The function doesn't use it and all of its callers say in a comment that
their respective arguments are to be non-NULL only in auto-translated
mode. Since xen_remap_domain_mfn_array() isn't supposed to be used by
non-PV, drop the parameter there as well. It was bogusly passed as non-
NULL (PRIV_VMA_LOCKED) by its only caller anyway. For
xen_remap_domain_gfn_range(), otoh, it's not clear at all why this
wouldn't want / might not need to gain auto-translated support down the
road, so the parameter is retained there despite now remaining unused
(and the only caller passing NULL); correct a respective comment as
well.

The initial observation was that in PV mode under Xen 32-bit user space
didn't work anymore. Attempts of system calls ended in #GP(0x402). All
of the sudden the vector 0x80 handler was not in place anymore. As it
turns out up to 5.13 redundant initialization did occur: Once from
cpu_initialize_context() (through its VCPUOP_initialise hypercall) and a
2nd time while each CPU was brought fully up. This 2nd initialization is
now gone, uncovering that the 1st one was flawed: Unlike for the
set_trap_table hypercall, a full virtual IDT needs to be specified here;
the "vector" fields of the individual entries are of no interest. With
many (kernel) IDT entries still(?) (i.e. at that point at least) empty,
the syscall vector 0x80 ended up in slot 0x20 of the virtual IDT, thus
becoming the domain's handler for vector 0x20.
Make xen_convert_trap_info() fit for either purpose, leveraging the fact
that on the xen_copy_trap_info() path the table starts out zero-filled.
This includes moving out the writing of the sentinel, which would also
have lead to a buffer overrun in the xen_copy_trap_info() case if all
(kernel) IDT entries were populated. Convert the writing of the sentinel
to clearing of the entire table entry rather than just the address
field.
(I didn't bother trying to identify the commit which uncovered the issue
in 5.14; the commit named below is the one which actually introduced the
bad code.)

In order to allow objtool to make sense of all the various paravirt
functions, it needs to either parse whole pv_ops[] tables, or observe
individual assignments in the form:
bf87: 48 c7 05 00 00 00 00 00 00 00 00 movq $0x0,0x0(%rip)
bf92 <xen_init_spinlocks+0x5f>
bf8a: R_X86_64_PC32 pv_ops+0x268
As is, xen_cpu_ops[] is at offset +0 in pv_ops[] and could thus be
parsed as a 'normal' pv_ops[] table, however xen_irq_ops[] and
xen_mmu_ops[] are not.
Worse, both the latter two are compiled into the individual assignment
for by current GCC, but that's not something one can rely on.
Therefore, convert all three into full pv_ops[] tables. This has the
benefit of not needing to teach objtool about the offsets and
resulting in more conservative code-gen.

Because hypercall_page is page-aligned, the assembler inexplicably adds
an unreachable jump from after the end of the previous code to the
beginning of hypercall_page.
That confuses objtool, understandably. It also creates significant text
fragmentation. As a result, much of the object file is wasted text
(nops).
Move hypercall_page to the beginning of the file to both prevent the
text fragmentation and avoid the dead jump instruction.

Xen PV guests are specifying the highest used PFN via the max_pfn
field in shared_info. This value is used by the Xen tools when saving
or migrating the guest.
Unfortunately this field is misnamed, as in reality it is specifying
the number of pages (including any memory holes) of the guest, so it
is the highest used PFN + 1. Renaming isn't possible, as this is a
public Xen hypervisor interface which needs to be kept stable.
The kernel will set the value correctly initially at boot time, but
when adding more pages (e.g. due to memory hotplug or ballooning) a
real PFN number is stored in max_pfn. This is done when expanding the
p2m array, and the PFN stored there is even possibly wrong, as it
should be the last possible PFN of the just added P2M frame, and not
one which led to the P2M expansion.
Fix that by setting shared_info->max_pfn to the last possible PFN + 1.

kernel.h is being used as a dump for all kinds of stuff for a long time.
Here is the attempt to start cleaning it up by splitting out panic and
oops helpers.
There are several purposes of doing this:
- dropping dependency in bug.h
- dropping a loop by moving out panic_notifier.h
- unload kernel.h from something which has its own domain
At the same time convert users tree-wide to use new headers, although for
the time being include new header back to kernel.h to avoid twisted
indirected includes for existing users.

Revert "xen: fix p2m size in dom0 for disabled memory hotplug case"
xen/x86: make XEN_BALLOON_MEMORY_HOTPLUG_LIMIT depend on MEMORY_HOTPLUG
This partially reverts commit 882213990d ("xen: fix p2m size in dom0
for disabled memory hotplug case")
There's no need to special case XEN_UNPOPULATED_ALLOC anymore in order
to correctly size the p2m. The generic memory hotplug option has
already been tied together with the Xen hotplug limit, so enabling
memory hotplug should already trigger a properly sized p2m on Xen PV.
Note that XEN_UNPOPULATED_ALLOC depends on ZONE_DEVICE which pulls in
MEMORY_HOTPLUG.

The Xen memory hotplug limit should depend on the memory hotplug
generic option, rather than the Xen balloon configuration. It's
possible to have a kernel with generic memory hotplug enabled, but
without Xen balloon enabled, at which point memory hotplug won't work
correctly due to the size limitation of the p2m.
Rename the option to XEN_MEMORY_HOTPLUG_LIMIT since it's no longer
tied to ballooning.

On 32-bit kernels, the stackprotector canary is quite nasty -- it is
stored at %gs:(20), which is nasty because 32-bit kernels use %fs for
percpu storage. It's even nastier because it means that whether %gs
contains userspace state or kernel state while running kernel code
depends on whether stackprotector is enabled (this is
CONFIG_X86_32_LAZY_GS), and this setting radically changes the way
that segment selectors work. Supporting both variants is a
maintenance and testing mess.
Merely rearranging so that percpu and the stack canary
share the same segment would be messy as the 32-bit percpu address
layout isn't currently compatible with putting a variable at a fixed
offset.
Fortunately, GCC 8.1 added options that allow the stack canary to be
accessed as %fs:__stack_chk_guard, effectively turning it into an ordinary
percpu variable. This lets us get rid of all of the code to manage the
stack canary GDT descriptor and the CONFIG_X86_32_LAZY_GS mess.
(That name is special. We could use any symbol we want for the
%fs-relative mode, but for CONFIG_SMP=n, gcc refuses to let us use any
name other than __stack_chk_guard.)
Forcibly disable stackprotector on older compilers that don't support
the new options and turn the stack canary into a percpu variable. The
"lazy GS" approach is now used for all 32-bit configurations.
Also makes load_gs_index() work on 32-bit kernels. On 64-bit kernels,
it loads the GS selector and updates the user GSBASE accordingly. (This
is unchanged.) On 32-bit kernels, it loads the GS selector and updates
GSBASE, which is now always the user base. This means that the overall
effect is the same on 32-bit and 64-bit, which avoids some ifdeffery.

To improve TLB shootdown performance, flush the remote and local TLBs
concurrently. Introduce flush_tlb_multi() that does so. Introduce
paravirtual versions of flush_tlb_multi() for KVM, Xen and hyper-v (Xen
and hyper-v are only compile-tested).
While the updated smp infrastructure is capable of running a function on
a single local core, it is not optimized for this case. The multiple
function calls and the indirect branch introduce some overhead, and
might make local TLB flushes slower than they were before the recent
changes.
Before calling the SMP infrastructure, check if only a local TLB flush
is needed to restore the lost performance in this common case. This
requires to check mm_cpumask() one more time, but unless this mask is
updated very frequently, this should impact performance negatively.

xen: fix p2m size in dom0 for disabled memory hotplug case
xen-netback: respect gnttab_map_refs()'s return value
Xen/gnttab: handle p2m update errors on a per-slot basis
Since commit 9e2369c06c ("xen: add helpers to allocate unpopulated
memory") foreign mappings are using guest physical addresses allocated
via ZONE_DEVICE functionality.
This will result in problems for the case of no balloon memory hotplug
being configured, as the p2m list will only cover the initial memory
size of the domain. Any ZONE_DEVICE allocated address will be outside
the p2m range and thus a mapping can't be established with that memory
address.
Fix that by extending the p2m size for that case. At the same time add
a check for a to be created mapping to be within the p2m limits in
order to detect errors early.
While changing a comment, remove some 32-bit leftovers.

Its sibling (set_foreign_p2m_mapping()) as well as the sibling of its
only caller (gnttab_map_refs()) don't clean up after themselves in case
of error. Higher level callers are expected to do so. However, in order
for that to really clean up any partially set up state, the operation
should not terminate upon encountering an entry in unexpected state. It
is particularly relevant to notice here that set_foreign_p2m_mapping()
would skip setting up a p2m entry if its grant mapping failed, but it
would continue to set up further p2m entries as long as their mappings
succeeded.
Arguably down the road set_foreign_p2m_mapping() may want its page state

a Xen PV guest will nevertheless use the IRET hypercall, as there
is no sysret PV hypercall defined.
So instead of testing all the prerequisites for doing a sysret and
then mangling the stack for Xen PV again for doing an iret just use
the iret exit from the beginning.
This can easily be done via an ALTERNATIVE like it is done for the
sysenter compat case already.
It should be noted that this drops the optimization in Xen for not
restoring a few registers when returning to user mode, but it seems
as if the saved instructions in the kernel more than compensate for
this drop (a kernel build in a Xen PV guest was slightly faster with
this patch applied).
While at it remove the stale sysret32 remnants.

SWAPGS is used only for interrupts coming from user mode or for
returning to user mode. So there is no reason to use the PARAVIRT
framework, as it can easily be replaced by an ALTERNATIVE depending
on X86_FEATURE_XENPV.
There are several instances using the PV-aware SWAPGS macro in paths
which are never executed in a Xen PV guest. Replace those with the
plain swapgs instruction. For SWAPGS_UNSAFE_STACK the same applies.

xen: Set platform PCI device INTX affinity to CPU0
xen: Fix event channel callback via INTX/GSI
xen/privcmd: allow fetching resource sizes
Only the IPI-related functions in the smp_ops should be conditional
on the vector callback being available. The rest should still happen:
• xen_hvm_smp_prepare_boot_cpu()
This function does two things, both of which should still happen if
there is no vector callback support.
The call to xen_vcpu_setup() for vCPU0 should still happen as it just
sets up the vcpu_info for CPU0. That does happen for the secondary
vCPUs too, from xen_cpu_up_prepare_hvm().
The second thing it does is call xen_init_spinlocks(), which perhaps
counter-intuitively should *also* still be happening in the case
without vector callbacks, so that it can clear its local xen_pvspin
flag and disable the virt_spin_lock_key accordingly.
Checking xen_have_vector_callback in xen_init_spinlocks() itself
would affect PV guests, so set the global nopvspin flag in
xen_hvm_smp_init() instead, when vector callbacks aren't available.
• xen_hvm_smp_prepare_cpus()
This does some IPI-related setup by calling xen_smp_intr_init() and
xen_init_lock_cpu(), which can be made conditional. And it sets the
xen_vcpu_id to XEN_VCPU_ID_INVALID for all possible CPUS, which does
need to happen.
• xen_smp_cpus_done()
This offlines any vCPUs which doesn't fit in the global shared_info
page, if separate vcpu_info placement isn't available. That part also
needs to happen regardless of vector callback support.
• xen_hvm_cpu_die()
This doesn't actually do anything other than commin_cpu_die() right
right now in the !vector_callback case; all three teardown functions
it calls should be no-ops. But to guard against future regressions
it's useful to call it anyway, and for it to explicitly check for
xen_have_vector_callback before calling those additional functions.

A Xen PVH domain doesn't have a PCI bus or devices, so it doesn't need
PCI support built in. Currently, XEN_PVH depends on XEN_PVHVM which
depends on PCI.
Introduce XEN_PVHVM_GUEST as a toplevel item and change XEN_PVHVM to a
hidden variable. This allows XEN_PVH to depend on XEN_PVHVM without PCI
while XEN_PVHVM_GUEST depends on PCI.
In drivers/xen, compile platform-pci depending on XEN_PVHVM_GUEST since
that pulls in the PCI dependency for linking.

struct apic has two members which store information about the destination
mode: dest_logical and irq_dest_mode.
dest_logical contains a mask which was historically used to set the
destination mode in IPI messages. Over time the usage was reduced and the
logical/physical functions were seperated.
There are only a few places which still use 'dest_logical' but they can
use 'irq_dest_mode' instead.
irq_dest_mode is actually a boolean where 0 means physical destination mode
and 1 means logical destination mode. Of course the name does not reflect
the functionality. This will be cleaned up in a subsequent change.
Remove apic::dest_logical and fixup the remaining users.

Patch series "device-dax: Support sub-dividing soft-reserved ranges", v5.
The device-dax facility allows an address range to be directly mapped
through a chardev, or optionally hotplugged to the core kernel page
allocator as System-RAM. It is the mechanism for converting persistent
memory (pmem) to be used as another volatile memory pool i.e. the current
Memory Tiering hot topic on linux-mm.
In the case of pmem the nvdimm-namespace-label mechanism can sub-divide
it, but that labeling mechanism is not available / applicable to
soft-reserved ("EFI specific purpose") memory [3]. This series provides a
sysfs-mechanism for the daxctl utility to enable provisioning of
volatile-soft-reserved memory ranges.
The motivations for this facility are:
1/ Allow performance differentiated memory ranges to be split between
kernel-managed and directly-accessed use cases.
2/ Allow physical memory to be provisioned along performance relevant
address boundaries. For example, divide a memory-side cache [4] along
cache-color boundaries.
3/ Parcel out soft-reserved memory to VMs using device-dax as a security
/ permissions boundary [5]. Specifically I have seen people (ab)using
memmap=nn!ss (mark System-RAM as Persistent Memory) just to get the
device-dax interface on custom address ranges. A follow-on for the VM
use case is to teach device-dax to dynamically allocate 'struct page' at
runtime to reduce the duplication of 'struct page' space in both the
guest and the host kernel for the same physical pages.

Split out all the bits that are purely for dma_map_ops implementations
and related code into a new <linux/dma-map-ops.h> header so that they
don't get pulled into all the drivers. That also means the architecture
specific <asm/dma-mapping.h> is not pulled in by <linux/dma-mapping.h>
any more, which leads to a missing includes that were pulled in by the
x86 or arm versions in a few not overly portable drivers.

- Untangle the header spaghetti which causes build failures in various
situations caused by the lockdep additions to seqcount to validate that
the write side critical sections are non-preemptible.
- The seqcount associated lock debug addons which were blocked by the
above fallout.
seqcount writers contrary to seqlock writers must be externally
serialized, which usually happens via locking - except for strict per
CPU seqcounts. As the lock is not part of the seqcount, lockdep cannot
validate that the lock is held.
This new debug mechanism adds the concept of associated locks.
sequence count has now lock type variants and corresponding
initializers which take a pointer to the associated lock used for
writer serialization. If lockdep is enabled the pointer is stored and
write_seqcount_begin() has a lockdep assertion to validate that the
lock is held.
Aside of the type and the initializer no other code changes are
required at the seqcount usage sites. The rest of the seqcount API is
unchanged and determines the type at compile time with the help of
_Generic which is possible now that the minimal GCC version has been
moved up.
Adding this lockdep coverage unearthed a handful of seqcount bugs which
have been addressed already independent of this.
While generaly useful this comes with a Trojan Horse twist: On RT
kernels the write side critical section can become preemtible if the
writers are serialized by an associated lock, which leads to the well
known reader preempts writer livelock. RT prevents this by storing the
associated lock pointer independent of lockdep in the seqcount and
changing the reader side to block on the lock when a reader detects
that a writer is in the write side critical section.
- Conversion of seqcount usage sites to associated types and initializers.

Pull locking updates from Thomas Gleixner:
"A set of locking fixes and updates:
- Untangle the header spaghetti which causes build failures in
various situations caused by the lockdep additions to seqcount to
validate that the write side critical sections are non-preemptible.
- The seqcount associated lock debug addons which were blocked by the
above fallout.
seqcount writers contrary to seqlock writers must be externally
serialized, which usually happens via locking - except for strict
per CPU seqcounts. As the lock is not part of the seqcount, lockdep
cannot validate that the lock is held.
This new debug mechanism adds the concept of associated locks.
sequence count has now lock type variants and corresponding
initializers which take a pointer to the associated lock used for
writer serialization. If lockdep is enabled the pointer is stored
and write_seqcount_begin() has a lockdep assertion to validate that
the lock is held.
Aside of the type and the initializer no other code changes are
required at the seqcount usage sites. The rest of the seqcount API
is unchanged and determines the type at compile time with the help
of _Generic which is possible now that the minimal GCC version has
been moved up.
