f669b8a683 scsi: core: Fix the scsi_set_resid() documentation
Because scsi_finish_command() subtracts the residual from the buffer
length, residual overflows must not be reported. Reflect this in the SCSI
documentation. See also commit 9237f04e12 ("scsi: core: Fix
scsi_get/set_resid() interface")

deef1be18e scsi: core: Remove reserved request time-out handling
The SCSI core code does not currently support reserved commands. As such,
requests which time-out would never be reserved, and scsi_timeout()
'reserved' arg should never be set.

Remove handling for reserved requests, drop the wrapper scsi_timeout()
as it now just calls scsi_times_out() always, and finally rename
scsi_times_out() -> scsi_timeout() to match the blk_mq_ops method name.

Commit 91ebc1facd ("scsi: core: remove Scsi_Cmnd typedef") removed the
Scsi_cmnd typedef but it was still mentioned in a paragraph in the "SCSI
mid_level - lower_level driver interface" documentation page.  Remove this
obsolete paragraph.
