The 8020i protocol (also 8070i and QIC-157) uses 12-byte commands;
shorter commands must be padded. Simon Detheridge reports that his
3-TB USB disk drive claims to use the 8020i protocol (which is
normally meant for ATAPI devices like CD drives), and because of its
large size, the disk drive requires the use of 16-byte commands.
However the usb_stor_pad12_command() routine in usb-storage always
sets the command length to 12, making the drive impossible to use.
Since the SFF-8020i specification allows for 16-byte commands in
future extensions, we may as well accept them. This patch (as1490)
changes usb_stor_pad12_command() to leave commands larger than 12
bytes alone rather than truncating them.

Below are codes for accessing usb sysif_regs in driver:
usb_sys_regs = (struct usb_sys_interface *)
((u32)dr_regs + USB_DR_SYS_OFFSET);
these codes work in 32-bit, but in 64-bit, use u32 to type cast the address

call trace, so use (void *) for both 32-bit and 64-bit.

There's a race between the USB disconnect handler and the TTY close
handler which may cause the acm object to be freed while it's still
being used. This may lead to things like
http://article.gmane.org/gmane.linux.usb.general/54250
and
https://lkml.org/lkml/2011/5/29/64
This is the simplest fix I could come up with. Holding on to open_mutex
while closing the TTY device prevents acm_disconnect() from freeing the
acm object between acm->port.count drops to 0 and the TTY side of the
cleanups are finalized.

The current TT scheduling doesn't allow to play and then record on a
full-speed device connected to a high speed hub.
The IN iso stream can only start on the first uframe (0-2 for a 165 us)
because of CSPLIT transactions.
For the OUT iso stream there no such restriction. uframe 0-5 are possible.
The idea of this patch is that the first uframe are precious (for IN TT iso
stream) and we should allocate the last uframes first if possible.
For that we reverse the order of uframe allocation (last uframe first).
Here an example :
hid interrupt stream
----------------------------------------------------------------------
uframe | 0 | 1 | 2 | 3 | 4 | 5 | 6 | 7 |
----------------------------------------------------------------------
max_tt_usecs | 125 | 125 | 125 | 125 | 125 | 125 | 30 | 0 |
----------------------------------------------------------------------
used usecs on a frame | 13 | 0 | 0 | 0 | 0 | 0 | 0 | 0 |
----------------------------------------------------------------------
iso OUT stream
----------------------------------------------------------------------
uframe | 0 | 1 | 2 | 3 | 4 | 5 | 6 | 7 |
----------------------------------------------------------------------
max_tt_usecs | 125 | 125 | 125 | 125 | 125 | 125 | 30 | 0 |
----------------------------------------------------------------------
used usecs on a frame | 13 | 125 | 39 | 0 | 0 | 0 | 0 | 0 |
----------------------------------------------------------------------
There no place for iso IN stream (uframe 0-2 are used) and we got "cannot
submit datapipe for urb 0, error -28: not enough bandwidth" error.
With the patch this become.
iso OUT stream
----------------------------------------------------------------------
uframe | 0 | 1 | 2 | 3 | 4 | 5 | 6 | 7 |

max_tt_usecs | 125 | 125 | 125 | 125 | 125 | 125 | 30 | 0 |

This patch (as1494) fixes a problem in xhci-hcd's resume routine.
When the controller is runtime-resumed, this can only mean that one of
the two root hubs has made a wakeup request and therefore needs to be
resumed as well. Rather than try to determine which root hub requires
attention (which might be difficult in the case where a new
non-SuperSpeed device has been plugged in), the patch simply resumes
both root hubs.
Without this change, there is a race: The controller might be put back
to sleep before it can activate its IRQ line, and the wakeup condition
might never get handled.
The patch also simplifies the logic in xhci_resume a little, combining
some repeated flag settings into a single pair of statements.

This patch (as1491) works around a bug in GCC-3.4.6, which is still
supposed to be supported. The number of microseconds in the udelay()
call in quirk_usb_disable_ehci() is fixed at 100, but the compiler
doesn't understand this and generates a link-time error. So we
replace the otherwise unused variable "delta" with a simple constant
100. This same pattern is already used in other delay loops in that
source file.

The inline documentation of _gadget_stop_activity() states that the
function should be called holding the udc->lock. This however will
result in a deadlock, because _gadget_stop_activity() takes the udc->lock.
During normal operation _gadget_stop_activity() is always called unlocked,
but in ci13xxx_stop() it's called locked, this results in the following
deadlock during rmmod of a gadget driver.
This patch fixes the deadlock by calling _gadget_stop_activity() always
unlocked, the inline documentation is adjusted accordingly.

The current driver sets the request's dma addr (mReq->req.dma) to 0 to
mark the DMA address as not valid. However some gadget drivers
(e.g. gadgetfs) set the request's dma addr to DMA_ADDR_INVALID to mark
the address as invalid. This leads to bogus data send because the
ci13xxx_udc driver assumes the request has already been mapped.
This patch fixes the problem, by using DMA_ADDR_INVALID instead of 0
to mark the request's DMA address as invalid.

The dynamic jack allocation of the MIDI gadget currently links all
external jacks to one single instance of an embedded jack. According to
the spec, this is only valid if these streams always carry the same data
stream, as described in the USB MIDI 1.0 spec, chapter 3.3.1.
Also, genius Windows 7(tm) terminates it's life cycle instantly with a
blue screen of death once a device with more than one input and output
port with the current implementation is connected.

commit 27641c3f (drm/vblank: Add support for precise vblank

section with the comments:
* Disable preemption, so vblank_time_lock is held as short as
* possible, even under a kernel with PREEMPT_RT patches.
/* Disable preemption while holding vblank_time_lock. Do
* it explicitely to guard against PREEMPT_RT kernel.
Just that this has never been tested on a RT kernel which would have
granted that nonsense with a might_sleep() warning because
dev->vblank_time_lock is converted to a "sleeping" spinlock on RT.

Commit 31a3ddda16 introduced
a use after free in virtio-pci. The main issue is
that the release method signals removal of the virtio device,
while remove signals removal of the pci device.
For example, on driver removal or hot-unplug,
virtio_pci_release_dev is called before virtio_pci_remove.
We then might get a crash as virtio_pci_remove tries to use the
device freed by virtio_pci_release_dev.
We allocate/free all resources together with the
pci device, so we can leave the release method empty.

After commit e978aa7d7d ("cpuidle: Move dev->last_residency update to
driver enter routine; remove dev->last_state") setting acpi_idle_suspend
to 1 by acpi_processor_suspend() causes the ACPI cpuidle routines to
return error codes continuously, which in turn causes cpuidle to lock up
(hard).
However, acpi_idle_suspend doesn't appear to be useful for any
particular purpose (it's racy and doesn't really provide any real
protection), so it can be removed, which makes the problem go away.

Real world year equals the value in vrtc YEAR register plus an offset.
We used 1960 as the offset to make leap year consistent, but for a
device's first use, its YEAR register is 0 and the system year will
be parsed as 1960 which is not a valid UNIX time and will cause many
applications to fail mysteriously. So we use 1972 instead to fix this
issue.

Aacraid controller can hang on some nodes if kernel uses non-default
(powersave) ASPM policy. Controller hangs shortly after successful load and
hardware detection. Scsi error handler detects this hang and tries to restart
hardware but it does not help.
Initially it was noticed on RHEL6-based openVZ kernel after backporting
aacraid driver from mainline (RHEL6 kernel with original driver works well)
http://bugzilla.openvz.org/show_bug.cgi?id=2043
This issue happens because default ASPM policy was changed in Red Hat
kernels. Therefore guys from Red Hat have noticed this problem long time ago:
on Fedora 12
https://bugzilla.redhat.com/show_bug.cgi?id=540478
on Fedora 14
https://bugzilla.redhat.com/show_bug.cgi?id=679385
In RHEL6 kernel this issue was fixed, ASPM was disabled in aacraid driver. In
kernel changelog I've found that seems it was done by Matthew Garrett: -
[scsi] aacraid: Disable ASPM by default (Matthew Garrett) [599735]
However seems this patch was not submitted to mainline. I've reproduced this
issue on vanilla 3.1.0 kernel booted with "pcie_aspm.policy=powersave" option,
So I believe it makes sense to do it now.

drm_wait_vblank must be DRM_UNLOCKED because otherwise it
will grab the drm_global_mutex and then go to sleep until the vblank
event it is waiting for. That can wreck havoc in the windowing system
because if one process issues this ioctl, it will block all other
processes for the duration of all vblanks between the current and the
one it is waiting for. In some cases it can block the entire windowing
system.
v2: incorporate comments received from Daniel Vetter and
Michel Daenzer.
v3/v4: after a lengty discussion with Daniel Vetter, it was concluded
that the only thing not yet protected with locks and atomic
ops is the write to dev->last_vblank_wait. It's only used in a
debug file in proc, and the current code already employs no
correct locking: the proc file only takes dev->struct_mutex,
whereas drm_wait_vblank implicitly took the drm_global_mutex.
Given all this, it's not worth bothering to try to fix
the locks at this time.

This was only the case if the GPU reset was triggered from the CS ioctl,
otherwise other processes could happily enter the CS ioctl and wreak havoc
during the GPU reset.
This is a little complicated because the GPU reset can be triggered from the
CS ioctl, in which case we're already holding the mutex, or from other call
paths, in which case we need to lock the mutex. AFAICT the mutex API doesn't
allow recursive locking or finding out the mutex owner, so we need to handle
this with helper functions which allow recursive locking from the same
process.

The runtime PM platform support stub in use by ARM-based SH/R-Mobile
platforms contains nothing that's specifically ARM-related and instead of
wholly generic to anything using the clock framework.
The recent runtime PM changes interact rather badly with the lazy
disabling of clocks late in the boot process through the clock framework,
leading to situations where the runtime suspend/resume paths are entered
without a clock being actively driven due to having been lazily gated
off.
In order to correct this we can trivially tie in the aforementioned stub
as a general fallback for all SH platforms that don't presently have
their own runtime PM implementations (the corner case being SH-based
SH-Mobile platforms, which have their own stub through the hwblk API --
which in turn has bitrotted and will be subsequently adapted to use the
same stub as everyone else), regardless of whether the platforms choose
to define power domains of their own or not.
This fixes up regressions for clock framework users who also build in
runtime PM support without any specific power domains of their own, which
was previously causing the serial console to be lost when warring with
lazy clock disabling.

Setting sev->fh to NULL causes problems for the del op added in the next
patch of this series, since this op needs a way to get to its own data
structures, and typically this will be done by using container_of on an
embedded v4l2_fh struct.
The reason the original code is setting sev->fh to NULL is to signal
to users of the event framework that the unsubscription has happened,
but since their is no shared lock between the event framework and users
of it, this is inherently racy, and it also turns out to be unnecessary
as long as both the event framework and the user of the framework do their
own locking properly and the user guarantees that it holds no references
to the subcribed_event structure after its del operation has been called.
This is best explained by looking at the only code currently checking for
sev->fh being set to NULL on unsubscribe, which is the v4l2-ctrls.c send_event
function. Here is the relevant code from v4l2-ctrls: send_event():
if (sev->fh && (sev->fh != fh ||
(sev->flags & V4L2_EVENT_SUB_FL_ALLOW_FEEDBACK)))
v4l2_event_queue_fh(sev->fh, &ev);
Now lets say that v4l2_event_unsubscribe and v4l2-ctrls: send_event() race
on the same sev, then the following could happens:
1) send_event checks sev->fh, finds it is not NULL
<thread switch>
2) v4l2_event_unsubscribe sets sev->fh NULL
3) v4l2_event_unsubscribe calls v4l2_ctrls del_event function, this blocks

The kev pointers inside the pending events queue (the available queue) of the
fh point to data inside the sev, unsubscribing frees the sev, thus making these
pointers point to freed memory!
This patch fixes these dangling pointers in the available queue by removing

Rx byte count was off; instead use the hardware's count. Tx packet
count was counting pre-TSO packets; instead count on-the-wire packets.
Report hardware dropped frame count as rx_fifo_errors.
- The count of transmitted packets reported by the forcedeth driver
reports pre-TSO (TCP Segmentation Offload) packet counts and not the
count of the number of packets sent on the wire. This change fixes
the forcedeth driver to report the correct count. Fixed the code by
copying the count stored in the NIC H/W to the value reported by the
driver.
- Count rx_drop_frame errors as rx_fifo_errors:
We see a lot of rx_drop_frame errors if we disable the rx bottom-halves
for too long. Normally, rx_fifo_errors would be counted in this case.
The rx_drop_frame error count is private to forcedeth and is not
reported by ifconfig or sysfs. The rx_fifo_errors count is currently
unused in the forcedeth driver. It is reported by ifconfig as overruns.
This change reports rx_drop_frame errors as rx_fifo_errors.

Function ndo_get_stats() updates most of the stats from hardware
registers, making the manual updates un-needed. This change removes
these manual updates. Main exception is rx_missed_errors which needs
manual update.
Another exception is rx_packets, still updated manually in this commit
to make sure this patch doesn't change behavior of driver. This will

Fix up trivial conflict in drivers/net/ethernet/stmicro/stmmac/Kconfig
as per Michal: the STMMAC_DUAL_MAC config variable is still unused and
should be deleted.
This removes the use of the special "macbookair_fn_keys" keyboard
translation table for the MacBookAir4,x models (ie the 2011 refresh).
They use the standard apple_fn_keys[] translation. Apparently only the
old MacBook Air's need a different translation table.
This mirrors the change that commit da617c7cb9 ("HID: consolidate
MacbookAir 4,1 mappings") did for the WELLSPRING6A ones, but does it for
the WELLSPRING6 model used on the MacBookAir4,2.

This patch makes the cpuidle_states structure global (single copy)
instead of per-cpu. The statistics needed on per-cpu basis
by the governor are kept per-cpu. This simplifies the cpuidle
subsystem as state registration is done by single cpu only.
Having single copy of cpuidle_states saves memory. Rare case
of asymmetric C-states can be handled within the cpuidle driver
and architectures such as POWER do not have asymmetric C-states.
Having single/global registration of all the idle states,
dynamic C-state transitions on x86 are handled by
the boot cpu. Here, the boot cpu would disable all the devices,
re-populate the states and later enable all the devices,
irrespective of the cpu that would receive the notification first.

This is the first step towards global registration of cpuidle
states. The statistics used primarily by the governor are per-cpu
and have to be split from rest of the fields inside cpuidle_state,
which would be made global i.e. single copy. The driver_data field
is also per-cpu and moved.

The cpuidle_device->prepare() mechanism causes updates to the
cpuidle_state[].flags, setting and clearing CPUIDLE_FLAG_IGNORE
to tell the governor not to chose a state on a per-cpu basis at
run-time. State demotion is now handled by the driver and it returns
the actual state entered. Hence, this mechanism is not required.
Also this removes per-cpu flags from cpuidle_state enabling
it to be made global.

Cpuidle governor only suggests the state to enter using the
governor->select() interface, but allows the low level driver to
override the recommended state. The actual entered state
may be different because of software or hardware demotion. Software
demotion is done by the back-end cpuidle driver and can be accounted
correctly. Current cpuidle code uses last_state field to capture the
actual state entered and based on that updates the statistics for the
state entered.
Ideally the driver enter routine should update the counters,
and it should return the state actually entered rather than the time
spent there. The generic cpuidle code should simply handle where
the counters live in the sysfs namespace, not updating the counters.

There are a lot userspace approaches to detect the usage of the
platform (laptop, workstation, server, ...) and adjust kernel tunables
accordingly (io/process scheduler, power management, ...).
These approaches need constant maintaining and are ugly to implement
(detect PCMCIA controller -> laptop,
does not work on recent systems anymore, ...)
On ACPI systems there is an easy and reliable way (if implemented
in BIOS and most recent platforms have this value set).
-> export it to userspace.

The attached patch simplifies 29df8d8f87. As
the "pnp_xxx" structs are not designed to cope with IORESOURCE_DISABLED, and
hence no code can test for this value, setting this value is actually a "no op"
and can be skipped altogether. It is sufficient to remove the checks for
"empty" resources and continue processing.
The patch is applied against 3.1.

Calling pm-suspend might trigger a recursive lock in it's code path.
In function acpi_hw_clear_acpi_status, acpi_os_acquire_lock holds
the lock acpi_gbl_hardware_lock before calling acpi_hw_register_write(),
then without releasing acpi_gbl_hardware_lock, this function calls
acpi_ev_walk_gpe_list, which tries to hold acpi_gbl_gpe_lock.
Both acpi_gbl_hardware_lock and acpi_gbl_gpe_lock are at same
lock-class and which might cause lock recursion deadlock.
Following patch fixes this scenario by just releasing
acpi_gbl_hardware_lock before calling acpi_ev_walk_gpe_list.
