[PATCH] paravirt: Add APIC accessors to paravirt-ops.

Add APIC accessors to paravirt-ops.  Unfortunately, we need two write
functions, as some older broken hardware requires workarounds for
Pentium APIC errata - this is the purpose of apic_write_atomic.
