All modern drivers can support extra partitions using the extended
dev_t. In fact except for the ioctl method drivers never even see
partitions in normal operation.
So remove the GENHD_FL_EXT_DEVT and allow extra partitions for all
block devices that do support partitions, and require those that
do not support partitions to explicit disallow them using
GENHD_FL_NO_PART.

It is a bit confusing that there is BLKDEV_MAX_RQ and MAX_SCHED_RQ, as
the name BLKDEV_MAX_RQ would imply the max requests always, which it is
not.
Rename to BLKDEV_MAX_RQ to BLKDEV_DEFAULT_RQ, matching its usage - that being
the default number of requests assigned when allocating a request queue.

Currently rbd_quiesce_lock() holds lock_rwsem for read while blocking
on releasing_wait completion. On the I/O completion side, each image
request also needs to take lock_rwsem for read. Because rw_semaphore
implementation doesn't allow new readers after a writer has indicated
interest in the lock, this can result in a deadlock if something that
needs to take lock_rwsem for write gets involved. For example:
1. watch error occurs
2. rbd_watch_errcb() takes lock_rwsem for write, clears owner_cid and
releases lock_rwsem
3. after reestablishing the watch, rbd_reregister_watch() takes
lock_rwsem for write and calls rbd_reacquire_lock()
4. rbd_quiesce_lock() downgrades lock_rwsem to for read and blocks on
releasing_wait until running_list becomes empty
5. another watch error occurs
6. rbd_watch_errcb() blocks trying to take lock_rwsem for write
7. no in-flight image request can complete and delete itself from
running_list because lock_rwsem won't be granted anymore
A similar scenario can occur with "lock has been acquired" and "lock
has been released" notification handers which also take lock_rwsem for
write to update owner_cid.
We don't actually get anything useful from sitting on lock_rwsem in
rbd_quiesce_lock() -- owner_cid updates certainly don't need to be
synchronized with. In fact the whole owner_cid tracking logic could
probably be removed from the kernel client because we don't support
proxied maintenance operations.

Skipping the "lock has been released" notification if the lock owner
is not what we expect based on owner_cid can lead to I/O hangs.
One example is our own notifications: because owner_cid is cleared
in rbd_unlock(), when we get our own notification it is processed as
unexpected/duplicate and maybe_kick_acquire() isn't called. If a peer
that requested the lock then doesn't go through with acquiring it,
I/O requests that came in while the lock was being quiesced would
be stalled until another I/O request is submitted and kicks acquire
from rbd_img_exclusive_lock().
This makes the comment in rbd_release_lock() actually true: prior to
this change the canceled work was being requeued in response to the
"lock has been acquired" notification from rbd_handle_acquired_lock().

The BDI_CAP_STABLE_WRITES is one of the few bits of information in the
backing_dev_info shared between the block drivers and the writeback code.
To help untangling the dependency replace it with a queue flag and a
superblock flag derived from it. This also helps with the case of e.g.
a file system requiring stable writes due to its own checksumming, but
not forcing it on other users of the block device like the swap code.
One downside is that we an't support the stable_pages_required bdi
attribute in sysfs anymore. It is replaced with a queue attribute which
also is writable for easier testing.

It turns out that currently we rely only on sysfs attribute
permissions:

This means that images can be mapped and unmapped (i.e. block devices
can be created and deleted) by a UID 0 process even after it drops all
privileges or by any process with CAP_DAC_OVERRIDE in its user namespace
as long as UID 0 is mapped into that user namespace.
Be consistent with other virtual block devices (loop, nbd, dm, md, etc)
and require CAP_SYS_ADMIN in the initial user namespace for mapping and
unmapping, and also for dumping the configuration string and refreshing
the image header.

revalidate_disk is a relative awkward helper for driver use, as it first
calls an optional driver method and then updates the block device size,
while most callers either don't need the method call at all, or want to
keep state between the caller and the called method.
Add a revalidate_disk_size helper that just performs the update of the
block device size from the gendisk one, and switch all drivers that do
not implement ->revalidate_disk to use the new helper instead of
revalidate_disk()

Using uninitialized_var() is dangerous as it papers over real bugs[1]
(or can in the future), and suppresses unrelated compiler warnings
(e.g. "unused variable"). If the compiler thinks it is uninitialized,
either simply initialize the variable or make compiler changes.
In preparation for removing[2] the[3] macro[4], remove all remaining
needless uses with the following script:

osd_req_flags is overly general and doesn't suit its only user
(read_from_replica option) well:
- applying osd_req_flags in account_request() affects all OSD
requests, including linger (i.e. watch and notify). However,
linger requests should always go to the primary even though
some of them are reads (e.g. notify has side effects but it
is a read because it doesn't result in mutation on the OSDs).
- calls to class methods that are reads are allowed to go to
the replica, but most such calls issued for "rbd map" and/or
exclusive lock transitions are requested to be resent to the
primary via EAGAIN, doubling the latency.
Get rid of global osd_req_flags and set read_from_replica flag
only on specific OSD requests instead.

rbd_dev_unprobe() is supposed to undo most of rbd_dev_image_probe(),
including rbd_dev_header_info(), which means that rbd_dev_header_info()
isn't supposed to be called after rbd_dev_unprobe().
However, rbd_dev_image_release() calls rbd_dev_unprobe() before
rbd_unregister_watch(). This is racy because a header update notify
can sneak in:

rbd_unregister_watch() flushes notifies and therefore cannot be called
under header_rwsem because a header update notify takes header_rwsem to
synchronize with "rbd map". If mapping an image fails after the watch
is established and a header update notify sneaks in, we deadlock when
erroring out from rbd_dev_image_probe().
Move watch registration and unregistration out of the critical section.
The only reason they were put there was to make header_rwsem management
slightly more obvious.

Avoid making allocations for !IMG_REQ_CHILD image requests. Only
IMG_REQ_CHILD image requests need to be freed now.
Move the initial request checks to rbd_queue_rq(). Unfortunately we
can't fill the image request and kick the state machine directly from
rbd_queue_rq() because ->queue_rq() isn't allowed to block.

Currently header_rwsem is acquired twice: once in rbd_dev_parent_get()
when the image request is being created and then in rbd_queue_workfn()
to capture mapping_size and snapc. Introduce rbd_img_capture_header()
and move image request allocation so that header_rwsem can be acquired
just once.

rbd_img_request_destroy() directly and drop the kref.

IMG_REQ_LAYERED is set in rbd_img_request_create(), and tested and
cleared in rbd_img_request_destroy() when the image request is about to
be destroyed. The barriers are unnecessary.

Pull vfs file system parameter updates from Al Viro:
"Saner fs_parser.c guts and data structures. The system-wide registry
of syntax types (string/enum/int32/oct32/.../etc.) is gone and so is
the horror switch() in fs_parse() that would have to grow another case
every time something got added to that system-wide registry.
New syntax types can be added by filesystems easily now, and their
namespace is that of functions - not of system-wide enum members. IOW,
they can be shared or kept private and if some turn out to be widely
useful, we can make them common library helpers, etc., without having
to do anything whatsoever to fs_parse() itself.
And we already get that kind of requests - the thing that finally
pushed me into doing that was "oh, and let's add one for timeouts -
things like 15s or 2h". If some filesystem really wants that, let them
do it. Without somebody having to play gatekeeper for the variants
blessed by direct support in fs_parse(), TYVM.
Quite a bit of boilerplate is gone. And IMO the data structures make a
lot more sense now. -200LoC, while we are at it"

fs_parse() analogue taking p_log instead of fs_context.
fs_parse() turned into a wrapper, callers in ceph_common and rbd
switched to __fs_parse().
As the result, fs_parse() never gets NULL fs_context and neither
do fs_context-based logging primitives

As it is, vfs_parse_fs_string() makes "foo" and "foo=" indistinguishable;
both get fs_value_is_string for ->type and NULL for ->string. To make
it even more unpleasant, that combination is impossible to produce with
fsconfig().
Much saner rules would be
"foo" => fs_value_is_flag, NULL
"foo=" => fs_value_is_string, ""
"foo=bar" => fs_value_is_string, "bar"
All cases are distinguishable, all results are expressable by fsconfig(),
->has_value checks are much simpler that way (to the point of the field
being useless) and quite a few regressions go away (gfs2 has no business
accepting -o nodebug=, for example).

The rbd driver already provides additional information in sysfs
under /sys/bus/rbd, so we should set the 'device' link in the block
device to reference this information.

gcc -O3 warns about a dummy variable that is passed
down into rbd_img_fill_nodata without being initialized:

Convert the ceph filesystem to the new internal mount API as the old
one will be obsoleted and removed. This allows greater flexibility in
communication of mount parameters between userspace, the VFS and the
filesystem.

For a read-only mapping, ask for a set of features that make the image
only unwritable rather than both unreadable and unwritable by a client
that doesn't understand them. As of today, the difference between them
for krbd is journaling (JOURNALING) and live migration (MIGRATING).
get_features method supports read_only parameter since hammer, ceph.git
commit 6176ec5fde2a ("librbd: differentiate between R/O vs R/W RBD
features").

Since infernalis, ceph.git commit 281f87f9ee52 ("cls_rbd: get_features
on snapshots returns HEAD image features"), querying and checking that
is pointless. Userspace support for manipulating image features after
image creation came also in infernalis, so a snapshot with a different
set of features wasn't ever possible.

RBD_DEV_FLAG_EXISTS check in rbd_queue_workfn() is racy and leads to
inconsistent behaviour. If the object (or its snapshot) isn't there,
the OSD returns ENOENT. A read submitted before the snapshot removal
notification is processed would be zero-filled and ended with status
OK, while future reads would be failed with IOERR. It also doesn't
handle a case when an image that is mapped read-only is removed.
On top of this, because watch is no longer established for read-only
mappings, we no longer get notifications, so rbd_exists_validate() is
effectively dead code. While failing requests rather than returning
zeros is a good thing, RBD_DEV_FLAG_EXISTS is not it.

A read-only mapping should be usable with read-only OSD caps, so
neither the header lock nor the object map lock can be acquired.
Unfortunately, this means that images mapped read-only lose the
advantage of the object map.
Snapshots, however, can take advantage of the object map without
any exclusionary locks, so if the object map is desired, snapshot
the image and map the snapshot instead of the image.
