PCI: Drop msi_lower_address_reg() and msi_upper_address_reg() macros
msi_lower_address_reg() and msi_upper_address_reg() don't provide any useful abstraction, so drop them. Signed-off-by: Bjorn Helgaas <bhelgaas@google.com>
