[PATCH] IB/mthca: add support for new MT25204 HCA
Decouple table of HCA features from exact HCA device type. Add a current FW version field so we can warn when someone is using old FW. Add support for new MT25204 HCA. Remove the warning about mem-free support, since it should be pretty solid at this point.
