struct bus_type should never be modified in a sysfs callback as there is
nothing in the structure to modify, and frankly, the structure is almost
never used in a sysfs callback, so mark it as constant to allow struct
bus_type to be moved to read-only memory.

Some storage, such as AIX VDASD (virtual storage) and IBM 2076 (front
end), fail as a result of commit c92a6b5d63 ("scsi: core: Query VPD
size before getting full page").
That commit changed getting SCSI VPD pages so that we now read just
enough of the page to get the actual page size, then read the whole
page in a second read. The problem is that the above mentioned
hardware returns zero for the page size, because of a firmware
error. In such cases, until the firmware is fixed, this new blacklist
flag says to revert to the original method of reading the VPD pages,
i.e. try to read a whole buffer's worth on the first try.

The LLDD and the stack currently process FPINs received from the fabric,
but the stack is not aware of any action taken by the driver to alleviate
congestion. The current interface between the driver and the SCSI stack is
limited to passing the notification mainly for statistics and heuristics.
The reaction to an FPIN could be handled either by the driver or by the
stack (marginal path and failover). Amend the interface to indicate if
action on an FPIN has already been reacted to by the LLDDs or not. Add an
additional flag to fc_host_fpin_rcv() to indicate if the FPIN has been
acknowledged/reacted to by the driver.
Also added a new event code FCH_EVT_LINK_FPIN_ACK to notify to the user
that the event has been acknowledged/reacted by the LLDD driver

Move the SCSI execution functions to use a struct for passing in optional
args. This commit adds the new struct, temporarily converts scsi_execute()
and scsi_execute_req() ands a new helper, scsi_execute_cmd(), which takes
the scsi_exec_args struct.
There should be no change in behavior. We no longer allow users to pass in
any request->rq_flags value, but they were only passing in RQF_PM which we
do support by allowing users to pass in the BLK_MQ_REQ flags used by
blk_mq_alloc_request().
Subsequent commits will convert scsi_execute() and scsi_execute_req() users
to the new helpers then remove scsi_execute() and scsi_execute_req().

Factor out sas_ata_add_dev() and put it in sas_ata.c since it is a SATA
related interface. Also follow the standard coding style to define an

The coding style where calling this interface is inconsistent with other
interfaces for SATA devices. The standard style for other SATA interfaces

It was observed that the kernel would potentially send
ISCSI_KEVENT_UNBIND_SESSION multiple times. Introduce 'target_state' in
iscsi_cls_session() to make sure session will send only one unbind session
event.
This introduces a regression wrt. the issue fixed in commit 13e60d3ba2
("scsi: iscsi: Report unbind session event when the target has been
removed"). If iscsid dies for any reason after sending an unbind session to
kernel, once iscsid is restarted, the kernel's ISCSI_KEVENT_UNBIND_SESSION
event is lost and userspace is then unable to logout. However, the session
is actually in invalid state (its target_id is INVALID) so iscsid should
not sync this session during restart.
Consequently we need to check the session's target state during iscsid
restart. If session is in unbound state, do not sync this session and
perform session teardown. This is OK because once a session is unbound, we
can not recover it any more (mainly because its target id is INVALID).

This converts the SCSI errors we commonly see during PR handling to PR_STS
errors or -Exyz errors. pr_ops callers can then handle SCSI and NVMe errors

ZBC Zoned Block Commands specification mandates SYNCHRONIZE CACHE(16) for
host-managed zoned block devices, but does not mandate SYNCHRONIZE
CACHE(10). Call SYNCHRONIZE CACHE(16) in place of SYNCHRONIZE CACHE(10) to
ensure that the command is always supported. For this purpose, add
use_16_for_sync flag to struct scsi_device in same manner as use_16_for_rw
flag.
To be precise, ZBC does not mandate SYNCHRONIZE CACHE(16) for host-aware
zoned block devices. However, modern devices should support 16-byte
commands. Hence, call SYNCHRONIZE CACHE (16) on both types of ZBC devices,
host-aware and host-managed. Of note is that READ(16) and WRITE(16) have
same story and they are already called for both types of ZBC devices.
Another note is that this patch depends on the fix commit ea045fd344
("ata: libata-scsi: fix SYNCHRONIZE CACHE (16) command failure").

The current behavior for SCSI commands submitted while error recovery is
ongoing is to retry command submission after error recovery has finished.
See also the scsi_host_in_recovery() check in scsi_host_queue_ready(). Add
support for failing SCSI commands while host recovery is in progress. This
functionality will be used to fix a deadlock in the UFS driver.

Commit 6600593cbd ("block: rename BLK_EH_NOT_HANDLED to BLK_EH_DONE")
made it impossible for .eh_timed_out() implementations to call
scsi_done() without causing a crash.
Restore support for SCSI timeout handlers to call scsi_done() as follows:
* Change all .eh_timed_out() handlers as follows:
- Change the return type into enum scsi_timeout_action.
- Change BLK_EH_RESET_TIMER into SCSI_EH_RESET_TIMER.
- Change BLK_EH_DONE into SCSI_EH_NOT_HANDLED.
* In scsi_timeout(), convert the SCSI_EH_* values into BLK_EH_* values.

blk-mq already provides a unique tag per request. Some libsas LLDDs - like
hisi_sas - already use this tag as the unique per-I/O HW tag.
Add a common function to provide the request associated with a sas_task for
all libsas LLDDs.

All upstream scsi_device_put() calls happen from thread context. Hence
simplify scsi_device_put() by always calling the release function
synchronously. This commit prepares for constifying the SCSI host template
by removing an assignment that clears the module pointer in the SCSI host
template.
scsi_device_dev_release_usercontext() was introduced in 2006 via
commit 65110b2168 ("[SCSI] fix wrong context bugs in SCSI").

Instead of using scsi_host_template members to track the SCSI proc
directory entries, track these entries in a list. This changes the time
needed for looking up the proc dir pointer from O(1) into O(n). This is
considered acceptable since the number of SCSI host adapter types per host
is usually small (less than ten).
This change has been tested by attaching two USB storage devices to a qemu
host:

Similar to how AHCI handles NCQ errors in ahci_error_intr() ->
ata_port_abort() -> ata_do_link_abort(), add an NCQ error handler for LLDDs
to call to initiate a link abort.
This will mark all outstanding QCs as failed and kick-off EH.
Note:
A "force reset" argument is added for drivers which require the ATA error
handling to always reset the device.
A driver may require this feature for when SATA device per-SCSI cmnd
resources are only released during reset for ATA EH. As such, we need an
option to force reset to be done, regardless of what any EH autopsy
decides.
The SATA device FIS fields are set to indicate a device error from
ata_eh_analyze_tf().

The passthrough structure is declared off of the stack, so it needs to be
set to zero before copied back to userspace to prevent any unintentional
data leakage. Switch things to be statically allocated which will fill the
unused fields with 0 automatically.

There are two .exit_cmd_priv implementations. Both implementations use
resources associated with the SCSI host. Make sure that these resources are
still available when .exit_cmd_priv is called by waiting inside

This commit prevents that the following sequence triggers a kernel crash:
- Deletion of a SCSI device is requested via sysfs. Device removal takes
some time because blk_cleanup_queue() is waiting for the SCSI error
handler.
- The SCSI target associated with that SCSI device is removed.
- scsi_remove_target() returns and its caller frees the resources
associated with the SCSI target.
- The error handler makes progress and invokes an LLD callback that
dereferences the SCSI target pointer.

Streaming DMA mappings may be considerably slower when mappings go through
an IOMMU and the total mapping length is somewhat long. This is because the
IOMMU IOVA code allocates and free an IOVA for each mapping, which may
affect performance.
New member Scsi_Host.opt_sectors is added, which is the optimal host
max_sectors, and use this value to cap the request queue max_sectors when
set.
It could be considered to have request queues io_opt value initially
set at Scsi_Host.opt_sectors in __scsi_init_queue(), but that is not
really the purpose of io_opt.
Finally, even though Scsi_Host.opt_sectors value should never be greater
than the request queue max_hw_sectors value, continue to limit to this
value for safety.

The conn_send_pdu API is evil in that it returns a pointer to an
iscsi_task, but that task might have been freed already so you can't touch
it. This patch splits the task allocation and transmission, so functions
like iscsi_send_nopout() can access the task before its sent and do
whatever bookkeeping is needed before it is sent.

We currently require that the back_lock is held when calling the functions
that manipulate the iscsi_task refcount. The only reason for this is to
handle races where we are handling SCSI-ml EH callbacks and the cmd is
completing at the same time the normal completion path is running, and we
can't return from the EH callback until the driver has stopped accessing
the cmd. Holding the back_lock while also accessing the task->state made it
simple to check that a cmd is completing and also get/put a refcount at the
same time, and at the time we were not as concerned about performance.
The problem is that we don't want to take the back_lock from the xmit path
for normal I/O since it causes contention with the completion path if the
user has chosen to try and split those paths on different CPUs (in this
case abusing the CPUs and ignoring caching improves perf for some uses).
Begins to remove the back_lock requirement for iscsi_get/put_task by
removing the requirement for the get path. Instead of always holding the
back_lock we detect if something has done the last put and is about to call
iscsi_free_task(). A subsequent commit will then allow iSCSI code to do the
last put on a task and only grab the back_lock if the refcount is now zero
and it's going to call iscsi_free_task().

When the system is shutting down, iscsid is not running so we will not get
a response to the ISCSI_ERR_INVALID_HOST error event. The system shutdown
will then hang waiting on userspace to remove the session.
This has libiscsi force the destruction of the session from the kernel when
iscsi_host_remove() is called from a driver's shutdown callout.
This fixes a regression added in qedi boot with commit d1f2ce7763 ("scsi:
qedi: Fix host removal with running sessions") which made qedi use the
common session removal function that waits on userspace instead of rolling
its own kernel based removal.

During qedi shutdown we need to stop the iSCSI layer from sending new nops
as pings and from responding to target ones and make sure there is no
running connection cleanups. Commit d1f2ce7763 ("scsi: qedi: Fix host
removal with running sessions") converted the driver to use the libicsi
helper to drive session removal, so the above issues could be handled. The
problem is that during system shutdown iscsid will not be running so when
we try to remove the root session we will hang waiting for userspace to
reply.
Add a helper that will drive the destruction of sessions like these during
system shutdown.

fcoe_get_paged_crc_eof() relies on the caller having preemption disabled to
ensure the per-CPU fcoe_percpu context remains valid throughout the
call. This is done by either holding spinlocks (such as bnx2fc_global_lock
or qedf_global_lock) or the get_cpu() from fcoe_alloc_paged_crc_eof(). This
last one breaks PREEMPT_RT semantics as there can be memory allocation and
end up sleeping in atomic contexts.
Introduce a local_lock_t to struct fcoe_percpu that will keep the non-RT
case the same, mapping to preempt_disable/enable, while RT will use a
per-CPU spinlock allowing the region to be preemptible but still maintain
CPU locality. The other users of fcoe_percpu are already safe in this
regard and do not require local_lock()ing.

The SCSI disk driver consults VPD pages b0 (Block Limits), b1 (Block Device
Characteristics), and b2 (Logical Block Provisioning). Instead of having
sd.c request these pages every revalidate cycle, cache them along with the
other commonly used VPDs.

We currently default to 255 bytes when fetching VPD pages during discovery.
However, we have had a few devices that are known to wedge if the requested
buffer exceeds a certain size. See commit af73623f5f ("[SCSI] sd: Reduce
buffer size for vpd request") which works around one example of this
problem in the SCSI disk driver.
With commit d188b0675b ("scsi: core: Add sysfs attributes for VPD pages
0h and 89h") we now risk triggering the same issue in the generic midlayer
code.
The problem with the ATA VPD page in particular is that the SCSI portion of
the page is trailed by 512 bytes of verbatim ATA Identify Device
information. However, not all controllers actually provide the additional
512 bytes and will lock up if one asks for more than the 64 bytes
containing the SCSI protocol fields.
Instead of picking a new, somewhat arbitrary, number of bytes for the VPD
buffer size, start fetching the 4-byte header for each page. The header
contains the size of the page as far as the device is concerned. We can use
the reported size to specify the correct allocation length when
subsequently fetching the full page.
The header validation is done by a new helper function scsi_get_vpd_size()
and both scsi_get_vpd_page() and scsi_get_vpd_buf() now rely on this to
query the page size.
In addition, scsi_get_vpd_page() is simplified to mirror the logic in
scsi_get_vpd_page(). This involves removing the Supported VPD Pages lookup
prior to attempting to query a page. There does not appear any evidence,
even in the oldest SCSI specs, that this step is required. We already rely
on scsi_get_vpd_page() throughout the stack and this function never
consulted the Supported VPD Pages. Since this has not caused any problems
it should be safe to remove the precondition from scsi_get_vpd_page().
Instrumented runs also revealed that the Supported VPD Pages lookup had
little effect since the device page index often was larger than the
supplied buffer size. As a result, inquiries frequently bypassed the index
check and went through the "If we ran off the end of the buffer, give us
the benefit of the doubt" code path which assumed the page was present
despite not being listed. The revised code takes both the page size
reported by the device as well as the size of the buffer provided by the
scsi_get_vpd_page() caller into account.

ZBC-2 allows host-managed disks to report gap zones. This allow zoned disks
to report an offset between data zone starts that is a power of two even if
the number of logical blocks with data per zone is not a power of two.
Another new feature in ZBC-2 is support for constant zone starting LBA
offsets. For zoned disks that report a constant zone starting LBA offset,
hide the gap zones from the block layer. Report the offset between data
zone starts as zone size and report the number of logical blocks with data
per zone as the zone capacity.

These flags are supposed to be bit numbers. Right now they cause a double
shift bug where we use BIT(BIT(2)) instead of BIT(2). Fortunately, the bit
numbers are small and it's done consistently so it does not cause an issue
at run time.

If a offload driver doesn't use the xmit workqueue, then when we are doing
ep_disconnect libiscsi can still inject PDUs to the driver. This adds a
check for if the connection is bound before trying to inject PDUs.

If iscsid is doing a stop_conn at the same time the kernel is starting
error recovery we can hit a race that allows the cleanup work to run on a
valid connection. In the race, iscsi_if_stop_conn sees the cleanup bit set,
but it calls flush_work on the clean_work before iscsi_conn_error_event has
queued it. The flush then returns before the queueing and so the
cleanup_work can run later and disconnect/stop a conn while it's in a
connected state.
The patch:

kernel space")
added the late stop_conn call bug originally, and the patch:
Commit 23d6fefbb3 ("scsi: iscsi: Fix in-kernel conn failure handling")
attempted to fix it but only fixed the normal EH case and left the above
race for the iscsid restart case. For the normal EH case we don't hit the
race because we only signal userspace to start recovery after we have done
the queueing, so the flush will always catch the queued work or see it
completed.
For iscsid restart cases like boot, we can hit the race because iscsid will
call down to the kernel before the kernel has signaled any error, so both
code paths can be running at the same time. This adds a lock around the
setting of the cleanup bit and queueing so they happen together.

We can't release the endpoint ID until all references to the endpoint have
been dropped or it could be allocated while in use. This has us use an idr
instead of looping over all conns to find a free ID and then free the ID
when all references have been dropped instead of when the device is only
deleted.

iscsi_create_conn() exposed iscsi_cls_conn to sysfs prior to initialization
of iscsi_conn's dd_data. When userspace tried to access an attribute such
as the connect address, a NULL pointer dereference was observed.
Do not add iscsi_cls_conn to sysfs until it has been initialized. Remove
iscsi_create_conn() since it is no longer used.

Use the common libsas internal abort functionality.
In addition, this driver has special handling for internal abort timeouts -
specifically whether to reset the controller in that instance, so extend
the API for that.
Timeout is now increased to 20 * Hz from 6 * Hz.
We also retry for failure now, but this should not make a difference.

The internal abort feature is common to hisi_sas and pm8001 HBAs, and the
driver support is similar also, so add a common handler.
Two modes of operation will be supported:
- single: Abort a single tagged command
- device: Abort all commands associated with a specific domain device
A new protocol is added, SAS_PROTOCOL_INTERNAL_ABORT, so the common queue
command API may be re-used.
Only add "single" support as a first step.

Requiring every ULP to have the scsi_drive as first member of the
private data is rather fragile and not necessary anyway. Just use
the driver hanging off the SCSI device instead.

Kernel messages produced during runtime PM can cause a never-ending cycle
because user space utilities (e.g. journald or rsyslog) write the messages
back to storage, causing runtime resume, more messages, and so on.
Messages that tell of things that are expected to happen are arguably
unnecessary, so add a flag to suppress them. This flag is used by the UFS
driver.

We currently allocate a workqueue per host and only use it for removing the
target. For the session per host case we could be using this workqueue to
be able to do recoveries (block, unblock, timeout handling) in parallel. To
also allow offload drivers to do their session recoveries in parallel, this
drops the per host workqueue and replaces it with a per session one.

Now that each scsi_request is backed by a scsi_cmnd, there is no need to
indirect the CDB storage. Change all submitters of SCSI passthrough
requests to store the CDB information directly in the scsi_cmnd, and while
doing so allocate the full 32 bytes that cover all Linux supported SCSI
hosts instead of requiring dynamic allocation for > 16 byte CDBs. On
64-bit systems this does not change the size of the scsi_cmnd at all, while
on 32-bit systems it slightly increases it for now, but that increase will
be made up by the removal of the remaining scsi_request fields.

Nobody checks the return codes, so make them return void. Indeed, if the
LLDD cannot send an event, nothing much can be done in the LLDD about it.
Also remove prototype for sas_notify_phy_event() in sas_internal.h, which

Add a function to execute an ATA command using the TMF code, and use in the
hisi_sas driver. That driver needs to be able to issue the command on a
specific phy, so add an interface for that.
With that, hisi_sas_exec_internal_tmf_task() may be deleted.

Some of the LLDDs which use libsas have their own definition of a struct
to hold TMF info, so add a common struct for libsas.
Also add an interim force phy id field for hisi_sas driver, which will be
removed once the STP "TMF" code is factored out.
Even though some LLDDs (pm8001) use a u32 for the tag, u16 will be adequate,
as that named driver only uses tags in range [0, 1024).

field, which makes reading the code easier.

Add scsi_done_direct() which behaves like scsi_done() except that it
invokes blk_mq_complete_request_direct() in order to complete the request.
Callers from process context can complete the request directly instead

kernel/sysctl.c is a kitchen sink where everyone leaves their dirty
dishes, this makes it very difficult to maintain.
To help with this maintenance let's start by moving sysctls to places
where they actually belong. The proc sysctl maintainers do not want to
know what sysctl knobs you wish to add for your own piece of code, we
just care about the core logic.
So move the sg-big-buff sysctl from kernel/sysctl.c to drivers/scsi/sg.c
and use register_sysctl() to register the sysctl interface.

For the hisi_sas driver, if a directly attached disk is removed during
suspend, a hang will occur in the resume process:
The background is that in commit 16fd4a7c59 ("scsi: hisi_sas: Add device
link between SCSI devices and hisi_hba"), it is ensured that the HBA device
cannot be runtime suspended when any SCSI device associated is active.
Other drivers which use libsas don't worry about this as none support
runtime suspend.
The mentioned hang occurs when an disk is removed during suspend. In the
removal process - from PHYE_RESUME_TIMEOUT event processing - we call into
scsi_remove_device(), which is being processed in the HA event workqueue.
Here we wait for all suppliers of the SCSI device to resume, which includes
the HBA device (from the above commit). However the HBA device cannot
resume, as it is waiting for the PHYE_RESUME_TIMEOUT to be processed (from
calling sas_resume_ha() -> sas_drain_work()). This is the deadlock.
There does not appear to be any need for the sas_drain_work() to be called
at all in sas_resume_ha() as it is not syncing against anything, so allow
LLDDs to avoid this by providing a variant of sas_resume_ha() which does
"sync", i.e. doesn't drain the event workqueue.

For fixing queue quiesce race between driver and block layer(elevator
switch, update nr_requests, ...), we need to support concurrent quiesce
and unquiesce, which requires the two call balanced.
It isn't easy to audit that in all scsi drivers, especially the two may
be called from different contexts, so do it in scsi core with one
per-device atomic variable to balance quiesce and unquiesce.

The changes to issue the abort from the scmd->abort_work instead of the EH
thread introduced a problem if eh_deadline is used. If aborting the
command(s) is successful, and there are never any scmds added to the
shost->eh_cmd_q, there is no code path which will reset the ->last_reset
value back to zero.
The effect of this is that after a successful abort with no EH thread
activity, a subsequent timeout, perhaps a long time later, might
immediately be considered past a user-set eh_deadline time, and the host
will be reset with no attempt at recovery.
Fix this by resetting ->last_reset back to zero in scmd_eh_abort_handler()
if it is determined that the EH thread will not run to do this.

this work is part of the larger series[0], but depends on all the false
positives and buffer overflow bug fixes to have landed already and those
that depend on this series to land.
As part of the FORTIFY_SOURCE refactoring, a set of both a compile-time
and run-time tests are added for FORTIFY_SOURCE and the mem*()-family
functions respectively. The compile time tests have found a legitimate
(though corner-case) bug[6] already.
Please note that the appearance of "panic" and "BUG" in the
FORTIFY_SOURCE refactoring are the result of relocating existing code,
and no new use of those code-paths are expected nor desired.
Finally, there are two tree-wide conversions for 0-element arrays and
flexible array unions to gain sane compiler introspection coverage that
result in no known object code differences.
After this series (and the changes that have now landed via netdev
and usb), we are very close to finally being able to build with
-Warray-bounds and -Wzero-length-bounds. However, due corner cases in
GCC[3] and Clang[4], I have not included the last two patches that turn
on these options, as I don't want to introduce any known warnings to
the build. Hopefully these can be solved soon.

v4.17 commit 86b87cde0b ("scsi: core: host template attribute groups")
introduced explicit sysfs_create_groups() in scsi_sysfs_add_sdev()
and sysfs_remove_groups() in __scsi_remove_device(), both for sdev_gendev,
based on a new field const struct attribute_group **sdev_groups
of struct scsi_host_template.
Commit 92c4b58b15 ("scsi: core: Register sysfs attributes earlier")
removed above explicit (de)registration of scsi_device attribute groups.
It also converted all scsi_device attributes and attribute_groups to
end up in a new field const struct attribute_group *gendev_attr_groups[6]
of struct scsi_device. However, that new field was not used anywhere.
Surprisingly, this only caused missing LLDD specific scsi_device sysfs
attributes. Whereas, scsi core attributes from scsi_sdev_attr_groups
did continue to exist because of scsi_dev_type.groups.
We separate scsi core attibutes from LLDD specific attributes.
Hence, we keep the initializing assignment scsi_dev_type =
{ .groups = scsi_sdev_attr_groups, } as this takes care of core
attributes. Without the separation, it would cause attribute double
registration due to scsi_dev_type.groups and sdev_gendev.groups.
Julian suggested to assign the sdev_groups pointer of the
scsi_host_template directly to the groups pointer of sdev_gendev.
This way we can delete the container scsi_device.gendev_attr_groups
and the loop copying each entry from hostt->sdev_groups to
sdev->gendev_attr_groups.
Alternative approaches ruled out:
Assigning gendev_attr_groups to sdev_dev has no visible effect.
Assigning sdev->gendev_attr_groups to scsi_dev_type.groups
caused scsi_device of all scsi host types to get LLDD specific
attributes of the LLDD for which the last sdev alloc happened to occur,
as that overwrote scsi_dev_type.groups,
e.g. scsi_debug had zfcp-specific scsi_device attributes.

Add a new helper that calls blk_get_request and initializes the
scsi_request to avoid the indirect call through ->.initialize_rq_fn.
Note that this makes the pktcdvd driver depend on the SCSI core, but
given that only SCSI devices support SCSI passthrough requests that
is not a functional change.

All SCSI drivers have been converted to use shost_groups and sdev_groups
instead of shost_attrs or sdev_attrs. Hence remove shost_attrs and
sdev_attrs. Additionally, remove the 'lld_attr_group' members and also
the scsi_convert_dev_attrs() function.

strict expectations on when attributes get created. When a new device is
registered in the kernel, a uevent is generated to notify userspace (like
udev) that a new device is available. If attributes are added after the
device is registered, then userspace won't get notified and userspace will
not know about the new attributes."
Hence register SCSI host sysfs attributes before the SCSI host shost_dev
uevent is emitted instead of after that event has been emitted.

The DEF_SCSI_QCMD() macro passes the addresses of the SCSI host lock and
also that of the scsi_done function to the queuecommand_lck() function
implementations. Remove the 'scsi_done' argument since its address is
now a constant and instead call 'scsi_done' directly from inside the
queuecommand_lck() functions.

Since the removal of the legacy block layer there is only one completion
function left in the SCSI core, namely scsi_mq_done(). Rename it into
scsi_done(). Export that function to allow SCSI LLDs to call it directly.

Conditional statements are faster than indirect calls. Use a structure
member to track the SCSI command submitter such that later patches can call
scsi_done(scmd) instead of scmd->scsi_done(scmd).
The asymmetric behavior that scsi_send_eh_cmnd() sets the submission
context to the SCSI error handler and that it does not restore the
submission context to the SCSI core is retained.
