SA will query and cache class port info as part of
its initialization. SA will also invalidate and
refresh the cache based on specific events. Callers such
as IPoIB and CM can query the SA to get the classportinfo
information. Apart from making the caller code much simpler,
this change puts the onus on the SA to query and maintain
classportinfo much like how it maitains the address handle to the SM.

Before calling ipoib_stop, rtnl_lock should be taken, then
the flow clears the IPOIB_FLAG_ADMIN_UP and IPOIB_FLAG_OPER_UP
flags, and waits for mcast completion if IPOIB_MCAST_FLAG_BUSY

On the other hand, the flow of multicast join task initializes
a mcast completion, sets the IPOIB_MCAST_FLAG_BUSY and calls
ipoib_mcast_join. If IPOIB_FLAG_OPER_UP flag is not set, this
call returns EINVAL without setting the mcast completion and

Update the broadcast address in the priv->broadcast object when the
Pkey value changes in index 0, otherwise the multicast GID value will
keep the previous value of the PKey, and will not be updated.
This leads to interface state down because the interface will keep the
old PKey value.
For example, in SR-IOV environment, if the PF changes the value of PKey
index 0 for one of the VFs, then the VF receives PKey change event that
triggers heavy flush. This flush calls update_parent_pkey that update the
broadcast object and its relevant members. If in this case the multicast
GID will not be updated, the interface state will be down.

This patch is preparing the netdev part at the IPoIB driver to be able
to use the ipoib_options.
It deals with the two flows from the .ndo: ipoib_open and ipoib_stop.
The code is rearranged as follows:
* All operations which deal with the hardware resources, (for example
change QP state, post-receive etc.) are performed in one place.
* All operations that are control oriented (like restart multicast task,
start the reap_ah etc.) are performed in separate place.
The functions that deal with the hardware resources now located at
__ipoib_ib_dev_open for the ipoib_open flow and __ipoib_ib_dev_stop
for ipoib_stop.

This patch prepares init and teardown flows so we can call them
through ipoib_options function pointers.
It arranges that area of code as the following:
* All operations which deal with the resource allocation/deletion
are performed in one place.
* All operations that are control oriented, meaning that they are not
connected to a specific hardware, are performed in a separate place.
The operations for allocation of hardware resources are now in the
function ipoib_dev_init_default, and the deletion of all the resources
are in ipoib_dev_uninit_default
The only exception is the creation of the PD object,
which is used both for resource allocation (create QP etc.)
and for control flows like creating AH.
It also does:
* Move creation of rx_ring and tx_ring to be in the resources
allocation area.
* Move the function ipoib_ib_dev_open that does the open device
to the control area instead of the dev_init which creates resources.

When sending packet to destination that was not resolved yet
via path query, the driver keeps the skb and tries to re-send it
again when the path is resolved.
But when re-sending via dev_queue_xmit the kernel doesn't call
to dev_hard_header, so IPoIB needs to keep 20 bytes in the skb
and to put the destination address inside them.
In that way the dev_start_xmit will have the correct destination,
and the driver won't take the destination from the skb->data, while
nothing exists there, which causes to packet be be dropped.
The test flow is:
1. Run the SM on remote node,
2. Restart the driver.
4. Ping some destination,
3. Observe that first ICMP request will be dropped.

Pull rdma updates from Doug Ledford:
"This is the complete update for the rdma stack for this release cycle.
Most of it is typical driver and core updates, but there is the
entirely new VMWare pvrdma driver. You may have noticed that there
were changes in DaveM's pull request to the bnxt Ethernet driver to
support a RoCE RDMA driver. The bnxt_re driver was tentatively set to
be pulled in this release cycle, but it simply wasn't ready in time
and was dropped (a few review comments still to address, and some
multi-arch build issues like prefetch() not working across all
arches).
Summary:
- shared mlx5 updates with net stack (will drop out on merge if
Dave's tree has already been merged)
- driver updates: cxgb4, hfi1, hns-roce, i40iw, mlx4, mlx5, qedr, rxe
- debug cleanups
- new connection rejection helpers
- SRP updates
- various misc fixes
- new paravirt driver from vmware"

After the commit 9207f9d45b ("net: preserve IP control block
during GSO segmentation"), the GSO CB and the IPoIB CB conflict.
That destroy the IPoIB address information cached there,
causing a severe performance regression, as better described here:
http://marc.info/?l=linux-kernel&m=146787279825501&w=2
This change moves the data cached by the IPoIB driver from the
skb control lock into the IPoIB hard header, as done before
the commit 936d7de3d7 ("IPoIB: Stop lying about hard_header_len
and use skb->cb to stash LL addresses").
In order to avoid GRO issue, on packet reception, the IPoIB driver
stash into the skb a dummy pseudo header, so that the received
packets have actually a hard header matching the declared length.
To avoid changing the connected mode maximum mtu, the allocated
head buffer size is increased by the pseudo header length.
After this commit, IPoIB performances are back to pre-regression
value.

alloc_ordered_workqueue() with WQ_MEM_RECLAIM set, replaces
deprecated create_singlethread_workqueue(). This is the identity

The workqueue "wq" queues mulitple work items viz &priv->restart_task,
&priv->cm.rx_reap_task, &priv->cm.skb_task, &priv->neigh_reap_task,
&priv->ah_reap_task, &priv->mcast_task and &priv->carrier_on_task.
The work items require strict execution ordering.

This fix solves a race between light flush and on the fly joins.
Light flush doesn't set the device to down and unset IPOIB_OPER_UP
flag, this means that if while flushing we have a MC join in progress
and the QP was attached to BC MGID we can have a mismatches when
re-attaching a QP to the BC MGID.
The light flush would set the broadcast group to NULL causing an on
the fly join to rejoin and reattach to the BC MCG as well as adding
the BC MGID to the multicast list. The flush process would later on
remove the BC MGID and detach it from the QP. On the next flush
the BC MGID is present in the multicast list but not found when trying
to detach it because of the previous double attach and single detach.

When a new CM connection is being requested, ipoib driver copies data
from the path pointer in the CM/tx object, the path object might be
invalid at the point and memory corruption will happened later when now
the CM driver will try using that data.
The next scenario demonstrates it:
neigh_add_path --> ipoib_cm_create_tx -->
queue_work (pointer to path is in the cm/tx struct)
#while the work is still in the queue,
#the port goes down and causes the ipoib_flush_paths:
ipoib_flush_paths --> path_free --> kfree(path)
#at this point the work scheduled starts.
ipoib_cm_tx_start --> copy from the (invalid)path pointer:
(memcpy(&pathrec, &p->path->pathrec, sizeof pathrec);)
-> memory corruption.
To fix that the driver now starts the CM/tx connection only if that
specific path exists in the general paths database.
This check is protected with the relevant locks, and uses the gid from
the neigh member in the CM/tx object which is valid according to the ref
count that was taken by the CM/tx.

ipoib_neigh_get unconditionally updates the "alive" variable member on
any packet send. This prevents the neighbor garbage collection from
cleaning out a dead neighbor entry if we are still queueing packets
for it. If the queue for this neighbor is full, then don't update the
alive timestamp. That way the neighbor can time out even if packets
are still being queued as long as none of them are being sent.

Pull more rdma updates from Doug Ledford:
"This is the second group of code for the 4.7 merge window. It looks
large, but only in one sense. I'll get to that in a minute. The list
of changes here breaks down as follows:
- Dynamic counter infrastructure in the IB drivers
This is a sysfs based code to allow free form access to the
hardware counters RDMA devices might support so drivers don't need
to code this up repeatedly themselves
- SendOnlyFullMember multicast support
- IB router support
- A couple misc fixes
- The big item on the list: hfi1 driver updates, plus moving the hfi1
driver out of staging
There was a group of 15 patches in the hfi1 list that I thought I had
in the first pull request but they weren't. So that added to the
length of the hfi1 section here.
As far as these go, everything but the hfi1 is pretty straight
forward.
The hfi1 is, if you recall, the driver that Al had complaints about
how it used the write/writev interfaces in an overloaded fashion. The
write portion of their interface behaved like the write handler in the
IB stack proper and did bi-directional communications. The writev
interface, on the other hand, only accepts SDMA request structures.
The completions for those structures are sent back via an entirely
different event mechanism.
With the security patch, we put security checks on the write
interface, however, we also knew they would be going away soon. Now,
we've converted the write handler in the hfi1 driver to use ioctls
from the IB reserved magic area for its bidirectional communications.
With that change, Intel has addressed all of the items originally on
their TODO when they went into staging (as well as many items added to
the list later).
As such, I moved them out, and since they were the last item in the
staging/rdma directory, and I don't have immediate plans to use the
staging area again, I removed the staging/rdma area.
Because of the move out of staging, as well as a series of 5 patches
in the hfi1 driver that removed code people thought should be done in
a different way and was optional to begin with (a snoop debug
interface, an eeprom driver for an eeprom connected directory to their
hfi1 chip and not via an i2c bus, and a few other things like that),
the line count, especially the removal count, is high"

In IB networks, and specifically in IPoIB/rdmacm traffic, the device
address of an IPoIB interface is used as a means to exchange information
between nodes needed for communication.
Currently an IPoIB interface will always be created with a device
address based on its node GUID without a way to change that.
This change adds the ability to set the device address of an IPoIB
interface by value. We use the set mac address ndo to do that.
The flow should be broken down to two:
1) The GID value is already in the GID table,
in this case the interface will be able to set carrier up.
2) The GID value is not yet in the GID table,
in this case the interface won't try to join the multicast group
and will wait (listen on GID_CHANGE event) until the GID is inserted.
In order to track those changes, we add a new flag:
* IPOIB_FLAG_DEV_ADDR_SET.
When set, it means the dev_addr is a based on a value in the gid
table. this bit will be cleared upon a dev_addr change triggered
by the user and set after validation.
Per IB spec the port GUID can't change if the module is loaded.
port GUID is the basis for GID at index 0 which is the basis for
the default device address of a ipoib interface.
The issue is that there are devices that don't follow the spec,
they change the port GUID while HCA is powered on, so in order
not to break userspace applications. We need to check if the
user wanted to control the device address and we assume that
if he sets the device address back to be based on GID index 0,
he no longer wishs to control it.
In order to track this, we add an additional flag:
* IPOIB_FLAG_DEV_ADDR_CTRL
When setting the device address, there is no validation of the upper
twelve bytes of the device address (flags, qpn, subnet prefix) as those
bytes are not under the control of the user.

- in struct net_device (legacy)

Instead of open-coding dev->trans_start usage to obtain the current
trans_start value, use dev_trans_start() instead.
This is not exactly the same, as dev_trans_start also considers
the trans_start values of the netdev queues owned by the device
and provides the most recent one.
For legacy devices this doesn't matter as dev_trans_start can cope
with netdev trans_start values of 0 (they are ignored).
This is a prerequisite to eventual removal of dev->trans_start.

IPoIB converts skb-fragments to sge adding 1 extra sge when SG is enabled.
Current codepath assumes that the max number of sge a device support
is at least MAX_SKB_FRAGS+1, there is no interaction with upper layers
to limit number of fragments in an skb if a device suports fewer
sges. The assumptions also lead to requesting a fixed number of sge
when IPoIB creates queue-pairs with SG enabled.
A fallback/slowpath is implemented using skb_linearize to

This adds an abstraction that allows ULPs to simply pass a completion
object and completion callback with each submitted WR and let the RDMA
core handle the nitty gritty details of how to handle completion
interrupts and poll the CQ.
In detail there is a new ib_cqe structure which just contains the
completion callback, and which can be used to get at the containing
object using container_of. It is pointed to by the WR and WC as an
alternative to the wr_id field, similar to how many ULPs already use
the field to store a pointer using casts.
A driver using the new completion callbacks allocates it's CQs using
the new ib_create_cq API, which in addition to the number of CQEs and
the completion vectors also takes a mode on how we poll for CQEs.
Three modes are available: direct for drivers that never take CQ
interrupts and just poll for them, softirq to poll from softirq context
using the to be renamed blk-iopoll infrastructure which takes care of
rearming and budgeting, or a workqueue for consumer who want to be
called from user context.
Thanks a lot to Sagi Grimberg who helped reviewing the API, wrote

Since IPoIB should, as much as possible, emulate how multicast
sends work on Ethernet for regular TCP/IP apps, there should be
no requirement to subscribe to a multicast group before your
sends are properly sent. However, due to the difference in how
multicast is handled on InfiniBand, we must join the appropriate
multicast group before we can send to it. Previously we tried
not to trigger the auto-create feature of the subnet manager when
doing this because we didn't have tracking of these sendonly
groups and the auto-creation might never get undone. The previous
patch added timing to these sendonly joins and allows us to
leave them after a reasonable idle expiration time. So supply

An ib_client callback that is called with the lists_rwsem locked only for
read is protected from changes to the IB client lists, but not from
ib_unregister_device() freeing its client data. This is because
ib_unregister_device() will remove the device from the device list with
lists_rwsem locked for write, but perform the rest of the cleanup,
including the call to remove() without that lock.
Mark client data that is undergoing de-registration with a new going_down
flag in the client data context. Lock the client data list with lists_rwsem
for write in addition to using the spinlock, so that functions calling the
callback would be able to lock only lists_rwsem for read and let callbacks
sleep.
Since ib_unregister_client() now marks the client data context, no need for
remove() to search the context again, so pass the client data directly to
remove() callbacks.

By default, IPoIB-CM driver uses 64k MTU. Larger MTU gives better
performance.
This MTU plus overhead puts the memory allocation for IP based packets at
32 4k pages (order 5), which have to be contiguous.
When the system memory under pressure, it was observed that allocating 128k
contiguous physical memory is difficult and causes serious errors (such as
system becomes unusable).
This enhancement resolve the issue by removing the physically contiguous
memory requirement using Scatter/Gather feature that exists in Linux stack.
With this fix Scatter-Gather will be supported also in connected mode.
This change reverts some of the change made in commit e112373fd6

We needed the mcast_mutex when we had to prevent the join completion
callback from having the value it stored in mcast->mc overwritten
by a delayed return from ib_sa_join_multicast. By storing the return
of ib_sa_join_multicast in an intermediate variable, we prevent a
delayed return from ib_sa_join_multicast overwriting the valid
contents of mcast->mc, and we no longer need a mutex to force the
join callback to run after the return of ib_sa_join_multicast. This
allows us to do away with the mutex entirely and protect our critical
sections with a just a spinlock instead. This is highly desirable
as there were some places where we couldn't use a mutex because the
code was not allowed to sleep, and so we were currently using a mix
of mutex and spinlock to protect what we needed to protect. Now we
only have a spin lock and the locking complexity is greatly reduced.

Allow the ipoib layer to attempt to join all outstanding multicast
groups at once. The ib_sa layer will serialize multiple attempts to
join the same group, but will process attempts to join different groups
in parallel. Take advantage of that.
In order to make this happen, change the mcast_join_thread to loop
through all needed joins, sending a join request for each one that we
still need to join. There are a few special cases we handle though:
1) Don't attempt to join anything but the broadcast group until the join
of the broadcast group has succeeded.
2) No longer restart the join task at the end of completion handling.
If we completed successfully, we are done. The join task now needs kicked
either by mcast_send or mcast_restart_task or mcast_start_thread, but
should not need started anytime else except when scheduling a backoff
attempt to rejoin.
3) No longer use separate join/completion routines for regular and
sendonly joins, pass them all through the same routine and just do the
right thing based on the SENDONLY join flag.
4) Only try to join a SENDONLY join twice, then drop the packets and
quit trying. We leave the mcast group in the list so that if we get a
new packet, all that we have to do is queue up the packet and restart
the join task and it will automatically try to join twice and then
either send or flush the queue again.

Commit a9c8ba5884 ("IPoIB: Fix usage of uninitialized multicast
objects") added a new flag MCAST_JOIN_STARTED, but was not very strict
in how it was used. We didn't always initialize the completion struct
before we set the flag, and we didn't always call complete on the
completion struct from all paths that complete it. And when we did
complete it, sometimes we continued to touch the mcast entry after
the completion, opening us up to possible use after free issues.
This made it less than totally effective, and certainly made its use
confusing. And in the flush function we would use the presence of this
flag to signal that we should wait on the completion struct, but we never
cleared this flag, ever.
In order to make things clearer and aid in resolving the rtnl deadlock
bug I've been chasing, I cleaned this up a bit.
1) Remove the MCAST_JOIN_STARTED flag entirely
2) Change MCAST_FLAG_BUSY so it now only means a join is in-flight
3) Test mcast->mc directly to see if we have completed
ib_sa_join_multicast (using IS_ERR_OR_NULL)
4) Make sure that before setting MCAST_FLAG_BUSY we always initialize
the mcast->done completion struct
5) Make sure that before calling complete(&mcast->done), we always clear
the MCAST_FLAG_BUSY bit
6) Take the mcast_mutex before we call ib_sa_multicast_join and also
take the mutex in our join callback. This forces
ib_sa_multicast_join to return and set mcast->mc before we process
the callback. This way, our callback can safely clear mcast->mc
if there is an error on the join and we will do the right thing as
a result in mcast_dev_flush.
7) Because we need the mutex to synchronize mcast->mc, we can no
longer call mcast_sendonly_join directly from mcast_send and
instead must add sendonly join processing to the mcast_join_task
8) Make MCAST_RUN mean that we have a working mcast subsystem, not that
we have a running task. We know when we need to reschedule our
join task thread and don't need a flag to tell us.
9) Add a helper for rescheduling the join task thread
A number of different races are resolved with these changes. These
races existed with the old MCAST_FLAG_BUSY usage, the
MCAST_JOIN_STARTED flag was an attempt to address them, and while it
helped, a determined effort could still trip things up.
One race looks something like this:
Thread 1 Thread 2
ib_sa_join_multicast (as part of running restart mcast task)
alloc member
call callback
ifconfig ib0 down
wait_for_completion
callback call completes
wait_for_completion in
mcast_dev_flush completes
mcast->mc is PTR_ERR_OR_NULL
so we skip ib_sa_leave_multicast
return from callback
return from ib_sa_join_multicast
set mcast->mc = return from ib_sa_multicast
We now have a permanently unbalanced join/leave issue that trips up the
refcounting in core/multicast.c
Another like this:
Thread 1 Thread 2 Thread 3
ib_sa_multicast_join
ifconfig ib0 down
priv->broadcast = NULL
join_complete
wait_for_completion
mcast->mc is not yet set, so don't clear
return from ib_sa_join_multicast and set mcast->mc
complete
return -EAGAIN (making mcast->mc invalid)
call ib_sa_multicast_leave
on invalid mcast->mc, hang
forever
By holding the mutex around ib_sa_multicast_join and taking the mutex
early in the callback, we force mcast->mc to be valid at the time we
run the callback. This allows us to clear mcast->mc if there is an
error and the join is going to fail. We do this before we complete
the mcast. In this way, mcast_dev_flush always sees consistent state
in regards to mcast->mc membership at the time that the
wait_for_completion() returns.

Various places in the IPoIB code had a deadlock related to flushing

flushing the device specific workqueues and we can do so unilaterally.
Signed-off-by: Doug Ledford <dledford@redhat.com>
During my recent work on the rtnl lock deadlock in the IPoIB driver, I
saw that even once I fixed the apparent races for a single device, as
soon as that device had any children, new races popped up. It turns
out that this is because no matter how well we protect against races
on a single device, the fact that all devices use the same workqueue,
and flush_workqueue() flushes *everything* from that workqueue means
that we would also have to prevent all races between different devices
(for instance, ipoib_mcast_restart_task on interface ib0 can race with
ipoib_mcast_flush_dev on interface ib0.8002, resulting in a deadlock on
the rtnl_lock).
There are several possible solutions to this problem:
Make carrier_on_task and mcast_restart_task try to take the rtnl for
some set period of time and if they fail, then bail. This runs the
real risk of dropping work on the floor, which can end up being its
own separate kind of deadlock.
Set some global flag in the driver that says some device is in the
middle of going down, letting all tasks know to bail. Again, this can
drop work on the floor.
Or the method this patch attempts to use, which is when we bring an
interface up, create a workqueue specifically for that interface, so
that when we take it back down, we are flushing only those tasks
associated with our interface. In addition, keep the global
workqueue, but now limit it to only flush tasks. In this way, the
flush tasks can always flush the device specific work queues without
having deadlock issues.

We blindly assume that we can just take the rtnl lock and that will
prevent races with downing this interface. Unfortunately, that's not
the case. In ipoib_mcast_stop_thread() we will call flush_workqueue()
in an attempt to clear out all remaining instances of ipoib_join_task.
But, since this task is put on the same workqueue as the join task,
the flush_workqueue waits on this thread too. But this thread is
deadlocked on the rtnl lock. The better thing here is to use trylock
and loop on that until we either get the lock or we see that
FLAG_OPER_UP has been cleared, in which case we don't need to do
anything anyway and we just return.
While investigating which flag should be used, FLAG_ADMIN_UP or
FLAG_OPER_UP, it was determined that FLAG_OPER_UP was the more
appropriate flag to use. However, there was a mix of these two flags in
use in the existing code. So while we check for that flag here as part
of this race fix, also cleanup the two places that had used the less
appropriate flag for their tests.

The ipoib_mcast_flush_dev routine is called with the rtnl_lock held and
needs to keep it held. It also needs to call flush_workqueue() to flush
out any outstanding work. In the past, we've had to try and make sure
that we didn't flush out any outstanding join completions because they
also wanted to grab rtnl_lock() and that would deadlock. It turns out
that the only thing in the join completion handler that needs this lock
can be safely moved to our carrier_on_task, thereby reducing the
potential for the join completion code and the flush code to deadlock
against each other.

Create a an ipoib_flush_ah and ipoib_stop_ah routines to use at
appropriate times to flush out all remaining ah entries before we shut
the device down.
Because neighbors and mcast entries can each have a reference on any
given ah, we must make sure to free all of those first before our ah
will actually have a 0 refcount and be able to be reaped.
This factoring is needed in preparation for having per-device work
queues. The original per-device workqueue code resulted in the following
error message:
<ibdev>: ib_dealloc_pd failed
That error was tracked down to this issue. With the changes to which
workqueues were flushed when, there were no flushes of the per device
workqueue after the last ah's were freed, resulting in an attempt to
dealloc the pd with outstanding resources still allocated. This code
puts the explicit flushes in the needed places to avoid that problem.

the multicast_list and while adding entries to the remove_list, but
they also both drop their locks prior to doing the actual removes.
The mcast_dev_flush routine is run entirely under the rtnl lock and so
has at least some locking. The actual race condition is like this:
Thread 1 Thread 2
ifconfig ib0 up
start multicast join for broadcast
multicast join completes for broadcast
start to add more multicast joins
call mcast_restart_task to add new entries
ifconfig ib0 down
mcast_dev_flush
mcast_leave(mcast A)
mcast_leave(mcast A)
As mcast_leave calls ib_sa_multicast_leave, and as member in
core/multicast.c is ref counted, we run into an unbalanced refcount
issue. To avoid stomping on each others removes, take the rtnl lock
specifically when we are deleting the entries from the remove list.

This made it less
than totally effective, and certainly made its use confusing. And in
the flush function we would use the presence of this flag to signal
that we should wait on the completion struct, but we never cleared
this flag, ever. This is further muddied by the fact that we overload
the MCAST_FLAG_BUSY flag to mean two different things: we have a join
in flight, and we have succeeded in getting an ib_sa_join_multicast.
In order to make things clearer and aid in resolving the rtnl deadlock
bug I've been chasing, I cleaned this up a bit.
1) Remove the MCAST_JOIN_STARTED flag entirely
2) Un-overload MCAST_FLAG_BUSY so it now only means a join is in-flight
3) Test on mcast->mc directly to see if we have completed
ib_sa_join_multicast (using IS_ERR_OR_NULL)
4) Make sure that before setting MCAST_FLAG_BUSY we always initialize
the mcast->done completion struct
A number of different races are resolved with these changes. These
races existed with the old MCAST_FLAG_BUSY usage, the
MCAST_JOIN_STARTED flag was an attempt to address them, and while it
helped, a determined effort could still trip things up.
One race looks something like this:
Thread 1 Thread 2
ib_sa_join_multicast (as part of running restart mcast task)
alloc member
call callback
ifconfig ib0 down
wait_for_completion
callback call completes
wait_for_completion in
mcast_dev_flush completes
mcast->mc is PTR_ERR_OR_NULL
so we skip ib_sa_leave_multicast
return from callback
return from ib_sa_join_multicast
set mcast->mc = return from ib_sa_multicast
We now have a permanently unbalanced join/leave issue that trips up the
refcounting in core/multicast.c
Another like this:
Thread 1 Thread 2 Thread 3
ib_sa_multicast_join
ifconfig ib0 down
priv->broadcast = NULL
join_complete
wait_for_completion
mcast->mc is not yet set, so don't clear
return from ib_sa_join_multicast and set mcast->mc
complete
return -EAGAIN (making mcast->mc invalid)
call ib_sa_multicast_leave
on invalid mcast->mc, hang
forever
By holding the mutex around ib_sa_multicast_join and taking the mutex
early in the callback, we force mcast->mc to be valid at the time we
run the callback. This allows us to clear mcast->mc if there is an
error and the join is going to fail. We do this before we complete
the mcast. In this way, mcast_dev_flush always sees consistent state
in regards to mcast->mc membership at the time that the
wait_for_completion() returns.

The better thing here is to use trylock
and loop on that until we either get the lock or we see that
FLAG_ADMIN_UP has been cleared, in which case we don't need to do
anything anyway and we just return.

Testing xmit_more support with netperf and connected UDP sockets,
I found strange dst refcount false sharing.
Current handling of IFF_XMIT_DST_RELEASE is not optimal.
Dropping dst in validate_xmit_skb() is certainly too late in case
packet was queued by cpu X but dequeued by cpu Y
The logical point to take care of drop/force is in __dev_queue_xmit()
before even taking qdisc lock.
As Julian Anastasov pointed out, need for skb_dst() might come from some
packet schedulers or classifiers.
This patch adds new helper to cleanly express needs of various drivers
or qdiscs/classifiers.
Drivers that need skb_dst() in their ndo_start_xmit() should call
following helper in their setup instead of the prior :
dev->priv_flags &= ~IFF_XMIT_DST_RELEASE;
->
netif_keep_dst(dev);
Instead of using a single bit, we use two bits, one being
eventually rebuilt in bonding/team drivers.
The other one, is permanent and blocks IFF_XMIT_DST_RELEASE being
rebuilt in bonding/team. Eventually, we could add something
smarter later.

Currently, the parent interface keeps sending broadcast group join
requests even if p_key index 0 is invalid, which is possible/common in
virtualized environments where a VF has been probed to VM but the
actual P_key configuration has not yet been assigned by the management
software. This creates unnecessary noise on the fabric and in the
kernel logs:
ib0: multicast join failed for ff12:401b:8000:0000:0000:0000:ffff:ffff, status -22
The original code run the multicast task regardless of the actual
P_key value, which can be avoided. The fix is to re-init resources and
bring interface up only if P_key index 0 is valid either when starting
up or on PKEY_CHANGE event.

This addresses a problem where NFS client writes over IPoIB connected
mode may deadlock on memory allocation/writeback.
The problem is not directly memory reclamation. There is an indirect
dependency between network filesystems writing back pages and
ipoib_cm_tx_init() due to how a kworker is used. Page reclaim cannot
make forward progress until ipoib_cm_tx_init() succeeds and it is
stuck in page reclaim itself waiting for network transmission.
Ordinarily this situation may be avoided by having the caller use
GFP_NOFS but ipoib_cm_tx_init() does not have that information.
To address this, take a general approach and add a new QP creation
flag that tells the low-level hardware driver to use GFP_NOIO for the
memory allocations related to the new QP.
Use the new flag in the ipoib connected mode path, and if the driver
doesn't support it, re-issue the QP creation without the flag.

Then after connecting and disconnecting the link, which should result
in exactly the same state as before, it shows:

7: mlx4_ib1.8003@mlx4_ib1: <NO-CARRIER,BROADCAST,MULTICAST,UP> mtu 2044 qdisc
pfifo_fast state LOWERLAYERDOWN qlen 256
...
Notice the (now correct) LOWERLAYERDOWN operstate shown for the
mlx4_ib1.8003 interface. Ideally the identical state would be shown
right after boot.
The problem is related to the calling of netif_carrier_off() in
network drivers. For a long time it was known that doing
netif_carrier_off() before registering the netdevice would result in
the interface's operstate being shown as UNKNOWN if the device was
brought up without a working link. This problem was fixed in commit
8f4cccbbd9 ('net: Set device operstate at registration time'), but
still there remains the minor inconsistency demonstrated above.
This patch fixes it by moving ipoib's call to netif_carrier_off() into
the .ndo_open method, which is where network drivers ordinarily do it.
With the patch when doing the same test as above, the operstate of
mlx4_ib1.8003 is shown as LOWERLAYERDOWN right after boot.

The driver starts the mcast_join task whenever the netdev interface is
UP without relation to the underlying IB port state.
Until the port state is ACTIVE all the join requests are irrelevant,
and the IB core returns -EINVAL. So the user will see errors such as:
"multicast join failed for ff12:401b:... , status -22".
Instead, have ipoib_mcast_join_task() return when the port is not active.

The driver should avoid calling ib_sa_free_multicast on the mcast->mc
object until it finishes its initialization state. Otherwise we can
crash when ipoib_mcast_dev_flush() attempts to use the uninitialized
multicast object.
Instead, only call wait_for_completion() for multicast entries that
started the join process, meaning that ib_sa_join_multicast() finished.

When ipoib interface is going down it takes all of its children with
it, under mutex.
For each child, dev_change_flags() is called. That function calls
ipoib_stop() via the ndo, and causes flush of the workqueue.
Sometimes in the workqueue an __ipoib_dev_flush work() is waiting and
when invoked tries to get the same mutex, which leads to a deadlock,
as seen below.
The solution is to switch to rw-sem instead of mutex.
The deadlock:

If napi has never been enabled when calling ipoib_ib_dev_stop, a
kernel crash occurs, because the verbs layer completion handler
(ipoib_ib_completion) calls napi_schedule unconditionally.
If the napi structure passed in the napi_schedule call has not
been initialized, napi will crash.
The cleanest solution is to simply enable napi before calling
ipoib_ib_dev_stop in the dev_open error flow. (dev_stop then
immediately disables napi).

In several places, this snippet is used when removing neigh entries:
list_del(&neigh->list);
ipoib_neigh_free(neigh);
The list_del() removes neigh from the associated struct ipoib_path, while
ipoib_neigh_free() removes neigh from the device's neigh entry lookup
table. Both of these operations are protected by the priv->lock
spinlock. The table however is also protected via RCU, and so naturally
the lock is not held when doing reads.
This leads to a race condition, in which a thread may successfully look
up a neigh entry that has already been deleted from neigh->list. Since
the previous deletion will have marked the entry with poison, a second
list_del() on the object will cause a panic:

IPoIB's required behaviour w.r.t to the pkey used by the device is the following:
- For "parent" interfaces (e.g ib0, ib1, etc) who are created
automatically as a result of hot-plug events from the IB core, the
driver needs to take whatever pkey vlaue it finds in index 0, and
stick to that index.
- For child interfaces (e.g ib0.8001, etc) created by admin directive,
the driver needs to use and stick to the value provided during its
creation.
In SR-IOV environment its possible for the VF probe to take place
before the cloud management software provisions the suitable pkey for
the VF in the paravirtualed PKEY table index 0. When this is the case,
the VF IB stack will find in index 0 an invalide pkey, which is all
zeros.
Moreover, the cloud managment can assign the pkey value at index 0 at
any time of the guest life cycle.
The correct behavior for IPoIB to address these requirements for
parent interfaces is to use PKEY_CHANGE event as trigger to optionally
re-init the device pkey value and re-create all the relevant resources
accordingly, if the value of the pkey in index 0 has changed (from
invalid to valid or from valid value X to invalid value Y).
This patch enhances the heavy flushing code which is triggered by pkey
change event, to behave correctly for parent devices. For child
devices, the code remains the same, namely chases pkey value and not
index.

Support TIPC in the IPoIB driver. Since IPoIB now keeps track of its own
neighbour entries and doesn't require the packet to have a dst_entry
anymore, the only necessary changes are to:
- not drop multicast TIPC packets because of the unknown ethernet type
- handle unicast TIPC packets similar to IPv4/IPv6 unicast packets
in ipoib_start_xmit().
An alternative would be to remove all ethertype limitations since they're
not necessary anymore, all TIPC needs to know about is ARP and RARP since
it wants to always perform "path find", even if a path is already known.

If you have a patched up dhcp server (and dhclient), they will use
AF_PACKET/SOCK_DGRAM pair to send dhcp packets over IPoIB.
However, when testing an upstream kernel, this has been broken for a
very long time (I tested 2.6.34, 2.6.38, 3.0, 3.1, 3.8, HEAD).
It turns out that the hard_header routine in ipoib is not following
the API and is returning 0 even when it pushed data onto the skb.
This then causes af_packet.c to overwrite the header just pushed with
data from user space.
Fixing this gets DHCP working on IPoIB.

Commit f0dc117abd ("IPoIB: Fix TX queue lockup with mixed UD/CM
traffic") attempts to solve an issue where unprocessed UD send
completions can deadlock the netdev.
The patch doesn't fully resolve the issue because if more than half
the tx_outstanding's were UD and all of the destinations are RC
reachable, arming the CQ doesn't solve the issue.
This patch uses the IB_CQ_REPORT_MISSED_EVENTS on the
ib_req_notify_cq(). If the rc is above 0, the UD send cq completion
callback is called directly to re-arm the send completion timer.
This issue is seen in very large parallel filesystem deployments
and the patch has been shown to correct the issue.

If IPoIB fails to look up a path record (eg if it tries during an SM
failover when one SM is dead but the new one hasn't taken over yet), the
driver ends up with a neighbour structure but no address handle (AH).
There's no mechanism to recover from this: any further packets sent to
this destination will be silently dumped in ipoib_start_xmit().
Fix this by freeing the neighbour structures when a path rec query
fails, so that the next packet queued to be sent will trigger a new path
record query.

The hash function introduced in commit b63b70d877 ("IPoIB: Use a
private hash table for path lookup in xmit path") was designd to use
the 3 octets of the IPoIB HW address that holds the remote QPN.
However, this currently isn't the case on little-endian machines,
because the the code there uses the flags part (octet[0]) and not the
last octet of the QPN (octet[3]). Fix this.

After commit b13912bbb4 ("IPoIB: Call skb_dst_drop() once skb is
enqueued for sending"), using connected mode and running multithreaded
iperf for long time, ie
iperf -c <IP> -P 16 -t 3600
results in a crash.
After the above-mentioned patch, the driver is calling skb_orphan() and
skb_dst_drop() after calling post_send() in ipoib_cm.c::ipoib_cm_send()
(also in ipoib_ib.c::ipoib_send())
The problem with this is, as is written in a comment in both routines,
"it's entirely possible that the completion handler will run before we
execute anything after the post_send()." This leads to running the
skb cleanup routines simultaneously in two different contexts.
The solution is to always perform the skb_orphan() and skb_dst_drop()
before queueing the send work request. If an error occurs, then it
will be no different than the regular case where dev_free_skb_any() in
the completion path, which is assumed to be after these two routines.

Currently, IPoIB delays collecting send completions for TX packets in
order to batch work more efficiently. It does skb_orphan() right after
queuing the packets so that destructors run early, to avoid problems
like holding socket send buffers for too long (since we might not
collect a send completion until a long time after the packet is
actually sent).
However, IPoIB clears IFF_XMIT_DST_RELEASE because it actually looks
at skb_dst() to update the PMTU when it gets a too-long packet. This
means that the packets sitting in the TX ring with uncollected send
completions are holding a reference on the dst. We've seen this lead
to pathological behavior with respect to route and neighbour GC. The
easy fix for this is to call skb_dst_drop() when we call skb_orphan().
Also, give packets sent via connected mode (CM) the same skb_orphan()
/ skb_dst_drop() treatment that packets sent via datagram mode get.

Merge tag 'rdma-for-linus' of git://git.kernel.org/pub/scm/linux/kernel/git/roland/infiniband
Pull infiniband updates from Roland Dreier:
"First batch of InfiniBand/RDMA changes for the 3.7 merge window:
- mlx4 IB support for SR-IOV
- A couple of SRP initiator fixes
- Batch of nes hardware driver fixes
- Fix for long-standing use-after-free crash in IPoIB
- Other miscellaneous fixes"
This merge also removes a new use of __cancel_delayed_work(), and
replaces it with the regular cancel_delayed_work() that is now irq-safe
thanks to the workqueue updates.
That said, I suspect the sequence in question should probably use
"mod_delayed_work()". I just did the minimal "don't use deprecated
functions" fixup, though.

Fix a crash in ipoib_mcast_join_task(). (with help from Or Gerlitz)
Commit c8c2afe360 ("IPoIB: Use rtnl lock/unlock when changing device
flags") added a call to rtnl_lock() in ipoib_mcast_join_task(), which
is run from the ipoib_workqueue, and hence the workqueue can't be
flushed from the context of ipoib_stop().
In the current code, ipoib_stop() (which doesn't flush the workqueue)
calls ipoib_mcast_dev_flush(), which goes and deletes all the
multicast entries. This takes place without any synchronization with
a possible running instance of ipoib_mcast_join_task() for the same
ipoib device, leading to a crash due to NULL pointer dereference.
Fix this by making sure that the workqueue is flushed before
ipoib_mcast_dev_flush() is called. To make that possible, we move the
RTNL-lock wrapped code to ipoib_mcast_join_finish().

Add rtnl_link_ops to IPoIB, with the first usage being child device
create/delete through them. Childs devices are now either legacy ones,
created/deleted through the ipoib sysfs entries, or RTNL ones.
Adding support for RTNL childs involved refactoring of ipoib_vlan_add
which is now used by both the sysfs and the link_ops code.
Also, added ndo_uninit entry to support calling unregister_netdevice_queue
from the rtnl dellink entry. This required removal of calls to
ipoib_dev_cleanup from the driver in flows which use unregister_netdevice,
since the networking core will invoke ipoib_uninit which does exactly that.

Lockdep points out a circular locking dependency betwwen the ipoib
device priv spinlock (priv->lock) and the neighbour table rwlock
(ntbl->rwlock).
In the normal path, ie neigbour garbage collection task, the neigh
table rwlock is taken first and then if the neighbour needs to be
deleted, priv->lock is taken.
However in some error paths, such as in ipoib_cm_handle_tx_wc(),
priv->lock is taken first and then ipoib_neigh_free routine is called
which in turn takes the neighbour table ntbl->rwlock.
The solution is to get rid the neigh table rwlock completely and use
only priv->lock.

If the neighbours hash table is empty when unloading the module, then
ipoib_flush_neighs(), the cleanup routine, isn't called and the
memory used for the hash table itself leaked.
To fix this, ipoib_flush_neighs() is allways called, and another
completion object is added to signal when the table is freed.
Once invoked, ipoib_flush_neighs() flushes all the neighbours (if
there are any), calls the the hash table RCU free routine, which now
signals completion of the deletion process, and waits for the last
neighbour to be freed.

Commit b63b70d877 ("IPoIB: Use a private hash table for path lookup
in xmit path") introduced a bug where in ipoib_cm_destroy_tx() a CM
object is moved between lists without any supported locking. Under a
stress test, this eventually leads to list corruption and a crash.
Previously when this routine was called, callers were taking the
device priv lock. Currently this function is called from the RCU
callback associated with neighbour deletion. Fix the race by taking
the same lock we used to before.

Dave Miller <davem@davemloft.net> provided a detailed description of
why the way IPoIB is using neighbours for its own ipoib_neigh struct
is buggy:
Any time an ipoib_neigh is changed, a sequence like the following is made:
spin_lock_irqsave(&priv->lock, flags);
/*
* It's safe to call ipoib_put_ah() inside
* priv->lock here, because we know that
* path->ah will always hold one more reference,
* so ipoib_put_ah() will never do more than
* decrement the ref count.
*/
if (neigh->ah)
ipoib_put_ah(neigh->ah);
list_del(&neigh->list);
ipoib_neigh_free(dev, neigh);
spin_unlock_irqrestore(&priv->lock, flags);
ipoib_path_lookup(skb, n, dev);
This doesn't work, because you're leaving a stale pointer to the freed up
ipoib_neigh in the special neigh->ha pointer cookie. Yes, it even fails
with all the locking done to protect _changes_ to *ipoib_neigh(n), and
with the code in ipoib_neigh_free() that NULLs out the pointer.
The core issue is that read side calls to *to_ipoib_neigh(n) are not
being synchronized at all, they are performed without any locking. So
whether we hold the lock or not when making changes to *ipoib_neigh(n)
you still can have threads see references to freed up ipoib_neigh
objects.
cpu 1 cpu 2
n = *ipoib_neigh()
*ipoib_neigh() = NULL
kfree(n)
n->foo == OOPS
[..]
Perhaps the ipoib code can have a private path database it manages
entirely itself, which holds all the necessary information and is
looked up by some generic key which is available easily at transmit
time and does not involve generic neighbour entries.
See <http://marc.info/?l=linux-rdma&m=132812793105624&w=2> and
<http://marc.info/?l=linux-rdma&w=2&r=1&s=allows+references+to+freed+memory&q=b>
for the full discussion.
This patch aims to solve the race conditions found in the IPoIB driver.
The patch removes the connection between the core networking neighbour
structure and the ipoib_neigh structure. In addition to avoiding the
race described above, it allows us to handle SKBs carrying IP packets
that don't have any associated neighbour.
We add an ipoib_neigh hash table with N buckets where the key is the
destination hardware address. The ipoib_neigh is fetched from the
hash table and instead of the stashed location in the neighbour
structure. The hash table uses both RCU and reference counting to
guarantee that no ipoib_neigh instance is ever deleted while in use.
Fetching the ipoib_neigh structure instance from the hash also makes
the special code in ipoib_start_xmit that handles remote and local
bonding failover redundant.
Aged ipoib_neigh instances are deleted by a garbage collection task
that runs every M seconds and deletes every ipoib_neigh instance that
was idle for at least 2*M seconds. The deletion is safe since the
ipoib_neigh instances are protected using RCU and reference count
mechanisms.
The number of buckets (N) and frequency of running the GC thread (M),
are taken from the exported arb_tbl.

This will be used so that we can compose a full flow key.
Even though we have a route in this context, we need more. In the
future the routes will be without destination address, source address,
etc. keying. One ipv4 route will cover entire subnets, etc.
In this environment we have to have a way to possess persistent storage
for redirects and PMTU information. This persistent storage will exist
in the FIB tables, and that's why we'll need to be able to rebuild a
full lookup flow key here. Using that flow key will do a fib_lookup()
and create/update the persistent entry.

IPoIB indeed allocates a full page to store a fragment, but only
accounts in skb->truesize the used part of the page (frame length)

also fixes a performance issue, because RX skbs have not enough tailroom

Use a bit in wc_flags rather then a whole integer to hold the
"checksum OK" flag. By itself, this change doesn't reduce the size of
struct ib_wc on 64bit machines -- it stays on 56 bytes because of
padding. However, it will allow to add more fields in the future
without enlarging the struct. Also, it will let us have a unified
approach with future libibverbs checksum offload reporting, because a
bit flag doesn't break the library ABI.

Handle dst_get_neighbour_noref() returning NULL in ipoib_start_xmit()

want it to fall through into the ARP/RARP/multicast handling, since
