When running on an AMD vIOMMU, it is better to avoid TLB flushes
of unmodified PTEs. vIOMMUs require the hypervisor to synchronize the
virtualized IOMMU's PTEs with the physical ones. This process induce
overheads.
AMD IOMMU allows us to flush any range that is aligned to the power of
2. So when running on top of a vIOMMU, break the range into sub-ranges
that are naturally aligned, and flush each one separately. This apporach
is better when running with a vIOMMU, but on physical IOMMUs, the
penalty of IOTLB misses due to unnecessary flushed entries is likely to
be low.
Repurpose (i.e., keeping the name, changing the logic)
domain_flush_pages() so it is used to choose whether to perform one
flush of the whole range or multiple ones to avoid flushing unnecessary
ranges. Use NpCache, as usual, to infer whether the IOMMU is physical or
virtual.

On virtual machines, software must flush the IOTLB after each page table
entry update.
The iommu_map_sg() code iterates through the given scatter-gather list
and invokes iommu_map() for each element in the scatter-gather list,
which calls into the vendor IOMMU driver through iommu_ops callback. As
the result, a single sg mapping may lead to multiple IOTLB flushes.
Fix this by adding amd_iotlb_sync_map() callback and flushing at this
point after all sg mappings we set.

AMD's IOMMU can flush efficiently (i.e., in a single flush) any range.
This is in contrast, for instnace, to Intel IOMMUs that have a limit on
the number of pages that can be flushed in a single flush. In addition,
AMD's IOMMU do not care about the page-size, so changes of the page size
do not need to trigger a TLB flush.
So in most cases, a TLB flush due to disjoint range is not needed for
AMD. Yet, vIOMMUs require the hypervisor to synchronize the virtualized
IOMMU's PTEs with the physical ones. This process induce overheads, so
it is better not to cause unnecessary flushes, i.e., flushes of PTEs
that were not modified.
Implement and use amd_iommu_iotlb_gather_add_page() and use it instead
of the generic iommu_iotlb_gather_add_page(). Ignore disjoint regions
unless "non-present cache" feature is reported by the IOMMU
capabilities, as this is an indication we are running on a physical
IOMMU. A similar indication is used by VT-d (see "caching mode"). The
new logic retains the same flushing behavior that we had before the
introduction of page-selective IOTLB flushes for AMD.
On virtualized environments, check if the newly flushed region and the
gathered one are disjoint and flush if it is.

Do not use flush-queue on virtualized environments, where the NpCache
capability of the IOMMU is set. This is required to reduce
virtualization overheads.
This change follows a similar change to Intel's VT-d and a detailed

Recent patch attempted to enable selective page flushes on AMD IOMMU but
neglected to adapt amd_iommu_iotlb_sync() to use the selective flushes.
Adapt amd_iommu_iotlb_sync() to use selective flushes and change
amd_iommu_unmap() to collect the flushes. As a defensive measure, to
avoid potential issues as those that the Intel IOMMU driver encountered
recently, flush the page-walk caches by always setting the "pde"
parameter. This can be removed later.

For the printing of RMP_HW_ERROR / RMP_PAGE_FAULT / IO_PAGE_FAULT
events, the AMD IOMMU code uses such logic:

This means that if we receive an event for a PCI devid which actually
does have a struct pci_dev and an attached struct iommu_dev_data, but
rate limiting kicks in, we'll fall back to the non-PCI branch of the
test, and print the event in a different format.

If people are going to insist on calling iommu_iova_to_phys()
pointlessly and expecting it to work, we can at least do ourselves a
favour by handling those cases in the core code, rather than repeatedly
across an inconsistent handful of drivers.
Since all the existing drivers implement the internal callback, and any
future ones are likely to want to work with iommu-dma which relies on
iova_to_phys a fair bit, we may as well remove that currently-redundant
check as well and consider it mandatory.

Make IOMMU_DEFAULT_LAZY default for when AMD_IOMMU config is set, which
matches current behaviour.
For "fullflush" param, just call iommu_set_dma_strict(true) directly.
Since we get a strict vs lazy mode print already in iommu_subsys_init(),
and maintain a deprecation print when "fullflush" param is passed, drop the
prints in amd_iommu_init_dma_ops().
Finally drop global flag amd_iommu_unmap_flush, as it has no longer has any
purpose.

Passing a 64-bit address width to iommu_setup_dma_ops() is valid on
virtual platforms, but isn't currently possible. The overflow check in
iommu_dma_init_domain() prevents this even when @dma_base isn't 0. Pass
a limit address instead of a size, so callers don't have to fake a size
to work around the check.
The base and limit parameters are being phased out, because:
* they are redundant for x86 callers. dma-iommu already reserves the
first page, and the upper limit is already in domain->geometry.
* they can now be obtained from dev->dma_range_map on Arm.
But removing them on Arm isn't completely straightforward so is left for
future work. As an intermediate step, simplify the x86 callers by
passing dummy limits.

A recent commit introduced this section mismatch warning:

Now that DMA ops are part of the core API via iommu-dma, fold the
vestigial remains of the IOMMU_DMA_OPS init state into the IOMMU API
phase, and clean up a few other leftovers. This should also close the
race window wherein bus_set_iommu() effectively makes the DMA ops state
visible before its nominal initialisation - it seems this was previously
fairly benign, but since commit a250c23f15 ("iommu: remove
DOMAIN_ATTR_DMA_USE_FLUSH_QUEUE") it can now lead to the strict flush
queue policy inadvertently being picked for default domains allocated
during that window, with a corresponding unexpected perfomance impact.

print_iommu_info prints the EFR register and then the decoded list of
features on a separate line:

The second line is emitted via 'pr_cont', which causes it to have a
different ('warn') loglevel compared to the previous line ('info').
Commit 9a295ff0ff attempted to rectify this by removing the newline
from the pci_info format string, but this doesn't work, as pci_info
calls implicitly append a newline anyway.
Printing the decoded features on the same line would make it quite long.
Instead, change pci_info() to pr_info() to omit PCI bus location info,
which is also shown in the preceding message. This results in:

The logic to determine the mask of page-specific invalidations was
tested in userspace. As the code was copied into the kernel, the
parentheses were mistakenly set in the wrong place, resulting in the
wrong mask.

Since commit 08a27c1c3e ("iommu: Add support to change default domain
of an iommu group") a user can switch a device between IOMMU and direct
DMA through sysfs. This doesn't work for AMD IOMMU at the moment because
dev->dma_ops is not cleared when switching from a DMA to an identity
IOMMU domain. The DMA layer thus attempts to use the dma-iommu ops on an
identity domain, causing an oops:

In early AMD desktop/mobile platforms (during 2013), when the IOMMU
Performance Counter (PMC) support was first introduced in
commit 30861ddc9c ("perf/x86/amd: Add IOMMU Performance Counter
resource management"), there was a HW bug where the counters could not
be accessed. The result was reading of the counter always return zero.
At the time, the suggested workaround was to add a test logic prior
to initializing the PMC feature to check if the counters can be programmed
and read back the same value. This has been working fine until the more
recent desktop/mobile platforms start enabling power gating for the PMC,
which prevents access to the counters. This results in the PMC support
being disabled unnecesarily.
Unfortunatly, there is no documentation of since which generation
of hardware the original PMC HW bug was fixed. Although, it was fixed
soon after the first introduction of the PMC. Base on this, we assume
that the buggy platforms are less likely to be in used, and it should
be relatively safe to remove this legacy logic.

This reverts commit 6778ff5b21.
The original commit tries to address an issue, where PMC power-gating
causing the IOMMU PMC pre-init test to fail on certain desktop/mobile
platforms where the power-gating is normally enabled.
There have been several reports that the workaround still does not
guarantee to work, and can add up to 100 ms (on the worst case)
to the boot process on certain platforms such as the MSI B350M MORTAR
with AMD Ryzen 3 2200G.
Therefore, revert this commit as a prelude to removing the pre-init
test.

Currently, IOMMU invalidations and device-IOTLB invalidations using
AMD IOMMU fall back to full address-space invalidation if more than a
single page need to be flushed.
Full flushes are especially inefficient when the IOMMU is virtualized by
a hypervisor, since it requires the hypervisor to synchronize the entire
address-space.
AMD IOMMUs allow to provide a mask to perform page-specific
invalidations for multiple pages that match the address. The mask is
encoded as part of the address, and the first zero bit in the address
(in bits [51:12]) indicates the mask size.
Use this hardware feature to perform selective IOMMU and IOTLB flushes.
Combine the logic between both for better code reuse.
The IOMMU invalidations passed a smoke-test. The device IOTLB
invalidations are untested.

A few functions that were intentended for the perf events support are
currently declared in arch/x86/events/amd/iommu.h, which mens they are
not in scope for the actual function definition. Also amdkfd has started
using a few of them using externs in a .c file. End that misery by
moving the prototypes to the proper header.

Instead make the global iommu_dma_strict paramete in iommu.c canonical by
exporting helpers to get and set it and use those directly in the drivers.
This make sure that the iommu.strict parameter also works for the AMD and
Intel IOMMU drivers on x86. As those default to lazy flushing a new
IOMMU_CMD_LINE_STRICT is used to turn the value into a tristate to
represent the default if not overriden by an explicit parameter.

increase_address_space() calls get_zeroed_page(gfp) under spin_lock with

Certain AMD platforms enable power gating feature for IOMMU PMC,
which prevents the IOMMU driver from updating the counter while
trying to validate the PMC functionality in the init_iommu_perf_ctr().
This results in disabling PMC support and the following error message:
"AMD-Vi: Unable to read/write to IOMMU perf counter"
To workaround this issue, disable power gating temporarily by programming
the counter source to non-zero value while validating the counter,
and restore the prior state afterward.

IOMMU Extended Feature Register (EFR) is used to communicate
the supported features for each IOMMU to the IOMMU driver.
This is normally read from the PCI MMIO register offset 0x30,
and used by the iommu_feature() helper function.
However, there are certain scenarios where the information is needed
prior to PCI initialization, and the iommu_feature() function is used
prematurely w/o warning. This has caused incorrect initialization of IOMMU.
This is the case for the commit 6d39bdee23 ("iommu/amd: Enforce 4k
mapping for certain IOMMU data structures")
Since, the EFR is also available in the IVHD header, and is available to
the driver prior to PCI initialization. Therefore, default to using
the IVHD EFR instead.

The AMD IOMMU initialisation registers the IRQ remapping domain for
each IOMMU before doing the final sanity check that every I/OAPIC is
covered.
This means that the AMD irq_remapping_select() function gets invoked
even when IRQ remapping has been disabled, eventually leading to a NULL
pointer dereference in alloc_irq_table().
Unfortunately, the IVRS isn't fully parsed early enough that the sanity
check can be done in time to registering the IRQ domain altogether.
Doing that would be nice, but is a larger and more error-prone task. The
simple fix is just for irq_remapping_select() to refuse to report a
match when IRQ remapping has disabled.

When I made the INTCAPXT support stop gratuitously pretending to be MSI,
I missed the fact that iommu_setup_msi() also sets the ->int_enabled
flag. I missed this in the iommu_setup_intcapxt() code path, which means
that a resume from suspend will try to allocate the IRQ domains again,
accidentally re-enabling interrupts as it does, resulting in much sadness.
Lift out the bit which sets iommu->int_enabled into the iommu_init_irq()
function which is also where it gets checked.

Currently, macros related to the interrupt remapping table length are
defined separately. This has resulted in an oversight in which one of
the macros were missed when changing the length. To prevent this,
redefine the macros to add built-in sanity check.
Also, rename macros to use the name of the DTE[IntTabLen] field as
specified in the AMD IOMMU specification. There is no functional change.

AMD IOMMU requires 4k-aligned pages for the event log, the PPR log,
and the completion wait write-back regions. However, when allocating
the pages, they could be part of large mapping (e.g. 2M) page.
This causes #PF due to the SNP RMP hardware enforces the check based
on the page level for these data structures.
So, fix by calling set_memory_4k() on the allocated pages.

The AMD IOMMU has two modes for generating its own interrupts.
The first is very much based on PCI MSI, and can be configured by Linux
precisely that way. But like legacy unmapped PCI MSI it's limited to
8 bits of APIC ID.
The second method does not use PCI MSI at all in hardawre, and instead
configures the INTCAPXT registers in the IOMMU directly with the APIC ID
and vector.
In the latter case, the IOMMU driver would still use pci_enable_msi(),
read back (through MMIO) the MSI message that Linux wrote to the PCI MSI
table, then swizzle those bits into the appropriate register.
Historically, this worked because__irq_compose_msi_msg() would silently
generate an invalid MSI message with the high bits of the APIC ID in the
high bits of the MSI address. That hack was intended only for the Intel
IOMMU, and I recently enforced that, introducing a warning in
__irq_msi_compose_msg() if it was invoked with an APIC ID above 255.
Fix the AMD IOMMU not to depend on that hack any more, by having its own
irqdomain and directly putting the bits from the irq_cfg into the right
place in its ->activate() method.

Registering the remapping irq domain unconditionally is potentially
allowing I/O-APIC and MSI interrupts to be parented in the IOMMU IR domain

Certain device drivers allocate IO queues on a per-cpu basis.
On AMD EPYC platform, which can support up-to 256 cpu threads,
this can exceed the current MAX_IRQ_PER_TABLE limit of 256,
and result in the error message:
AMD-Vi: Failed to allocate IRTE
This has been observed with certain NVME devices.
AMD IOMMU hardware can actually support upto 512 interrupt
remapping table entries. Therefore, update the driver to
match the hardware limit.
Please note that this also increases the size of interrupt remapping
table to 8KB per device when using the 128-bit IRTE format.

The I/O-APIC generates an MSI cycle with address/data bits taken from its
Redirection Table Entry in some combination which used to make sense, but
now is just a bunch of bits which get passed through in some seemingly
arbitrary order.
Instead of making IRQ remapping drivers directly frob the I/OA-PIC RTE, let
them just do their job and generate an MSI message. The bit swizzling to
turn that MSI message into the I/O-APIC's RTE is the same in all cases,
since it's a function of the I/O-APIC hardware. The IRQ remappers have no
real need to get involved with that.
The only slight caveat is that the I/OAPIC is interpreting some of those
fields too, and it does want the 'vector' field to be unique to make EOI
work. The AMD IOMMU happens to put its IRTE index in the bits that the
I/O-APIC thinks are the vector field, and accommodates this requirement by
reserving the first 32 indices for the I/O-APIC. The Intel IOMMU doesn't
actually use the bits that the I/O-APIC thinks are the vector field, so it
fills in the 'pin' value there instead.

Having two seperate structs for the I/O-APIC RTE entries (non-remapped and
DMAR remapped) requires type casts and makes it hard to map.
Combine them in IO_APIC_routing_entry by defining a union of two 64bit
bitfields. Use naming which reflects which bits are shared and which bits
are actually different for the operating modes.

there are defines for initializing these variables and struct members, they
are not used consequently and the meaning of 'trigger' and 'polarity' is
opaque and confusing at best.
Rename them to 'is_level' and 'active_low' and make them boolean in various
structs so it's entirely clear what the meaning is.

Commit 387caf0b75 ("iommu/amd: Treat per-device exclusion
ranges as r/w unity-mapped regions") accidentally overwrites
the 'flags' field in IVMD (struct ivmd_header) when the I/O
virtualization memory definition is associated with the
exclusion range entry. This leads to the corrupted IVMD table
(incorrect checksum). The kdump kernel reports the invalid checksum:

Fix the above-mentioned issue by modifying the 'struct unity_map_entry'

When the IOMMU SNP support bit is set in the IOMMU Extended Features
register, hardware re-purposes the following registers:
1. IOMMU Exclusion Base register (MMIO offset 0020h) to
Completion Wait Write-Back (CWWB) Base register
2. IOMMU Exclusion Range Limit (MMIO offset 0028h) to
Completion Wait Write-Back (CWWB) Range Limit register
and requires the IOMMU CWWB semaphore base and range to be programmed
in the register offset 0020h and 0028h accordingly.

IOMMU SNP support requires the completion wait write-back semaphore to be
implemented using a 4K-aligned page, where the page address is to be
programmed into the newly introduced MMIO base/range registers.
This new scheme uses a per-iommu atomic variable to store the current
semaphore value, which is incremented for every completion wait command.
Since this new scheme is also compatible with non-SNP mode,
generalize the driver to use 4K page for completion-wait semaphore in
both modes.

Commit e52d58d54a ("iommu/amd: Use cmpxchg_double() when updating
128-bit IRTE") removed an assumption that modify_irte_ga always set
the valid bit, which requires the callers to set the appropriate value
for the struct irte_ga.valid bit before calling the function.
Similar to the commit 26e495f341 ("iommu/amd: Restore IRTE.RemapEn
bit after programming IRTE"), which is for the function
amd_iommu_deactivate_guest_mode().
The same change is also needed for the amd_iommu_activate_guest_mode().
Otherwise, this could trigger IO_PAGE_FAULT for the VFIO based VMs with
AVIC enabled.

PASID is defined as a few different types in iommu including "int",
"u32", and "unsigned int". To be consistent and to match with uapi
definitions, define PASID and its variations (e.g. max PASID) as "u32".
"u32" is also shorter and a little more explicit than "unsigned int".
No PASID type change in uapi although it defines PASID as __u64 in
some places.

As the next step to make X86 utilize the direct MSI irq domain operations
store the irq domain pointer in the device struct when a device is probed.
It only overrides the irqdomain of devices which are handled by a regular
PCI/MSI irq domain which protects PCI devices behind special busses like
VMD which have their own irq domain.
No functional change.
It just avoids the redirection through arch_*_msi_irqs() and allows the
PCI/MSI core to directly invoke the irq domain alloc/free functions instead
of having to look up the irq domain for every single MSI interupt.

Convert the interrupt remap drivers to retrieve the pci device from the msi
descriptor and use info::hwirq.
This is the first step to prepare x86 for using the generic MSI domain ops.

Move the IOAPIC specific fields into their own struct and reuse the common
devid. Get rid of the #ifdeffery as it does not matter at all whether the
alloc info is a couple of bytes longer or not.
