The helper crypto_tfm_ctx is only used by the Crypto API algorithm
code and should really be in algapi.h. However, for historical
reasons many files relied on it to be in crypto.h. This patch
changes those files to use algapi.h instead in prepartion for a
move.

Eliminate anonymous module_init() and module_exit(), which can lead to
confusion or ambiguity when reading System.map, crashes/oops/bugs,
or an initcall_debug log.
Give each of these init and exit functions unique driver-specific
names to eliminate the anonymous names.

This prefixes all crypto module loading with "crypto-" so we never run
the risk of exposing module auto-loading to userspace via a crypto API,
as demonstrated by Mathias Krause:

Initialization of cra_list is currently mixed, most ciphers initialize this
field and most shashes do not. Initialization however is not needed at all
since cra_list is initialized/overwritten in __crypto_register_alg() with
list_add(). Therefore perform cleanup to remove all unneeded initializations
of this field in 'arch/x86/crypto/'.

x86 has fast unaligned accesses, so twofish-x86_64/i586 does not need to enforce
alignment.

This needed by 3-way twofish patch to be able to easily use one block
assembler functions. As glue code is shared between i586/x86_64 apply
change to i586 assembler too. Also export assembler functions for
3-way parallel twofish module.
