Add DT nodes for Tegra USB PHY along with related documentation.
Also added a phandle property to controller DT node, for referring
to connected PHY instance.

As Tegra USB host driver is using instance number for resetting
PORT0 twice, adding a new DT property for handling this.

Rename all Tegra binding documentation according to the first or
primary compatible value they describe. This provides a clear naming
scheme for the files, and prevents any naming conflicts should future
SoC versions require their own binding.
