ef19470ef8 fs/fat/inode.c: clean up string initializations (char[] instead of char *)
Initializations like 'char *foo = "bar"' will create two variables: a
static string and a pointer (foo) to that static string.  Instead 'char
foo[] = "bar"' will declare a single variable and will end up in shorter
assembly (according to Jeff Garzik on the KernelJanitor's TODO list).

190a8843de fs/fat/: add support for DOS 1.x formatted volumes
Add structure for parsed BPB information, struct fat_bios_param_block,
and move all of the deserialization and validation logic from
fat_fill_super() into fat_read_bpb().

Add a 'dos1xfloppy' mount option to infer DOS 2.x BIOS Parameter Block
defaults from block device geometry for ancient floppies and floppy
images, as a fall-back from the default BPB parsing logic.

When fat_read_bpb() finds an invalid FAT filesystem and dos1xfloppy is
set, fall back to fat_read_static_bpb().  fat_read_static_bpb()
validates that the entire BPB is zero, and that the floppy has a
DOS-style 8086 code bootstrapping header.  Then it fills in default BPB
values from media size and a table.[0]

Media size is assumed to be static for archaic FAT volumes.  See also:

91b0abe36a mm + fs: store shadow entries in page cache
Reclaim will be leaving shadow entries in the page cache radix tree upon
evicting the real page.  As those pages are found from the LRU, an
iput() can lead to the inode being freed concurrently.  At this point,
reclaim must no longer install shadow pages because the inode freeing
code needs to ensure the page tree is really empty.

Add an address_space flag, AS_EXITING, that the inode freeing code sets
under the tree lock before doing the final truncate.  Reclaim will check
for this flag before installing shadow pages.

02b9984d64 fs: push sync_filesystem() down to the file system's remount_fs()
Previously, the no-op "mount -o mount /dev/xxx" operation when the
file system is already mounted read-write causes an implied,
unconditional syncfs().  This seems pretty stupid, and it's certainly
documented or guaraunteed to do this, nor is it particularly useful,
except in the case where the file system was mounted rw and is getting
remounted read-only.

However, it's possible that there might be some file systems that are
actually depending on this behavior.  In most file systems, it's
probably fine to only call sync_filesystem() when transitioning from
read-write to read-only, and there are some file systems where this is
not needed at all (for example, for a pseudo-filesystem or something
like romfs).

6e5b93ee55 fatfs: add FAT_IOCTL_GET_VOLUME_ID
This patch, originally from Android kernel, adds vfat ioctl command
FAT_IOCTL_GET_VOLUME_ID, with this command we can get the vfat volume ID
using following code:

	ioctl(fd, FAT_IOCTL_GET_VOLUME_ID, &volume_ID)

This patch is a modified version of the patch by Mike Lockwood, with
changes from Dmitry Pervushin, who noticed the original patch makes some
volume IDs abiguous with error returns: for example, if volume id is
0xFFFFFDAD, that matches -ENOIOCTLCMD, we get "FFFFFFFF" from the user
space.

So add a parameter to ioctl to get the correct volume ID.

Android uses vfat volume ID to identify different sd card, when a new sd
card is inserted to device, android can scan the media on it and pop up
new contents.

da53be12bb Don't pass inode to ->d_hash() and ->d_compare()
Instances either don't look at it at all (the majority of cases) or
only want it to find the superblock (which can be had as dentry->d_sb).
A few cases that want more are actually safe with dentry->d_inode -
the only precaution needed is the check that it hadn't been replaced with
NULL by rmdir() or by overwriting rename(), which case should be simply
treated as cache miss.

f1e6fb0ab4 fat (exportfs): rebuild directory-inode if fat_dget()
This patch enables rebuilding of directory inodes which are not present in
the cache.This is done by traversing the disk clusters to find the
directory entry of the parent directory and using its i_pos to build the
inode.

The traversal is done by fat_scan_logstart() which is similar to
fat_scan() but matches i_pos values instead of names.fat_scan_logstart()
needs an inode parameter to work, for which a dummy inode is created by
it's caller fat_rebuild_parent().  This dummy inode is destroyed after the
traversal completes.

All this is done  only if the nostale_ro nfs mount option is specified.

If the cache lookups fail,use the i_pos value to find the directory entry
of the inode and rebuild the inode.Since this involves accessing the FAT

ea3983ace6 fat: restructure export_operations
Define two nfs export_operation structures,one for 'stale_rw' mounts and
the other for 'nostale_ro'.  The latter uses i_pos as a basis for encoding
and decoding file handles.

Also, assign i_pos to kstat->ino.  The logic for rebuilding the inode is
added in the subsequent patches.

2628b7a6ac fat: introduce 2 new values for the -o nfs mount option
This patchset eliminates the client side ESTALE errors when a FAT
partition exported over NFS has its dentries evicted from the cache.  The
idea is to find the on-disk location_'i_pos' of the dirent of the inode
that has been evicted and use it to rebuild the inode.

This patch:

Provide two possible values 'stale_rw' and 'nostale_ro' for the -o nfs
mount option.The first one allows all file operations but does not reduce
ESTALE errors on memory constrained systems.  The second one eliminates
ESTALE errors but mounts the filesystem as read-only.  Not specifying a
value defaults to 'stale_rw'.

7f78e03513 fs: Limit sys_mount to only request filesystem modules.
Modify the request_module to prefix the file system type with "fs-"
and add aliases to all of the filesystems that can be built as modules
to match.

A common practice is to build all of the kernel code and leave code
that is not commonly needed as modules, with the result that many
users are exposed to any bug anywhere in the kernel.

Looking for filesystems with a fs- prefix limits the pool of possible
modules that can be loaded by mount to just filesystems trivially
making things safer with no real cost.

Using aliases means user space can control the policy of which
filesystem modules are auto-loaded by editing /etc/modprobe.d/*.conf
with blacklist and alias directives.  Allowing simple, safe,
well understood work-arounds to known problematic software.

This also addresses a rare but unfortunate problem where the filesystem
name is not the same as it's module name and module auto-loading
would not work.  While writing this patch I saw a handful of such
cases.  The most significant being autofs that lives in the module
autofs4.

This is relevant to user namespaces because we can reach the request
module in get_fs_type() without having any special permissions, and
people get uncomfortable when a user specified string (in this case
the filesystem type) goes all of the way to request_module.

After having looked at this issue I don't think there is any
particular reason to perform any filtering or permission checks beyond
making it clear in the module request that we want a filesystem
module.  The common pattern in the kernel is to call request_module()
without regards to the users permissions.  In general all a filesystem
module does once loaded is call register_filesystem() and go to sleep.
Which means there is not much attack surface exposed by loading a
filesytem module unless the filesystem is mounted.  In a user
namespace filesystems are not mounted unless .fs_flags = FS_USERNS_MOUNT,
which most filesystems do not set today.

I'm not sure why, but the hlist for each entry iterators were conceived

        list_for_each_entry(pos, head, member)

The hlist ones were greedy and wanted an extra parameter:

        hlist_for_each_entry(tpos, pos, head, member)

Why did they need an extra pos parameter? I'm not quite sure. Not only
they don't really need it, it also prevents the iterator from looking
exactly like the list iterator, which is unfortunate.

Besides the semantic patch, there was some manual work required:

 - Fix up the actual hlist iterators in linux/list.h
 - Fix up the declaration of other iterators based on the hlist ones.
 - A very small amount of places were using the 'node' parameter, this
 was modified to use 'obj->member' instead.
 - Coccinelle didn't handle the hlist_for_each_entry_safe iterator
 properly, so those had to be fixed up manually.

b88a105802 fat: mark fs as dirty on mount and clean on umount
There is no documented methods to mark FAT as dirty.  Unofficially MS
started to use reserved Byte in boot sector for this purpose, at least
since Win 2000.  With Win 7 user is warned if fs is dirty and asked to
clean it.

Different versions of Win, handle it in different ways, but always have
same meaning:

- Win 2000 and XP, set it on write operations and
  remove it after operation was finnished
- Win 7, set dirty flag on first write and remove it on umount.

We will do it as follows:

- set dirty flag on mount. If fs was initially dirty, warn user,
  remember it and do not do any changes to boot sector.
- clean it on umount. If fs was initially dirty, leave it dirty.
- do not do any thing if fs mounted read-only.
- TODO: leave fs dirty if we found some error after mount.

c6c20372bb fs/fat: strip "cp" prefix from codepage in display
Option parsing code expects an unsigned integer for the codepage option,
but prefixes and stores this option with "cp" before passing to
load_nls().  This makes the displayed option in /proc an invalid one.
Strip the prefix when printing so that the displayed option is valid for
reuse.

58156c8fbf fat: provide option for setting timezone offset
So far FAT either offsets time stamps by sys_tz.minuteswest or leaves them
as they are (when tz=UTC mount option is used).  However in some cases it
is useful if one can specify time stamp offset on his own (e.g.  when time
zone of the camera connected is different from time zone of the computer,
or when HW clock is in UTC and thus sys_tz.minuteswest == 0).

So provide a mount option time_offset= which allows user to specify offset
in minutes that should be applied to time stamps on the filesystem.

akpm: this code would work incorrectly when used via `mount -o remount',
because cached inodes would not be updated.  But fatfs's fat_remount() is
basically a no-op anyway.

Maintain an index of directory inodes by starting cluster, so that
fat_get_parent() can return the proper cached inode rather than inventing
one that cannot be traced back to the filesystem root.

Add a new msdos/vfat binary mount option "nfs" so that FAT filesystems
that are _not_ exported via NFS are not saddled with maintenance of an
index they will never use.

Finally, simplify NFS file handle generation and lookups.  An
ext2-congruent implementation is adequate for FAT needs.

21b6633d51 fat (exportfs): move NFS support code
Under memory pressure, the system may evict dentries from cache.  When the
FAT driver receives a NFS request involving an evicted dentry, it is
unable to reconnect it to the filesystem root.  This causes the request to
fail, often with ENOENT.

This is partially due to ineffectiveness of the current FAT NFS
implementation, and partially due to an unimplemented fh_to_parent method.
 The latter can cause file accesses to fail on shares exported with
subtree_check.

This patch set provides the FAT driver with the ability to
reconnect dentries.  NFS file handle generation and lookups are simplified
and made congruent with ext2.

Testing has involved a memory-starved virtual machine running 3.5-rc5 that
exports a ~2 GB vfat filesystem containing a kernel tree (~770 MB, ~40000
files, 9 levels).  Both 'cp -r' and 'ls -lR' operations were performed
from a client, some overlapping, some consecutive.  Exports with
'subtree_check' and 'no_subtree_check' have been tested.

Note that while this patch set improves FAT's NFS support, it does not
eliminate ESTALE errors completely.

The following should be considered for NFS clients who are sensitive to ESTALE:

* Mounting with lookupcache=none
  Unfortunately this can degrade performance severely, particularly for deep
  filesystems.

* Incorporating VFS patches to retry ESTALE failures on the client-side,
  such as https://lkml.org/lkml/2012/6/29/381

* Handling ESTALE errors in client application code

This patch:

Move NFS-related code into its own C file.  No functional changes.

There's no reason to call rcu_barrier() on every
deactivate_locked_super().  We only need to make sure that all delayed rcu
free inodes are flushed before we destroy related cache.

Removing rcu_barrier() from deactivate_locked_super() affects some fast
paths.  E.g.  on my machine exit_group() of a last process in IPC
namespace takes 0.07538s.  rcu_barrier() takes 0.05188s of that time.

78491189dd fat: switch to fsinfo_inode
Currently FAT file-system maps the VFS "superblock" abstraction to the
FSINFO block.  The FSINFO block contains non-essential data about the
amount of free clusters and the next free cluster.  FAT file-system can
always find out this information by scanning the FAT table, but having it
in the FSINFO block may speed things up sometimes.  So FAT file-system
relies on the VFS superblock write-out services to make sure the FSINFO
block is written out to the media from time to time.

The whole "superblock write-out" VFS infrastructure is served by the
'sync_supers()' kernel thread, which wakes up every 5 (by default) seconds
and writes out all dirty superblock using the '->write_super()' call-back.
 But the problem with this thread is that it wastes power by waking up the
system every 5 seconds no matter what.  So we want to kill it completely
and thus, we need to make file-systems to stop using the '->write_super'
VFS service, and then remove it together with the kernel thread.

This patch switches the FAT FSINFO block management from
'->write_super()'/'->s_dirt' to 'fsinfo_inode'/'->write_inode'.  Now,
instead of setting the 's_dirt' flag, we just mark the special
'fsinfo_inode' inode as dirty and let VFS invoke the '->write_inode'
call-back when needed, where we write-out the FSINFO block.

This patch also makes sure we do not mark the 'fsinfo_inode' inode as
dirty if we are not FAT32 (FAT16 and FAT12 do not have the FSINFO block)
or if we are in R/O mode.

As a bonus, we can also remove the '->sync_fs()' and '->write_super()' FAT
call-back function because they become unneeded.

330fe3c4c6 fat: mark superblock as dirty less often
Preparation for further changes.  It touches few functions in fatent.c and
prevents them from marking the superblock as dirty unnecessarily often.
Namely, instead of marking it as dirty in the internal tight loops - do it
only once at the end of the functions.  And instead of marking it as dirty
while holding the FAT table lock, do it outside the lock.

The reason for this patch is that marking the superblock as dirty will
soon become a little bit heavier operation, so it is cleaner to do this
only when it is necessary.

020ac5b6be fat: introduce special inode for managing the FSINFO block
This is patchset makes fatfs stop using the VFS '->write_super()' method
for writing out the FSINFO block.

The final goal is to get rid of the 'sync_supers()' kernel thread.  This
kernel thread wakes up every 5 seconds (by default) and calls
'->write_super()' for all mounted file-systems.  And the bad thing is that
this is done even if all the superblocks are clean.  Moreover, some
file-systems do not even need this end they do not register the
'->write_super()' method at all (e.g., btrfs).

So 'sync_supers()' most often just generates useless wake-ups and wastes
power.  I am trying to make all file-systems independent of
'->write_super()' and plan to remove 'sync_supers()' and '->write_super'
completely once there are no more users.

The '->write_supers()' method is mostly used by baroque file-systems like
hfs, udf, etc.  Modern file-systems like btrfs and xfs do not use it.
This justifies removing this stuff from VFS completely and make every FS
self-manage own superblock.

Tested with xfstests.

This patch:

Preparation for further changes.  It introduces a special inode
('fsinfo_inode') in FAT file-system which we'll later use for managing the
FSINFO block.  Note, this there is already one special inode ('fat_inode')
which is used for managing the FAT tables.

Introduce new 'MSDOS_FSINFO_INO' constant for this special inode.  It is
safe to do because FAT file-system does not store inode numbers on the
media but generates them run-time.

I've also cleaned up the comment to existing 'MSDOS_ROOT_INO' constant,
while on it.

Seeing that just about every destructor got that INIT_LIST_HEAD() copied into
it, there is no point whatsoever keeping this INIT_LIST_HEAD in inode_init_once();
the cost of taking it into inode_init_always() will be negligible for pipes
and sockets and negative for everything else.  Not to mention the removal of
boilerplate code from ->destroy_inode() instances...

0720a06a75 NLS: improve UTF8 -> UTF16 string conversion routine
The utf8s_to_utf16s conversion routine needs to be improved.  Unlike
its utf16s_to_utf8s sibling, it doesn't accept arguments specifying
the maximum length of the output buffer or the endianness of its
16-bit output.

This patch (as1501) adds the two missing arguments, and adjusts the
only two places in the kernel where the function is called.  A
follow-on patch will add a third caller that does utilize the new
capabilities.

The two conversion routines are still annoyingly inconsistent in the
way they handle invalid byte combinations.  But that's a subject for a
different patch.

FAT16 support maximum 4GB vol/file size with 64KB cluster size.

Win NT/XP/7 increased the maximum cluster size to 64KB, and file/vol
size increased 4GB also.  Although increasing, the file size of linux
FAT is still limited at 2GB.

I found that it is limited by sb->maxbytes(0x7fffffff) when partition
is formatted by FAT16.  sb->s_maxbytes in fill_super should be set to
0xffffffff like fat32.

02c24a8218 fs: push i_mutex and filemap_write_and_wait down into ->fsync() handlers
Btrfs needs to be able to control how filemap_write_and_wait_range() is called
in fsync to make it less of a painful operation, so push down taking i_mutex and
the calling of filemap_write_and_wait() down into the ->fsync() handlers.  Some
file systems can drop taking the i_mutex altogether it seems, like ext3 and
ocfs2.  For correctness sake I just pushed everything down in all cases to make
sure that we keep the current behavior the same for everybody, and then each
individual fs maintainer can make up their mind about what to do from there.

aacfc19c62 fs: simplify the blockdev_direct_IO prototype
Simple filesystems always pass inode->i_sb_bdev as the block device
argument, and never need a end_io handler.  Let's simply things for
them and for my grepping activity by dropping these arguments.  The
only thing not falling into that scheme is ext4, which passes and
end_io handler without needing special flags (yet), but given how
messy the direct I/O code there is use of __blockdev_direct_IO
in one instead of two out of three cases isn't going to make a large
difference anyway.

fa0d7e3de6 fs: icache RCU free inodes
RCU free the struct inode. This will allow:

- Subsequent store-free path walking patch. The inode must be consulted for
  permissions when walking, so an RCU inode reference is a must.
- sb_inode_list_lock to be moved inside i_lock because sb list walkers who want
  to take i_lock no longer need to take sb_inode_list_lock to walk the list in
  the first place. This will simplify and optimize locking.
- Could remove some nested trylock loops in dcache code
- Could potentially simplify things a bit in VM land. Do not need to take the
  page lock to follow page->mapping.

The downsides of this is the performance cost of using RCU. In a simple
creat/unlink microbenchmark, performance drops by about 10% due to inability to
reuse cache-hot slab objects. As iterations increase and RCU freeing starts
kicking over, this increases to about 20%.

In cases where inode lifetimes are longer (ie. many inodes may be allocated
during the average life span of a single inode), a lot of this cache reuse is
not applicable, so the regression caused by this patch is smaller.

The cache-hot regression could largely be avoided by using SLAB_DESTROY_BY_RCU,
however this adds some complexity to list walking and store-free path walking,
so I prefer to implement this at a later date, if it is shown to be a win in
real situations. I haven't found a regression in any non-micro benchmark so I
doubt it will be a problem.

does put significant restrictions on what may be done from the callback,
however there don't seem to have been any problems with in-tree fses.
If some strange use case pops up that _really_ cannot cope with the
rcu-walk rules, we can just add new rcu-unaware callbacks, which would
cause name lookup to drop out of rcu-walk mode.

For in-tree filesystems, this is just a mechanical change.

The lock_kernel in fat_put_super is not needed because
it only protects the super block itself and we know that
no other thread can reach it because we are about to
kfree the object.

In the two fill_super functions, this converts the locking

us on the safe side.

db71922217 BKL: Explicitly add BKL around get_sb/fill_super
This patch is a preparation necessary to remove the BKL from do_new_mount().
It explicitly adds calls to lock_kernel()/unlock_kernel() around
get_sb/fill_super operations for filesystems that still uses the BKL.

I've read through all the code formerly covered by the BKL inside
do_kern_mount() and have satisfied myself that it doesn't need the BKL
any more.

do_kern_mount() is already called without the BKL when mounting the rootfs
and in nfsctl. do_kern_mount() calls vfs_kern_mount(), which is called
from various places without BKL: simple_pin_fs(), nfs_do_clone_mount()
through nfs_follow_mountpoint(), afs_mntpt_do_automount() through
afs_mntpt_follow_link(). Both later functions are actually the filesystems
follow_link inode operation. vfs_kern_mount() is calling the specified
get_sb function and lets the filesystem do its job by calling the given
fill_super function.

Therefore I think it is safe to push down the BKL from the VFS to the
low-level filesystems get_sb/fill_super operation.

dd3932eddf block: remove BLKDEV_IFL_WAIT
All the blkdev_issue_* helpers can only sanely be used for synchronous
caller.  To issue cache flushes or barriers asynchronously the caller needs
to set up a bio by itself with a completion callback to move the asynchronous
state machine ahead.  So drop the BLKDEV_IFL_WAIT flag that is always
specified when calling blkdev_issue_* and also remove the now unused flags
argument to blkdev_issue_flush and blkdev_issue_zeroout.  For
blkdev_issue_discard we need to keep it for the secure discard flag, which
gains a more descriptive name and loses the bitops vs flag confusion.

9cb569d601 remove SWRITE* I/O types
These flags aren't real I/O types, but tell ll_rw_block to always
lock the buffer instead of giving up on a failed trylock.

Instead add a new write_dirty_buffer helper that implements this semantic
and use it from the existing SWRITE* callers.  Note that the ll_rw_block
code had a bug where it didn't promote WRITE_SYNC_PLUG properly, which
this patch fixes.

In the ufs code clean up the helper that used to call ll_rw_block
to mirror sync_dirty_buffer, which is the function it implements for
compound buffers.

2c27c65ed0 check ATTR_SIZE contraints in inode_change_ok
Make sure we check the truncate constraints early on in ->setattr by adding
those checks to inode_change_ok.  Also clean up and document inode_change_ok
to make this obvious.

As a fallout we don't have to call inode_newsize_ok from simple_setsize and
simplify it down to a truncate_setsize which doesn't return an error.  This
simplifies a lot of setattr implementations and means we use truncate_setsize
almost everywhere.  Get rid of fat_setsize now that it's trivial and mark
ext2_setsize static to make the calling convention obvious.

Keep the inode_newsize_ok in vmtruncate for now as all callers need an
audit for its removal anyway.

Note: setattr code in ecryptfs doesn't call inode_change_ok at all and
needs a deeper audit, but that is left for later.

eafdc7d190 sort out blockdev_direct_IO variants
Move the call to vmtruncate to get rid of accessive blocks to the callers
in prepearation of the new truncate calling sequence.  This was only done
for DIO_LOCKING filesystems, so the __blockdev_direct_IO_newtrunc variant
was not needed anyway.  Get rid of blockdev_direct_IO_no_locking and
its _newtrunc variant while at it as just opencoding the two additional
paramters is shorted than the name suffix.

1b061d9247 rename the generic fsync implementations
We don't name our generic fsync implementations very well currently.
The no-op implementation for in-memory filesystems currently is called
simple_sync_file which doesn't make too much sense to start with,
the the generic one for simple filesystems is called simple_fsync
which can lead to some confusion.

This patch renames the generic file fsync method to generic_file_fsync
to match the other generic_file_* routines it is supposed to be used
with, and the no-op implementation to noop_fsync to make it obvious
what to expect.  In addition add some documentation for both methods.

When using the string representation of a random counter as part of the base
name, ensure that it is no longer than 4 bytes.

Since we are repeatedly decrementing the counter in a loop until we have found a
unique base name, the counter may wrap around zero; therefore, it is not enough
to mask its higher bits before entering the loop, this must be done inside the
loop.

5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

After d_find_alias(), vfat_lookup() checks !(->d_flags & DCACHE_DISCONNECTED)
without IS_ROOT().  This means it hits non-anonymous but disconnected
dentry. (NOTE: d_splice_alias() doesn't clear DCACHE_DISCONNECTED)

But, vfat_lookup() has interest to alias if it was non-anonymous. So,
this adds vfat_d_anon_disconn() helper to check it correctly.

Another bug is refcnt leak. It needs dput() for uninterested alias.

If we didn't check sb->s_dirt, it will update the FSINFO
unconditionally. It will reduce the filetime of flash base device.

So, this checks sb->s_dirt. sb->s_dirt is racy, however FSINFO is just
hint. So even if there is race, and we hit it, it would not become big
problem.

And this also is as workaround of suspend problem.

Because, with "shortname=lower", copying one FAT filesystem tree to
another FAT filesystem tree using Linux results in semantically
different filesystems. (E.g.: Filenames which were once "all
uppercase" are now "all lowercase").

So, this changes the default of "shortname=lower" to "shortname=mixed".

74675a5850 NLS: update handling of Unicode
This patch (as1239) updates the kernel's treatment of Unicode.  The
character-set conversion routines are well behind the current state of
the Unicode specification: They don't recognize the existence of code
points beyond plane 0 or of surrogate pairs in the UTF-16 encoding.

The old wchar_t 16-bit type is retained because it's still used in
lots of places.  This shouldn't cause any new problems; if a
conversion now results in an invalid 16-bit code then before it must
have yielded an undefined code.

Difficult-to-read names like "utf_mbstowcs" are replaced with more
transparent names like "utf8s_to_utf16s" and the ordering of the
parameters is rationalized (buffer lengths come immediate after the
pointers they refer to, and the inputs precede the outputs).
Fortunately the low-level conversion routines are used in only a few
places; the interfaces to the higher-level uni2char and char2uni
methods have been left unchanged.

Push down lock_super into ->write_super instances and remove it from the
caller.

Following filesystem don't need ->s_lock in ->write_super and are skipped:

 * bfs, nilfs2 - no other uses of s_lock and have internal locks in
	->write_super
 * ext2 - uses BKL in ext2_write_super and has internal calls without s_lock
 * reiserfs - no other uses of s_lock as has reiserfs_write_lock (BKL) in
 	->write_super
 * xfs - no other uses of s_lock and uses internal lock (buffer lock on
	superblock buffer) to serialize ->write_super.  Also xfs_fs_write_super
	is superflous and will go away in the next merge window

6cfd014842 push BKL down into ->put_super
Move BKL into ->put_super from the only caller.  A couple of
filesystems had trivial enough ->put_super (only kfree and NULLing of
s_fs_info + stuff in there) to not get any locking: coda, cramfs, efs,
hugetlbfs, omfs, qnx4, shmem, all others got the full treatment.  Most
of them probably don't need it, but I'd rather sort that out individually.
Preferably after all the other BKL pushdowns in that area.

removed since we don't do lock_super() at all in generic_shutdown_super()
now]

8c85e12512 remove ->write_super call in generic_shutdown_super
We just did a full fs writeout using sync_filesystem before, and if
that's not enough for the filesystem it can perform it's own writeout
in ->put_super, which many filesystems already do.

Move a call to foofs_write_super into every foofs_put_super for now to
guarantee identical behaviour until it's cleaned up by the individual
filesystem maintainers.

Exceptions:

 - affs already has identical copy & pasted code at the beginning of
   affs_put_super so no need to do it twice.
 - xfs does the right thing without it and I have changes pending for
   the xfs tree touching this are so I don't really need conflicts
   here..

85c7859190 FAT: add 'errors' mount option
On severe errors FAT remounts itself in read-only mode. Allow to
specify FAT fs desired behavior through 'errors' mount option:
panic, continue or remount read-only.

`mount -t [fat|vfat] -o errors=[panic,remount-ro,continue] \
	<bdev> <mount point>`

This is analog to ext2 fs 'errors' mount option.

the task_struct during the introduction of COW creds.

Change some task->e?[ug]id to task_e?[ug]id().  In some places it makes more
sense to use RCU directly rather than a convenient wrapper; these will be
addressed by later patches.

2bdf67eb16 fat: mmu_private race fix
mmu_private is 64bits value, hence it's not atomic to update.

So, the access rule for mmu_private is we must hold ->i_mutex.  But,
fat_get_block() path doesn't follow the rule on non-allocation path.

FAT has the ATTR_RO (read-only) attribute. But on Windows, the ATTR_RO
of the directory will be just ignored actually, and is used by only
applications as flag. E.g. it's setted for the customized folder by
Explorer.

This adds "rodir" option. If user specified it, ATTR_RO is used as
read-only flag even if it's the directory. Otherwise, inode->i_mode

9183482f5d fat: Fix ATTR_RO in the case of (~umask & S_WUGO) == 0
If inode->i_mode doesn't have S_WUGO, current code assumes it means
ATTR_RO.  However, if (~[ufd]mask & S_WUGO) == 0, inode->i_mode can't
hold S_WUGO. Therefore the updated directory entry will always have
ATTR_RO.

This adds fat_mode_can_hold_ro() to check it. And if inode->i_mode
can't hold, uses -i_attrs to hold ATTR_RO instead.

With this, we don't set ATTR_RO unless users change it via ioctl() if
(~[ufd]mask & S_WUGO) == 0.

And on FAT_IOCTL_GET_ATTRIBUTES path, this adds ->i_mutex to it for
not returning the partially updated attributes by FAT_IOCTL_SET_ATTRIBUTES
to userland.

9c0aa1b87b fat: Cleanup FAT attribute stuff
This adds three helpers:

fat_make_attrs() - makes FAT attributes from inode.
fat_make_mode()  - makes mode_t from FAT attributes.
fat_save_attrs() - saves FAT attributes to inode.

Then this replaces: MSDOS_MKMODE() by fat_make_mode(), fat_attr() by
fat_make_attrs(), ->i_attrs = attr & ATTR_UNUSED by fat_save_attrs().
And for root inode, those is used with ATTR_DIR instead of bogus
ATTR_NONE.

1c13a243a4 fat: Kill d_invalidate() in vfat_lookup()
d_invalidate() for positive dentry doesn't work in some cases
(vfsmount, nfsd, and maybe others). shrink_dcache_parent() by
d_invalidate() is pointless for vfat usage at all.

So, this kills it, and intead of it uses d_move().

To save old behavior, this returns alias simply for directory (don't
change pwd, etc..). the directory lookup shouldn't be important for
performance.

1b52467243 fat: Fix/Cleanup dcache handling for vfat
- Add comments for handling dcache of vfat.

- Separate case-sensitive case and case-insensitive to
  vfat_revalidate() and vfat_ci_revalidate().

  vfat_revalidate() doesn't need to drop case-insensitive negative
  dentry on creation path.

- Current code is missing to set ->d_revalidate to the negative dentry
  created by unlink/etc..

  This sets ->d_revalidate always, and returns 1 for positive
  dentry. Now, we don't need to change ->d_op dynamically anymore,
  so this just uses sb->s_root->d_op to set ->d_op.

- d_find_alias() may return DCACHE_DISCONNECTED dentry. It's not
  the interesting dentry there. This checks it.

- Add missing LOOKUP_PARENT check. We don't need to drop the valid
  negative dentry for (LOOKUP_CREATE | LOOKUP_PARENT) lookup.

- For consistent filename on creation path, this drops negative dentry
  if we can't see intent.

In fat_search_long() if fat_parse_long() returns a -ve value we return
without first freeing unicode.  This patch free's them on this error path.

7decd1cb03 fat: Fix and cleanup timestamp conversion
This cleans date_dos2unix()/fat_date_unix2dos() up. New code should be
much more readable.

And this fixes those old functions. Those doesn't handle 2100
correctly. 2100 isn't leap year, but old one handles it as leap year.
Also, with this, centi sec is handled and is fixed.

a447c09324 vfs: Use const for kernel parser table
This is a much better version of a previous patch to make the parser
tables constant. Rather than changing the typedef, we put the "const" in
all the various places where its required, allowing the __initconst
exception for nfsroot which was the cause of the previous trouble.

This was posted for review some time ago and I believe its been in -mm
since then.

5f22ca9b13 vfat: fix 'sync' mount deadlock due to BKL->lock_super conversion
There was another FAT BKL conversion deadlock reported by Bart
Trojanowski due to the BKL being used as a recursive lock by FAT, which
was missed because it only triggers with 'sync' (or 'dirsync') mounts.

The recursion worked for the BKL, but after the conversion to lock_super
(which uses a mutex), it just deadlocks.

Thanks to Bart for debugging this and testing the fix.  The lock
debugging information from the original report:

where the deadlock is due to the nesting of lock_super from vfat_unlink
to fat_write_inode:

and the fix is to simply remove the use of lock_super() in fat_write_inode.

The lock_super() there had been just an automatic conversion of the
kernel lock to the superblock lock, but no locking was actually needed
there, since the code in fat_write_inode already protected all relevant
accesses with a spinlock (sbi->inode_hash_lock to be exact).  The only
code inside the BKL (and thus the superblock lock) was accesses tp local
variables or calls to functions that have long been SMP-safe (i.e.
sb_bread, mark_buffe_dirty and brlese).

Bart reports:
 "Looks good.  I ran 10 parallel processes creating 1M files truncating
  them, writing to them again and then deleting them.  This patch fixes
  the issue I ran into.

b1da47e29e [patch 3/4] fat: dont call notify_change
The FAT_IOCTL_SET_ATTRIBUTES ioctl() calls notify_change() to change
the file mode before changing the inode attributes.  Replace with
explicit calls to security_inode_setattr(), fat_setattr() and
fsnotify_change().

This is equivalent to the original.  The reason it is needed, is that
later in the series we move the immutable check into notify_change().
That would break the FAT_IOCTL_SET_ATTRIBUTES ioctl, as it needs to
perform the mode change regardless of the immutability of the file.

[Fix error if fat is built as a module.  Thanks to OGAWA Hirofumi for
noticing.]

b271e067c8 fatfs: add UTC timestamp option
Provide a new mount option ("tz=UTC") for DOS (vfat/msdos) filesystems,
allowing timestamps to be in coordinated universal time (UTC) rather than
local time in applications where doing this is advantageous.

In particular, portable devices that use fat/vfat (such as digital
cameras) can benefit from using UTC in their internal clocks, thus
avoiding daylight saving time errors and general time ambiguity issues.
The user of the device does not have to worry about changing the time when
moving from place or when daylight saving changes.

The new mount option, when set, disables the counter-adjustment that Linux
currently makes to FAT timestamp info in anticipation of the normal
userspace time zone correction.  When used in this new mode, all daylight
saving time and time zone handling is done in userspace as is normal for
many other filesystems (like ext3).  The default mode, which remains
unchanged, is still appropriate when mounting volumes written in Windows
(because of its use of local time).

I originally based this patch on one submitted last year by Paul Collins,
but I updated it to work with current source and changed variable/option
naming.  Ogawa Hirofumi (who maintains these filesystems) and I discussed
this patch at length on lkml, and he suggested using the option name in
the attached version of the patch.  Barry Bouwsma pointed out a good
addition to the patch as well.

8f5934278d Replace BKL with superblock lock in fat/msdos/vfat
This replaces the use of the BKL in the FAT family of filesystems with the
existing superblock lock instead.

The code already appears to do mostly proper locking with its own private
spinlocks (and mutexes), but while the BKL could possibly have been
dropped entirely, converting it to use the superblock lock (which is just
a regular mutex) is the conservative thing to do.

As a per-filesystem mutex, it not only won't have any of the possible
latency issues related to the BKL, but the lock is obviously private to
the particular filesystem instance and will thus not cause problems for
entirely unrelated users like the BKL can.

2d518f84e5 fat: relax the permission check of fat_setattr()
New chmod() allows only acceptable permission, and if not acceptable, it
returns -EPERM.  Old one allows even if it can't store permission to on
disk inode.  But it seems too strict for users.

E.g.  https://bugzilla.redhat.com/show_bug.cgi?id=449080: With new one,
rsync couldn't create the temporary file.

So, this patch allows like old one, but now it doesn't change the
permission if it can't store, and it returns 0.

Also, this patch fixes missing check.

The on-disk media specification field in FAT is only 8-bits, so testing for
<=0xff is pointless, and can generate a "comparison is always true due to
limited range of data type" warning.

While we're there, convert FAT_VALID_MEDIA() into a C function - the present
implementation is buggy: it generates either one or two references to its
argument.

This patch fix the problem that the buffer allocated for convert of unicode to
utf8 in fat/dir.c is too small.

And cannot handle filename with 255 asian characters when mounted with utf8
options.

Also it fix the filename length limitation checking in vfat/namei.c that the
filename length should be checked against the number of converted unicode
characters.

Not the length before NLS/UTF8 converted.

Currently, free_clusters is not updated until it is trusted, because
Windows doesn't update it correctly.

But if user is using FAT driver of Linux, it updates free_clusters
correctly.  Instead, this updates it even if it's untrusted, so if
free_clustes is correct, now keep correct value.

e97e8de388 fat: fat_setattr() fix
Fix fat_setattr() on the case of showexec option. If user specified
showexec option, inode->i_mode may not have S_IXUGO. This just use
inode->i_mode to fix it.

And with this patch, we don't allow chmod() on memory inode, it's just
bad behaviour. IOW, we allow changing S_IWUGO only which can be stored
to disk.

19c561a60f fs/fat/: refine chmod checks
Prohibit mode changes in non-quiet mode that cannot be stored reliably with
the on-disk format.

Suppose a vfat filesystem is mounted with umask=0 and [not-quiet].  Then
all files will have mode 0777.  Trying to change the owner will fail,
because fat does not know about owners or groups.  chmod 0770, on the other
hand, will succeed, even though fat does not know about the permission
triplet [user/group/other].

So this patch changes fat's not-quiet behavior so that only UNIX modes are
accepted that can be mapped lossless between the fat disk format and the
local system.  There is only one attribute, and that is the readonly
attribute, which is mapped to the UNIX write permission bit(s).  chmod 0555
is therefore valid (taking away the +w bits <=> setting the readonly
attribute).  Since chmod 0775 and chmod 0755 is an ambiguous case as to
whether to set or clear the readonly bit, these modes are also denied.

In quiet mode, chmod and chown will continue to "succeed" as they did
before, meaning that a subsequent stat() will temporarily return the new
mode as long as the inode is not reread from disk, and chown will silently
do nothing, not even return the new uid/gid in stat().

9f966be899 fat: optimize fat_count_free_clusters()
On large partition, scanning the free clusters is very slow if users
doesn't use "usefree" option.

For optimizing it, this patch uses sb_breadahead() to read of FAT
sectors. On some user's 15GB partition, this patch improved it very
much (1min => 600ms).

The following is the result of 2GB partition on my machine.

4ba9b9d0ba Slab API: remove useless ctor parameter and reorder parameters
Slab constructors currently have a flags parameter that is never used.  And
the order of the arguments is opposite to other slab functions.  The object
pointer is placed before the kmem_cache pointer.

Convert

        ctor(void *object, struct kmem_cache *s, unsigned long flags)

to

        ctor(struct kmem_cache *s, void *object)

throughout the kernel

20c2df83d2 mm: Remove slab destructors from kmem_cache_create().
Slab destructors were no longer supported after Christoph's
c59def9f22 change. They've been
BUGs for both slab and slub, and slob never supported them
either.

This rips out support for the dtor pointer from kmem_cache_create()
completely and fixes up every single callsite in the kernel (there were
about 224, not including the slab allocator definitions themselves,
or the documentation references).

fs.h.  fs.h is already far too large and there are very few places needing the

98283bb49c fat: Fix the race of read/write the FAT12 entry
FAT12 entry is 12bits, so it needs 2 phase to update the value.  And
writer and reader access it without any lock, so reader can get the
half updated value.

This fixes the long standing race condition by adding a global
spinlock to only FAT12 for avoiding any impact against FAT16/32.

They can use generic_file_splice_read() instead. Since sys_sendfile() now
