Set the queue dying flag and call blk_mq_exit_queue from del_gendisk for
all disks that do not have separately allocated queues, and thus remove
the need to call blk_cleanup_queue for them.
Rename blk_cleanup_disk to blk_mq_destroy_queue to make it clear that
this function is intended only for separately allocated blk-mq queues.
This saves an extra queue freeze for devices without a separately
allocated queue.

The function get_capabilities() has the possibility of failing to allocate
the transfer buffer but it does not currently handle this. This may lead to
exceptions when accessing the buffer.

Commit 2e27f576ab ("scsi: scsi_ioctl: Call scsi_cmd_ioctl() from
scsi_ioctl()") seems to have a typo as it is checking ret instead of cmd in
the if statement checking for CDROMCLOSETRAY and CDROMEJECT. This changes
the behaviour of these ioctls as the cdrom_ioctl handling of these is more
restrictive than the scsi_ioctl version.

Commit 01d0c69853 ("sr: implement ->free_disk to simplify refcounting")
refactored sr_block_open(), initialized one variable with a duplicate
assignment (probably an unintended copy & paste duplication) and turned one
error case into an early return, which makes the initialization of the
return variable needless.
So, simplify the local variable initialization in sr_block_open() to make
the code a bit more clear.
No functional change. No change in resulting object code.

Now that each scsi_request is backed by a scsi_cmnd, there is no need to
indirect the CDB storage. Change all submitters of SCSI passthrough
requests to store the CDB information directly in the scsi_cmnd, and while
doing so allocate the full 32 bytes that cover all Linux supported SCSI
hosts instead of requiring dynamic allocation for > 16 byte CDBs. On
64-bit systems this does not change the size of the scsi_cmnd at all, while
on 32-bit systems it slightly increases it for now, but that increase will
be made up by the removal of the remaining scsi_request fields.

All modern drivers can support extra partitions using the extended
dev_t. In fact except for the ioctl method drivers never even see
partitions in normal operation.
So remove the GENHD_FL_EXT_DEVT and allow extra partitions for all
block devices that do support partitions, and require those that
do not support partitions to explicit disallow them using
GENHD_FL_NO_PART.

GENHD_FL_CD marks a gendisk as a vaguely CD-ROM like device.
Besides being used internally inside of sunvdc.c an xen-blkfront it
is used by xen-blkback as a hint to claim a device exported to a
guest is a CD-ROM like device. Just check for disk->cdi instead
which is the right indicator for "real" CD-ROM or DVD drivers. This
will miss the paravirtualized guest drivers, but those make little
sense to report anyway.

Add a new helper that calls blk_get_request and initializes the
scsi_request to avoid the indirect call through ->.initialize_rq_fn.
Note that this makes the pktcdvd driver depend on the SCSI core, but
given that only SCSI devices support SCSI passthrough requests that
is not a functional change.

request field request with a macro, allowing us to offset map to it

Most drivers should use and have been converted to use blk_alloc_disk
and blk_mq_alloc_disk. Only the scsi ULPs and dasd still allocate
a disk separately from the request_queue, so don't bother with
convenience macros for something that should not see significant
new users and remove these wrappers.

cdrom_read_cdda_bpc() relies on sending SCSI command to the low level
driver using a REQ_OP_SCSI_IN request. This isn't generic block layer
functionality, so move the actual low-level code into the sr driver and
call it through a new read_cdda_bpc method in the cdrom_device_ops
structure.
With this the CDROM code does not have to pull in scsi_normalize_sense()
and depend on CONFIG_SCSI_COMMON.

Media event code 3 is defined in the MMC-6 spec as follows:
"MediaRemoval: The media has been removed from the specified slot, and
the Drive is unable to access the media without user intervention. This
applies to media changers only."
This indicated that treating the condition as an EJECT_REQUEST was
appropriate. However, doing so had the unfortunate side-effect of causing
the drive tray to be physically ejected on resume. Instead treat the event
as a MEDIA_CHANGE request.

Handle a reported media event code of 3. This indicates that the media has
been removed from the drive and user intervention is required to proceed.
Return DISK_EVENT_EJECT_REQUEST in that case.

Originally the SCSI subsystem has been using 'special' SCSI status codes,
which were the SAM-specified ones but shifted by 1. As most drivers have
now been modified to use the SAM-specified ones, having two nearly
identical sets of definitions only causes confusion.
The Linux-specifed SCSI status codes have been marked obsolete for several
years so drop them and use the SAM-specified status codes throughout.

The description for scsi_mode_sense() claims to return the number of valid
bytes on success, which is not what the code does. Additionally there is
no gain in returning the SCSI status, as everything the callers do is to
check against scsi_result_is_good(), which is what scsi_mode_sense() does
already. So change the calling convention to return a standard error code
on failure, and 0 on success, and adapt the description and all callers.

The in_interrupt() check in sr_init_command() is a leftover from the
past, pre v2.3.16 era to be exact. Back then the ioctl() was served by
`sr' itself and sector size changes by CDROMREADMODE2 (as noted in the
comment) were accounted within sr's data structures which allowed a
"lazy" reset so it could be skipped on the next request and reset back
to the default value once the device node was closed or before a command
from the blockqueue was issued.
This does not work like that anymore. The CDROMREADMODE2 is served by
cdrom's mmc_ioctl() function which may change the sector size but the
`sr' driver does not learn about it and so its ->sector_size is not
updated.
The ioctl() resets the changed sector size back to 2048.
sr_read_sector() also resets the sector size back to the default once it
is done.
Remove the conditional sector size update from sr_init_command() and
sr_release() because it is not needed.
