In commit 8caab75fd2 ("spi: Generalize SPI "master" to "controller"")
some functions and struct members were renamed. To not break all drivers
compatibility macros were provided.
To be able to remove these compatibility macros push the renaming into
the SPI bitbang controller drivers.

Commit c7299fea67 ("spi: Fix spi device unregister flow") changed the
SPI core's behavior if the ->setup() hook returns an error upon adding
an spi_device: Before, the ->cleanup() hook was invoked to free any
allocations that were made by ->setup(). With the commit, that's no
longer the case, so the ->setup() hook is expected to free the
allocations itself.
I've identified 5 drivers which depend on the old behavior and am fixing
them up hereinafter: spi-bitbang.c spi-fsl-spi.c spi-omap-uwire.c
spi-omap2-mcspi.c spi-pxa2xx.c
Importantly, ->setup() is not only invoked on spi_device *addition*:
It may subsequently be called to *change* SPI parameters. If changing
these SPI parameters fails, freeing memory allocations would be wrong.
That should only be done if the spi_device is finally destroyed.
I am therefore using a bool "initial_setup" in 4 of the affected drivers
to differentiate between the invocation on *adding* the spi_device and
any subsequent invocations: spi-bitbang.c spi-fsl-spi.c spi-omap-uwire.c
spi-omap2-mcspi.c
In spi-pxa2xx.c, it seems the ->setup() hook can only fail on spi_device
addition, not any subsequent calls. It therefore doesn't need the bool.
It's worth noting that 5 other drivers already perform a cleanup if the
->setup() hook fails. Before c7299fea67, they caused a double-free
if ->setup() failed on spi_device addition. Since the commit, they're
fine. These drivers are: spi-mpc512x-psc.c spi-pl022.c spi-s3c64xx.c
spi-st-ssc4.c spi-tegra114.c
(spi-pxa2xx.c also already performs a cleanup, but only in one of
several error paths.)

The ->chipselect() callback on the bit-banged SPI library
master is optional if using GPIO descriptors: when using
descriptors exclusively without any native chipselects,
the core does not even call out the the native ->set_cs()
and therefore ->chipselect() on a bit-banged SPI master
will not even be called in this case.
Make sure to respect the SPI_MASTER_GPIO_SS as used by
e.g. spi-gpio.c though: this setting will make the core
handle the chip select using GPIO descriptors *AND* call
the local chipselect handler.

spi_setup() already call spi_set_cs() right after calling the
controller setup method, so there is no need for the bitbang driver to
do that. Because of this the chipselect() callback was confusingly
still called when CS is GPIO based.

Change the bitbang driver to use the generic implementation of
transfer_one_message. This simplifies the bitbang driver code and
provides benefits like the statistics in the generic implementation.
Successfully tested on a IMX6-based system (spi-imx) and on a MIPS-based
router (OpenWRT with spi-ath79).

chipselect (in the case of spi-gpio: spi_gpio_chipselect, which
calls gpiod_set_raw_value_cansleep) can sleep, so we should not
hold a spinlock while calling it from spi_bitbang_setup.
This issue was introduced by this commit, which converted spi-gpio
to cansleep variants:
d9dda5a191 "spi: spi-gpio: Use 'cansleep' variants to access GPIO"
Replacing the lock variable by a mutex fixes the issue: This is
safe as all instances where the lock is used are called from
contexts that can sleep.
Finally, update spi-ppc4xx and and spi-s3c24xx to use mutex
functions, as they directly hold the lock for similar purpose.

Many drivers that use bitbang library have a leak on probe error paths.
This is because once a spi_master_get() call succeeds, we need an additional
spi_master_put() call to free the memory.
Fix this issue by moving the code taking a reference to master to
spi_bitbang_start(), so spi_bitbang_start() will take a reference to master on
success. With this change, the caller is responsible for calling
spi_bitbang_stop() to decrement the reference and spi_master_put() as
counterpart of spi_alloc_master() to prevent a memory leak.
So now we have below patten for drivers using bitbang library:
probe:
spi_alloc_master -> Init reference count to 1
spi_bitbang_start -> Increment reference count
remove:
spi_bitbang_stop -> Decrement reference count
spi_master_put -> Decrement reference count (reference count reaches 0)
Fixup all users accordingly.

Currently no drivers use the ability to override spi_bitbang_transfer()
and if any started this would make it harder to convert the bitbang code
to use transfer_one_message() so remove the export in order to prevent
anyone starting.

The spi core make sure that each transfer structure have the proper
setting for bits_per_word before calling low level transfer APIs.
Hence it is no more require to check again in low level driver for
this field whether this is set correct or not. Removing such code
from low level driver.
The txx9 change also removes a test for bits_per_word set to 0, and
forcing it to 8 in that case. This can also be removed now since
spi_setup() ensures spi->bits_per_word is not zero.

Sort the SPI makefile and enforce the naming convention spi_*.c for
spi drivers.
This change also rolls the contents of atmel_spi.h into the .c file
since there is only one user of that particular include file.
