Pull block driver updates from Jens Axboe:
"This is the block driver pull request for 3.13. As with the core pull
request just sent out, this was rebased on top of the core branch
again after the immutable series was pulled. This also means that
bcache gets to sit the initial pull over. I will send a second driver
pull request in the merge window to get those fixes in, once they have
been rebased and tested on top of the non-immutable stack.
This pull request contains:
- Add support for the sTec Kronos pci-e flash card from sTec. Also
has various cleanups for this driver, from myself, Bart, Mike
Snizter, and Wei Yongjun.
- Add surprise removal support for the micron mtip32xx driver from
Micron.
- Floppy documentation fix from Ben Harris.

changed the semantics of elevator_init() in a way that now enforces to hold
the corresponding request queue's sysfs_lock when calling elevator_init()
to fix a race.
The patch did not convert the s390 dasd device driver which is the only
device driver which also calls elevator_init(). So add the missing locking.

The while loop only peeks at the top request in the queue but does
not yet consume it. Since we only handle fs requests, we need to
dequeue and complete all other request command types with error
just in case we would ever receive such an unforeseen request.

The prefix command is used instead of a define extent to make use of
PAV alias devices during format. On some older storage servers the
prefix command may not be available and the IO request will fail.
Check for availability of prefix command and use define extent if
not available.

When a recovery cqr is cleaned up, copy the start time, stop time,
and start device to the original cqr. These times are needed later
when the original request is finalized and counted in the DASD
statistics.

The ECKD protocol supports reading of tracks with arbitrary format as
raw track images. The DASD device driver supports this in its
raw_track_access mode. In this mode it maps each track to sixteen 4096
byte sectors and rejects all requests that are not properly aligned to
this mapping.
An application that wants to use a DASD in raw_track_access mode will
usually use direct I/O to make sure that properly aligned requests are
directly submitted to the driver. However, applications that are not
aware of this mode, e.g. udev, will encounter I/O errors.
To make the use without direct I/O possible and avoid this kind of
alignment errors, we now pad unaligned read requests with a dummy
page, so that we can always read full tracks. Please note that
writing is still only possible for full track images that are properly
aligned.

Whenever a DASD request encounters a timeout we might
need to abort all outstanding requests on this or
even other devices.
This is especially useful if one wants to fail all
devices on one side of a RAID10 configuration, even
though only one device exhibited an error.
To handle this I've introduced a new device flag
DASD_FLAG_ABORTIO.
This flag is evaluated in __dasd_process_request_queue()
and will invoke blk_abort_request() for all
outstanding requests with DASD_CQR_FLAGS_FAILFAST set.
This will cause any of these requests to be aborted
immediately if the blk_timeout function is activated.
The DASD_FLAG_ABORTIO is also evaluated in
__dasd_process_request_queue to abort all
new request which would have the
DASD_CQR_FLAGS_FAILFAST bit set.
The flag can be set with the new ioctls 'BIODASDABORTIO'
and removed with 'BIODASDALLOWIO'.

This patch adds a 'timeout' attibute to the DASD driver.
When set to non-zero, the blk_timeout function will
be enabled with the timeout specified in the attribute.
Setting 'timeout' to '0' will disable block timeouts.

The DASD driver is using FASTFAIL as an equivalent to the
transport errors in SCSI. And the 'steal lock' function maps
roughly to a reservation error. So we should be returning the
appropriate error codes when completing a request.

Whenever a request has been aborted internally by the driver
there is no sense data to be had. And printing lots of messages
stalls the system, so better to print out a short one-liner.

This patch implements generic block layer timeout handling
callbacks for DASDs. When the timeout expires the respective
cqr is aborted.
With this timeout handler time-critical request abort
is guaranteed as the abort does not depend on the internal
state of the various DASD driver queues.

Originally the DASD device tasklet would process the entries on
the ccw_queue until the first non-final request was found.
Which was okay as long as all requests have the same retries and
expires parameter.
However, as we're now allowing to modify both it is possible to
have requests _after_ the first request which already have expired.
So we need to check all requests in the device tasklet.

When a path is gone and dasd_generic_path_event is called with a
PE_PATH_GONE event, we must assume that any I/O request on that
subchannel is still running. This is unlike the dasd_generic_notify
handler and the CIO_NO_PATH event, which implies that the subchannel
has been cleared.
If dasd_generic_path_event finds that the path has been the last
usable path, it must not call dasd_generic_last_path_gone (which would
reset the state of running requests), but just set the
DASD_STOPPED_DC_WAIT bit.

The value passed is 0 in all but "it can never happen" cases (and those
only in a couple of drivers) *and* it would've been lost on the way

If too many ccw requests are pre-build before a suspend/resume cycle
the device might not get enough memory to do path verification
during resume.
Requeue requests to the block device request queue on suspend and free
pre-build ccw requests.

This is the pull request for the earlier patchset[1] with the same
name. It's only three patches (the first one was committed to
workqueue tree) but the merge strategy is a bit involved due to the
dependencies.
* Because the conversion needs features from wq/for-3.10,
block/for-3.10/core is based on rc3, and wq/for-3.10 has conflicts
with rc3, I pulled mainline (rc5) into wq/for-3.10 to prevent those
workqueue conflicts from flaring up in block tree.
* Resolving the issue that Jan and Dave raised about debugging
requires arch-wide changes. The patchset is being worked on[2] but
it'll have to go through -mm after these changes show up in -next,
and not included in this pull request.

Pulling it into block/for-3.10/core produces a conflict in
drivers/md/raid5.c between the following two commits.

The conflict is trivial - one removes an "if ()" conditional while the
other removes "rbi->bi_next = NULL" right above it. We just need to
remove both. The merged branch is available at
git://git.kernel.org/pub/scm/linux/kernel/git/tj/wq.git block-test-merge
so that you can use it for verification. The test merge commit has
proper merge description.
While these changes are a bit of pain to route, they make code simpler
and even have, while minute, measureable performance gain[3] even on a
workload which isn't particularly favorable to showing the benefits of
this conversion.

If a block device driver cannot fetch all requests from the blocklayer
it's in his responsibility to call the request function at a later time.
Normally this would be done after the next irq for the underlying device
is handled. However in situations where we have no outstanding request
we have to schedule the request function for a later time.
This is determined using an internal counter of requests issued to the
hardware.
In some cases where we give a request back to the block layer unhandled
the number of queued requests was not adjusted. Fix this class of
failures by adjusting queued_requests in all functions used to give
a request back to the block layer.

When the DASD devices are detached from the driver, then the
dasd_generic_remove function is called. One of the things this
function should do is to remove the DASD specific sysfs attributes,
but this is not done in all cases. This is likely to cause an oops
when at a later point sysfs stumbles over the stale pointers. In
particular this happens when when the modules are unloaded and loaded
again.

Now that irq sum accounting for /proc/stat's "intr" line works again we
have the oddity that the sum field (first field) contains only the sum
of the second (external irqs) and third field (I/O interrupts).
The reason for that is that these two fields are already sums of all other
fields. So if we would sum up everything we would count every interrupt
twice.
This is broken since the split interrupt accounting was merged two years
ago: 052ff461c8 "[S390] irq: have detailed
statistics for interrupt types".
To fix this remove the split interrupt fields from /proc/stat's "intr"
line again and only have them in /proc/interrupts.
This restores the old behaviour, seems to be the only sane fix and mimics
a behaviour from other architectures where /proc/interrupts also contains
more than /proc/stat's "intr" line does.

The regular behavior of the DASD device driver when setting a device
offline is to return all outstanding I/O as failed. This behavior is
different from that of other System z operating systems and may lead
to unexpected data loss. Adding an explicit 'safe' offline function
will allow customers to use DASDs in the way they expect them to work.

If a channel path is cabled incorrectly and the device is suspended and
resumed the device may be inaccessible afterwards.
Make the path connection check not interrupt the resume callback there
could be other valid paths available.

A common way to prepare a z/VM mini disk is to format the real device
with a z/VM tool like CPFMTXA and then define a mini disk that excludes
the first cylinder, i.e. the cylinder 0 of the virtual disk is located
at cylinder 1 of the real device.
The DASD device driver will recognize such a mini disk as formatted, as
the uniform record layout on the disk matches that of an LDL formatted
device. However, the cylinder value in the 'count' field of the ECKD
records matches the geometry of the real device, and not that of the
mini disk, so I/O requests will fail with 'record not found' errors.
To make the mini disk usable, it needs to be formatted with a tool like
dasdfmt. To enable tools like distribution installation tools to
recognize this situation, the DASD device driver should report such a
mini disk as 'not formatted'.
To this end we need to extend the device recognition code to check not
just for proper record sizes, but also for proper cylinder/head/record
values.

Read unit address is done for all devices during online processing to read
out LCU features. This is also done after disconnect/connect a LCU.
Some older storage hardware does not provide the capability to read unit
address configuration.
This leads to a loop trying to read unit address configuration every 30
seconds. The device is still operational but logs are flooded with error
messages.
Fix the loop by recognizing a command reject saying that the suborder
for ruac is not supported.

If a new path is available we need to verify the path data. If it is the
first path for a device the stop bits are removed after path verification.
If a pathgroup is established we need to set system characteristics for
the lcu. Therefore I/O has to be started.
If the device is stopped the set system characteristics worker may block
the path verification worker and the device is blocked.
Turn on failfast for set system characteristics CQR to prevent a deadlock
with the path verification worker.
If a pathgroup is established on a device that is not in use trigger path
verification. Maybe we were not informed about a working path.

Ensure that all work is done when the process waiting for a
dasd state change is woken up. With this change it is save
to assume that after a userspace triggered state change and
a udev settle invocation there are no unexpected users of a
dasd device.

Remove the file name from the comment at top of many files. In most
cases the file name was wrong anyway, so it's rather pointless.
Also unify the IBM copyright statement. We did have a lot of sightly
different statements and wanted to change them one after another
whenever a file gets touched. However that never happened. Instead
people start to take the old/"wrong" statements to use as a template
for new files.
So unify all of them in one go.

The external interrupt handlers have a parameter called ext_int_code.
Besides the name this paramter does not only contain the ext_int_code
but in addition also the "cpu address" (POP) which caused the external
interrupt.
To make the code a bit more obvious pass a struct instead so the called
function can easily distinguish between external interrupt code and
cpu address. The cpu address field however is named "subcode" since
some external interrupt sources do not pass a cpu address but a
different parameter (or none at all).

Remove the optimization that validate server is only called once per
LCU. If a device is set online we only know that we already know the
LCU. But if the pathgroup was lost in between we have to do a
validate server again to activate some features.
Since we have no indication when a pathgroup gets lost we have to do
a validate server every time a device is set online.

Move invalidate_bdev, block_sync_page into fs/block_dev.c. Export
kill_bdev as well, so brd doesn't have to open code it. Reduce
buffer_head.h requirement accordingly.
Removed a rather large comment from invalidate_bdev, as it looked a bit
obsolete to bother moving. The small comment replacing it says enough.

If an IO request is build on an alias device without prefix enabled
we try to calculate with zero data from the alias device. This
triggers a BUG statement with fixpoint divide exception.
This case is very unlikely and can only happen if the pathgroup is
lost with an alias device already in use.
Prevent the alias device from being used in this case.

The 'expires' value of a ccw requests defines how long the device
driver should wait for a response from the evice after the request has
been submitted to the channel subsystem. After the expiration time
(e.g. 30 seconds) the waiting request will be cancelled and started
again. This protects the DASD devices from beeing blocked by errors
that cause the answering I/O interrupt to be lost.
In case of error recovery requests, this 'expires' value used to be
set to 0, so in case of a lost interrupt, such a recovery request
would never expire and block the device. To prevent this kind of
problem, all recovery requests need to have an expires value > 0 as
well. If not specified otherwise, this should be the same expires
value as for the original request.

The buffer for read configuration data has to be initialized with an
EBCDIC string to show support for extended UIDs to z/VM.
If this read configuration data CQR needs to be retried, the buffer
may have changed in between. So re-initialize the buffer to get a
correct extended UID under z/VM.

Current IRQ statistics support does not show detail counts for I/O
interrupts which are processed internally only. The result is a

There is very little benefit in allowing to let a ->make_request
instance update the bios device and sector and loop around it in
__generic_make_request when we can archive the same through calling
generic_make_request from the driver and letting the loop in
generic_make_request handle it.
Note that various drivers got the return value from ->make_request and
returned non-zero values for errors.

To use raw track access some special storage server commands are
needed. Older storage hardware may not support these commands.
So check if raw track access is possible while setting the DASD
online.

The size of the buffer that is used to store DASD statistics input
strings depends on the user input. If the input string is to large,
the write operation could fail with -ENOMEM. To avoid this, use
vmalloc instead of kmalloc.

This patch extends the DASD statistics to allow for a more detailed
analysis of DASD I/O operations. In particular we want the statistics
to provide answers to the following questions:
- How many requests used a PAV alias?
- How many requests used High Performance FICON?
- How do read request perform versus write requests?
The existing DASD statistics interface has several shortcomings
- The interface for global data is a formatted text table in procfs
(/proc/dasd/statistics). The layout is meant for human readers and
is not to easy to parse. If values get to large for the table
layout, they get scaled down.
- The statistics which are collected per block device can be
accessed via an ioctl interface, which can only be extended by
defining a new ioctl.
- There is no statistics interface for individual PAV base and alias
devices.
To overcome theses shortcomings we create a new DASD statistics
interface in debugfs. This interface will contain one entry for global
data, one per DASD block device, and one per DASD base and alias
device. Each file contains the statistic data in easy to parse
name/value and name/array pairs. The existing interfaces will remain
functional, but they will not be extended.

Interrupt sources like pfault, sclp, dasd_diag and virtio all use the
service signal external interrupt subclass mask in control register 0
to enable and disable the corresponding interrupt.
Because no reference counting is implemented each subsystem thinks it
is the only user of subclass and sets and clears the bit like it wants.
This leads to case that unloading the dasd diag module under z/VM
causes both sclp and pfault interrupts to be masked. The result will
be locked up system sooner or later.
Fix this by introducing a new way to set (register) and clear
(unregister) the service signal subclass mask bit in cr0.
Also convert all drivers.

pfault, dasd diag and virtio all use the same external interrupt number.
The respective interrupt handlers decide by the subcode if they are
meant to handle the interrupt.
Counting is currently done before looking at the subcode which means
each handler counts an interrupt even if it is not handling it.
Fix this by moving the kstat code after the code which looks at the
subcode.

The dasd_open function uses the private_data pointer of the gendisk to
find the dasd_block structure that matches the gendisk. When a DASD
device is set offline, we set the private_data pointer of the gendisk
to NULL and later remove the dasd_block structure, but there is still
a small race window, in which dasd_open could first read a pointer
from the private_data field and then try to use it, after the structure
has already been freed.
To close this race window, we will store a pointer to the dasd_devmap
structure of the base device in the private_data field. The devmap
entries are not deleted, and we already have proper locking and
reference counting in place, so that we can safely get from a devmap
pointer to the dasd_device and dasd_block structures of the device.

The module parameter 'devs' and 'sizes' are marked as __initdata. The
memory for the parameters are freed after module_init completed. This
can lead to kernel crashes in param_free_charp. Remove the __initdata
attribute to fix the problem.

If a device is not resumed correctly the system crashes when this
device is set offline. This may happen if it gets disconnected
during suspend.
Check if the device is already removed from alias handling and skip
these steps to prevent the kernel panic.

In case the DASD driver needs to term a running I/O the retry counter
is decreased twice.

Normal I/O operations through the DASD device driver give only access
to the data fields of an ECKD device even for track based I/O.
This patch extends the DASD device driver to give access to whole
ECKD tracks including count, key and data fields.

The freeze callback may set a stop bit so that a worker thread could
not start I/O. The discipline specific freeze function waits for the
worker to be completed.
Set the stop_bit after the discipline specific freeze function has
returned and no worker is running.

If a DASD device has been reserved by a Linux system, and later
this reservation is ‘stolen’ by a second system by means of an
unconditional reserve, then the first system receives a
notification about this fact. With this patch such an event can
be either ignored, as before, or it can be used to let the device
fail all I/O request, so that the device will not block anymore.

When a new path is added at runtime, the CIO layer will call the drivers
path_event callback. The DASD device driver uses this callback to trigger
a path verification for the new path. The driver will use only those
paths for I/O, which have been successfully verified.

Up to now /proc/interrupts only has statistics for external and i/o
interrupts but doesn't split up them any further.
This patch adds a line for every single interrupt source so that it
is possible to easier tell what the machine is/was doing.

Over time, block layer has accumulated a set of APIs dealing with bdev
open, close, claim and release.
* blkdev_get/put() are the primary open and close functions.
* bd_claim/release() deal with exclusive open.
* open/close_bdev_exclusive() are combination of open and claim and
the other way around, respectively.
* bd_link/unlink_disk_holder() to create and remove holder/slave
symlinks.
* open_by_devnum() wraps bdget() + blkdev_get().
The interface is a bit confusing and the decoupling of open and claim
makes it impossible to properly guarantee exclusive access as
in-kernel open + claim sequence can disturb the existing exclusive
open even before the block layer knows the current open if for another
exclusive access. Reorganize the interface such that,
* blkdev_get() is extended to include exclusive access management.
@holder argument is added and, if is @FMODE_EXCL specified, it will
gain exclusive access atomically w.r.t. other exclusive accesses.
* blkdev_put() is similarly extended. It now takes @mode argument and
if @FMODE_EXCL is set, it releases an exclusive access. Also, when
the last exclusive claim is released, the holder/slave symlinks are
removed automatically.
* bd_claim/release() and close_bdev_exclusive() are no longer
necessary and either made static or removed.
* bd_link_disk_holder() remains the same but bd_unlink_disk_holder()
is no longer necessary and removed.
* open_bdev_exclusive() becomes a simple wrapper around lookup_bdev()
and blkdev_get(). It also has an unexpected extra bdev_read_only()
test which probably should be moved into blkdev_get().
* open_by_devnum() is modified to take @holder argument and pass it to
blkdev_get().
Most of bdev open/close operations are unified into blkdev_get/put()
and most exclusive accesses are tested atomically at the open time (as
it should). This cleans up code and removes some, both valid and
invalid, but unnecessary all the same, corner cases.
open_bdev_exclusive() and open_by_devnum() can use further cleanup -
rename to blkdev_get_by_path() and blkdev_get_by_devt() and drop
special features. Well, let's leave them for another day.
Most conversions are straight-forward. drbd conversion is a bit more
involved as there was some reordering, but the logic should stay the
same.

The BIODASDSNID ioctl executes a 'Sense Path Group ID'
command on a DASD ECKD device. The returned path group data
allows user space programs to determine path state and
path group ID of the channel paths to the device.

For the DASD DIAG discipline IO is started through special diagnose
calls. Unsolicited interrupts may contain information about the device
itself. But this information is not needed because the device is not
used directly.
Fix the case that an unimplemented dicipline function may be called
by ignoring unsolicited interrupts for the DIAG disciplin.

The dasd interrupt handler needs to distinguish solicited from
unsolicited interrupts, as unsolicited interrupts may require special
handling (e.g. summary unit checks) and solicited interrupts require
proper error recovery for the failed I/O request.
The interrupt handler needs to check several bit fields in the
interrupt response block (irb) to make this distinction.
So far our check of the status control bits has not been specific
enough, which may lead to a failed request getting just retried
instead of the necessary error recovery.

Writing to /proc/dasd/statistics while the debug level of the
generic dasd debug entry is set to DBF_DEBUG will lead to an
use after free when accessing the debug entry later.
Since for the format string "%s" in the s390 dbf only a pointer
to the string is stored in the debug feature and the buffer used
here is freed afterwards.
To fix this just remove the debug message.

The usual way to recover a failed DASD ECKD request (cqr) is to create
a new request with an appropriate recovery CCW program. Certain
features, e.g. failfast, can be enabled per request and are stored in
the requests flags. These flags have to be copied from the failed to
the recovery request, to let the recovery request use the same
features as the original one.

All file_operations should get a .llseek operation so we can make
nonseekable_open the default for future file operations without a
.llseek pointer.
The three cases that we can automatically detect are no_llseek, seq_lseek
and default_llseek. For cases where we can we can automatically prove that
the file offset is always ignored, we use noop_llseek, which maintains
the current behavior of not returning an error from a seek.
New drivers should normally not use noop_llseek but instead use no_llseek
and call nonseekable_open at open time. Existing drivers can be converted
to do the same when the maintainer knows for certain that no user code
relies on calling seek on the device file.
The generated code is often incorrectly indented and right now contains
comments that clarify for each added line why a specific variant was
chosen. In the version that gets submitted upstream, the comments will
be gone and I will manually fix the indentation, because there does not
seem to be a way to do that using coccinelle.
Some amount of new code is currently sitting in linux-next that should get
the same modifications, which I will do at the end of the merge window.

The dasd and dcssblk drivers gained the big
kernel lock in the recent pushdown from the
block layer, but they don't really need it,
so remove the calls without a replacement.

Barrier is deemed too heavy and will soon be replaced by FLUSH/FUA
requests. Deprecate barrier. All REQ_HARDBARRIERs are failed with
-EOPNOTSUPP and blk_queue_ordered() is replaced with simpler
blk_queue_flush().
blk_queue_flush() takes combinations of REQ_FLUSH and FUA. If a
device has write cache and can flush it, it should set REQ_FLUSH. If
the device can handle FUA writes, it should also set REQ_FUA.
All blk_queue_ordered() users are converted.
* ORDERED_DRAIN is mapped to 0 which is the default value.
* ORDERED_DRAIN_FLUSH is mapped to REQ_FLUSH.
* ORDERED_DRAIN_FLUSH_FUA is mapped to REQ_FLUSH | REQ_FUA.

The DASD reserve and release ioctls use the preallocated memory pool
of the respective device to build their CCW requests. However, when
the device is busy, the pool may already be empty and the ioctl fails.
Usually this can be recovered by calling the ioctl again, but in
a situation in which we need to issue an unconditional reserve to
make a device operational again, this would be not recoverable.
To avoid a failure due to lack of memory, DASD device driver will
preallocate enough memory for a single reserve/release request, which
can be used if normal allocation fails.

The open and release block_device_operations are currently
called with the BKL held. In order to change that, we must
first make sure that all drivers that currently rely
on this have no regressions.
This blindly pushes the BKL into all .open and .release
operations for all block drivers to prepare for the
next step. The drivers can subsequently replace the BKL
with their own locks or remove it completely when it can
be shown that it is not needed.
The functions blkdev_get and blkdev_put are the only
remaining users of the big kernel lock in the block
layer, besides a few uses in the ioctl code, none
of which need to serialize with blkdev_{get,put}.
Most of these two functions is also under the protection
of bdev->bd_mutex, including the actual calls to
->open and ->release, and the common code does not
access any global data structures that need the BKL.
