e96d9a938e nvdimm/pfn_dev: Prevent the creation of zero-sized namespaces
On architectures that have different page size values used for kernel
direct mapping and userspace mappings, the user can end up creating zero-sized
namespaces as shown below

:/sys/bus/nd/devices/region1# cat align

    "align":65536
  },
  "align":65536
}
similarily for fsdax

 $ ndctl create-namespace -r region1 -m fsdax  -s 18M --align 64K
{
  "dev":"namespace1.0",
  "mode":"fsdax",
  "map":"dev",
  "size":0,

  "sector_size":512,
  "align":65536,
  "blockdev":"pmem1"
}

In commit 9ffc1d19fc ("mm/memremap_pages: Introduce memremap_compat_align()")
memremap_compat_align was added to make sure the kernel always creates
namespaces with 16MB alignment. But the user can still override the
region alignment and no input validation is done in the region alignment
values to retain the flexibility user had before. However, the kernel
ensures that only part of the namespace that can be mapped via kernel
direct mapping page size is enabled. This is achieved by tracking the
unmapped part of the namespace in pfn_sb->end_trunc. The kernel also
ensures that the start address of the namespace is also aligned to the
kernel direct mapping page size.

Depending on the user request, the kernel implements userspace mapping
alignment by updating pfn device alignment attribute and this value is
used to adjust the start address for userspace mappings. This is tracked
in pfn_sb->dataoff. Hence the available size for userspace mapping is:

usermapping_size = size of the namespace - pfn_sb->end_trun - pfn_sb->dataoff

If the kernel finds the user mapping size zero then don't allow the
creation of namespace.

After fix:
$ ndctl create-namespace -f  -r region1 -m devdax  -s 18M --align 64K
libndctl: ndctl_dax_enable: dax1.1: failed to enable
  Error: namespace1.2: failed to enable

failed to create namespace: No such device or address

And existing zero sized namespace will be marked disabled.

fd774e36fe nvdimm: Explicitly include correct DT includes
The DT of_device.h and of_platform.h date back to the separate
of_platform_bus_type before it as merged into the regular platform bus.
As part of that merge prepping Arm DT support 13 years ago, they
"temporarily" include each other. They also include platform_device.h
and of.h. As a result, there's a pretty much random mix of those include
files used throughout the tree. In order to detangle these headers and
replace the implicit includes with struct declarations, users need to
explicitly include the correct includes.

The root cause is that submit_bio_noacct() needs bio_op() is either
WRITE or ZONE_APPEND for flush bio and async_pmem_flush() doesn't assign
REQ_OP_WRITE when allocating flush bio, so submit_bio_noacct just fail
the flush bio.

Simply fix it by adding the missing REQ_OP_WRITE for flush bio. And we
could fix the flush order issue and do flush optimization later.

44f23dabdc nvdimm: Use kstrtobool() instead of strtobool()
strtobool() is the same as kstrtobool().

1ea7ca1b09 dax: enable dax fault handler to report VM_FAULT_HWPOISON
When multiple processes mmap() a dax file, then at some point,
a process issues a 'load' and consumes a hwpoison, the process
receives a SIGBUS with si_code = BUS_MCEERR_AR and with si_lsb
set for the poison scope. Soon after, any other process issues
a 'load' to the poisoned page (that is unmapped from the kernel
side by memory_failure), it receives a SIGBUS with
si_code = BUS_ADRERR and without valid si_lsb.

This is confusing to user, and is different from page fault due
to poison in RAM memory, also some helpful information is lost.

Channel dax backend driver's poison detection to the filesystem
such that instead of reporting VM_FAULT_SIGBUS, it could report
VM_FAULT_HWPOISON.

If user level block IO syscalls fail due to poison, the errno will
be converted to EIO to maintain block API consistency.

Pull Compute Express Link (CXL) updates from Dan Williams:
 "To date Linux has been dependent on platform-firmware to map CXL RAM
  regions and handle events / errors from devices. With this update we
  can now parse / update the CXL memory layout, and report events /
  errors from devices. This is a precursor for the CXL subsystem to
  handle the end-to-end "RAS" flow for CXL memory. i.e. the flow that
  for DDR-attached-DRAM is handled by the EDAC driver where it maps
  system physical address events to a field-replaceable-unit (FRU /
  endpoint device). In general, CXL has the potential to standardize
  what has historically been a pile of memory-controller-specific error
  handling logic.

  Another change of note is the default policy for handling RAM-backed
  device-dax instances. Previously the default access mode was "device",
  mmap(2) a device special file to access memory. The new default is
  "kmem" where the address range is assigned to the core-mm via
  add_memory_driver_managed(). This saves typical users from wondering
  why their platform memory is not visible via free(1) and stuck behind
  a device-file. At the same time it allows expert users to deploy
  policy to, for example, get dedicated access to high performance
  memory, or hide low performance memory from general purpose kernel
  allocations. This affects not only CXL, but also systems with
  high-bandwidth-memory that platform-firmware tags with the
  EFI_MEMORY_SP (special purpose) designation.

  Summary:

   - CXL RAM region enumeration: instantiate 'struct cxl_region' objects
     for platform firmware created memory regions

   - CXL RAM region provisioning: complement the existing PMEM region

a93e884edf Driver core changes for 6.3-rc1
Here is the large set of driver core changes for 6.3-rc1.

 There's a lot of changes this development cycle, most of the work falls
 into two different categories:
   - fw_devlink fixes and updates. This has gone through numerous review
     cycles and lots of review and testing by lots of different devices.
     Hopefully all should be good now, and Saravana will be keeping a
     watch for any potential regression on odd embedded systems.
   - driver core changes to work to make struct bus_type able to be moved
     into read-only memory (i.e. const) The recent work with Rust has
     pointed out a number of areas in the driver core where we are
     passing around and working with structures that really do not have
     to be dynamic at all, and they should be able to be read-only making
     things safer overall. This is the contuation of that work (started
     last release with kobject changes) in moving struct bus_type to be
     constant. We didn't quite make it for this release, but the
     remaining patches will be finished up for the release after this
     one, but the groundwork has been laid for this effort.

 Other than that we have in here:
   - debugfs memory leak fixes in some subsystems
   - error path cleanups and fixes for some never-able-to-be-hit
     codepaths.
   - cacheinfo rework and fixes
   - Other tiny fixes, full details are in the shortlog

 All of these have been in linux-next for a while with no reported
 problems.

3222d8c2a7 block: remove ->rw_page
The ->rw_page method is a special purpose bypass of the usual bio handling
path that is limited to single-page reads and writes and synchronous which
causes a lot of extra code in the drivers, callers and the block layer.

The only remaining user is the MM swap code. Switch that swap code to
simply submit a single-vec on-stack bio an synchronously wait on it based
on a newly added QUEUE_FLAG_SYNCHRONOUS flag set by the drivers that
currently implement ->rw_page instead. While this touches one extra cache
line and executes extra code, it simplifies the block layer and drivers
and ensures that all feastures are properly supported by all drivers, e.g.
right now ->rw_page bypassed cgroup writeback entirely.

c91d713630 nvdimm: Support sizeof(struct page) > MAX_STRUCT_PAGE_SIZE
Commit 6e9f05dc66 ("libnvdimm/pfn_dev: increase MAX_STRUCT_PAGE_SIZE")

...updated MAX_STRUCT_PAGE_SIZE to account for sizeof(struct page)
potentially doubling in the case of CONFIG_KMSAN=y. Unfortunately this
doubles the amount of capacity stolen from user addressable capacity for
everyone, regardless of whether they are using the debug option. Revert
that change, mandate that MAX_STRUCT_PAGE_SIZE never exceed 64, but
allow for debug scenarios to proceed with creating debug sized page maps
with a compile option to support debug scenarios.

Note that this only applies to cases where the page map is permanent,
i.e. stored in a reservation of the pmem itself ("--map=dev" in "ndctl
create-namespace" terms). For the "--map=mem" case, since the allocation
is ephemeral for the lifespan of the namespace, there are no explicit
restriction. However, the implicit restriction, of having enough
available "System RAM" to store the page map for the typically large
pmem, still applies.

dc370b28c8 nvdimm/region: Move cache management to the region driver
Now that cpu_cache_invalidate_memregion() is generically available, use

sensitive dirty data in the cache being written back to media after a

15a8348707 libnvdimm: Introduce CONFIG_NVDIMM_SECURITY_TEST flag
nfit_test overrode the security_show() sysfs attribute function in nvdimm
dimm_devs in order to allow testing of security unlock. With the
introduction of CXL security commands, the trick to override
security_show() becomes significantly more complicated. By introdcing a
security flag CONFIG_NVDIMM_SECURITY_TEST, libnvdimm can just toggle the
check via a compile option. In addition the original override can can be
removed from tools/testing/nvdimm/.

The flag will also be used to bypass cpu_cache_invalidate_memregion() when
set in a different commit. This allows testing on QEMU with nfit_test or
cxl_test since cpu_cache_has_invalidate_memregion() checks whether
X86_FEATURE_HYPERVISOR cpu feature flag is set on x86.

6e9f05dc66 libnvdimm/pfn_dev: increase MAX_STRUCT_PAGE_SIZE
KMSAN adds extra metadata fields to struct page, so it does not fit into
64 bytes anymore.

This change leads to increased memory consumption of the nvdimm driver,
regardless of whether the kernel is built with KMSAN or not.

2e5021cc42 libnvdimm/region: Allow setting align attribute on regions without mappings
The alignment constraint for namespace creation in a region was
increased, from 2M to 16M, for non-PowerPC architectures in v5.7 with
commit 2522afb86a ("libnvdimm/region: Introduce an 'align'
attribute"). The thought behind the change was that region alignment
should be uniform across all architectures and, since PowerPC had the
largest alignment constraint of 16M, all architectures should conform to
that alignment.

The change regressed namespace creation in pre-defined regions that
relied on 2M alignment but a workaround was provided in the form of a
sysfs attribute, named 'align', that could be adjusted to a non-default
alignment value.

However, the sysfs attribute's store function returned an error (-ENXIO)
when userspace attempted to change the alignment of a region that had no
mappings. This affected 2M aligned regions of volatile memory that were
defined in a device tree using "pmem-region" and created by the
of_pmem_region_driver, since those regions do not contain mappings
(ndr_mappings is 0).

Allow userspace to set the align attribute on pre-existing regions that
do not have mappings so that namespaces can still be within those
regions, despite not being aligned to 16M.

5d66322b2b virtio_pmem: set device ready in probe()
The NVDIMM region could be available before the virtio_device_ready()
that is called by virtio_dev_probe(). This means the driver tries to
use device before DRIVER_OK which violates the spec, fixing this by
set device ready before the nvdimm_pmem_region_create().

Note that this means the virtio_pmem_host_ack() could be triggered
before the creation of the nd region, this is safe since the pmem_lock
has been initialized and whether or not any available buffer is added
before is validated by virtio_pmem_host_ack().

020e1aede7 virtio_pmem: initialize provider_data through nd_region_desc
We used to initialize the provider_data manually after
nvdimm_pemm_region_create(). This seems to be racy if the flush is
issued before the initialization of provider_data[1]. Fixing this by
initializing the provider_data through nd_region_desc to make sure the
provider_data is ready after the pmem is created.

 "Most of the MM queue. A few things are still pending.

  Liam's maple tree rework didn't make it. This has resulted in a few
  other minor patch series being held over for next time.

  Multi-gen LRU still isn't merged as we were waiting for mapletree to
  stabilize. The current plan is to merge MGLRU into -mm soon and to
  later reintroduce mapletree, with a view to hopefully getting both
  into 6.1-rc1.

  Summary:

   - The usual batches of cleanups from Baoquan He, Muchun Song, Miaohe
     Lin, Yang Shi, Anshuman Khandual and Mike Rapoport

   - Some kmemleak fixes from Patrick Wang and Waiman Long

   - DAMON updates from SeongJae Park

   - memcg debug/visibility work from Roman Gushchin

   - vmalloc speedup from Uladzislau Rezki

   - more folio conversion work from Matthew Wilcox

   - enhancements for coherent device memory mapping from Alex Sierra

   - addition of shared pages tracking and CoW support for fsdax, from
     Shiyang Ruan

   - hugetlb optimizations from Mike Kravetz

   - Mel Gorman has contributed some pagealloc changes to improve
     latency and realtime behaviour.

   - mprotect soft-dirty checking has been improved by Peter Xu

   - Many other singleton patches all over the place"

04ad63f086 cxl/region: Introduce cxl_pmem_region objects
The LIBNVDIMM subsystem is a platform agnostic representation of system
NVDIMM / persistent memory resources. To date, the CXL subsystem's
interaction with LIBNVDIMM has been to register an nvdimm-bridge device
and cxl_nvdimm objects to proxy CXL capabilities into existing LIBNVDIMM
subsystem mechanics.

With regions the approach is the same. Create a new cxl_pmem_region
object to proxy CXL region details into a LIBNVDIMM definition. With
this enabling LIBNVDIMM can partition CXL persistent memory regions with
legacy namespace labels. A follow-on patch will add CXL region label and
CXL namespace label support to persist region configurations across
driver reload / system-reset events.

33a8f7f2b3 pagemap,pmem: introduce ->memory_failure()
When memory-failure occurs, we call this function which is implemented by
each kind of devices. For the fsdax case, pmem device driver implements
it. Pmem device driver will find out the filesystem in which the
corrupted page located in.

With dax_holder notify support, we are able to notify the memory failure
from pmem driver to upper layers. If there is something not support in
the notify routine, memory_failure will fall back to the generic hanlder.

Pull cxl updates from Dan Williams:
 "Compute Express Link (CXL) updates for this cycle.

  The highlight is new driver-core infrastructure and CXL subsystem
  changes for allowing lockdep to validate device_lock() usage. Thanks
  to PeterZ for setting me straight on the current capabilities of the
  lockdep API, and Greg acked it as well.

  On the CXL ACPI side this update adds support for CXL _OSC so that
  platform firmware knows that it is safe to still grant Linux native
  control of PCIe hotplug and error handling in the presence of CXL
  devices. A circular dependency problem was discovered between suspend
  and CXL memory for cases where the suspend image might be stored in
  CXL memory where that image also contains the PCI register state to
  restore to re-enable the device. Disable suspend for now until an
  architecture is defined to clarify that conflict.

  Lastly a collection of reworks, fixes, and cleanups to the CXL
  subsystem where support for snooping mailbox commands and properly
  handling the "mem_enable" flow are the highlights.

  Summary:

   - Add driver-core infrastructure for lockdep validation of

f42e8e5088 pmem: implement pmem_recovery_write()
The recovery write thread started out as a normal pwrite thread and
when the filesystem was told about potential media error in the
range, filesystem turns the normal pwrite to a dax_recovery_write.

The recovery write consists of clearing media poison, clearing page
HWPoison bit, reenable page-wide read-write permission, flush the
caches and finally write. A competing pread thread will be held
off during the recovery process since data read back might not be
valid, and this is achieved by clearing the badblock records after
the recovery write is complete. Competing recovery write threads
are already serialized by writer lock held by dax_iomap_rw().

5898b43af9 mce: fix set_mce_nospec to always unmap the whole page
The set_memory_uc() approach doesn't work well in all cases.
As Dan pointed out when "The VMM unmapped the bad page from

"The guest gets virtual #MC on an access to that page. When

bb7bf697fe nvdimm: Allow overwrite in the presence of disabled dimms
It is not clear why the original implementation of overwrite support
required the dimm driver to be active before overwrite could proceed. In
fact that can lead to cases where the kernel retains an invalid cached
copy of the labels from before the overwrite. Unfortunately the kernel
has not only allowed that case, but enforced it.

Going forward, allow for overwrite to happen while the label area is
offline, and follow-on with updates to 'ndctl sanitize-dimm --overwrite'
to trigger the label area invalidation by default.

e6829d1bd3 nvdimm: Fix firmware activation deadlock scenarios
Lockdep reports the following deadlock scenarios for CXL root device
power-management, device_prepare(), operations, and device_shutdown()
operations for 'nd_region' devices:

 Chain exists of:
   &nvdimm_region_key --> &nvdimm_bus->reconfig_mutex --> system_transition_mutex

  Possible unsafe locking scenario:

        CPU0                    CPU1
        ----                    ----
   lock(system_transition_mutex);
                                lock(&nvdimm_bus->reconfig_mutex);
                                lock(system_transition_mutex);
   lock(&nvdimm_region_key);

 Chain exists of:
   &cxl_nvdimm_bridge_key --> acpi_scan_lock --> &cxl_root_key

  Possible unsafe locking scenario:

        CPU0                    CPU1
        ----                    ----
   lock(&cxl_root_key);
                                lock(acpi_scan_lock);
                                lock(&cxl_root_key);
   lock(&cxl_nvdimm_bridge_key);

These stem from holding nvdimm_bus_lock() over hibernate_quiet_exec()
which walks the entire system device topology taking device_lock() along
the way. The nvdimm_bus_lock() is protecting against unregistration,
multiple simultaneous ops callers, and preventing activate_show() from
racing activate_store(). For the first 2, the lock is redundant.
Unregistration already flushes all ops users, and sysfs already prevents
multiple threads to be active in an ops handler at the same time. For
the last userspace should already be waiting for its last
activate_store() to complete, and does not need activate_show() to flush
the write side, so this lock usage can be deleted in these attributes.

4a0079bc7a nvdimm: Replace lockdep_mutex with local lock classes
In response to an attempt to expand dev->lockdep_mutex for device_lock()

the ability to assign a dedicated lock class per subsystem device-type.

Use lockdep_set_class() to override the default device_lock()
'__lockdep_no_validate__' class for each NVDIMM subsystem device-type. This
enables lockdep to detect deadlocks and recursive locking within the
device-driver core and the subsystem.

de7a9e949f drivers/nvdimm: Fix build failure when CONFIG_PERF_EVENTS is not set
The following build failure occurs when CONFIG_PERF_EVENTS is not set
as generic pmu functions are not visible in that scenario.

|-- s390-randconfig-r044-20220313
|   |-- nd_perf.c:(.text):undefined-reference-to-perf_pmu_migrate_context
|   |-- nd_perf.c:(.text):undefined-reference-to-perf_pmu_register
|   `-- nd_perf.c:(.text):undefined-reference-to-perf_pmu_unregister

Similar build failure in nds32 architecture:
nd_perf.c:(.text+0x21e): undefined reference to `perf_pmu_migrate_context'
nd_perf.c:(.text+0x434): undefined reference to `perf_pmu_register'
nd_perf.c:(.text+0x57c): undefined reference to `perf_pmu_unregister'

Fix this issue by adding check for CONFIG_PERF_EVENTS config option
and disabling the nvdimm perf interface incase this config is not set.

Also remove function declaration of perf_pmu_migrate_context,
perf_pmu_register, perf_pmu_unregister functions from nd.h as these are
common pmu functions which are part of perf_event.h and since we
are disabling nvdimm perf interface incase CONFIG_PERF_EVENTS option
is not set, we not need to declare them in nd.h

Also move the platform_device header file addition part from nd.h to
nd_perf.c and add stub functions for register_nvdimm_pmu and
unregister_nvdimm_pmu functions to handle CONFIG_PERF_EVENTS=n
case.

f8669f1d6a nvdimm/blk: Delete the block-aperture window driver
Block Aperture Window support was an attempt to layer an error model
over PMEM for platforms that did not support machine-check-recovery.
However, it was abandoned before it ever shipped, and only ever existed
in the ACPI specification. Meanwhile Linux has carried a large pile of
dead code for non-shipping infrastructure. For years it has been off to
the side out of the way, but now CXL and recent directions with DAX
support have the potential to collide with this code.

In preparation for adding discontiguous namespace support, a
pre-requisite for the nvdimm subsystem to replace device-mapper for
striping + concatenation use cases, delete BLK aperture support.

On the obscure chance that some hardware vendor shipped support for this
mode, note that the driver will still keep BLK space reserved in the
label area. So an end user in this case would still have the opportunity
to report the regression to get BLK-mode support restored without
risking the data they have on that device.

3c5b903955 cxl: Prove CXL locking
When CONFIG_PROVE_LOCKING is enabled the 'struct device' definition gets
an additional mutex that is not clobbered by
lockdep_set_novalidate_class() like the typical device_lock(). This
allows for local annotation of subsystem locks with mutex_lock_nested()
per the subsystem's object/lock hierarchy. For CXL, this primarily needs
the ability to lock ports by depth and child objects of ports by their
parent parent-port lock.

dc104f4bb2 nvdimm/blk: add error handling support for add_disk()
We never checked for errors on add_disk() as this function
returned void. Now that this is fixed, use the shiny new
error handling.

Since nvdimm/blk uses devm we just need to move the devm
registration towards the end. And in hindsight, that seems
to also provide a fix given del_gendisk() should not be
called unless the disk was already added via add_disk().
The probably of that issue happening is low though, like
OOM while calling devm_add_action(), so the fix is minor.

We manually unwind in case of add_disk() failure prior
to the devm registration.

b7421afcec nvdimm/blk: avoid calling del_gendisk() on early failures
If nd_integrity_init() fails we'd get del_gendisk() called,
but that's not correct as we should only call that if we're
done with device_add_disk(). Fix this by providing unwinding
prior to the devm call being registered and moving the devm
registration to the very end.

This should fix calling del_gendisk() if nd_integrity_init()
fails. I only spotted this issue through code inspection. It
does not fix any real world bug.

3dd60fb9d9 nvdimm/pmem: stop using q_usage_count as external pgmap refcount
Originally all DAX access when through block_device operations and thus
needed a queue reference. But since commit cccbce6715
("filesystem-dax: convert to dax_direct_access()") all this happens at
the DAX device level which uses its own refcounting. Having the external
refcount thus wasn't needed but has otherwise been harmless for long
time.

But now that "block: drain file system I/O on del_gendisk" waits for
q_usage_count to reach 0 in del_gendisk this whole scheme can't work
anymore (and pmem is the only driver abusing q_usage_count like that).
So switch to the internal reference and remove the unbalanced
blk_freeze_queue_start that is taken care of by del_gendisk.

3e08773c38 block: switch polling to be bio based
Replace the blk_poll interface that requires the caller to keep a queue
and cookie from the submissions with polling based on the bio.

Polling for the bio itself leads to a few advantages:

 - the cookie construction can made entirely private in blk-mq.c
 - the caller does not need to remember the request_queue and cookie
   separately and thus sidesteps their lifetime issues
 - keeping the device and the cookie inside the bio allows to trivially
   support polling BIOs remapping by stacking drivers
 - a lot of code to propagate the cookie back up the submission path can
   be removed entirely.

e765f13ed1 nvdimm/pmem: move dax_attribute_group from dax to pmem
dax_attribute_group is only used by the pmem driver, and can avoid the
completely pointless lookup by the disk name if moved there. This
leaves just a single caller of dax_get_by_host, so move dax_get_by_host
into the same ifdef block as that caller.

5af96835e4 libnvdimm/labels: Introduce CXL labels
Now that all of use sites of label data have been converted to nsl_*
helpers, introduce the CXL label format. The ->cxl flag in
nvdimm_drvdata indicates the label format the device expects. A
follow-on patch allows a bus provider to select the label style.

Note that the EFI definition of the labels represents the Linux "claim
class" with a GUID. The CXL definition of the labels stores the same
identifier in UUID byte order.

42e192aa98 libnvdimm/labels: Introduce the concept of multi-range namespace labels
The CXL specification defines a mechanism for namespaces to be comprised
of multiple dis-contiguous ranges. Introduce that concept to the legacy
NVDIMM namespace implementation with a new nsl_set_nrange() helper, that
sets the number of ranges to 1. Once the NVDIMM subsystem supports CXL
labels and updates its namespace capacity provisioning for
dis-contiguous support nsl_set_nrange() can be updated, but in the
meantime CXL label validation requires nrange be non-zero.
