f16f584350 [S390] dasd: fix timeout handling in interrupt handler
When the dasd_int_handler is called with an error code instead of
an irb, the associated request should be restarted. This handling
was missing from the -ETIMEDOUT case. In fact it should be done in
any case.

64a87b244b [SCSI] Let scsi_cmnd->cmnd use request->cmd buffer
- struct scsi_cmnd had a 16 bytes command buffer of its own.
   This is an unnecessary duplication and copy of request's
   cmd. It is probably left overs from the time that scsi_cmnd
   could function without a request attached. So clean that up.

 - Once above is done, few places, apart from scsi-ml, needed
   adjustments due to changing the data type of scsi_cmnd->cmnd.

 - Lots of drivers still use MAX_COMMAND_SIZE. So I have left
   that #define but equate it to BLK_MAX_CDB. The way I see it
   and is reflected in the patch below is.
   MAX_COMMAND_SIZE - means: The longest fixed-length (*) SCSI CDB
                      as per the SCSI standard and is not related
                      to the implementation.
   BLK_MAX_CDB.     - The allocated space at the request level

 - I have audit all ISA drivers and made sure none use ->cmnd in a DMA
   Operation. Same audit was done by Andi Kleen.

(*)fixed-length here means commands that their size can be determined
   by their opcode and the CDB does not carry a length specifier, (unlike
   the VARIABLE_LENGTH_CMD(0x7f) command). This is actually not exactly
   true and the SCSI standard also defines extended commands and
   vendor specific commands that can be bigger than 16 bytes. The kernel
   will support these using the same infrastructure used for VARLEN CDB's.
   So in effect MAX_COMMAND_SIZE means the maximum size command
   scsi-ml supports without specifying a cmd_len by ULD's

17f3458085 [S390] Convert to SPARSEMEM & SPARSEMEM_VMEMMAP
Convert s390 to SPARSEMEM and SPARSEMEM_VMEMMAP. We do a select
of SPARSEMEM_VMEMMAP since it is configurable. This is because
SPARSEMEM without SPARSEMEM_VMEMMAP gives us a hell of broken
include dependencies that I don't want to fix.

c74c120a21 proc: remove proc_root from drivers
Remove proc_root export.  Creation and removal works well if parent PDE is
supplied as NULL -- it worked always that way.

So, one useless export removed and consistency added, some drivers created
PDEs with &proc_root as parent but removed them as NULL and so on.

30afcb4bd2 return pfn from direct_access, for XIP
Alter the block device ->direct_access() API to work with the new
get_xip_mem() API (that requires both kaddr and pfn are returned).

Some architectures will not do the right thing in their virt_to_page() for use
by XIP (to translate from the kernel virtual address returned by
direct_access(), to a user mappable pfn in XIP's page fault handler.

However, we can't switch it to just return the pfn and not the kaddr, because
we have no good way to get a kva from a pfn, and XIP requires the kva for its
read(2) and write(2) handlers.  So we have to return both.

6d9d63b948 [SCSI] zfcp: Add some statistics provided by the FCP adapter to the sysfs
The new FCP adapter statistics provide a variety of information about
the virtual adapter (subchannel). In order to collect this information
the zfcp driver is extended to query this information.

The information provided by the new FCP adapter statistics can be
fetched by reading from the following files in the sysfs filesystem

/sys/class/scsi_host/host<n>/seconds_active
/sys/class/scsi_host/host<n>/requests
/sys/class/scsi_host/host<n>/megabytes
/sys/class/scsi_host/host<n>/utilization

These are the statistics on a virtual adapter (subchannel) level.

The information provided is raw and not modified or interpreted by any
means.  No interpretation or modification of the values is done by the
zfcp driver.

ec258fe4b7 [SCSI] zfcp: Print some messages only during ERP
When statistics are polled from sysfs, the statistics use the same
commands as the adapter initialization. Change the messages printed
here, so they are only printed during initialization and not for each
poll of adapter data.

aee6ef1859 [SCSI] zfcp: Wait for free SBAL during exchange config
When sending a exchange config data command, wait for a free SBAL.
This does not matter during adapter initialization, but this is
required for pulling adapter statistics during high I/O load.

e976a2b997 s390: KVM guest: virtio device support, and kvm hypercalls
This patch implements kvm guest kernel support for paravirtualized devices
and contains two parts:
o a basic virtio stub using virtio_ring and external interrupts and hypercalls
o full hypercall implementation in kvm_para.h

Currently we dont have PCI on s390. Making virtio_pci usable for s390 seems
more complicated that providing an own stub. This virtio stub is similar to
the lguest one, the memory for the descriptors and the device detection is made
via additional mapped memory on top of the guest storage. We use an external
interrupt with extint code 0x2603 for host->guest notification.

The hypercall definition uses the diag instruction for issuing a hypercall. The
parameters are written in R2-R7, the hypercall number is written in R1. This is
similar to the system call ABI (svc) which can use R1 for the number and R2-R6
for the parameters.

d3135846f6 drivers: Remove unnecessary inclusions of asm/semaphore.h
None of these files use any of the functionality promised by
asm/semaphore.h.  It's possible that they rely on it dragging in some
unrelated header file, but I can't build all these files, so we'll have

a806170e29 [S390] Fix a lot of sparse warnings.
Most noteable part of this commit is the new local header file entry.h
which contains all the function declarations of functions that get only
called from asm code or are arch internal. That way we can avoid extern
declarations in C files.
This is more or less the same that was done for sparc64.

aa24f7f08b [S390] KVM preparation: split sysinfo definitions for kvm use
drivers/s390/sysinfo.c uses the store system information intruction to query
the system about information of the machine, the LPAR and additional
hypervisors. KVM has to implement the host part for this instruction.

To avoid code duplication, this patch splits the common definitions from
sysinfo.c into a separate header file include/asm-s390/sysinfo.h for KVM use.

a695f16729 [S390] vmur: Use wait queue instead of mutex to serialize open
If user space opens a unit record device node then vmur is leaving the kernel
with lock open_mutex still held to prevent other processes from opening the
device simultaneously. This causes lockdep to complain about a lock held when
returning to user space.
Now the mutex is replaced by a wait queue to serialize device open.

92bf435f38 [S390] tape: duplicate sysfs filename when setting tape device online
When a tape device is set online, offline and online again, the following
error message is printed on the console: "sysfs: duplicate filename
'non-rewinding' can not be created". The reason is that when setting a
device online, the tape driver creates a sysfs symlink from the tape device
to the tape class device. Unfortunately the symlink is not removed
correctly, when the device is set offline. Instead of passing the
tape device object to sysfs_remove_link, the class device object is used.
This patch fixes this problem and uses the correct tape device object now.

35b58b028d [S390] qdio: Unrecognized inbound traffic if many FCP devices are online
Problem:
Usually every FCP device has its own indicator field the adapter
uses to signal outstanding work. Once a certain limit of devices
is reached, a common indicator field is used. In certain scenarios
qdio resets this common indicator field, but handles only part of
the FCP-devices sharing the common indicator field. Thus inbound
traffic on the non-processed shared FCP-devices is not recognized
immediately.

Solution:
Make sure common indicator field is reset only, if all FCP-devices
sharing the indicator are processed.

parameter to vt220 write function, so we can let the write function

fe6173d9b3 [S390] cio: Trigger verification on device/path not operational.
Currently, we don't do much on no path or no device situations during
normal user I/O, since we rely on reports regarding those events by
the machine. If we trigger a path verification to bring our device
state up-to-date, we (a) may recover from path failures earlier and
(b) better handle situations where the hardware/hypervisor doesn't
give us enough notifications.

3caa4af834 qeth: keep ip-address after LAN_OFFLINE failure
Problem:     If setting of an ip-address fails with LAN_OFFLINE,
             qeth does not save the ip-address in its internal
             list of set ip-addresses. qeth recovers after a
             following STARTLAN event, but cannot set the unsaved
             ip-address.
Solution:    save the ip-address in the qeth-maintained list of
             ip-addresses after a LAN_OFFLINE failure for SETIP.

Symptom:     slow CCL response time
Problem:     non-ETH_P_802_2 packets are not delivered to NDH for
             CCL. But CCL detects missing sequence numbers, which
             cause a serious performance problem with CCL.
Solution:    assign sequence numbers only to 802.2 packets.

922dc0624e qeth: set lan_online flag after a received STARTLAN
Problem:     A STARTLAN command from the adapter may arrive while a
             qeth recovery is currently running with a failed qeth
             STARTLAN. Usually qeth schedules a recovery when
             receiving a STARTLAN command from the adapter. But
             another recovery scheduled while a recovery is already
             running never starts. Thus the qeth-administered
             lan_online flag remains zero in this scenario, even
             though the adapter-STARTLAN has happened.
Solution:    Set lan_online flag for a received STARTLAN from the
             adapter in case scheduled recovery does not start.

OSA-adapters do not have an address limitation for the qdio queue
structures except the MAX storage level of the current processor.
And due to a recent z/VM APAR there is no longer a restriction to

b75db73159 [SCSI] zfcp: Add qtcb dump to hba debug trace
This patch adds per request hardware debugging data to the trace
record which is written per request. It's a replacement for some sad
kernel message based debugging code. Considering the amount of trace
data, printk() is not suitable for this stuff. Writing binary traces
is more efficient. In addition we got all information in one place.

The QTCB trace data is only dumped for requests other than SCSI
requests.  Otherwise we would flood the trace ring buffer. We are
mostly interested in non-SCSI, recovery related requests here anyway.

This patch also works around a known hardware bug. It truncates QTCB
traces so that we do not save unused areas of the hardware trace.

[based on proposal from Mike Christie <michaelc@cs.wisc.edu>, this
patch adds some simplifications to the handler functions]

With the new target reset handler callback in the SCSI midlayer, the
device reset handler in zfcp can be split in two parts. Now, zfcp does
not have to track anymore whether the device supports LUN resets, so
remove this flag and let the SCSI midlayer decide what to do.

The device reset handler simply issues a LUN reset and the target
reset handler a target reset.

Using the /proc/dasd/devices interface leaves the reference counter
of alias devices in an inconsistent state. A process that tries to set
such a device offline afterwards will hang.

When a request fails that was started on an alias device then the
first recovery step is to retry it on the base device. If the
recovery request fails again with the same symptoms, the next step
should not be a simple retry, but should be a proper recovery based
on sense data, etc. To do so, the dasd recovery functions need to
recognize the alias recovery step in the erp chain by comparing
the start devices.

b32ecdb725 [S390] qdio: fix qdio_activate timeout handling.
Current code in qdio_activate waits for at least 5 seconds
until it returns. It may return earlier if an error occurs,
but not if everything is ok. This large timeout value
became visible with commit dfa77f611f
"qdio: set QDIO_ACTIVATE_TIMEOUT to 5s", which intended to
fix the timeout value which was zero. In turn setting an
FCP adapter online took 5 seconds.

In practice waiting for 5ms before continuing is sufficient
as pointed out by Utz Bacher and Cornelia Huck.

03513bccad [S390] dasd: fix locking in __dasd_device_process_final_queue
After setting the status of the cqr and releasing the lock for the
block cqr queue, we call the cqr callback function, which will usually
just trigger the dasd_block_tasklet. But when the tasklet is already
running the cqr might be processed before we invoke the callback
function. In rare cases the callback pointer may already be invalid
by the time we want to call it, which will result in a panic.
Solution: Call the callback function first and then release the lock.

59eb1ca7a8 [S390] sclp_vt220: Fix vt220 initialization
There are two problems in the vt220 intialization:

o Currently the vt220 console looses early printk events until the
  the vt220 tty is registered.
o console should work if tty_register fails

sclp_vt220_con_init calls __sclp_vt220_init and register_console.
It does not register the driver with the sclp core code via
sclp_register. That results in an sclp_send_mask=0. Therefore,
__sclp_vt220_emit will reject buffers with EIO. Unfortunately
register_console will cause the printk buffer to be sent to the
console and, therefore, every early message gets dropped. The
sclp_send_mask is set later during boot, when sclp_vt220_tty_init
calls sclp_register.

The solution is to move the sclp_register call from sclp_vt220_tty_init
to __sclp_vt220_init. This makes sure that the console is properly
registered with the sclp subsystem before the first log buffer messages
are passed to the vt220 console.

We also adopt the cleanup on error to keep the console alive if
tty_register fails.

bf3f837804 [S390] qdio: avoid hang when establishing qdio queues
If qdio establish runs in parallel with a channel error,
ccw_device_start_timeout may not trigger the qdio_timeout_handler.
In this case neither QDIO_IRQ_STATE_ESTABLISHED nor
QDIO_IRQ_STATE_ERR is reached and the following wait_event hangs
forever.
Solution: do not make use of the timeout option with
ccw_device_start, but add a timeout to the following wait_event.

6c81c32f96 calibrate_delay() must be __cpuinit
calibrate_delay() must be __cpuinit, not __{dev,}init.

I've verified that this is correct for all users.

While doing the latter, I also did the following cleanups:
- remove pointless additional prototypes in C files
- ensure all users #include <linux/delay.h>

c5411dba58 [S390] dcss: Initialize workqueue before using it.
In case a dcss segment cannot be loaded blk_cleanup_queue
will be called before blk_queue_make_request, leaving the
struct work unplug_work of the request queue uninitialized
before it is used.
That leads also to the lockdep message below.
To avoid that call blk_queue_make_request right after the
request_queue has been allocated.
This makes sure that the struct work is always initialized
before it is used.

The problem seems to be, that with a full console buffer, release_console_sem
disables interrupts with spin_lock_irqsave and then calls the console function
without enabling interrupts. __sclp_vt220_write checks for in_interrupt, to
decide if it can schedule. It should check for in_atomic instead.

The same is true for sclp_tty.c.

fe6b8e76d9 [S390] dasd: fix panic caused by alias device offline
When an alias device is set offline while it is in use this may
result in a panic in the cleanup part of the dasd_block_tasklet.
The problem here is that there may exist some ccw requests that were
originally created for the alias device and transferred to the base
device when the alias was set offline. When these request are
cleaned up later, the discipline pointer in the alias device may not
be valid anymore. To fix this use the base device discipline to find
the cleanup function.

6f52ac2971 [S390] cio: make sense id procedure work with partial hardware response
In some cases the current sense id procedure trips over incomplete
hardware responses. In these cases, checking against the preset value
of 0xFFFF is not enough. More critically, the VM DIAG call will always be
considered to have provided data after such an incident, even if it was not
successful at all.

The solution is to always initialize the control unit data before doing a
sense id call. Check the condition code before considering the control unit
data. And initialize again, before evaluating the VM data.

6b175b26c1 [NETNS]: Add netns parameter to inet_(dev_)add_type.
The patch extends the inet_addr_type and inet_dev_addr_type with the
network namespace pointer. That allows to access the different tables
relatively to the network namespace.

The modification of the signature function is reported in all the
callers of the inet_addr_type using the pointer to the well known
init_net.

8e09f21574 [S390] dasd: add hyper PAV support to DASD device driver, part 1
Parallel access volumes (PAV) is a storage server feature, that allows
to start multiple channel programs on the same DASD in parallel. It
defines alias devices which can be used as alternative paths to the
same disk. With the old base PAV support we only needed rudimentary
functionality in the DASD device driver. As the mapping between base
and alias devices was static, we just had to export an identifier
(uid) and could leave the combining of devices to external layers
like a device mapper multipath.
Now hyper PAV removes the requirement to dedicate alias devices to
specific base devices. Instead each alias devices can be combined with
multiple base device on a per request basis. This requires full
support by the DASD device driver as now each channel program itself
has to identify the target base device.
The changes to the dasd device driver and the ECKD discipline are:
- Separate subchannel device representation (dasd_device) from block
  device representation (dasd_block). Only base devices are block
  devices.
- Gather information about base and alias devices and possible
  combinations.
- For each request decide which dasd_device should be used (base or
  alias) and build specific channel program.
- Support summary unit checks, which allow the storage server to
  upgrade / downgrade between base and hyper PAV at runtime (support
  is mandatory).

08d0796827 [S390] Standby cpu activation/deactivation.
Add a new interface so that cpus can be put into standby state and
configured state.
Only offline cpus can be put into standby state or configured state.
For that the new percpu sysfs attribute "configure" must be used.
To put a cpu in standby state a "0" must be written to the attribute.
In order to switch it into configured state a "1" must be written to
the attribute.
Only cpus in configured state can be brought online.
In addition this patch introduces a static mapping of physical to
logical cpus. As a result only the sysfs directories of present cpus
will be created. To scan for new cpus the new sysfs attribute "rescan"
must be used.
Writing to /sys/devices/system/cpu/rescan will trigger a rescan of
cpus and will create directories for new cpus.

On IPL only configured cpus will be used. And on reboot/shutdown all
cpus will remain in their current state (configured/standby).

There are two ways for a ccwgroup device to be unregistered:
Via the ungroup attribute, or when on of the slave devices is killed
by a machine check. As we have to unregister the ccwgroup device

86ef5c9a8e cpu-hotplug: replace lock_cpu_hotplug() with get_online_cpus()
Replace all lock_cpu_hotplug/unlock_cpu_hotplug from the kernel and use
get_online_cpus and put_online_cpus instead as it highlights the
refcount semantics in these operations.

The new API guarantees protection against the cpu-hotplug operation, but
it doesn't guarantee serialized access to any of the local data
structures. Hence the changes needs to be reviewed.

In case of pseries_add_processor/pseries_remove_processor, use
cpu_maps_update_begin()/cpu_maps_update_done() as we're modifying the
cpu_present_map there.

d1ad09db2f [SCSI] zfcp: fix use after free bug.
zfcp_erp_strategy_check_fsfreq() checks if it is safe to access the
fsf_req associated with the erp_action that gets passed. To test if
it is safe it accesses the fsf_req in order to get its index into
the hash list. This is broken since the fsf_req might be freed already
and the read index has no meaning. It could lead to memory corruption.
Fix this by introducing a new zfcp_reqlist_find_safe() method which
just checks if addresses are equal. This is slower, but only gets
called in case of error recovery.

3f48985823 [SCSI] zfcp: Reduce flood on hba trace
Remove tracing for request with a "qualifier" field set in the
response.  The protocol status qualifier now contains measurement
data for "good" commands, so this check would trace every response
by default.

The fix is to simply remove the "qual" tracing: The responses with an
interesting status are also traced as "ferr" or "perr" and all
responses can be traced as "norm" with a higher trace level.

1de1b43b5f [SCSI] zfcp: Fix deadlock when adding invalid LUN
When adding an invalid LUN, there is a deadlock between the add
via scsi_scan_target and the slave_destroy handler: The handler
waits for the scan to complete, but for an invalid unit,
scsi_scan_target directly calls the slave_destroy handler.

Fix the deadlock by removing the wait in the slave_destroy
handler, it was not necessary anyway.

The common I/O layer can call remove a handler to inform zfcp
that a device disappeared. The handler zfcp_ccw_remove then

Recently, Wang Chen submitted a patch
(d30f53aeb3) to move a call to netif_rx(skb)
after a subsequent reference to skb, because netif_rx may call kfree_skb on
its argument.  netif_rx_ni calls netif_rx, so the same problem occurs in
the files below.

I have left the updating of dev->last_rx after the calls to netif_rx_ni
because it seems time dependent, but moved the other field updates before.

When loading a dcss segment with the dcssblk driver, sometimes the
following kind of message appears:

Buffer I/O error on device dcssblk0, logical block 172016

The fix is to move the disk registration after setting the
make_request function, to avoid calls into generic_make_request
for dcssblock without having the make_request function set up
properly.

c94dec99f9 [S390] cio: Issue SenseID per path.
We may receive a unit check for every path when we issue a SenseID.
Unfortunately, the channel subsystem will try on a different path
every time if we use a lpm of 0xff, which will exhaust our retry
counter.

Therefore, revert SenseID to its previous per-path behaviour and
just leave out the suspend multipath reconnect.

erp_running_head. Instead it should just go through the down() operation,
which balances the first up(), and run through zfcp_erp_strategy one more
time for the second up(), which eventually cleans up erp_action. Which
is similar to the normal processing of an event for erp_action doing
something asynchronously (e.g. waiting for the completion of an fsf_req).

This only works if we make sure that a dismissed erp_action is passed to
zfcp_erp_strategy() prior to the other action, which caused actions to be
dismissed. Therefore the patch implements this rule: running actions go to
the head of the ready list; new actions go to the tail of the ready list;
the erp thread picks actions to be processed from the ready list's head.

d0076f7754 [SCSI] zfcp: fix dismissal of error recovery actions
zfcp_erp_action_dismiss() used to ignore any actions in the ready list. This
is a bug. Any action superseded by a stronger action needs to be dismissed.
This patch changes zfcp_erp_action_dismiss() so that it dismisses actions
regardless of their list affiliation. The ERP thread is able to handle this.
It is important to kick the erp thread only for actions in the running list,
though, as an imbalance of wakeup signals would confuse the erp thread
otherwise.
