Commit 523e1d399c ("block: make gendisk hold a reference to its queue")
modified add_disk() and disk_release() but did not update any of the
error paths that trigger a put_disk() call after disk->queue has been
assigned. That introduced the following behavior in the pktcdvd driver
if pkt_new_dev() fails:
Kernel BUG at 00000000e98fd882 [verbose debug info unavailable]
Since disk_release() calls blk_put_queue() anyway if disk->queue != NULL,
fix this by removing the blk_cleanup_queue() call from the pkt_setup_dev()
error path.

This way we don't need a block_device structure to submit I/O. The
block_device has different life time rules from the gendisk and
request_queue and is usually only available when the block device node
is open. Other callers need to explicitly create one (e.g. the lightnvm
passthrough code, or the new nvme multipathing code).
For the actual I/O path all that we need is the gendisk, which exists
once per block device. But given that the block layer also does
partition remapping we additionally need a partition index, which is
used for said remapping in generic_make_request.
Note that all the block drivers generally want request_queue or
sometimes the gendisk, so this removes a layer of indirection all
over the stack.

Here is the big driver core update for 4.13-rc1.
The large majority of this is a lot of cleanup of old fields in the
driver core structures and their remaining usages in random drivers.
All of those fixes have been reviewed by the various subsystem

kobject uevent api interface that makes userspace interaction easier,

Instead of explicitly calling scsi_req_init() after blk_get_request(),
call that function from inside blk_get_request(). Add an
.initialize_rq_fn() callback function to the block drivers that need
it. Merge the IDE .init_rq_fn() function into .initialize_rq_fn()
because it is too small to keep it as a separate function. Keep the
scsi_req_init() call in ide_prep_sense() because it follows a

pktcdvd doesn't change the bi_io_vec of the clone bio,
so it is more efficient to use bio_clone_fast(), and not clone
the bi_io_vec.
This requires providing a bio_set, and it is safest to
provide a dedicated bio_set rather than sharing
fs_bio_set, which filesytems use.
This new bio_set, pkt_bio_set, can also be use for the bio_split()
call as the two allocations (bio_clone_fast, and bio_split) are

Hi Peter, hi Jens,
I've been looking over the multi page bio vec work again recently, and
one of the stumbling blocks is raw biovec access in the pktcdvd.
The first issue is that it directly sets up the page and offset pointers
in the biovec just before calling bio_add_page. As bio_add_page already
does the setup it's trivial to just switch it to stack variables for the
arguments.
The second issue is the copy code in pkt_make_local_copy, which
effectively is an opencoded version of bio_copy_data except that it
skips pages that already are the same in the ѕource and destination.
But we look at the only calleer we just set up the bio using bio_add_page
to point exactly to the page array that pkt_make_local_copy compares,
so the pages will always be the same and we can just remove this function.
Note that all this is done based on code inspection, I don't have any
packet writing hardware myself.

use to the block layer and a few drivers that dig into struct request

This patch adds a new req_flags_t rq_flags field to struct request for

also removes the unfortunate situation where we have to fit the fields
from the same enum into 32 bits for struct bio and 64 bits for

clear __GFP_DIRECT_RECLAIM so kswapd will still wake. As clearing
__GFP_WAIT behaves differently, there is a risk that people will clear the

indicate what it does -- setting it allows all reclaim activity, clearing

The way the block layer is currently written, it goes to great lengths
to avoid having to split bios; upper layer code (such as bio_add_page())
checks what the underlying device can handle and tries to always create
bios that don't need to be split.
But this approach becomes unwieldy and eventually breaks down with
stacked devices and devices with dynamic limits, and it adds a lot of
complexity. If the block layer could split bios as needed, we could
eliminate a lot of complexity elsewhere - particularly in stacked
drivers. Code that creates bios can then create whatever size bios are
convenient, and more importantly stacked drivers don't have to deal with
both their own bio size limitations and the limitations of the
(potentially multiple) devices underneath them. In the future this will
let us delete merge_bvec_fn and a bunch of other code.
We do this by adding calls to blk_queue_split() to the various
make_request functions that need it - a few can already handle arbitrary
size bios. Note that we add the call _after_ any call to
blk_queue_bounce(); this means that blk_queue_split() and
blk_recalc_rq_segments() don't need to be concerned with bouncing
affecting segment merging.
Some make_request_fn() callbacks were simple enough to audit and verify
they don't need blk_queue_split() calls. The skipped ones are:
* nfhd_make_request (arch/m68k/emu/nfblock.c)
* axon_ram_make_request (arch/powerpc/sysdev/axonram.c)
* simdisk_make_request (arch/xtensa/platforms/iss/simdisk.c)
* brd_make_request (ramdisk - drivers/block/brd.c)
* mtip_submit_request (drivers/block/mtip32xx/mtip32xx.c)
* loop_make_request
* null_queue_bio
* bcache's make_request fns
Some others are almost certainly safe to remove now, but will be left
for future patches.

Currently we have two different ways to signal an I/O error on a BIO:
(1) by clearing the BIO_UPTODATE flag
(2) by returning a Linux errno value to the bi_end_io callback
The first one has the drawback of only communicating a single possible
error (-EIO), and the second one has the drawback of not beeing persistent
when bios are queued up, and are not passed along from child to parent
bio in the ever more popular chaining scenario. Having both mechanisms
available has the additional drawback of utterly confusing driver authors
and introducing bugs where various I/O submitters only deal with one of
them, and the others have to add boilerplate code to deal with both kinds
of error returns.
So add a new bi_error field to store an errno value directly in struct
bio and remove the existing mechanisms to clean all this up.

up to the user allocating the request.
Add a blk_rq_set_block_pc() that sets the command type to
REQ_TYPE_BLOCK_PC, and properly initializes the members associated
with this type of request. Update callers to use this function instead
of manipulating rq->cmd_type directly.
Includes fixes from Christoph Hellwig <hch@lst.de> for my half-assed
attempt.

Set the return variable to an error code as done elsewhere in the function.
A simplified version of the semantic match that finds this problem is as
follows: (http://coccinelle.lip6.fr/)
// <smpl>
(
if@p1 (\(ret < 0\|ret != 0\))
{ ... return ret; }
|
ret@p1 = 0
)
... when != ret = e1
when != &ret
*if(...)
{
... when != ret = e2
when forall
return ret;
}
// </smpl>

The new bio_split() can split arbitrary bios - it's not restricted to
single page bios, like the old bio_split() (previously renamed to
bio_pair_split()). It also has different semantics - it doesn't allocate
a struct bio_pair, leaving it up to the caller to handle completions.
Then convert the existing bio_pair_split() users to the new bio_split()
- and also nvme, which was open coding bio splitting.
(We have to take that BUG_ON() out of bio_integrity_trim() because this
bio_split() needs to use it, and there's no reason it has to be used on
bios marked as cloned; BIO_CLONED doesn't seem to have clearly
documented semantics anyways.)

My static checker complains correctly that this is potential NULL
dereference because debugfs functions return NULL on error. They return
an ERR_PTR if they are configured out.
We don't need to check for ERR_PTR because if debugfs is stubbed out the
dummy functions won't complain about that. We don't need to check the
values before calling debugfs_remove() because that accepts ERR_PTRs and
NULL pointers.
We don't need to set pkt->dfs_f_info to NULL in pkt_debugfs_dev_new()
because it was initialized with kzalloc() so I have removed that.

Static checkers complain about widening the binary "not" operations here
because sectors are u64 and "(pd)->settings.size" is unsigned int.
It unintentionally clears the high 32 bits of the sector. This means
that the driver won't work for devices with over 2TB of space. Since
this is a DVD drive, we're unlikely to reach that limit, but we may as
well silence the warning.
