Look for the udev generated persistent device name for NVMe devices
in addition to the SCSI ones and the Redhat-specific device mapper
name.
This is the client side implementation of RFC 9561 "Using the Parallel
NFS (pNFS) SCSI Layout to Access Non-Volatile Memory Express (NVMe)
Storage Devices".
Note that the udev rules for nvme are a bit of a mess and udev will only
create a link for the uuid if the NVMe namespace has one, and not the
NGUID. As the current RFCs don't support UUID based identifications this
means the layout can't be used on such namespaces out of the box. A
small tweak to the udev rules can work around it, and as the real fix I
will submit a draft to the IETF NFSv4 working group to support UUID-based
identifiers for SCSI and NVMe.

An administrator cannot take action on these messages, but the
reported errors might be helpful for troubleshooting. Transition
them to trace points so these events appear in the trace log and
can be easily lined up with other traced NFS client operations.

Since commit f931d8374c ("nfs/blocklayout: refactor block device
opening"), an error is reported when no multi-path device is found.
But this isn't a fatal error if the subsequent device open is
successful. On systems without multi-path devices, this message
always appears whether there is a problem or not.
Instead, generate less system journal noise by reporting an error
only when both open attempts fail. The new error message is more
actionable since it indicates that there is a real configuration
issue to be addressed.

nfs4_find_get_deviceid() does a lookup without the spin lock first.
If it can't find a matching deviceid, it creates a new device_info
(which calls bl_alloc_deviceid_node, and that registers the device's
PR key).
Then it takes the nfs4_deviceid_lock and looks up the deviceid again.
If it finds it this time, bl_find_get_deviceid() frees the spare
(new) device_info, which unregisters the PR key for the same device.
Any subsequent I/O from this client on that device gets EBADE.
The umount later unregisters the device's PR key again.
To prevent this problem, register the PR key after the deviceid_node
lookup.

We noticed a SCSI device that refused to allow READ CAPACITY when the
device had a PR with exclusive access, registrants only. The result of
this situation is that the blocklayout driver adds a pnfs_block_dev of zero
length which always fails the offset_in_map tests. Instead of continuously
trying to do pNFS for this case, just mark the device as unavailable which
will allow the client to fallback to the MDS for the duration of
PNFS_DEVICE_RETRY_TIMEOUT.

The only overlap between the block open flags mapped into the fmode_t and
other uses of fmode_t are FMODE_READ and FMODE_WRITE. Define a new
blk_mode_t instead for use in blkdev_get_by_{dev,path}, ->open and
->ioctl and stop abusing fmode_t.

The current interface for exclusive opens is rather confusing as it
requires both the FMODE_EXCL flag and a holder. Remove the need to pass
FMODE_EXCL and just key off the exclusive open off a non-NULL holder.
For blkdev_put this requires adding the holder argument, which provides
better debug checking that only the holder actually releases the hold,
but at the same time allows removing the now superfluous mode argument.

Pass the block_device and operation that we plan to use this bio for to
bio_alloc to optimize the assignment. NULL/0 can be passed, both for the
passthrough case on a raw request_queue and to temporarily avoid
refactoring some nasty code.
Also move the gfp_mask argument after the nr_vecs argument for a much
more logical calling convention matching what most of the kernel does.

SUNRPC has two sorts of credentials, both of which appear as
"struct rpc_cred".
There are "generic credentials" which are supplied by clients
such as NFS and passed in 'struct rpc_message' to indicate
which user should be used to authorize the request, and there
are low-level credentials such as AUTH_NULL, AUTH_UNIX, AUTH_GSS
which describe the credential to be sent over the wires.
This patch replaces all the generic credentials by 'struct cred'
pointers - the credential structure used throughout Linux.
For machine credentials, there is a special 'struct cred *' pointer
which is statically allocated and recognized where needed as
having a special meaning. A look-up of a low-level cred will
map this to a machine credential.

The page cache offers the ability to search for a miss in the previous or
next N locations. Rather than teach the XArray about the page cache's
definition of a miss, use xas_prev() and xas_next() to search the page
array. This should be more efficient as it does not have to start the
lookup from the top for each index.

"dev->nr_children" is the number of children which were parsed
successfully in bl_parse_stripe(). It could be all of them and then, in
that case, it is equal to v->stripe.volumes_count. Either way, the >
should be >= so that we don't go beyond the end of what we're supposed
to.

PNFS block/SCSI layouts should gracefully handle cases where block devices
are not available when a layout is retrieved, or the block devices are
removed while the client holds a layout.
While setting up a layout segment, keep a record of an unavailable or
un-parsable block device in cache with a flag so that subsequent layouts do
not spam the server with GETDEVINFO. We can reuse the current
NFS_DEVICEID_UNAVAILABLE handling with one variation: instead of reusing
the device, we will discard it and send a fresh GETDEVINFO after the
timeout, since the lookup and validation of the device occurs within the
GETDEVINFO response handling.
A lookup of a layout segment that references an unavailable device will
return a segment with the NFS_LSEG_UNAVAILABLE flag set. This will allow
the pgio layer to mark the layout with the appropriate fail bit, which
forces subsequent IO to the MDS, and prevents spamming the server with
LAYOUTGET, LAYOUTRETURN.
Finally, when IO to a block device fails, look up the block device(s)
referenced by the pgio header, and mark them as unavailable.

If there's an error doing I/O to block device, and the client resends the
I/O to the MDS, the MDS must recall the layout from the client before
processing the I/O. Let's preempt that exchange by returning the layout

The blocklayout module contains the client support for both block and SCSI
layouts. Add a module alias for the SCSI layout type so that the module
will be loaded for SCSI layouts.

Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.
By default all files without license information are under the default
license of the kernel, which is GPL version 2.
Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier. The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.
This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.
How this work was done:
Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
- file had no licensing information it it.
- file was a */uapi/* one with no licensing information in it,
- file was a */uapi/* one with existing licensing information,
Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.
The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne. Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.
The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed. Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.
Criteria used to select files for SPDX license identifier tagging was:
- Files considered eligible had to be source code files.
- Make and config files were included as candidates if they contained >5
lines of source
- File already had some variant of a license header in it (even if <5
lines).
All documentation files were explicitly excluded.
The following heuristics were used to determine which SPDX license
identifiers to apply.
- when both scanners couldn't find any license traces, file was
considered to have no license information in it, and the top level
COPYING file license applied.
For non */uapi/* files that summary was:

- when the two scanners agreed on the detected license(s), that became
the concluded license(s).
- when there was disagreement between the two scanners (one detected a
license but the other didn't, or they both detected different
licenses) a manual inspection of the file occurred.
- In most cases a manual inspection of the information in the file
resulted in a clear resolution of the license that should apply (and
which scanner probably needed to revisit its heuristics).
- When it was not immediately clear, the license identifier was
confirmed with lawyers working with the Linux Foundation.
- If there was any question as to the appropriate license identifier,
the file was flagged for further research and to be revisited later
in time.
In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.
Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights. The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.
Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.
In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.
Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
- a full scancode scan run, collecting the matched texts, detected
license ids and scores
- reviewing anything where there was a license detected (about 500+
files) to ensure that the applied SPDX license was correct
- reviewing anything where there was no detection but the patch license
was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
SPDX license was correct
This produced a worksheet with 20 files needing minor correction. This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.
These .csv files were then reviewed by Greg. Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected. This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.) Finally Greg ran the script using the .csv files to
generate the patches.

This way we don't need a block_device structure to submit I/O. The
block_device has different life time rules from the gendisk and
request_queue and is usually only available when the block device node
is open. Other callers need to explicitly create one (e.g. the lightnvm
passthrough code, or the new nvme multipathing code).
For the actual I/O path all that we need is the gendisk, which exists
once per block device. But given that the block layer also does
partition remapping we additionally need a partition index, which is
used for said remapping in generic_make_request.
Note that all the block drivers generally want request_queue or
sometimes the gendisk, so this removes a layer of indirection all
over the stack.

Replace bi_error with a new bi_status to allow for a clear conversion.
Note that device mapper overloaded bi_error with a private value, which
we'll have to keep arround at least for now and thus propagate to a
proper blk_status_t value.

Now that %z is standartised in C99 there is no reason to support %Z.
Unlike %L it doesn't even make format strings smaller.
Use BUILD_BUG_ON in a couple ATM drivers.
In case anyone didn't notice lib/vsprintf.o is about half of SLUB which
is in my opinion is quite an achievement. Hopefully this patch inspires
someone else to trim vsprintf.c more.

last written byte to the offset of the end of the extent rather than the
end of the write which incorrectly updates the inode's size for
partial-page writes.

Block/SCSI layout write completion may add committable extents to the
extent tree before updating the layout's last-written byte under the inode
lock. If a sync happens before this value is updated, then
prepare_layoutcommit may find and encode these extents which would produce
a LAYOUTCOMMIT request whose encoded extents are larger than the request's
loca_length.
Fix this by using a last-written byte value that is updated atomically with
the extent tree so that commitable extents always match.

When bl_parse_deviceid() fails in bl_alloc_deviceid_node() on
blkdev_get_by_*() step we get an pnfs_block_dev struct that is
uninitialized except for bdev field which is set to whatever error
blkdev_get_by_*() returns. bl_free_device() then tries to call
blkdev_put() if bdev is not 0 resulting in a wrong pointer dereference.
Fixing this by setting bdev in struct pnfs_block_dev only if we didn't
get an error from blkdev_get_by_*().
