
command will still be generated. This patch factors out a helper that
can be called from sd_revalidate_disk and cache_type_store.

This change enables to test read/write commands with huge transfer
length such as 1GB. For example:

The data type of max_sectors in scsi_host_template has been extended
to unsigned int by the previous change. So we can increase it from
0xffff to 0xffffffff to allow such huge transfer length.
Also, this increases sg_tablesize and max_segment_size, otherwise the
maximum transfer length is limited to 64MB.
(sg_tablesize * max_segment_size = 256 * 256KB)

This change makes the scsi disk driver handle the requests whose
transfer length is greater than 0xffff with READ_16 or WRITE_16.
However, this is a preparation for extending the data type of
max_sectors in struct Scsi_Host and scsi_host_template. So, it is
impossible to happen this condition for now, because SCSI low-level
drivers can not specify max_sectors greater than 0xffff due to the
data type limitation.

This prevents integer overflow when converting the request queue's
max_sectors from sectors to bytes. However, this is a preparation for
extending the data type of max_sectors in struct Scsi_Host and
scsi_host_template. So, it is impossible to happen this integer
overflow for now, because SCSI low-level drivers can not specify
max_sectors greater than 0xffff due to the data type limitation.

scsi_put_command() is either invoked before blk_start_request() or
after block layer processing has completed. scsi_cmnd.abort_work
is scheduled from inside the SCSI timeout handler. The block layer
guarantees that either the regular completion handler
(softirq_done_fn()) or the timeout handler (rq_timed_out_fn()) is
invoked but not both. This means that scsi_put_command() is never
invoked while abort_work is scheduled. Hence remove the
cancel_delayed_work() call from scsi_put_command().
Similarly, scsi_abort_command() is only invoked from the SCSI
timeout handler. If scsi_abort_command() is invoked for a SCSI
command with the SCSI_EH_ABORT_SCHEDULED flag set this means that
scmd_eh_abort_handler() has already invoked scsi_queue_insert() and
hence that scsi_cmnd.abort_work is no longer pending. Hence also
remove the cancel_delayed_work() call from scsi_abort_command().

There's several locations in the kernel that open code the calculation
of the next location in the trace_seq buffer. This is usually done with
p->buffer + p->len
Instead of having this open coded, supply a helper function in the

Even though the virtio-scsi spec guarantees that all requests related
to the TMF will have been completed by the time the TMF itself completes,
the request queue's callback might not have run yet. This causes requests
to be completed more than once, and as a result triggers a variety of
BUGs or oopses.

If a CRQ reset is triggered for some reason while in the middle
of performing VSCSI adapter initialization, we don't want to
call the done function for the initialization MAD commands as
this will only result in two threads attempting initialization
at the same time, resulting in failures.

Its due to the fact that we call a potentially sleeping function from the bnx2fc
rcv path with preemption disabled (via the get_cpu call embedded in the per-cpu
variable stats lookup in bnx2fc_l2_rcv_thread.
Easy enough fix, we can just move the stats collection later in the function
where we are sure we won't preempt or sleep. This also allows us to not have to
enable pre-emption when doing a per-cpu lookup, since we're certain not to get
rescheduled.

In case of of error, the bnx2fc_cmd_mgr_alloc() function will call
the bnx2fc_cmd_mgr_free() to perform the cleanup.
The problem is that in one case the latter may try to scan
some not-yet initialized lists, resulting in a kernel panic.
This patch prevents this from happening by freeing the lists
before calling bnx2fc_cmd_mgr_free().

Any callbacks in scsi_timeout_out() might return BLK_EH_RESET_TIMER,
in which case we should leave the result alone and not set

After scsi_try_to_abort_cmd returns, the eh_abort_handler may have
already found that the command has completed in the device, causing
the host_byte to be nonzero (e.g. it could be DID_ABORT). When
this happens, ORing DID_TIME_OUT into the host byte will corrupt
the result field and initiate an unwanted command retry.
Fix this by using set_host_byte instead, following the model of
commit 2082ebc45a.

In case protection information exists over the wire
iscsi header data length is required to include it.
Use protection information aware scsi helpers to set
the correct transfer length.
In order to avoid breakage, remove iser transfer length
checks for each task as they are not always true and

With the optimizations around not clearing the full request at alloc
time, we are leaving some of the needed init for REQ_TYPE_BLOCK_PC
up to the user allocating the request.
Add a blk_rq_set_block_pc() that sets the command type to
REQ_TYPE_BLOCK_PC, and properly initializes the members associated
with this type of request. Update callers to use this function instead
of manipulating rq->cmd_type directly.

This patch converts qla2xxx target code to use generic percpu_ida
tag allocation provided by target-core, thus removing the original
kmem_cache_zalloc() for each struct qla_tgt_cmd descriptor in the
incoming ATIO packet fast-path.
This includes the conversion of qlt_handle_cmd_for_atio() to perform
qla_tgt_sess lookup before dispatching a command descriptor into
qla_tgt_wq process context, along with handling the case where no
active session exists, and subsequently kicking off a seperate
process context for qlt_create_sess_from_atio() to create a new one.
It also includes moving tag allocation into generic code within
qlt_get_tag(), so that the same logic can be shared between
qlt_handle_cmd_for_atio() + qlt_create_sess_from_atio() contexts.
Also, __qlt_do_work() has been made generic between both normal
process context in qlt_do_work() + qlt_create_sess_from_atio().
Next, update qlt_free_cmd() to release the percpu-ida tags, and
drop the now-unused global qla_tgt_cmd_cachep.
Finally in tcm_qla2xxx code, tcm_qla2xxx_check_initiator_node_acl()
has been updated to use transport_init_session_tags() along with a
hardcoded TCM_QLA2XXX_DEFAULT_TAGS=2088 as the number of qla_tgt_cmd
descriptors to pre-allocate per qla_tgt_sess instance.
(Use ha->fw_xcb_count if available to calculate num_tags, and
also factor in extra pad tags - Quinn)

This patch updates virtscsi_probe() to setup necessary Scsi_Host
level protection resources. (currently hardcoded to 1)
It changes virtscsi_add_cmd() to attach outgoing / incoming
protection SGLs preceeding the data payload, and is using the
new virtio_scsi_cmd_req_pi->pi_bytes[out,in] field to signal
to signal to vhost/scsi bytes to expect for protection data.
(Add missing #include <linux/blkdev.h> for blk_integrity - sfr + nab)

"It's a big(ish) round this time, lots of development effort has gone
into blk-mq in the last 3 months. Generally we're heading to where
3.16 will be a feature complete and performant blk-mq. scsi-mq is
progressing nicely and will hopefully be in 3.17. A nvme port is in
progress, and the Micron pci-e flash driver, mtip32xx, is converted
and will be sent in with the driver pull request for 3.16.
This pull request contains:
- Lots of prep and support patches for scsi-mq have been integrated.
All from Christoph.
- API and code cleanups for blk-mq from Christoph.
- Lots of good corner case and error handling cleanup fixes for
blk-mq from Ming Lei.
- A flew of blk-mq updates from me:
* Provide strict mappings so that the driver can rely on the CPU
to queue mapping. This enables optimizations in the driver.
* Provided a bitmap tagging instead of percpu_ida, which never
really worked well for blk-mq. percpu_ida relies on the fact
that we have a lot more tags available than we really need, it
fails miserably for cases where we exhaust (or are close to
exhausting) the tag space.
* Provide sane support for shared tag maps, as utilized by scsi-mq
* Various fixes for IO timeouts.
* API cleanups, and lots of perf tweaks and optimizations.
- Remove 'buffer' from struct request. This is ancient code, from
when requests were always virtually mapped. Kill it, to reclaim
some space in struct request. From me.
- Remove 'magic' from blk_plug. Since we store these on the stack
and since we've never caught any actual bugs with this, lets just
get rid of it. From me.
- Only call part_in_flight() once for IO completion, as includes two
atomic reads. Hopefully we'll get a better implementation soon, as
the part IO stats are now one of the more expensive parts of doing
IO on blk-mq. From me.
- File migration of block code from {mm,fs}/ to block/. This
includes bio.c, bio-integrity.c, bounce.c, and ioprio.c. From me,
from a discussion on lkml.

CTLR_STATE_CHANGE_EVENT and CTLR_STATE_CHANGE_EVENT_REDUNDANT_CNTRL
do not require rescans to be initiated. Current firmware filters out
these events already, but some out of date firmware doesn't, so the
driver needs to filter them out too. Without this change and with out
of date firmware you may see the driver spending a lot of time
scanning devices unnecessarily on some Smart Arrays.

There's nothing the user can or should do about these messages,
the commands are retried down the normal RAID path, and the
messages just flood the logs and sap performance.

After 3.22 firmware, PMC firmware guys tell us the
previous 5 second delay after a reset now needs to
be 10 secs to avoid a PCIe error due to the driver
looking at the controller too soon after the reset.

The acornscsi driver was added in v2.1.88. It has always #undef-ed
CONFIG_SCSI_ACORNSCSI_LINK near the top of acornscsi.c. And, just to be
sure, it has also always triggered a preprocessor error if
CONFIG_SCSI_ACORNSCSI_LINK was still defined. But, as far as I can see,
it has never even been possible to set SCSI_ACORNSCSI_LINK through
kconfig, or its predecessors, in the first place.
Let's remove the code involved.

Reusing a smid quickly means it's still cache-hot. This yields a small
but noticable performance improvement in a well-known database benchmark.
This improvement is already present in the mpt3sas driver.

The mpt2sas_scsih_issue_tm() function does not use the 'serial_number'
argument passed to it. Removing it removes the last vestiges of the
scsi_cmnd's serial_number field from this driver.

During heavy IO in multipath environment with many active sessions
and port-bouncing happening, there is a race condition because of
which beiscsi_prcess_cqe() gets called for a connection whose
endpoint is freed.
Checking endpoint reference for a connection before processing in
beiscsi_process_cq().

EQ teardown should happen only after all CQ are destroyed.
In some FW config, adapter goes into a freeze state. This
fix moves teardown of MCC-Q before the EQ teardown happens.

Before probe for function was completed, iSCSI Daemon had initiated login
to target while OS was coming up. The targets which had node.startup=automatic,
login process was initiated.Since function specific initialization was still in
progress this lead to kernel panic.
Fixed the issue by moving iscsi_host_add() call after adapter initialization
is done.

Getting WRB for MCCQ posting was done before looking if tag is
available or not. This lead to increase phba->ctrl.mcc_obj.q.used
variable and the WARN_ON message was coming from wrb_from_mccq().
Moved getting wrb from mccq after checking for the tag.
In wrb_from_mccq(), memset is done before returning wrb ptr.
Removed memset of mccq wrb from all other functions.

Error handling in UFS driver is broken and resets the host controller
for fatal errors without re-initialization. Correct the fatal error
handling sequence according to UFS Host Controller Interface (HCI)
v1.1 specification.
o Processed requests which are completed w/wo error are reported to
SCSI layer and any pending commands that are not started are aborted
in the controller and re-queued into scsi mid-layer queue.
o Upon determining fatal error condition the host controller may hang
forever until a reset is applied. Block SCSI layer for sending new
requests and apply reset in a separate error handling work.
o SCSI is informed about the expected Unit-Attention exception from the
device for the immediate command after a reset so that the SCSI layer
take necessary steps to establish communication with the device.

As of now SCSI initiated error handling is broken because,
the reset APIs don't try to bring back the device initialized and
ready for further transfers.
In case of timeouts, the scsi error handler takes care of handling aborts
and resets. Improve the error handling in such scenario by resetting the
device and host and re-initializing them in proper manner.

There is a possible race condition in the hardware when the abort
command is issued to terminate the ongoing SCSI command as described
below:
- A bit in the door-bell register is set in the controller for a
new SCSI command.
- In some rare situations, before controller get a chance to issue
the command to the device, the software issued an abort command.
- If the device recieves abort command first then it returns success
because the command itself is not present.
- Now if the controller commits the command to device it will be
processed.
- Software thinks that command is aborted and proceed while still
the device is processing it.
- The software, controller and device may go out of sync because of
this race condition.
To avoid this, query task presence in the device before sending abort
task command so that after the abort operation, the command is guaranteed
to be non-existent in both controller and the device.

Currently, sending Task Management (TM) command to the card might
be broken in some scenarios as listed below:
Problem: If there are more than 8 TM commands the implementation
returns error to the caller.
Fix: Wait for one of the slots to be emptied and send the command.
Problem: Sometimes it is necessary for the caller to know the TM service
response code to determine the task status.
Fix: Propogate the service response to the caller.
Problem: If the TM command times out no proper error recovery is
implemented.
Fix: Clear the command in the controller door-bell register, so that
further commands for the same slot don't fail.
Problem: While preparing the TM command descriptor, the task tag used
should be unique across SCSI/NOP/QUERY/TM commands and not the
task tag of the command which the TM command is trying to manage.
Fix: Use a unique task tag instead of task tag of SCSI command.
Problem: Since the TM command involves H/W communication, abruptly ending
the request on kill interrupt signal might cause h/w malfunction.
Fix: Wait for hardware completion interrupt with TASK_UNINTERRUPTIBLE
set.

Fix many warnings with incorrect endian assumptions
which makes the code unportable to new architectures.
The UFS specification defines the byte order as big-endian
for UPIU structure and little-endian for the host controller
transfer/task management descriptors.

The sun3 drivers suffer from a whole bunch of duplicated code. Fix this
by following the g_NCR5380_mmio example. (Notionally, sun3_scsi relates to
sun3_scsi_vme in the same way that g_NCR5380 relates to g_NCR5380_mmio.)
Dead code is also removed: we now have working debug macros so
SUN3_SCSI_DEBUG is undesirable. Dead code within #ifdef OLD_DMA is also
dropped, consistent with sun3_scsi_vme.c.

Move the #include "NCR5380.h" out of the sun3_scsi.h header file and into
the driver .c files, like all the other NCR5380 drivers in the tree.
This improves uniformity and reduces the depth of nested includes. The
sequence of #include's, #define's and #if's no longer does my head in.

All three NCR5380 core driver implementations share the same NCR5380.h
header file so they need to agree on certain macro definitions.
The flag bit used by the NDEBUG_MERGING macro in atari_NCR5380 and
sun3_NCR5380 collides with the bit used by NDEBUG_LISTS.
Moreover, NDEBUG_ABORT appears in NCR5380.c so it should be defined in
NCR5380.h rather than in each of the many drivers using that core.
An undefined NDEBUG_ABORT macro caused compiler errors and led to dodgy
workarounds in the core driver that can now be removed.

Move all of the NDEBUG_ABORT, NDEBUG_TAGS and NDEBUG_MERGING macro
definitions into NCR5380.h where all the other NDEBUG macros live.
Also, incorrect "#ifdef NDEBUG" becomes "#if NDEBUG" to fix the warning:

There are three implementations of the core NCR5380 driver and three sets
of debugging macro definitions. And all three implementations use the
NCR5380.h header as well.
Two of the definitions of the dprintk macro accept a variable argument list
whereas the third does not. Standardize on the variable argument list.

The change from cmd->target to cmd->device->id was apparently the purpose of
commit a7f251228390e87d86c5e3846f99a455517fdd8e in
kernel/git/tglx/history.git but some instances have been missed.
Also fix the "NDEBUG_LAST_WRITE_SENT" and "NDEBUG_ALL" typo's.
Also fix some format strings (%ul becomes %lu) that caused compiler warnings.

With the kernel running from FastRAM instead of ST-RAM, none of ST-RAM is
mapped by mem_init, and DMA-addressable buffer must be mapped by ioremap.
Use platform specific virt/phys translation helpers for this case.
