The current logic suffers from a slow response time to disable user DB
usage, and also fails to avoid DB FIFO drops under heavy load. This commit
fixes these deficiencies and makes the avoidance logic more optimal.
This is done by more efficiently notifying the ULDs of potential DB
problems, and implements a smoother flow control algorithm in iw_cxgb4,
which is the ULD that puts the most load on the DB fifo.
Design:
cxgb4:
Direct ULD callback from the DB FULL/DROP interrupt handler. This allows
the ULD to stop doing user DB writes as quickly as possible.
While user DB usage is disabled, the LLD will accumulate DB write events
for its queues. Then once DB usage is reenabled, a single DB write is
done for each queue with its accumulated write count. This reduces the
load put on the DB fifo when reenabling.
iw_cxgb4:
Instead of marking each qp to indicate DB writes are disabled, we create
a device-global status page that each user process maps. This allows
iw_cxgb4 to only set this single bit to disable all DB writes for all
user QPs vs traversing the idr of all the active QPs. If the libcxgb4
doesn't support this, then we fall back to the old approach of marking
each QP. Thus we allow the new driver to work with an older libcxgb4.
When the LLD upcalls iw_cxgb4 indicating DB FULL, we disable all DB writes
via the status page and transition the DB state to STOPPED. As user
processes see that DB writes are disabled, they call into iw_cxgb4
to submit their DB write events. Since the DB state is in STOPPED,
the QP trying to write gets enqueued on a new DB "flow control" list.
As subsequent DB writes are submitted for this flow controlled QP, the
amount of writes are accumulated for each QP on the flow control list.
So all the user QPs that are actively ringing the DB get put on this
list and the number of writes they request are accumulated.
When the LLD upcalls iw_cxgb4 indicating DB EMPTY, which is in a workq
context, we change the DB state to FLOW_CONTROL, and begin resuming all
the QPs that are on the flow control list. This logic runs on until
the flow control list is empty or we exit FLOW_CONTROL mode (due to
a DB DROP upcall, for example). QPs are removed from this list, and
their accumulated DB write counts written to the DB FIFO. Sets of QPs,
called chunks in the code, are removed at one time. The chunk size is 64.
So 64 QPs are resumed at a time, and before the next chunk is resumed, the
logic waits (blocks) for the DB FIFO to drain. This prevents resuming to
quickly and overflowing the FIFO. Once the flow control list is empty,
the db state transitions back to NORMAL and user QPs are again allowed
to write directly to the user DB register.
The algorithm is designed such that if the DB write load is high enough,
then all the DB writes get submitted by the kernel using this flow
controlled approach to avoid DB drops. As the load lightens though, we
resume to normal DB writes directly by user applications.

Without a shutdown handler, T4 cards behave very badly after a kexec.
Some firmware calls return errors indicating allocation failures, for
example. This is probably because thouse resources were not released by
a BYE message to the firmware, for example.
Using the remove handler guarantees we will use a well tested path.
With this patch I applied, I managed to use kexec multiple times and
probe and iSCSI login worked every time.

The adapter is freed before we check its flags. It was caused
by commit 144be3d ("net/cxgb4: Avoid disabling PCI device for
towice"). The problem was reported by Intel's "0-day" tool.
The patch fixes it to avoid reverting commit 144be3d.

We possibly retrieve the adapter's statistics during EEH recovery
and that should be disallowed. Otherwise, it would possibly incur
replicate EEH error and EEH recovery is going to fail eventually.
The patch reuses statistics lock and checks net_device is attached
before going to retrieve statistics, so that the problem can be
avoided.

If we have EEH error happens to the adapter and we have to remove
it from the system for some reasons (e.g. more than 5 EEH errors
detected from the device in last hour), the adapter will be disabled
for towice separately by eeh_err_detected() and remove_one(), which
will incur following unexpected backtrace. The patch tries to avoid
it.

None of these files are actually using any __init type directives
and hence don't need to include <linux/init.h>. Most are just a
left over from __devinit and __cpuinit removal, or simply due to
code getting copied from one driver to the next.
This covers everything under drivers/net except for wireless, which
has been submitted separately.

Since commit 52367a763d
("cxgb4/cxgb4vf: Code cleanup to enable T4 Configuration File support"),
we have failures like this during cxgb4 probe:
cxgb4 0000:01:00.4: bad SGE FL page buffer sizes [65536, 65536]
cxgb4: probe of 0000:01:00.4 failed with error -22
This happens whenever software parameters are used, without a
configuration file. That happens when the hardware was already
initialized (after kexec, or after csiostor is loaded).
It happens that these values are acceptable, rendering fl_pg_order equal
to 0, which is the case of a hard init when the page size is equal or
larger than 65536.
Accepting fl_large_pg equal to fl_small_pg solves the issue, and
shouldn't cause any trouble besides a possible performance reduction
when smaller pages are used. And that can be fixed by a configuration
file.

Adds API cxgb4_select_ntuple so as to enable Upper Level Drivers to correctly
calculate the tuple fields.
Adds constant definitions for TP_VLAN_PRI_MAP for the Compressed
Filter Tuple field widths and structures and uses them.
Also, the CPL Parameters field for T5 is 40 bits so we need to prototype
cxgb4_select_ntuple() to calculate and return u64 values.

IPv6 uses 2 TIDs with CLIP enabled and 4 TIDs without CLIP.
Currently we are incrementing STIDs in use by 1 for both IPv4 and IPv6 which
is wrong.
Further, driver currently does not have interface to query if CLIP is programmed
for particular IPv6 address. So, in this patch we increment/decrement TIDs in use
by 4 for IPv6 assuming absence of CLIP. Such assumption keeps us on safe side and
we don't end up allocating more stids for IPv6 than actually supported.

The LE workaround code is incorrectly reusing the TCAM TIDs
(meant for allocation by firmware in case of hash collisions) for filter
servers. This patch assigns the filter server TIDs properly starting from
sftid_base index.

We were creating LE Workaround Server Filters without specifying
IPPROTO_TCP (6) in the filters (when F_PROTOCOL is set in TP_VLAN_PRI_MAP).
This meant that UDP packets with matching IP Addresses/Ports would get
caught up in the filter and be delivered to ULDs like iw_cxgb4.
So, include the protocol information in the server filter properly.

When creating offload server entries, an IPv6 passive connection request
can trigger a reply with a null STID, whereas the driver would expect
the reply 'STID to match the value used for the request.
This happens due to h/w limitation on T4 and T5.
This patch ensures that STID 0 is never used if the stid range starts
from zero.

There are a mix of function prototypes with and without extern
in the kernel sources. Standardize on not using extern for
function prototypes.
Function prototypes don't need to be written with extern.
extern is assumed by the compiler. Its use is as unnecessary as
using auto to declare automatic/local variables in a block.

When driver registration fails, we need to clean up the resources allocated
before. cxgb4 missed to destroy the workqueue allocated at the very beginning.
This patch destroies the workqueue when registration fails.

As the tests PPC64 (powernv platform) show, IOMMU pages are leaking
when transferring big amount of small packets (<=64 bytes),
"ping -f" and waiting for 15 seconds is the simplest way to confirm the bug.

The Compressed LIP region is used to hold a limited number of Local
IPv6 addresses. This region is primarily used to reduce the TCAM
space consumed for an IPv6 offloaded connection. A 128-bit LIP will
be reduced to 13-bit and stored in the TCAM if there is a match
between the IPv6 tuple's LIP and the one stored in the CLIP region.

Add cxgb4_create_server6 and cxgb4_remove_server routines to create
and remove listening IPv6 servers.
Return success (0) from cxgb4_create_server in case of ctrl queue
congestion since in case of congestion, passive open request gets
queued and gets processed later. If a non-zero value were returned it
would be treated as an error and the ULD would free STID, which can
result in an error in passive open reply.
Add cpl structure for active open request with IPv6 address for T5.

net_device::dev_id should not be used merely to indicate a VI index,
as it affects the way the local part of IPv6 addresses is normally
generated.
This field was intended for use where multiple devices may share a
single assigned MAC address and need to have different IPv6 addresses.
T4 VIs each have their own MAC address.

This corrects an regression introduced by "net: Use 16bits for *_headers
fields of struct skbuff" when NET_SKBUFF_DATA_USES_OFFSET is not set. In
that case skb->tail will be a pointer whereas skb->transport_header
will be an offset from head. This is corrected by using wrappers that
ensure that comparisons and calculations are always made using pointers.

Forcing uninitialized state allows us to upgrade and reinitialize the adapter.
FW_VERSION_T4 = 1.4.0.0
FW_VERSION_T5 = 0.0.0.0
At this point driver supports above and greater than above version of firmware.
If it doesn't find the required firmware version than it forces the adapter to
be reinitialized as shown below.
1) If FW_ON_ADAPTER < FW_VERSION and we're the MASTER_PF force uninitialized
state and a FW upgrade if available.
- If FW_ON_ADAPTER < /lib/firmware/cxgb4/t*fw.bin we will update the
adapters FW.
- If FW_ON_ADAPTER >= /lib/firmware/cxgb4/t*fw.bin don't upgrade FW.
- If upgrade_fw() fails force reinitialization of the adapter anyways,
it might still work.
Either way forcing the uninitialized state allows cxgb4 reinitialize FW.
2) If FW_ON_ADAPTER >= FW_VERSION driver follows normal path.

Since commit 636f9d371f ("cxgb4: Add
support for T4 configuration file"), t4_fw_hello may return a positive
value instead of 0 for success. The recovery code tests only for zero
and fails recovery for any other value.
This fix tests for negative error values and fails only on those cases.
Error recovery after an error injection works after this change.

Add a protocol argument to the VLAN packet tagging functions. In case of HW
tagging, we need that protocol available in the ndo_start_xmit functions,
so it is stored in a new field in the skb. The new field fits into a hole
(on 64 bit) and doesn't increase the sks's size.

Rename the hardware VLAN acceleration features to include "CTAG" to indicate
that they only support CTAGs. Follow up patches will introduce 802.1ad
server provider tagging (STAGs) and require the distinction for hardware not
supporting acclerating both.

Drivers should reserve some headroom in skb used in receive path,
to avoid future head reallocation.
One possible way to do that is to use dev_alloc_skb() instead
of alloc_skb(), so that NET_SKB_PAD bytes are reserved.

It enables direct DMA by HW to memory region PBL arrays and fast register PBL
arrays from host memory, vs the T4 way of passing these arrays in the WR itself.
The result is lower latency for memory registration, and larger PBL array
support for fast register operations.
This patch also updates ULP_TX_MEM_WRITE command fields for T5. Ordering bit of
ULP_TX_MEM_WRITE is at bit position 22 in T5 and at 23 in T4.

All T5 adapters will only support VFs on PF0-3 despite the ability of the
hardware to support them on PF4-7. This keeps our T4 and T5 adapters more
similar which simplifies host driver software.

This patch implements a low latency Write Combining (aka Write Coalescing) work
request path. PCIE maps User Space Doorbell BAR2 region writes to the new
interface to SGE. SGE pulls a new message from PCIE new interface and if its a
coalesced write work request then pushes it for processing. This patch copies
coalesced work request to memory mapped BAR2 space.
