Take f_mutex around mmap() processing to protect against races with the
SG_SET_RESERVED_SIZE ioctl. Ensure the reserve buffer length remains
consistent during the mapping operation, and set the "mmap called" flag
to prevent further changes to the reserved buffer size as an atomic
operation with the mapping.

Commit 1bc0eb0446 ("scsi: sg: protect accesses to 'reserved' page
array") adds needed concurrency protection for the "reserve" buffer.
Some checks that are initially made outside the lock are replicated once
the lock is taken to ensure the checks and resulting decisions are made
using consistent state.
The check that a request with flag SG_FLAG_MMAP_IO set fits in the
reserve buffer also needs to be performed again under the lock to ensure
the reserve buffer length compared against matches the value in effect
when the request is linked to the reserve buffer. An -ENOMEM should be
returned in this case, instead of switching over to an indirect buffer
as for non-MMAP_IO requests.

If scsi_host_template->host_reset is NULL and the user requests an
adapter reset through
echo adapter > /sys/class/scsi_host/hostx/host_reset
-EINVAL will be returned even though the "adapter" argument is perfectly
valid.
Change this so that we only return -EINVAL if the provided string is
invalid. If the host does not implement a ->host_reset function we'll
return -EOPNOTSUPP.

Currently in the driver the qedf_ctx attribute vlan_hw_insert is used to
which whether to insert a VLAN tag in FIP frames (except for FIP VLAN
request which is explicitly sent out untagged at least from the driver's
point of view).
When we receive a FIP VLAN response, we set qedf->vlan_hw_insert to 0 which
makes the qedf_fip_send function insert the VLAN. However when we exhaust
our FIP VLAN retries, we do not set qedf->vlan_hw_insert to 0 which means
that the driver will not tag the FIP frame with the correct VLAN ID. The
result that was observed on the wire is that some entity either in the LL2
or L2 firmware is adding a NULL VLAN tag which can cause FIP solicitation
to fail.
The offload FCoE frame function, qedf_xmit, does not use the vlan_hw_insert
attribute to decide whether to tag frames with the FIP/FCoE VLAN. Instead
it unilaterially tags the offload frames with the VLAN ID stored in
qedf->vlan_id. This is the correct behavior so the driver can guarantee
that non-offload FIP frames go out with the correct VLAN ID.
Also use the Linux network layer helpers instead of doing the VLAN insert
manually.
Also fix setting the fallback VLAN so that it used the module parameter and
is not hardcoded to 1002 (though 1002 is the default).

Currently in the driver we've been using the fc_fcoe_set_mac() function to
set the source MAC for FCoE traffic. This works well in most cases as it
uses the spec. default FCF-MAC. However, if the administrator changes the
FCF-MAC switch, then any FCoE traffic we send will be dropped by the
switch.
Instead we should check the granted MAC from the FLOGI payload and use that
address if it is present. Otherwise, fall back to using the the default
FCF-MAC and the fabric ID of the port as the FCoE MAC address.
Once this address is known we need to set it when doing non-offload
traffic, offload traffic and setting the data_src_address libfcoe uses for
FIP keep alive messages.

Since switching to blk-mq as the default in commit 5c279bd9e4
("scsi: default to scsi-mq"), virtio-scsi LUNs consume about 10x as
much kernel memory.
qemu currently allocates a fixed 128 entry virtqueue. can_queue
currently is set to 1024. But with indirect descriptors, each command
in the queue takes 1 virtqueue entry, so the number of commands which
can be queued is equal to the length of the virtqueue.
Note I intend to send a patch to qemu to allow the virtqueue size to be
configured from the qemu command line.

At the beginning of 'qedf_srr_compl()' and of 'qedf_rec_compl()', we
check if 'orig_io_req' is NULL. If this happens, a NULL pointer
dereference will occur in the error handling path.
Fix it by adding an additional label in the error handling path in order
to avoid this NULL pointer dereference.

Defaulting to scsi-mq in 4.13-rc has shown various regressions
on setups that we didn't previously consider. Fixes for them are
in progress, but too invasive to make it in this cycle. So for
now revert the commit that defaults to blk-mq for SCSI. For 4.14
we'll plan to try again with these fixes.

Releasing a zone write lock only when the write commnand that acquired
the lock completes can cause deadlocks due to potential command
reordering if the lock owning request is requeued and not executed. This
problem exists only with the scsi-mq path as, unlike the legacy path,
requests are moved out of the dispatch queue before being prepared and
so before locking a zone for a write command.
Since sd_uninit_cmnd() is now always called when a request is requeued,
call sd_zbc_write_unlock_zone() from that function for write requests
that acquired a zone lock instead of from sd_done(). Acquisition of a zone
lock by a write command is indicated using the new command
flag SCMD_ZONE_WRITE_LOCK.

megasas_mgmt_info.max_index has increased by 1 before megasas_io_attach,
if megasas_io_attach return error, then goto fail_io_attach,

This patch is a fix related to freeing a device in v2 hw driver.
Before, we polled to ITCT CLR interrupt to check if a device is free.
This was error prone, as if the interrupt doesn't occur in 10us, we miss
processing it.
To avoid this situation, service this interrupt and sync the event with
a completion.

This patch removes some repeated configurations:
(1) The device id of the device is already set in the alloc function, so
we don't need to modify in free device function.
(2) Field dev_type and dev_status are configured in hisi_sas_dev_gone(),
so there is no need for repeated config in free_device_v3_hw.

This patch provides fixes for the following issues:
1. Fix issue of controller reset required to send commands. For reset
process, it may be required to send commands to the controller, but
not during soft reset. So add HISI_SAS_NOT_ACCEPT_CMD_BIT to prevent
executing a task during this period.
2. Send a broadcast event in rescan topology to detect any topology
changes during reset.
3. Previously it was not ensured that libsas has processed the PHY up
and down events after reset. Potentially this could cause an issue
that we still process the PHY event after reset. So resolve this by
flushing shot workqueue in LLDD reset.
4. Port ID requires refresh after reset. The port ID generated after
reset is not guaranteed to be the same as before reset, so it needs
to be refreshed for each device's ITCT.

Add PQI reset to driver shutdown callback to work around controller bug.
During an 1.) OS shutdown or 2.) kexec outside of a kdump, the Linux
kernel will clear BME on our controller.
If BME is cleared during a controller/host PCIe transfer, the controller
will lock up.
So we perform a PQI reset in the driver's shutdown callback function to
eliminate the possibility of a controller/host PCIe transfer being
active when the kernel clears BME immediately after calling the driver's
shutdown callback.

In function pm8001_pci_probe(), on errors that the control flow jumps to
label err_out_ha_free, function pm8001_free() is called. In pm8001_free(),
scsi_host_put() is called to release shost, which keeps the return value
of scsi_host_alloc(). After pm8001_free() returns, kfree() is called to
free shost again, resulting in a double free bug. This patch removes
scsi_host_put() from pm8001_free() and explicitly calls scsi_host_put()
to release Scsi_Host in need.

When in MESSAGE IN phase, the ESP device does not automatically
acknowledge each byte that is transferred by PIO. The mac_esp driver
neglects to explicitly ack them, which causes a timeout during messages
larger than one byte (e.g. tag bytes during reconnect). Fix this with an
ESP_CMD_MOK command after each byte.
The MESSAGE IN phase is also different in that each byte transferred
raises ESP_INTR_FDONE. So don't exit the transfer loop for this interrupt,
for this phase.
That resolves the "Reconnect IRQ2 timeout" error on those Macs which use
PIO transfers instead of PDMA. This patch also improves on the weak tests
for unexpected interrupts and phase changes during PIO transfers.

Currently, calls to nvmet_fc_rcv_fcp_req() always copied the
FC-NVME cmd iu to a temporary buffer before returning, allowing
the driver to immediately repost the buffer to the hardware.
To address timing conditions on queue element structures vs async
command reception, the nvmet_fc transport occasionally may need to
hold on to the command iu buffer for a short period. In these cases,
the nvmet_fc_rcv_fcp_req() will return a special return code
(-EOVERFLOW). In these cases, the LLDD must delay until the new
defer_rcv lldd callback is called before recycling the buffer back
to the hw.
This patch adds support for the new nvmet_fc transport defer_rcv
callback and recognition of the new error code when passing commands
to the transport.

If a SES device returns an error on a requested diagnostic page, we are

If blk_queue_get() in st_probe fails, disk->queue must not be set to

Scsi_cmnd is an unsuitable argument for eh_device_reset_handler(),
eh_target_reset_handler(), and eh_host_reset_handler() which do not have
the scope of one single SCSI command. These callbacks tend to use
fc_block_scsi_eh() requiring scsi_cmnd. In order to start decoupling
above eh callbacks from scsi_cmnd, introduce a new variant of the
function called fc_block_rport() taking an fc_rport as argument.
Refactor the old fc_block_scsi_eh() to simply delegate to
fc_block_rport().

Limit PDMA send to 512 B to avoid data corruption on DTC3181E. The
corruption is always the same: one byte missing at the beginning of a
128 B block. It happens only with slow Quantum LPS 240 drive, not with
faster IBM DORS-32160. It's not clear what causes this. Documentation
for the DTC436 chip has not been made available.

The polling loops in pread() and pwrite() can easily become infinite
loops and hang the machine.
Merge the IRQ check into host buffer wait loop and add polling limit.
Also place a limit on polling for 53C80 registers accessibility.
[Use NCR5380_poll_politely2() for register polling. Rely on polling for
gated IRQ rather than polling for phase error, like the algorithm in the
53c400 datasheet. Move DTC436 workarounds into a separate patch.
Factor-out common code as wait_for_53c80_access(). Rework the residual

When an IRQ arrives during PDMA transfer, pread() and pwrite() return
without waiting for the 53C80 registers to be ready and this ends up
messing up the chip state. This was observed with SONY CDU-55S which is
slow enough to disconnect during 4096-byte reads.
IRQ during PDMA is not an error so don't return -1. Instead, store the
remaining byte count for use by NCR5380_dma_residual().

generic_NCR5380_dma_xfer_len() incorrectly uses cmd->transfersize which
causes rescan-scsi-bus and CD-ROM access to hang the system. Use
cmd->SCp.this_residual instead, like other NCR5380 drivers.

When sending a reset fib we shouldn't rely on the scsi command,
but rather set the TMF status in the map_info->reset_state variable.
That allows us to send a TMF independent on a scsi command.

This patch drops two incorrect usages of tcm_qla2xxx_free_cmd()
during TMR ABORT within tcm_qla2xxx_handle_data_work() and
tcm_qla2xxx_aborted_task(), which where attempting to dispatch
into workqueue context to do tcm_qla2xxx_complete_free() and
subsequently invoke transport_generic_free_cmd().
This is incorrect because during TMR ABORT target-core will
drop the outstanding se_cmd->cmd_kref references once it has
quiesced the se_cmd via transport_wait_for_tasks(), and in
the case of qla2xxx it should not attempt to do it's own
transport_generic_free_cmd() once the abort has occured.
As reported by Pascal, this was originally manifesting as a
BUG_ON(cmd->cmd_in_wq) in qlt_free_cmd() during TMR ABORT,
with a LIO backend that had sufficently high enough WRITE
latency to trigger a host side TMR ABORT_TASK.

Don't make any assumptions on the sg_io_hdr_t::dxfer_direction or the
sg_io_hdr_t::dxferp in order to determine if it is a valid request. The
only way we can check for bad requests is by checking if the length
exceeds 256M.

1) Consolidating the callbacks into a single state. The CPU thread can be
torn down on the CPU which goes offline. There is no point in delaying
that to the CPU dead state
2) Let the core code invoke the online/offline callbacks and remove the
extra for_each_online_cpu() loops.

the cpu hotplug lock held. This worked with the old CPU locking
implementation which allowed recursive locking, but with the new percpu
rwsem based mechanism this is not longer allowed.
Use the _cpuslocked() variants to fix this.

bnx2fc_process_new_cqes() has protection against CPU hotplug, which relies
on the per cpu thread pointer. This protection is racy because it happens
only partially with the per cpu fp_work_lock held.
If the CPU is unplugged after the lock is dropped, the wakeup code can
dereference a NULL pointer or access freed and potentially reused memory.
Restructure the code so the thread check and wakeup happens with the
fp_work_lock held.

some performance drops on the hpsa and smartpqi cards. The cards
actually have an internal cache for request speed up but bypass it for
transfers > 1MB. Since 4.3 the efficiency of our merges has rendered
the cache mostly unused, so limit transfers to under 1MB to recover
the cache boost.

We shouldn't be writing over the "ret" variable. It means we return
ERR_PTR(0) which is NULL and it results in a NULL dereference in the

When LPFC is built-in but NVMe is a loadable module, we fail to link the
kernel:

When trying to delete a vport via 'vport_delete' sysfs attribute we
should be checking if the port is already in state VPORT_DELETING; if so

In error case it is possible that ->cleanup_task() gets called without
calling ->alloc_pdu() in this case cxgbi_task_data is not valid, so add
a check for for valid cxgbi_task_data in cxgbi_cleanup_task().

Building firmware with O=path was apparently broken in aic7 for ever.
Message of the previous commit to the Makefile (from 2008) mentions this
unfortunate state of affairs already. Fix this, mostly to make
randconfig builds more reliable.

qedi uses iscsi_boot_sysfs to export the targets used for boot to
sysfs. Select the config option to make sure the module is built.

dxfer_len is an unsigned int and we always assign a value > 0 to it, so
it doesn't make any sense to check if it is < 0. We can't really check
dxferp as well as we have both NULL and not NULL cases in the possible
call paths.
So just return true for SG_DXFER_FROM_DEV transfer in
sg_is_valid_dxfer().

1MB, so we should cap the request size to avoid any performance
degradation in kernels later than v4.3
This degradation is caused from d2be537c3b,
which changed max_sectors_kb to 1280k, but the hardware is able to

This patch adds support for Boot from SAN over iSCSI offload. The iSCSI
boot information in the NVRAM is populated under
/sys/firmware/iscsi_bootX/ using qed NVM-image reading API and further
exported to open-iscsi to perform iSCSI login enabling boot over offload
iSCSI interface in a Boot from SAN environment.

This patch drops two incorrect usages of tcm_qla2xxx_free_cmd()
during TMR ABORT within tcm_qla2xxx_handle_data_work() and
tcm_qla2xxx_aborted_task(), which where attempting to dispatch
into workqueue context to do tcm_qla2xxx_complete_free() and
subsequently invoke transport_generic_free_cmd().
This is incorrect because during TMR ABORT target-core will
drop the outstanding se_cmd->cmd_kref references once it has
quiesced the se_cmd via transport_wait_for_tasks(), and in
the case of qla2xxx it should not attempt to do it's own
transport_generic_free_cmd() once the abort has occured.
As reported by Pascal, this was originally manifesting as a
BUG_ON(cmd->cmd_in_wq) in qlt_free_cmd() during TMR ABORT,
with a LIO backend that had sufficently high enough WRITE
latency to trigger a host side TMR ABORT_TASK.
In addition, for the case in tcm_qla2xxx_write_pending_status()
and tcm_qla2xxx_handle_data_work() that waits for outstanding
FCP WRITE data transfer to complete before preceeding with a
TMR ABORT, avoid se_cmd->t_transport_stop_comp that is already
used by transport_wait_for_tasks() and use a qla2xxx internal
struct completion instead.

ibmvscsis always returned 0 for the tpg/tag, since it did not
parse the value passed in by the user.
When functions like ALUA members exports the value, it will
be incorrect because targetcli/rtslib starts the tpg numbering
at 1.

Here is the big driver core update for 4.13-rc1.
The large majority of this is a lot of cleanup of old fields in the
driver core structures and their remaining usages in random drivers.
All of those fixes have been reviewed by the various subsystem
maintainers. There's also some small firmware updates in here, a new
kobject uevent api interface that makes userspace interaction easier,
and a few other minor things.
All of these have been in linux-next for a long while with no reported
issues.

This patch fixes incorrect assignment for entry_type field for Continuation
Type iocb packet on BE system. This was caught by -Woverflow warning on BE
system compilation.
For Continuation Type iocb driver needs to write complete 32 bit value to

If lpfc_nvmet_unsol_fcp_issue_abort() fails then we accidentally
increment "tgtp->xmt_abort_rsp_error" and then two lines later we
increment it a second time.

We're calling spin_lock_irq() multiple times, the problem is that on the
first spin_unlock_irq() then we will re-enable IRQs and we don't want
that.

Currently we allocate 3 sets of DMA memories from separate pools for
each slot. This is inefficient in terms of memory usage
(buffers are less than 1 page in size, so we lose due to alignment),
and also time spent in doing 3 allocations + de-allocations per slot,
instead of 1.
To optimise, combine the 3 DMA buffers into a single buffer from a
single pool.

dev_pm_ops. So mark the non-const structs as const.

The device and host reset handler contain debug prints to help identify
the entities being reset. Today these reset handlers are based on a SCSI
EH design that uses a SCSI command reference as a means of identifying
the target entity. As such, the debug trace includes the SCSI command
pointer and associated CDB. This is not necessary as the SCSI command is
simply the messenger in these scenarios.
Refactor the debug prints in the host and reset handlers to only present
information that is applicable given the function scope.

The current send_tmf() implementation is based on the caller providing a
SCSI command reference. In reality all that is needed is a SCSI device
reference as the routine uses a private command.
Refactor send_tmf() to pass the private adapter configuration reference
and a SCSI device reference. As a nice side effect, this will ease the
burden of converting caller routines to be based solely off of a SCSI
device reference.

The device_unregister() service used when cleaning up the character
device is already responsible for the internal state associated with the
device upon successful creation. As the cxlflash driver does not obtain
a second reference to the character device, the explicit call to
put_device() is not required and can lead to an inconsistent sysfs among
other issues as the reference is no longer valid after the first
put_device() is performed.
Remove the unnecessary put_device() to remedy this issue.

The addition of the STARGET_REMOVE state had the side effect of
introducing a race condition that can cause a crash.
scsi_target_reap_ref_release() checks the starget->state to
see if it still in STARGET_CREATED, and if so, skips calling
transport_remove_device() and device_del(), because the starget->state
is only set to STARGET_RUNNING after scsi_target_add() has called
device_add() and transport_add_device().
However, if an rport loss occurs while a target is being scanned,
it can happen that scsi_remove_target() will be called while the
starget is still in the STARGET_CREATED state. In this case, the
starget->state will be set to STARGET_REMOVE, and as a result,
scsi_target_reap_ref_release() will take the wrong path. The end
result is a panic:

In qla2xx_start_scsi_mq() and qla2xx_dif_start_scsi_mq() we grab the
qpair->qp_lock but do access members of the qpair before having the lock.
Re-order the locking sequence to have all read and write access to qpair
members under the qpair->qp_lock.

Just wire up the generic TCG OPAL infrastructure to the SCSI disk driver
and the Security In/Out commands.
Note that I don't know of any actual SCSI disks that do support TCG OPAL,
but this is required to support ATA disks through libata.

Currently the queue command returns DID_NO_CONNECT anytime the rport is
not in RPORT_ST_READY state. Changing it to return DID_NO_CONNECT only
when the rport is in RPORT_ST_DELETE state. When the rport is in one of
the init states retruning DID_IMM_RETRY.

io_cmpl_skip keep track of number of completions to skip when stats are
reset. If a fw_reset happens immediately after stats reset it could put
it out of sync so need to reset io_cmpl_skip when fw reset is completed.
